Preferences of Foodservice Types for the Elderly Patients at the Long-term Care Facilities through Conjoint Analysis (컨조인트 분석에 의한 노인의료전문 병원의 급식서비스 선호도 연구)

  • The elderly population in Korea is growing rapidly and their needs for long-term care has also increased. By the year 2018, our society will be approaching aged society and by 2026 it will be a super-aged society. The purpose of this study was to employ conjoint analysis to establish the relative importance of foodservice encounters in terms of determining the utility values of hospital foodservice for elderly patients. According to the results pearson's R(0.420) and Kendall's tau(0.402) statistics showed that the model fits the data well(p<0.05). The relative importance scores of hospital foodservice encounters were as follows: dietary counseling with dietetics(51.2%), foodservice personnel(48.7%), and food(0.1%). A soft cooking method(0.001) was preferred to a general cooking method(0.001), and kind foodservice personnel(0.086) were preferred to quick service(-0.086). Finally, counseling with a dietitian once a week(-0.138) was preferred to counseling twice a week (-0.276). Based on this conjoint analysis, the most preferable model for foodservice at a long-term care facility would be; soft cooking methods, kind service by foodservice personnel, and dietetic counseling once a week. Overall, a better understanding of the specific needs of our institutionalized elderly is one of the key elements that can help our long-term care system develop improved foodservice programs.

A Study of the Spatial Composition and Departmental Area Distribution in Geriatric Hospital (노인전문병원의 공간구성과 부문별 면적배분에 관한 연구)

  • As the population of the elderly increases drastically year by year in Korea, the more welfare centers for them are greatly needed. There are, however, a small number of public geriatric hospitals currently available. And a limited number of private-founded geriatric centers are under construction throughout the country. And the systematic standards for the spacial composition and departmental area distribution for them are not satisfactorily set up yet. The analytical study of the space composition and area distribution of the hospitals in question shows that publicly-shared spaces, such as day-room and lobby, are much more useful for their communications than private spaces, such as shut-in living rooms. It also shows that in-yards or in-gardens which are located in the center of the wards are very helpful for their comfortable long-term hospitalization. The shared spaces are recommended to be in sight of and within earshot of their caretakers. In relation with out-patient departments, the programs for the local elderly residents, such as the day-care center and recreation facilities are recommended to be prepared for their physical care and emotional treatment at a time by municipal welfare centers rather than by private institutions. The analytic study carried out in terms of departmental area distributions reveals that the areas for the wards are generally wider and areas for out-patient/or diagnosis departments are relatively narrower than those in the general hospitals. These area distributions seem to have come from the considerations of their relatively long-term staying in the centers than general hospitals and their mental stability as well.

A Study on ASET Elongation & Notification Time to Fire Stations for the Escape Safety of Aged Bedridden Patients in Elderly Long-term Medical Care (노인의료복지시설 화재 시 와상노인의 피난안전성 제고를 위한 피난허용시간 연장과 소방기관으로의 통보시간 연구)

  • Recently, huge life losses occurred in the elderly long-term medical care fires due to lack of escape safety. As part of the measures to enhance the effectiveness of fire escape safety, while they prolong the available safe egress time (ASET) of non fire compartments, a measure to shorten fire-fighter's arrival time by fire alarm notifying device should be implemented in these facilities. The four categories from the aspects of fire prevention/protection engineering were provided with the necessary component technologies for carrying out these helper-guided evacuations. Fire prevention engineered technology was presented by two provisions; one for ensuring small compartment sections by installing the fire rated wall between bed rooms and another for ensuring the fire retardant or/and non-flammable performance of finishing materials. Also fire protection engineered technology was presented by two items; one for imposing cooling effects by sprinklers and another for providing automatic fire alarm notifying functions to fire stations. In order to improve the escape safety of these facilities in Korea, alternative revisions may presented by considering insufficient provisions in the architectural/fire law provisions by analyzing the provisions of Japanese and domestic laws in detail.

Expected Family Involvement of Family of Elderly Residents in Nursing Homes (요양시설 입소 노인 가족의 가족참여 요구도에 관한 연구)

  • Purpose: The purpose of this study was to investigate the expected family involvement of family members of elderly residents in nursing homes. Methods: This study utilized a descriptive design with a survey research method. Data were collected using the Expect Family Involvement Questionnaire Scale, which was completed by 112 family caregivers of older adults in nursing homes in Seoul and Gyeong-gi Province. The collected data were analyzed using descriptive statistics. Results: Responses to the questionnaire indicated the highest value was recorded for the item, 'use of a room for family privacy' with $2.41{\pm}0.43$ points, followed by the item, 'staff calling family for advice with resident problems' with $2.38{\pm}0.38$ points and 'education about nursing home programs' with $2.32{\pm}0.42$ points. Conclusion: In order to promote family involvement in long-term care facilities, various approaches, interventions, and government supports with the respective of the family are required. The nursing home placement process should include understanding discuss on the importance of the role of family, and should help promote positive relationships between the elderly residents and their families. Staffs and administrators of nursing home are to consider the family as a partner rather than visitor.

The Effect of the Long-Term Care Worker's Empowerment on Job Satisfaction, Organizational Commitment and Turnover Intention (요양보호사의 임파워먼트가 직무만족과 조직몰입 및 이직의도에 미치는 영향)

  • This study was designed to inquire into the effect of care worker's empowerment on job satisfaction, organizational commitment, and turnover intention. For this, a survey was carried out on 200 care workers in elderly care facilities located in Chungcheong area from May 16 to May 30, 2011. Analysis showed that their empowerment has a significant impact on the improvement of job satisfaction and organizational commitment, and the reduction of turnover intention. This means that their high empowerment has a high inclination for job satisfaction and organizational commitment, and a low inclination for turnover intention according to the positive job satisfaction and organizational commitment. Consequently, considering their low empowerment, the job productivity and specialty may be eventually raised by improving their empowerment that brings out their own special abilities and merits with individual personality and aptitude, and showing more concerns and consideration for the job.

A Study on Caregiver's Perception of Restraints (요양보호사의 억제대 사용에 대한 인식)

  • This is a descriptive study on visiting caregivers' perception of restraint use. Data were collected from 113 caregivers working at long-term facilities using a structured questionnaire from Feb to March 2016. The caregivers' perception of restraint use was the medium score. The items they considered the most important reasons for the restraints was "Protecting an older person from falling out of bed", which was primarily intended to prevent accidents of the elderly. The score of the knowledge of restraint use was 11.23 points out of 18 and those affected were more than 50 years of age. The attitude score to the use restraints was 9.19, and caregivers thought it should be possible to refrain from using restraints. Work experience of 3 years or less were higher. The perception of using restraints is affected by the knowledge and attitudes of restraint use. This is very important for the elderly in long-term care facilities and is related to health and human rights. Therefore, it is important to provide continuous education related to the use of restraints and regulations on their management.

  • Korea faces new social needs of community-based service for senior citizen as aging is getting fasten. Targeting elderly people who are beneficiaries of long-term care insurance and caring service, this paper aims to explore decent and proper services for senior citizens in terms of rehabilitation, health improvement and quality of life. Especially this research approaches from the perspective of service providers, including service facilities and care-service givers. Using FGI methods with care-providers and in-depth interviews with community social workers as well as nurses, convergence approaches are explored. As a result, welfare field and nursing practices are clearly divided in the community, so that the convergence education and team work tool would be developed in the near future.

Fuzzy Analysis of Consciousness Structure of Administrator for Determinative of Care Service Quality (요양서비스 질 결정요인에 대한 관리자의 의식구조 퍼지분석)

  • The aim of this study is to structuralize a model of the factors determining the quality of nursing care perceived by the director or manager of a long-term care facilities (hospitalization of patients) using FSM(Fuzzy Structural Modeling), employed in structuralizing social systems. The results were as follows: first, quality in the top tier was shown to be connected with job commitment, commitment to the organization, work experience, care skills, knowledge about the elderly, training and education, which are factors in the middle tier; and second, the structure of the middle tier (job commitment, commitment to the organization, work experience, care skills, knowledge about the elderly, training and education) either showed a connection with the lower tier, which includes employment type, job satisfaction, leadership, relationship with users and workplace relationships, or showed a connection among the factors within. These results confirmed the following: first, care skills and knowledge about the elderly, which demonstrate the job expertise of caregivers, showed a connection with service quality based on work experience; second, job commitment in the middle tier was observed to affect various factors in the same tier such as care skills, knowledge about the elderly, training and education amongst others, and it was determined that it is an important determining factor in service quality. Lastly, a meaningful result was shown in relation to leadership. The leadership skills of the director of the facilities had a connection with the care caregivers' commitment to the organization, which had a connection with service quality. This structure showed the kind of role the director must play in order to improve service quality.

Status of oral health care provided by care workers in accordance with their oral health awareness and knowledge (요양보호사의 구강보건인식 및 지식에 따른 구강보건제공현황)

  • By studying the status of oral health provided by caregivers working in care facilities in accordance with their oral health awareness and knowledge, this thesis sought to find a plan of development to enhance the oral health for the elderly in care facilities. A survey was carried out from the 16th of August to the 2nd of September 2016, with 200 care givers as the subjects, working in 14 long-term care facilities for elderly, located in the city of D. Frequency analysis and independent sample t-test were conducted of the collected data, using SPSS 18.0. The results of the study were as follows; for gender, female was 91.5% which was higher than that of male, and married for marital status, 50 for age, highschool for education, christian for religion were shown to be the highest. The highest number of people for daily care was 6~7. In terms of status for providing oral health and oral health awareness of caregivers, the existence of oral hygiene guideline was statistically meaningful, and in terms of the status of oral health provided and oral health knowledge, there were notable differences in storage of dentures, and usage of mouthwash(p<.05).

Oral health knowledge level on oral care and elder oral hygiene care of the nursing home staff in long-term care facilities (노인요양시설 요양보호사의 구강관리 및 노인구강위생관리에 따른 구강보건지식 수준)

  • Objectives : This research was conducted to investigate geriatric care workers' oral care, and the oral care conditions of residents of elder care facilities in relation to workers' knowledge of oral care. Methods : A survey was conducted from November 2011 to February 2012 with geriatric-care workers in elder care facilities located in Busan and other areas of Gyeongsangnam-do. Results : With regard to their oral self-care practices, 52.5% of the geriatric workers "brushed their teeth from top to bottom" and 58.1% brushed their teeth 3 times a day. Further, 52.0% did not use any oral care products, while 36.5% used Mouthrinese. The workers who brushed their teeth in their own ways or from top to bottom had significantly higher oral care knowledge scores than those who brushed from left to right (p<.05). Concerning oral hygiene care for residents, 72.6% of the geriatric workers recommended that elders visit dentists for oral care, 96.6% helped elders oral wash their teeth every day, and 54.7% used oral care products for the elders. The workers who did not recommend visits to dental clinics scored slightly higher than those who did (p<.05). Additionally, those who did not help elders oral wash their teeth scored significantly higher than those who did (p<.05). Except for oral wash to recuperated old man and recommendation of visiting the dentist, relation between oral care and knowledge about oral health and oral hygiene care is plus mutual relation. Conclusions : To manage the accuracy and quality of oral care work performed by geriatric care workers in elder care facilities, enhancing standard training and education on oral care is necessary. Providing systematic, regular education on such knowledge and practices would also be essential.
