The Effect of Isometric Hip Adduction and Abduction on the Muscle Activities of Vastus Medialis Oblique and Vastus Lateralis during Leg Squat Exercises (쪼그려 앉기(Leg Squat) 운동 시 등척성 고관절 내·외전이 내·외측광근의 근 활성도에 미치는 영향)

  • The purpose of this study was to investigate the effect of isometric hip adduction and abduction on the muscle activities of vastus medialis oblique(VMO) and vastus lateralis(VL) during leg squat exercises. This study consisted of 21 healthy subjects who had no medical history of anterior knee pain or lower extremity disorders. The ball and belt were used to isometrically adduct and abduct the hip joint during the leg squat exercise, respectively. The surface electromyograms of VMO and VL were analyzed, and the findings were used to calculate the VMO:VL ratio during 3 different quadriceps-strengthening exercises(leg squat, LS leg squat with isometric hip adduction, LSHD leg squat with isometric hip abduction, LSHB). The muscle activities of VMO and VL and the VMO:VL ratios were compared using the paired t-test with Bonferroni adjustment. The results showed that the muscle activities of VMO and VL during LSHD were greater than those during LSHB. The VMO:VL ratio was the highest during LSHD. This finding suggests that LSHD using a ball is more effective than LS and LSHB in selectively increasing the muscle activities of VMO. Therefore, we suggest that leg squat exercise with isometric hip adduction using a ball would be useful for maintaining correct patella tracking and for selectively strengthening VMO.

The Kinematic Analysis of Gliding Type and Delivery Phase in Each Trails during Shot-Putting - Focusing on Lee, Hyung-Keun, Player in Men's High School Youth Group - (고등부 남자 포환던지기 선수의 시기 별 글라이드 유형과 딜리버리 국면의 운동학적 분석 - 고등부 이형근 선수를 중심으로 -)

  • The purpose of this study was to provide information about kinematic variables of the gliding and delivery motion of Hyung-Keun Lee, a high school shot putter who was ranked 1st at the 2011 National Sports Festivals. Three-Dimensional motion analysis using a system of 4 video cameras at a sampling frequency of 60 Hz was conducted during shot-putting events at the 2011 National Sports Festivals. During the gliding and delivery phase of the player the results showed following characteristics; 1) The gliding technique types of the player appeared to be the short-long technique as the gliding and stance length ratio were $42.3{\pm}3.87$ % and $57.7{\pm}3.87$ %, respectively. In addition, the trajectory of shots during the gliding and delivery phase showed different trajectory patterns with "S-shaped" type of elite players due to the deviation from a central axis of the APSS (athletic-plus shot system). 2) The horizontal velocity of COG made from gliding should maintain the velocity during transition and release phase, but the player showed a small momentum for a gradual decrease of velocity. 3) Therefore, the player requires to adjust an appropriate ratio between gliding and stance length with a strong muscle power at the trunk, throwing arm, and the lower extremity during gliding and delivery phase.

  • A 36-year-old woman with a history of spontaneous abortion and photosensitivity was admitted to our hospital with periumbilical pain and lower extremity edema. The preoperative CT scan revealed massive inferior vena cava thrombus, which extended from the suprarenal portion of the vena cava to the hepatic vein. The laboratory data showed an elevated level of anticardiolipin antibody, which suggested the diagnosis of antiphospholipid syndrome. After medical management, she underwent an operation for removal of an vena cava thrombus by performing cavectomy and thrombectomy. After the operation, she has been taking oral anticoagulation and she is doing quite well at 9 months after her operation.

  • The purpose of this study was to identify biomechanical characteristics of glide and delivery motion of In-Sung Hwang, player who is a member of the national team among the finalists in the men's shot put at the 2010 National Sports Festivals. Three-Dimensional motion analysis using a system of 3 video cameras at a sampling frequency of 60 Hz was performed for this study. During the glide and delivery phase the results showed following characteristics; 1) The glide type was suitable for the short-long technique, but the trajectory of shot at the glide and delivery phase showed a different trajectory pattern with "S-shaped" type of elite players due to many deviating from central axis of the APSS(athletic-plus shot system). 2) Left knee was more flexed during failed trials compared to successful trials but COG was higher. Therefore, the player showed less stability of COG as he may not get enough breaking force at the left foot. 3) Furthermore, it would be required to have strong muscle power at the trunk, throwing arm, and the lower extremity in order to achieve maintain a low projection angle of the release.

  • As the population of elderly people and disabled people are increased, various demands for human welfare using robot system are raised. Especially autonomous rehabilitation system using robot could reduce the human effort while maintaining the its intrinsic efficacy. This study deals with mobile gait rehabilitation system which combined with BWS (Body Weight Support) for training of elderly and handicapped people who suffer the muscle force weakness of lower extremity. BWS which is designed by kinematic analysis of body lifting characteristics and walking guide system are integrated with main control system and wheeled platform. This mobile platform is operated by UCS (User Command System) and autonomous trajectory planning algorithm. Finally, through the EMG (Electromyography) signal measuring and its analysis for subject, performance and feasibility of developed system is verified.

The effect of Functional Performance Following an Ice Immersion to the Ankle, Foot (발목, 발에 냉적용이 기능 수행에 미치는 효과)

  • Purpose : The purpose of this study is to assess the effects of ice immersion to the ankle, foot on vertical jump, isokinetic exercise. Methods : Thirty volunteers from universal students who had not sustained an injury to the lower extremity within the past 6 months were randomly assigned to either an experimental or control group. Subjects in the experimental group performed vertical jump and isokinetic exercise before and after the application of a 15-minute ice immersion($50-55^{\circ}F$) to the ankle and foot. Subjects in the control group performed vertical jump and isokinetic exercise before and after the application of a 15-minute resting. Results : Shuttle run was not significantly decreased after than before the application of a 15-minute ice immersion($50-55^{\circ}F$) to the ankle and foot(p<0.05). Vertical jump was significantly decreased after than before the application of a 15-minute ice immersion($50-55^{\circ}F$) to the ankle and foot(p<0.05). Speed $60^{\circ}$ of peak torque was significantly decreased after than before the application of a 15-minute ice immersion($50-55^{\circ}F$) to the ankle and foot(p<0.05). Speed $120^{\circ}$ of peak torque was significantly decreased after than before the application of a 15-minute ice immersion($50-55^{\circ}F$) to the ankle and foot(p<0.05). Conclusion : we think because the ice immersion decrease active, physical therapist should carefully consider the consequence of cold therapy to increase active.

A Study on the Kinematic Variables in Different Safety Shoes and Applying Insole During Walking (안전화 형태와 인솔착용 유무에 따른 보행동작시 하지부위에 대한 운동학적 부하 분석)

  • This study was performed to compare the kinematics among three different safety shoes(type 1: ergonomically designed and high quality shoes, 2: curved and cushioned safety shoes, and 3: regular safety shoes) and to find the effect of insole during walking. Ten healthy subjects were recruited for this study. The range of motion of knee and ankle joint, angle of rear foot and angle of heel contact were measured using a three dimensional motion analysis system. In the second peak, the angle of heel contact showed statistically significant difference between safety shoes and insole, however, there was no statistical significance among three different safety shoes. The angle of ankle increased significantly at initial contact, first peak, the second peak and the toe off phase compared with type 1 and 2 safety shoes, and the angle of ankle showed statistically significant difference between with and without applying the insole. During the first peak, the second peak and the toe off phase, the angle of knee was statistical significance between safety shoes and insole. In heel contact, the angles of Achilles' tendon showed statistically significant difference between safety shoes and insole. The rear foot angles showed statistically significant difference between safety shoes and insole during heel contact and early heel contact. These results suggest that the type 1 safety shoes were superior to others in the statistics, and applying insole could be a possible method to prevent fatigue of lower extremity and musculoskeletal disorders. Further studies are needed to find the effect of ergonomically designed safety shoes and insole on practical value in prevention of musculoskeletal disorder, fatigue and satisfaction of workers.

  • Sit-to-stand movement is a basic movement in daily activities. On the basis of this movement, the biomechanical functions of a person can be evaluated. The study of the joint kinematics, moment, and muscle coordination is necessary to understand the characteristics of the sit-to-stand movement. We have developed a motion-based program for inverse dynamics analysis and the electromyogram-based program for muscle force prediction. The joint kinematics and the kinetic results estimated on the basis of obtained motion data, ground reaction force, and electromyogram signals were compared with those reported in previous studies, and the muscle forces determined by the two methods were compared with each other. The methods and programs developed in this study can be used to understand biomechanics and muscle coordination involved in basic movements in daily activities.

Study of Clinical Application of Pathology of Blood Stasis, Focused on 33 Prescriptions in 『Yilingaicuo』 (『醫林改錯』 처방의 현대 질병 범위에 관한 연구)

  • This paper researches the features of blood stasis theory of Wangqingren, who wrote 『Yilingaicuo』 that greatly contributed in the development of blood stasis theory at Qing dynasty period. And the disease cause, disease mechanism of blood stasis and scope of modern diseases related with blood stasis are studied by research on clinical papers which used 33 prescriptions in 『Yilingaicuo』 in modern times. Research on the features of blood stasis theory of Wangqingren is proceeded by referring to the annotations of 『Yilingaicuopingyi』 and the papers which related with blood stasis from Korea and China. And clinical papers are searched in China Academic Journals(CAJ) of China National Knowledge Infrastructure(CNKI) to analyse the scope of modern diseases related with blood stasis. The features of blood stasis theory in 『Yilingaicuo』 expanded the range of existing theory. Wangqingren thought that chronic disease, weird disease, the disease of no effect from normal treatments were related with blood stasis. And he attached great importance to qi and blood and thought that the main pathogenesis of blood stasis was qi deficiency. And a lot of Astragalus membranaceus Bunge were combined in many prescriptions to reinforce qi. He also used different herbs according to the location of the disease. Musk and Allium fistulosum were used for the disease located at head or upper part of the patient's trunk. Bupleurum falcatum L., Aurantii Fructus Pericarpium and Platycodon grandiflorum A. De Candolle were used for the disease located at thorax. Cyperus rotundus L., Linderae Radix and Aurantii Fructus Pericarpium were used for the disease located at the stomach or below the costal angle. Foeniculi Fructus and Corydalis remota were used for the disease located at belly or lower part of the patient's trunk. Trogopterorum Faeces, myrrha, Cyperus rotundus L. and Cnidium officinale were used for the disease located at extremity or joint.

Cross-Leg Achilles Tendon Reconstruction Using a Composite Flap of Dorsalis Pedis and Tendon Strips of the Extensor Digitorum Longus in a Vascular Compromised Wound (족지 신건이 포함된 족배부 도서형 교차하지 피판을 이용한 혈행장애 하지부의 일단계 아킬레스건 재건 -증례보고-)

  • Reconstructions of soft tissue defect of the posterior ankle including Achilles the tendon should take into account not only coverage but functional outcome. Various methods of tendon transfer and tendon graft have been reported as a single-stage procedure. With advances and refinements in microsurgical techniques, several free composite flaps including tendon, fascia, or nerve have been used in single-stage reconstructions of large defects in this area minimizing further damage to the traumatized leg. However, when free flap is not feasible for some reasons, this cannot be accomplished successfully. Here we present a patient with Achilles tendon and circumferential large soft tissue defect. Because of circulatory compromise of the lower extremity, free flap reconstruction could not be applied. Instead, cross-leg composite flap of the dorsalis pedis flap including the extensor hallucis brevis musle and tendon, and tendon strips of the Second, third and fourth extensor digitorum logus were employed, Functional reconstruction of the tendon and resurfacing were obtained at the same time. The flap was detached 3 weeks postoperatively, and the transplanted flap has survived without any complications. By 3 months after surgery, full weight bearing, tip-toe standing and even walking without crutch assistance was possible. When functional reconstruction with the free flap is unattainable in the large defect of the posterior ankle including the Achilles tendon, cross-leg composite island flap of dorsalis pedis flap and tendon strips of the extensor digitorum longus tendon is a viable alternative.
