Efficacy of Decompression and Fixation for Metastatic Spinal Cord Compression : Analysis of Factors Prognostic for Survival and Postoperative Ambulation

  • Objective : The goals of surgical intervention for metastatic spinal cord compression (MSCC) are prolonging survival and improving quality of life. Non-ambulatory paraplegic patients, either at presentation or after treatment, have a much shorter life expectancy than ambulatory patients. We therefore analyzed prognostic factors for survival and postoperative ambulation in patients surgically treated for MSCC. Methods : We assessed 103 patients with surgically treated MSCC who presented with lower extremity weakness between January 2001 and December 2008. Factors prognostic for overall survival (OS) and postoperative ambulation, including surgical method, age, sex, primary tumor site, metastatic spinal site, surgical levels, Tokuhashi score, and treatment with chemo- or radiation therapy, were analyzed retrospectively. Results : Median OS was significantly longer in the postoperatively ambulatory group [11.0 months; 95% confidence interval (CI), 9.29-12.71 months] than in the non-ambulatory group (5.0 months; 95% CI, 1.80-8.20 months) ($p$=0.035). When we compared median OS in patients with high (9-11) and low (0-8) Tokuhashi scores, they were significantly longer in the former (15.0 months; 95% CI, 9.29-20.71 months vs. 9.0 months; 95% CI, 7.48-10.52 months; $p$=0.003). Multivariate logistic regression analysis showed that preoperative ambulation with or without aid [odds ratio (OR) 5.35; 95% CI 1.57-18.17; $p$=0.007] and hip flexion power greater than grade III (OR 6.23; 95% CI, 1.29-7.35; $p$=0.038) were prognostic of postoperative ambulation. Conclusion : We found that postoperative ambulation and preoperative high Tokuhashi score were significantly associated with longer patient survival. In addition, preoperative hip flexion power greater than grade III was critical for postoperative ambulation.

Implantable Intrathecal Drug Delivery Pump in Complex Regional Pain Syndrome Patient -A case report- (복합부위통증증후군 환자에게 시행한 매몰식 펌프를 이용한 지주막하강 내 약물투입 -증례보고-)

  • Implantable intrathecal pump is one of the therapeutic options for intractable pain. A 24-year-old male with complex regional pain syndrome was suffering from right lower extremity pain. He had all modalities of treatment including spinal cord stimulator. However, his pain had been worse in the past 6 months. His visual analogue pain scale (VAS) was 8-10 and he could not sit or walk. Only opioid was thought to be effective. Then, intrathecal pump was considered. We estimated the minimal effective dose of spinal morphine before implantation. 0.3 mg of morphine was injected intrathecally as a starting dose. Dosage had been increased up to 0.8 mg in 10 days. His VAS score decreased from 8 to 5. He could sleep without pain and walk with crutch. Therefore, intrathecal pump was inserted. He could tolerate to pain. This case suggests that intrathecal morphine delivery can provide effective treatment for intractable non-malignant pain.

A Case of Adult onset Henoch-Sch$\ddot{o}$nlein Purpura with Acute Renal Failure (급성 신부전을 동반한 성인 Henoch-Sch$\ddot{o}$nlein 자반증 1예)

  • Henoch-Sch$\ddot{o}$nlein purpura (HSP) is a leukocytoclastic vasculitis of small vessels with deposition of IgA, commonly resulting in skin, joint, gastrointestinal, and kidney involvement. HSP is an uncommon disorder in adults and accounts for 0.6% to 2% of adult nephropathy. We report a case of HSP with acute renal failure successfully treated with corticosteroid. In this case, the patient presented with vasculitic purpuric rash on lower extremity, arthralgia in the wrist, abdominal pain, hematochezia, oliguria and azotemia. Abdominal CT showed wall thickening of the small and large bowels. Skin biopsy revealed leukocytoclastic vasculitis. Percutaneous renal biopsy showed no crescent formation, but mesangial IgA and $C_3$ deposits were observed by immunofluorescence. The patient was treated with corticosteroid (1mg/kg per day) and hemodialysis. After treatment, renal function improved and purpuric lesion, arthralgia and abdominal pain disappeared. Thus, when adults present with purpuric rash and rapidly progressive glomerulonephritis (RPGN), HSP should be a diagnostic consideration.

The Effect of Sports Elastic Compression Stockings on Lower Extremity Edema by High Intensity Exercise (스포츠용 탄력압박스타킹이 최대 운동 시 하지의 부종에 미치는 영향)

  • 본 연구는 스포츠용 탄력 압박 스타킹(Essential compression calf sleeve, SKINS, Australia)을 착용하고 최대 운동을 하였을 때, 하지 부종에 어떠한 효과가 있는지를 알아보고자 한다. 실험 대상자는 젊은 성인 남녀 30명으로(남자 15명, 여자15명) 근 골격계 및 심혈관계에 특이 병력이나 사고에 의한 외상 등이 없었으며, 규칙적으로 운동을 하는 자는 제외하였다. 대상자들의 신체적 특성을 파악하기 위해서 자동 신장계(BSM330, Biospace, Korea)를 사용하였다. 부종정도를 알기위해서 줄자로 운동 전후의 장딴지 근(종아리)의 둘레와 체성분 분석기(InBody 720, Biospace, Korea)를 사용하여 하지의 부종 수치를 측정하였다. 최대의 운동은 호흡가스 분석기와 연결된 트레이드밀(CPEX-1, IRS, Japan)을 사용하여 변형된 BRUCE Protocol TR-3에 따라 호흡 교환율(R) 값이 1.15이상 되거나 all-out 될 때까지 하였다. 실험 대상자들은 동일 집단으로 전이효과를 방지하기 위하여 일주일의 간격을 두고 스포츠 탄력 압박 스타킹 착용 유무에 따른 동일한 실험을 두 번 하였다. 측정은 최대 운동 전후의 장딴지 근(종아리)의 둘레와 부종 수치를 하였다. 스포츠용 탄력 압박 스타킹 착용 유무와 운동 전후를 비교하기 위하여 MANOVA(다변량 분산분석)를 사용하였고 유의수준은 .05로 설정하였다. 탄력 압박 스타킹을 착용한 상태에서 최대 운동 전후의 부종 정도를 측정한 결과 스타킹을 착용하지 않았을 때 운동 전후의 둘레가 유의하게 증가하였고(p<0.05), 스타킹을 착용 하였을 때 운동 전후의 둘레가 유의하게 증가하지 않았다(p>0.05). 부종 수치(세포외액/전체체액)는 탄력 압박 스타킹을 착용하였을 때 오른쪽 다리는 유의하지 않게 감소하였고(p>0.05), 왼쪽 다리는 유의하게 감소하였다(p>0.05). 탄력 압박 스타킹을 착용하고 운동할 경우 하지의 부종 경감에 어느 정도 효과가 있다고 사료된다.

Treatment of Intraoperative Thrombosis and Spasm in Free Tissue Transfers Using the Fogarty Catheter (포가티 카테터를 이용한 유리조직 전이술 시 수술 중 혈전 및 연축의 치료)

  • Purpose: Microvascular free tissue transfer has become a reliable technique for the reconstruction of complex wounds. Occasionally, unexpected intraoperative thrombosis and/or spasm of recipient artery might be annoying problems even for the technically competent microvascular surgeons. If such problems are not treated properly, they will inevitably cause to flap failure. Methods: From January 2006 to February 2007, soft tissue reconstructions by free tissue transfers were performed on 21 patients having complex defects in the lower extremity. Although segmental revision and various pharmacologic agents were repeatedly applied, arterial occlusions were not managed in 6 cases. For removal of thrombi and release of spasm, Fogarty No. 2 or No. 3 catheters were inserted into the lumen to the proximal recipient artery. Its balloon was then inflated after passing through a resistant area. Next, the catheter was gently withdrawn backward. Results: After the Fogarty catheter was inserted two or three times, the pulsatile arterial flow was restored. When the catheter was inserted into the lumen, a feeling of resistance existed in a 5-10cm more proximal portion that could not be easily accessed from the vascular end. After the reestablishment of blood flow, successful anastomoses were achieved and immediate rethrombosis or spasm did not occur. No long-term sequelae associated with balloon trauma to the arterial wall were observed. Conclusion: The use of the Fogarty catheter can be an effective method in treating pedicle thrombosis and spasm. This is a very simple and rapid technique that offers microvascular surgeons another option to increase the success rate of microvascular anastomosis in free tissue transfers.

Indications, Outcomes, and Complications of Pedicled Propeller Perforator Flaps for Upper Body Defects: A Systematic Review

  • Background The aim of this investigation was to systematically review the current literature to provide the best data for indications, outcomes, survival, and complication rates of pedicled propeller perforator flaps for upper body defects. Methods A comprehensive literature review for articles published from January 1991 to December 2011 was performed using the PubMed, Medline, and Cochrane Databases. Articles without available full-text, single case reports or papers with excessive missing data were excluded. Papers reporting pedicle-perforator (propeller) flaps used for lower extremity reconstruction were excluded from meta-analysis. Results From the initial 1,736 studies our search yielded, 343 studies qualified for the second stage of selection. Of 117 full-text reports screened, 41 studies, met the definitive inclusion and exclusion criteria. Of the selected 41 articles, 26 were case series, original papers or retrospective reviews and were included, whereas 15 were case report papers and therefore were excluded. Two hundred ninety-five propeller flaps were reported to have been used in a total of 283 patients. Indications include repair of trauma-induced injuries, post-trauma revision surgery, cancer resection, chronic infection, pressure sores, and chronic ulcers with a major complication rate (3.3%) comparable to that of free flaps. No specific exclusion criteria for the procedure were presented in the studies reviewed. Conclusions Pedicled propeller flaps are a versatile and safe reconstructive option that are easy and quick to raise and that provide unlimited clinical solutions because of the theoretical possibility of harvesting them based on any perforator chosen among those classified in the body.

Measurement of the CTA and Q-Angle with the Different Position of the Pronation and Supination of the Foot (발의 회내 $\cdot$ 회외 변화에 따른 슬개대퇴골각과 종경골각 측정)

  • An excessive Q-angle has been implicated in the development of knee injuries by altering the lower-extremity locomotion kinematics. The purpose of this study was measured the Q-angle and the CTA when the foot moves pronation and supination of the foot in the standing status. The participants of this examination were 60 adult(30 men and 30 women) who had no orthopaedic and neurological impairment, aged between 20 and 40years. The foot tilt(FT 1)is made of acrylic plate and the slope of the suface is altered as $0^{\circ}$, pronation ($10^{\circ},20^{\circ},30^{\circ}$)and supination($10^{\circ},20^{\circ},30^{\circ}$). The results were as follows : 1. The result about the left/right Q-angle and the left/right CAT There was no statistical significant difference between the left and the right side of the Q-angle with different position of the foot(P > 0.05). While significant difference in the left CTA at the $0^{\circ}$, pronation($10^{\circ},20^{\circ},30^{\circ}$) and supination($10^{\circ},20^{\circ}$) has been observed(P < 0.05). 2. The result about the Q-angle and the CTA between male and female There was significant difference in the Q-angle between male and female with different position of the foot(P < 0.05). while significant different in the right CTA at the $0^{\circ}$ pronation ($20^{\circ}$)(P < 0.05), no significant difference in the left CTA have been observed(P > 0.05). 3. The result about correlation between the left/right Q-angle and the left/right CAT There was statistical significant positive correlation between the left/right Q-angle and the left/right CAT with the different position of the foot(P < 0.01).

Effects of Wearing Spandex Pants on Impact Forces and Muscle Activities during Drop Landing (드롭 착지 동작 시 탄성 섬유 바지 착용이 충격력과 근육 활동에 미치는 영향)

  • The purpose of this study was to determine how spandex pants affect impact force and muscle activities in the lower extremity. Seven pairs of surface electrodes were attached to the right-hand side of the body. Paired t-test was performed to test if significant difference exist between two conditions(p< .05). The average IEMG activities in the TA, BF, and GM during the landing phase reduced significantly with spandex pants compared to regular pants. All peak IEMG activities except the RF in wearing spandex pants group were significantly greater than the corresponding values in wearing regular pants one. The greater muscle activity recorded in wearing spandex pants can be attributed to the greater motor unit recruitment needed to decelerate and stabilized their bodies. Impact force and loading rate were not significantly decreased with wearing spandex pants. Since the spandex pants used in this study were not custom-fit based on girth of each participants thigh and shank, compression and elasticity for the pants could not be optimized.

A Study of Motor Expertise about Kinematic and Kinetic Characteristics of Lower Extremity in the Seokmun Ilwol Martial Art Yin-yang Bo Gait Pattern (석문일월무예 음양보법의 숙련성에 따른 보행 패턴의 하지 운동학 및 운동역학적 특성)

  • The purpose of this study was to quantify kinematic and kinetic characteristics of Yin-yang Bo gait according to their motor expertise, one of the Seokmun Ilwol martial art gait patterns. Yin-yang Bo gait pattern shows initial forefoot contact instead of heel contact, and increased time of stance phase time, internal-external rotation of ankle-knee-hip joints and pelvic. It aims to produce and store the more energy through continuous homeostasis of center of gravity (COG) and performance of stretch-shortening cycle. Some of these characteristics also were similar to the gait modification strategies for reducing knee adduction moment such as toe-out progression, medial thrust, internal rotation of hip joint. To identify the characteristics, four factors of expert Yin-yang Bo gait performance group were compared to that of none expert group; 1) angles of COG displacement and rotation 2) distal joint pre-rotation in internal-external rotation of ankle-knee-hip joints and pelvic, 3) invariability pelvic potential and pelvic segment total energy 4) knee abduction moment. Six healthy(three male) subjects participated in the experiment to perform Yin-yang gait pattern. Three-dimensional and force plate data were collected. Kinematic and kinetic data were compared between two groups using t-tests. Results showed that 1) the peak point of COG internal rotation angle was reduced in expert group, 2) kneeexternal and hip joint -internal and pelvic rotation angle peak frames were more near points in expert group.

The Development of Rhythmic Balance Training Equipment and its Effect on Performance for Elderly

  • Objective: The aims of this study were 1) to develop easy-to-use rhythmic balance training equipment for the elderly and 2) to investigate the effect of training with the equipment on balance and physical function. Method: Twenty-one elderly individuals (age: $75.4{\pm}3.34yrs$, height: $152.07{\pm}4.81cm$, weight: $58.35{\pm}8.34kg$) participated in this study. Each participant underwent balance and physical function testing before and after 12 weeks of training with the equipment. Y-balance (i.e. dynamic balance) and one leg static balance tests were used for balance testing, and timed up- and-down-stairs and five times sit-to-stand tests were used for physical function testing. A paired t test was used to determine whether there was a significant pre- and post-training difference. Results: The rhythmic balance training equipment provided a fun and motivating training program with age-friendly music, dance movements for lower extremity strength training, and touch screen controls with simple features. Post-training left foot dynamic balance was significantly greater (p<.05), and static balance with eyes open was significantly improved (p<.05) compared to pre-training. Completion of the timed up-and-down-stairs and the five times sit-to-stand tests was significantly shorter (p<.05) compared to pre-training. Conclusion: Training using the equipment developed in this study improved balance and physical function in elderly participants.
