  • Introduction of new longterm care policy for elderly in Korea would change many aspects of elderly care service facilities. Especially elderly home care services like adult daycare centers will expand drastically after beginning of longterm care insurance. The purpose of this study is to estimate demand of adult daycare centers by comparing with the U.S and Japanese cases. Korean government is expecting that adult daycare centers will expand ten times within 4 years. This estimate is exceeding the facility demand estimate of the U.S. and Japan. The results of population study and expecting growth rate of adult daycare centers in Seoul indicate that more than 300 centers, 4 times of the number of existing centers, are in need based on Seoul elderly population in 2004. To supply these numbers of facilities in short period, more in depth study should be followed. Existing adult daycare facilities' in Seoul were analyzed by their building and management types. Interior spaces of adult daycare centers in Korea are similar to the U.S. and Japan in space arrangements but much smaller in size. In depth study of space programming as well as overall demand survey of adult daycare centers is urgently in need for more realistic expansion of adult daycare centers.

  • The coverage of the National Health Insurance for the elderly is expanding to denture and implants. Although the National Long-Term Care Insurance was just being settled, Oral health service was not provided to the Elderly in Long-Term Care Facilities. The long-term care facilities had part-time facility doctors. However, there is no dentist in the long-term care facility because of lack of long-term care insurance-related legislations. The amendments of long-term care insurance-related legislations for the introduction of part-time facility dentists are needed because the elderly in long-term care facility are vulnerable to oral health. For the substantial management of the National Long-Term Care Insurance, the development of oral health service model for the elderly and education materials for the dental team will be needed. Also, adequate dental service fee of the National longterm care insurance will be needed.

일본 고령자복합시설과 데이서비스센터의 복합유형에 관한 연구 (A Study on Types of Multi-Functional Welfare Facilities and Day Service Centers for the Elderly in Japan)

  • Due to the shortage of elderly care services in urban areas, multi-functional welfare facilities are proven to be very effective for delivering various service needs of elderly in Japan. Introduction of new longterm care policy for elderly in Korea would change many aspects of elderly care service facilities. Especially elderly home care services like adult daycare centers will expand drastically after beginning of elderly longterm care insurance. The purpose of this research is to study and analyze multi-functional welfare facilities in Japan focusing on the types of day service in those facilities. Planning of daycare centers in multi-functional welfare facilities for the elderly can be classified to 8 types. Those types are daycare centers with senior housing, longterm care insurance facilities, senior centers, small multi-function facility, medical facilities, educational facilities, community facilities and general housing projects. Each type has different benefits for the networking of services for the elderly. Design of daycare centers in multi-functional welfare facilities have distinctive features in entrance and user approach, space allocation and circulation planning. The study of daycare centers and multi-function planning should be followed to make better home care environments for the elderly in Korea.

  • The purpose of this study is to suggest optimum size of residents' bedrooms in nursing homes. Bedroom is an essential part of nursing homes because nursing home residents spend most of their times in bedrooms and occupy the largest part of facility area. Therefore it is necessary to set an adequate standard related to resident room area in nursing facilities, which have a great effect on planning longterm care facilities for the elderly. The body dimensions of elderly people and care givers have been collected from the previous study. Based on the body dimensions data, the behaviors of elderly as well as care givers have been analysed. The minimum areas-exclusive of toilets, closets, vestibules and so on - have been proposed from the results of the behavior analysis.

노인복지시설 수용자 특성별 장기 요양서비스 유형설정에 관한 연구 (A Study on the Classification of Institutional Long-term Care Based Upon Characteristics of Institutionalized Elderlies)

  • The objective of running a long-term care institution is to provide services helpful for maintaining, supporting, and improving elderlies' optimum level of physical, mental, and psychosocial functioning. For the purpose of analyzing the current situations of institutional long term care facilities in Korea, 27 facilities were selected proportionately from each of the cities and provinces, out of the total 152 facilities. About 20% of those who were institutionalized during 25 August through 2 Qctober 1993, the 391 elderlies were chosen on a systematic random basis. The instrument of this study was developed by modifying the tools of CARE, MAI and PCTC. A multivariate approach of discriminant analysis and clustering technique were employed for this study. The Stiudy reveals that there is no clear differentiation of goals and functions among the longterm care institutions in Korea. Staffing patte군 of long-term care facilities shows a shortage of nurses, physical therapists, and dieticians. The linkage between acute care facilities and long-term care is weak, and administration of long-term care faciltiy is carried out by non-professionals. They are responsible for assessing health status before entering the facility, and evaluating elderlies' care. Therefore, it is not surprising to find that most of the facilities have accommodated agede regardless of their real needs and health status. Based upon findings of the analysis, this study has classified long-term care facilities into four types : Type I is to help elderlies maintain independence in daily living activities. Type II facilities have the objective of maintaining and improving the current level of elderlies' function. Type III is to maintain maximum independence of elderlies in activities of daily living. And Type IV is identified for the group of facilities designed to restore or improve functional abilities of elderlies. In conclusion, the following suggestions are made : the need for long-term care should be assessed by multidimensional measurement. Institutional long-term care facilities should be classified and developed in response to type of type of care and service need. Both acute and long-term care facilities should be linked together in order to support the evaluation of service operation and program development.

노인장기요양시설 입소노인의 구강건강실태와 시사점 (Oral health status of institutionalized elderly in Korea)

  • Purpose: The institutionalized elderly have placed the most vulnerable state, and the report about the oral health status of the institutionalized elderly was scarce. The aims of the current study were to investigate the oral health status of institutionalized elderly. Materials and methods: The survey of the oral health status was carried out on 487 institutionalized elderly from 4 longterm care facilities of Seoul metropolitan city, Gyunggi province, and Gangwon province in Korea. Results: The prevalence of dental caries, root caries, and dental calculus of the institutionalized elderly was 19.2%, 15.3%, and 23.7%, respectively. The percentage of edentulism among the institutionalized elderly was 26.2%. Those who had poor oral hygiene and tongue coating were 43.9% and 90.5%, respectively. The percentages wearing complete and removable partial denture on maxilla were 12.8% and 3.3%, respectively. The percentages wearing complete and removable partial denture on mandible were 7.8% and 5.6%, respectively. The percentages of institutionalized elderly needing complete and removable partial denture on maxilla were 29.7% and 27.2%, respectively. The percentages of institutionalized elderly needing complete and removable partial denture on mandible were 24.5% and 30.9%, respectively. Conclusion: The oral health stuatus of institutionalized elderly was poor. For the oral health promotion of elderly in longterm care facilities, it is essential to educate nurses and caregivers about the importance of the oral health and oral hygiene method.
