Actinomycosis is rare, chronic, slowly progressive disease caused by gram-positive anaerobic organisms from the Actinomycosis family that normally colonizes the oral cavity. Actinomycotic osteomyelitis is even more rare and refractory disease because diagnosis by bacterial culture is not easy. In our case, 80-year-old man visited our clinic with a complaint of swelling and severe sinus tracts without teeth evolvement on anterior mandible. Computed tomography (CT) scan demonstrated erosive bone destruction on anterior mandible, clinically suspected actinomycotic osteomyelitis. The patient also had thoracic actinomycosis on Lt. lower lung. We could diagnosis actinomycosis by histopathologic examination. He treated by conservative surgery and long term antibiotics. After 2 year, no recurrence was seen in CT scan.

Pediatric dentists often meet children with abnormal in number of tooth. Presence of supernumerary teeth is frequent cause of malocclusion. Etiology for supernumerary teeth is not yet clearly defined, but it is thought to be caused by excessive proliferation of dental lamina by hereditary and environmental factors. Supernumerary teeth occur in the maxilla nine times more frequently than in the mandible. Most common supernumerary tooth is the mesiodens in the maxilla, and some are observed in the maxillary molar and mandibular premolar. It occurs rarely in the mandibular incisor region with the incidence of 1-2% among all supernumerary teeth. A six-year old boy visited the department of the pediatric dentistry, Yonsei University Dental Hospital, with the chief complaint of crowded supernumerary teeth on the mandibular incisor region. Clinical and radiographic examinations revealed six permanent mandibular incisors similar in size, shape, and length. Further investigation using computed tomography(CT) was proceeded on the mandible to measure and compare morphologic features and positions of the six incisors. Then, we decided to remove two incisors which were already erupted. Periodic check-up was followed to monitor the dental development and spontaneous positional enhancement of the remaining four incisors in the mandible.

The purpose of this study was to determine and compare the shear bond strength of two self-etching primer systems to primary teeth contaminated with saliva and blood. Clearfil SE Bond and AQ Bond were evaluated. One hundred specimens were made by seventy-five deciduous teeth(fifty anterior and twenty-five posterior teeth) and divided randomly into ten groups. Small flat dentinal surfaces were prepared by grinding the buccal, lingual and labial areas. Specific surface treatments were applied to each group: (1) a self-etching primer application(control group), (2) saliva contamination followed by primer(Group I), (3) primer curing followed by saliva contamination (Group II), (4) blood contamination followed by primer(Group III), (5) primer curing followed by blood contamination(Group IV). After bonding of composite resin(Z100, 3M, USA) to contaminated sample surfaces and thermocycling(1,000 cycles), shear bond strengths were measured using Universal Testing Machine(Zwick Z020, Zwick Co., Germany). The results were as follows; 1. Group I showed lower shear bond strength than control group but no statistically significant difference was found(P>0.05). 2. Group II and blood contamination group(Group III & IV) showed significantly lower shear bond strength than control group(P<0.01). 3. The shear bond strength of Clearfil SE Bond was significantly higher than that of AQ Bond(P<0.05).

Background: To introduce a novel technique for advancement genioplasty helping surgeons to avoid soft tissue difficulties especially in short-faced patients with deep mentolabial fold and everted lower lip. Case presentation: In a trapezius-shaped, osteotomy was performed in the chin region. The mobilized segment was advanced, and the existing gap was grafted using interpositional allograft materials. Each side had been fixated by three-hole plates and two screws. The outcomes revealed no change in lower anterior teeth vitality. The patients did not report any changes of sensation in lower lip and chin either. The measurements indicated no increase in depth of mentolabial fold in patients undergoing this surgical technique. The postoperative evaluation showed a successful esthetic outcome for the patient and the surgeon concurrently. Conclusion: Based on our experience, the authors concluded that the Balcony technique is a simple and reliable procedure for patients with a deep mentolabial fold.
The patient, 18-year-old girl, had a class III malocclusion with the lateral compression of the maxilla due to the premature loss of the canines, mild mandibular prognathism, and mouth breathing habit. The treatment plan consisted of 1) rapid maxillary expansion 2) a period of retention 3) extraction of the first mandibular premolars instead of surgical correction 4) completion of orthodontic treatment with multibanded system. The maxilla was separated in the midline by the application of orthopedic forces via a cemented rapid maxillary expansion device. After 18 months, She gained functional overbite-overjet relationship, good interdigitation of buccal segments, and facial harmony due to the retraction of lower anterior teeth.
The stability of teeth is an important measure of the periodontal health. This study was designed to determine if there was a relation between the patterns of disclusion and a tooth mobility. An evaluation was made on 117 persons with Angle's class I occlusion who were free from histories of orthodontics, removable or fixed prosthesis, and anterior crowding or open contacts. The results were as follows: 1. In this study, 17.52% of the subjects were exhibited canine protected occlusion. 2. In protrusive movements, 75.21% of the subjects were exhibited incisor guided occlusion and the subjects of canine guided occlusion was the smallest. 3. The canine tooth of mouths having canine-protected occlusions had slightly lower mean tooth mobility scores than the canine of mouths having group function occlsion, but there was no significances. 4. The tooth mobility score of central incisor had higher than that of lateral incisor.
Familial gigantiform cementoma is a rare fibre-cemento-osseous disease of the jaws which appears to be transmitted as an autosomal dominant trait with variable expressivity of the phenotype. A 7-year-old girl visited DKUDH complaining of the painless facial deformity. Clinically, significant facio-lingual expansion was observed at the left maxilla, left mandibular body and symphysis portion. Malposition of lower anterior teeth was found. Panoramic radiograph and CT scan showed the extensive expansile mixed lesion at maxilla and mandible. Bone scan revealed hot spot at the maxilla and left side of mandible. Histologic examination revealed moderately dense fibrous connective tissue with scattered masses resembling cementum. The patient's mother had a history of the mandibular resection due to benign tumor. Her younger brother had buccal expansion of right mandible. We report our finding of a family that has exhibited clinical, radiographic and histologic findings consistent with the familial gigantiform cementoma.

Self-injurious behavior is defined as deliberate harm to one's own body without suicidal intent. It usually occurs as head banging or hitting, skin cutting, or finger biting and includes ocular, genital and oral self-mutilation. Lesch-Nyhan syndrome is a rare X-linked recessively inherited disorder, caused by complete absence of hypoxanthin-guanine phosphoribosyl transferase(HPRT) activity. Clinical presentation is characterized by mental retardation, chorea, athetosis, hyperuricemia, uricosuria and self-mutilating behavior. In these patients, the most typical feature is loss of tissue from biting themselves, even though they are not insensitive to pain. The dental management of self-mutilation includes treatment with appliances such as soft mouthguard or lip bumper, extraction of all the teeth, and orthognathic surgery. We report a 25-month-old boy who was a known case of Lesch-Nyhan syndrome and presented with severe self-mutilation wound on his lower lip. Vital pulpectomy and coronal resection was done as a more conservative approach than extracting all primary anterior teeth. Due to maintaining the root portion of the teeth in the bone, it is expected that the normal growth of the alveolar bone will be achieved.
The purpose of this study was to examine and analyze the basic conditions of dental health of cleft lip and palate patients at early mixed dentition (7-year-old, experimental group A) and early permanent dentition period (12,13-year old, experimental group B) and compare them with those of normal children. The sample was consisted of 25 patients at the age of 7 years, 15 at the age of 12 years and 5 at the age of 13 years. Two trained dentists examined orthopantomographs and clinical photos at the first visit of each patient and distinguished the states by teeth and dental surface and filled them on the prescribed forms. DMF rate, DMF index, FT rate, mean number of permanent teeth with fissure sealants, DMFS index and mean percentage of caries experienced maxillary anterior 6 teeth by tooth type are calculated and compared to those of other studies examined normal children. Oral hygiene of experimental groups is poorer than control groups'. Preventive and treatment rate of experimental groups' are lower than control groups'. More concem and effort are needed about prevention and treatment of dental caries of cleft lip and palate patients.
Objective : To investigate the prevalence of dental anomalies and to determine the associations between dental anomalies in permanent teeth Materials and methods : The samples were 1,240 patients (760 females and 480 males, mean age=15.1 years) who visited the Samsung Medical Center. Dental anomalies were diagnosed using pre-treatment dental casts, radiographs, clinical examinations, and medical/dental histories. Prevalence and association were investigated according to gender and sidedness. The Chi-square test was performed for statistical analysis. Results : The most common missing tooth was the lower lateral incisor, followed by the lower and upper second premolars. This particular dental anomaly is characteristic of the East Asian population (prevalence of congenital missing tooth=12.3%). The upper anterior area was the most frequently affected area (prevalence of supernumerary tooth was 1.5%). The presence of a supernumerary tooth was more prevalent in males than in females (p<.05, odds ratio=3.2). The most frequently affected tooth was the upper canine (prevalence of impacted tooth=4.3%). Unilateral impaction of the upper canine occurred significantly more often compared to bilateral impaction (p<.001). The prevalence of peg lateralis was 2.7%. The presence of congenital missing tooth was closely associated with peg lateralis (p<.01). If children aged 7~8 years have peg lateralis, the rest of the teeth should be checked for congenital absences. Conclusion : The early detection of dental anomalies and understanding of their associations help clinicians determine the appropriate treatment timing and methods of dealing with these anomalies.
