Effect of tDCS and Lumbar Motor Control Exercise on Static Balance and Disability in Chronic Low Back Pain (tDCS와 요추 움직임 조절 운동이 만성요통환자의 정적균형과 요통장애지수에 미치는 영향)

  • Background: The purpose of this study was to investigate the effects on static balance and disability in chronic low back pain with lumbar rotation extension subgroup of trascranial direct current stimulation (tDCS) and lumbar motor control exercise (MCE). Methods: In 40 male low back pain with lumbar rotation extension subgroup subjects were recruited for the study. Subjects were randomly allocated into two groups. Experimental groups received tDCS and MCE, Control groups received sham-tDCS and MEC. Before and after intervention, measured in surface area, whole path length and Roland-Morris low back pain questionnaire (RMQ). Results: Showed a significant static balance and disability from the experimental groups compared to the control group. Showed a no significant RMQ score from experimental groups compared to the control group. Conclusions: tDCS and lumbar MCE showed the increased static balance in chronic low back pain with lumbar rotation extension subgroup.

Effects of the Gluteus Muscle Exercise combined with the Lumbar Stabilization Exercise on Pain, ODI, and Postpartum Depression in Women with Chronic Back Pain After C-section Delivery (허리 안정화 운동과 병행한 엉덩근육 강화 운동이 제왕절개 분만 후 만성허리통증을 가진 여성의 통증, 기능장애지수, 산후우울증에 미치는 영향)

  • Background: This study aimed to investigate the effects of combining the lumbar stabilization exercise with the gluteal muscle strength exercise on pain, the dysfunction index, and postnatal depression in women experiencing chronic low back pain after a cesarean section. Methods: The study used a randomized design with two groups: one group (n=15) performed the lumbar stabilization exercise along with the gluteal muscle strength exercise, while the other group (n=15) only did the lumbar stabilization exercise. The intervention consisted of 30 minute exercise sessions, twice a week, for 6 weeks. The pain was assessed using the visual analog scale (VAS), the dysfunction index with the Oswestry disability index (ODI), and postnatal depression using the edinburgh postnatal depression scale-Korean version (EPDS-K). Results: Both groups showed significant improvements in their VAS, ODI, and EPDS-K scores within their respective groups (p<.05). Additionally, the between-group analysis revealed significant differences in post-test results (p<.05). Conclusion: The combination of the lumbar stabilization exercise and the gluteal muscle strength exercise proved to be more effective in reducing pain, improving the dysfunction index, and alleviating postnatal depression in women with chronic low back pain after a cesarean section. Therefore, incorporating these exercises could be beneficial as an intervention program for women experiencing chronic low back pain after a cesarean section.

The Effect of Home Exercise Program on Pelvic Alignment and Lumbar Scoliosis of Chronic Low Back Pain (가정 운동 프로그램이 만성요통환자의 요추측만과 골반정렬에 미치는 영향)

  • Purpose:To investigate the effect of the home exercise program on pain, scoliosis, pelvic alignment of low back in chronic back pain patients, and suggest optimal method for home exercise program. Methods:I divided into two groups who has chronic back pain; one is control group who was given a treatment at the hospital only and the other is experimental group who did another exercise after treatment at the hospital, and there were 10 people in each group. The manual therapy were given to all the patients in each group after applying a stupe and an electric treatment, but the experimental group conducted another exercise program at homes. All the exercise programs were applied to patients 12 times for 4 weeks totally. Results:SPSS for win version 12 was used for statistic analysis and independent t-test was used to find changes between two groups. VAS scale was used to show changes in pain between each group. The grade of pain was decreased between pre&post test to -5.60 in control group and -4.80 in experimental group but there wasn't significant difference between each group. Cobb's degree was used to compare the changes of scoliosis in lumbar and it was improved in each group but there wasn't significant difference between each group. The measurement of pelvic misalignment was decreased between pre&post test to -1.00 in control group and -2.00 in experimental group but it wasn't enough to show significant difference between each group. Conclusion:As you read the results above, for a chronic low back pain patient, application of the manual therapy showed that it has effect on decrease of low back pain, improvement of scoliosis in lumbar and pelvic misalignment. However, the effect of home exercise treatment was not sure about improvements for chronic low back pain patient. So I think there should need further study about the effect of home exercise treatment except the treatments at hospital and the thorough education for the exercise of lumbar should be done before the study for the accurate experiment.

The effect of stabilization exercise on pain-related disability, sleep disturbance, and psychological status of patients with non-specific chronic low back pain

  • Background: Sleep disturbance and depression are becoming more recognized as important symptoms among individuals with chronic low back pain. This study evaluated the effect of stabilization exercise on pain-related disability, sleep disturbance and the psychological status of patients with non-specific chronic low back pain (NSCLBP). Methods: A total of 26 patients (M/F = 17/9) with NSCLBP with a mean age of $50.0{\pm}15.5$ took part in this study. Participants were recruited from selected hospitals in Lagos state. Ethical approval was sought and obtained from the health research and ethics committee of Lagos University Teaching Hospital Idi-araba, Lagos, Nigeria. Participants performed stabilization exercise for eight weeks consecutively and were assessed for pain-related disability, anxiety, depression, and sleep disturbance using the pain-disability index, hospital anxiety and depression scale, and the insomnia severity index at baseline, the 4th week, and the 8th week, postintervention. Results: The participants studied recorded significant reduction in pain-related disability (P = 0.001). There was also improvement in the sleep quality (P = 0.001), depression level (P = 0.001), and anxiety level (P = 0.001), post intervention. Conclusions: This study revealed that stabilization exercise is very useful in the management of sleep disturbance, pain-related disability, depression, and anxiety in NSCLBP patients.

The Effects of Gluteal Muscle Exercises Combined Lumbar Stabilization on Lumbar Stability in Chronic Low Back Pain Patients with Lumbar Instability (요부안정화운동과 병행한 둔근운동이 요부불안정성을 가진 만성요통환자의 요부안정성에 미치는 영향)

  • PURPOSE: The purpose of this study was to determine if the program focusing on gluteal muscle exercise combined lumbar stabilization had an effects on pain, functional disability and lumbar instability in chronic low back pain patients who had lumbar instability. METHODS: Thirty four patients were recruited in this study. The participants were randomly allocated stabilization exercise group (SEG) (n=17) and gluteal muscle exercise group (GEG) (n=17). The gluteal muscle exercise group conducted gluteal muscle exercises combined lumbar stabilization and stabilization exercise group did only lumbar stabilization exercise for 30 minutes three times a week for six weeks. Pre-exercise assessment tools, visual analogue scale (VAS) and Korean version Oswestry Disability Index level (KODI) were used for pain and functional disability each. Also, these were scored by the tool of lumbar instability. The pain, functional disability levels, and lumbar instability after six weeks with this exercise were re-evaluated. RESULTS: First, SEG showed a significant decrease in the lumbar pain, functional disability levels and lumbar instability. Second, GEG showed a significant decrease in the lumbar pain, functional disability levels and lumbar instability. The third, GEG showed even more significant decrease in the lumbar pain levels, functional disability levels and lumbar instability than SEG. CONCLUSION: Gluteal muscle exercises combined lumbar stabilization are more effective than only lumbar stabilization exercise in the pain levels, functional disability levels and lumbar instability for the chronic low back pain patients with lumbar instability.

Effects of Thoracic and Hip Joint Mobility Exercise and Lumbar Stability Exercise on Pain and Balance in Women with Chronic Back Pain (등뼈와 엉덩관절 가동성운동을 겸한 허리 안정성 운동이 만성 허리통증 여성 환자들의 통증과 균형에 미치는 영향)

  • Background: The purpose of this study was to evaluate the pain, static balance, and dynamic balance abilities of women with chronic low back pain by performing thoracic and hip joint mobility exercises and lumbar stability exercises. Methods: The subjects of this study were 20 adult women with low back pain who lived in C city for more than 12 weeks. The experimental group performed the thoracic and hip mobility exercises with lumbar stability exercises and the control group performed the lumbar stability exercise with general exercise program. Both groups participated in the exercise program three times a week for six weeks, from December 20, 2022 to March 7, 2023. The balance ability were measured using BT4, and pain was measured using visual analog scale (VAS). The collected data were analyzed by independent sample t-test and paired t-test using SPSS version 23.0 program. Results: After 6 weeks of intervention, there was a significant change in VAS between the experimental group and the control group, and there was no difference between the two groups (p>.05). In the case of balance ability, there was a no significant increase in the experimental group (p>.05). Conclusion: Thoracic and hip joint mobility exercises and lumbar stability exercises for adult female patients with chronic low back pain may be partially effective in static balance and dynamic balance.

Effect of lumbar stabilizing exercise using the gym ball on psychosocial status in elderly women with chronic low back pain (짐볼을 이용한 요부안정화 운동이 만성요통 여성노인의 정신사회적 건강수준에 미치는 영향)

  • Objective : The purpose of this study was to analyze the effect on psychosocial status in elderly women with chronic low back pain after practicing lumbar stabilizing exercise using the gym ball, and to developed and activated on exercise program for healthy senile life. Methods : The subjects were 23 elderly women with chronic low back pain. Mean age of subjects was $67{\pm}13$ years. The lumbar stabilizing exercise using the gym ball was performed 3 times a week during the 8 weeks study period. The research tool were ODI(Oswestry Disability Index), VAS(Visual Analogue Scale), SES(Self-esteem Scale), GDSSF-K(Geriatric Depression Scale Short Form Korea Version), PSS (perceived stress scale), Life satisfaction, and MMSE-K. The analysis of data was processed by the statistical program SPSS 12.0 and paired t-test and frequency analysis were used to compare psychosocial status between the pretest and post tests. Result : The lumbar stabilizing exercise using the gym ball increased significantly SES and Life satisfaction of subjects, and ODI, VAS(trunk flexion & trunk extention), GDSSF-K, and PSS were decreased significantly(p<.05). While VAS(trunk rotation) were not significantly. Conclusion : The lumbar stabilizing exercise using the gym ball improved psychosocial status in elderly women with chronic low back pain.

Effects of Multimodal vs. Stabilization Exercises on Pain Intensity, Disability, and Pain-induced Fear in People with Chronic Low Back Pain: A Randomized Controlled Trial (만성요통 대상자의 통증 강도, 장애, 통증 관련 두려움에 대한 복합운동과 안정화운동의 효과 비교: 무작위 대조시험)

  • Purpose: We investigated the effects of multimodal vs. stabilization exercises on chronic low back pain. Methods: Study participants were randomly assigned to a multimodal exercise (n = 20) or a stabilization exercise group (n = 20). Participants in the multimodal exercise group performed stabilization, stretching, and endurance exercises, whereas those in the stabilization exercise group performed only stabilization exercises. Participants in both groups performed the exercises for 1 hour thrice a week for 5 weeks. The following outcomes were evaluated: pain intensity (numeric rating scale), disability (the Oswestry Disability Index [ODI] and the Roland-Morris Disability Questionnaire [RMDQ]), pain-induced fear (the Fear of Daily Activities Questionnaire [FDAQ], the Fear-Avoidance Belief Questionnaire [FABQ], and the Tampa scale for kinesiophobia-11 [TSK-11]). Outcome measures were evaluated at baseline and after intervention. Results: Significant post-intervention improvement was observed in pain intensity and the RMDQ and FDAQ scores in both groups (p < 0.01). The post-intervention ODI, FABQ, and TSK-11 scores were improved in the multimodal exercise group (p < 0.01). Additionally, significant differences were observed in pain intensity, as well as in the ODI, FDAQ, and FABQ scores in the multimodal exercise group compared with these findings in the stabilization exercise group (p < 0.01). Conclusion: The multimodal and stabilization exercise programs reduced pain intensity, disability, and pain-induced fear. Compared with stabilization exercises, multimodal exercises more effectively reduced pain intensity, disability, and pain-induced fear. This study highlights that musculoskeletal rehabilitation for people with chronic low back pain should include a multimodal exercise program.

Effects of active stretching exercise of hip joint and lumbar stabilization exercise on the muscle thickness of piriformis and low back pain (엉덩관절 능동적 신장 운동과 허리 안정화 운동이 궁둥구멍근의 두께와 요통에 미치는 영향)

  • Background: This study was to investigate the effects of active stretching and stability exercise on piriformis muscle thickness and low back pain of male patients with chronic low back pain. Design: Randomized Controlled Trial. Methods: 45 male patients participated in this study. All subjects were randomly assigned. Subjects divided into 15 who underwent contract relaxation (CR) of the proprioceptive neuromuscular facilitation (PNF) to their hip joints, 15 who underwent a combination of isotonic (CI) of the PNF, and the rest 15 who underwent the two techniques (CR+CI). Real-time diagnostic ultrasound was used to measure thickness of the piriformis muscles. The visual analogue scale (VAS) was used to measure degrees of low back pain. A two-way repeated measures ANOVA was used to compare the average values. The SPSS 24.0 was used as the statistical program, and the significance level was set at .05. Results: The CR and the CR+CI groups had more decreased thickness of piriformis muscle when compared to the CI group. The CR+CI group had more decreased pain when compared to the CR and the CI groups. Conclusion: These results suggest that combination of stretching and stability exercise was effective on male patients with chronic low back pain showing limited hip medial rotation.

Effects of the Back Pain Management Program on Patients with Chronic Low Back Pain (만성요통 환자에 대한 요통관리프로그램의 효과)

  • Purpose: The purpose of this study was to explore the effects of the 8-weeks back pain management program (consisted of exercise and education for back pain) in patients with chronic low back pain. Methods: A total of 20 subjects who visited the pain clinic in a university hospital were randomly assigned either to a experimental or a control group. Ten subjects in the experimental group carried out the back pain management program for 8 weeks, and 10 in the control group did not. Results: After the back pain management program, back pain and disability were significantly reduced, and back pain management behavior and back muscle strength were significantly increased in the experimental group. However, radiating pain and flexibility were not significantly changed after the back pain management program. Conclusion: Findings of this study indicated that the 8-weeks back pain management program had favorable effects on back pain, degree of disability, back pain management behavior, and back muscle strength in patients with chronic low back pain. Future research needs to examine the optimal intensity and frequency of back muscle strengthen exercise, and the lasting effects of education for back pain.
