An Aanalysis of Predictive Factors of Medical Service Overuse for Inpatients Applied Out-of-Pocket Maximum in Long-Term Care Hospitals in South Korea (본인부담상한제 적용 요양병원 환자의 의료이용가수요 예측요인 분석)

  • Background: The out-of-pocket maximum is one of the distinctive healthcare systems which sets a ceiling on co-payment in order to reduce the burden of households from the unpredictable medical expenditure. However, this leads to an increase in the demand for healthcare services especially in long-term care hospitals (LTCHs) in Korea. Methods: This study analyzed the influence factor of medical service overuse of 165,592 inpatients in LTCHs which out-of-pocket maximum is applied, by utilizing data from the National Health Insurance Service (2016). Based on Anderson Model, the medical service overuse, as a dependent variable, was defined as long-stay admission more than 180 days at the LTCHs. Independent variable was comprised of predisposing factors (gender, age), enabling factors (income level, types of out-of-pocket maximum) and need factors (illness level, patient use of tertiary hospital). Results: The most powerful factor of medical service overuse in LTCHs was availability of pre-payment for the out-of-maximum (odds ratio [OR], 191.66; p<0.001). This tendency was found in high income level status (p<0.001). Furthermore, mild inpatients (OR, 1.50; p<0.001) which had no experience with the tertiary hospitals (OR, 2.06; p<0.001) were more relevant to the medical service overuse in LTCHs, compared to the severe inpatients. Conclusion: It is suggested that a separate standard of out-of pocket maximum with regards to LTCHs is required to secure the beneficial functions of long-term hospitals and prevent unnecessary financial leakage to achieve sustainable and financially sound National Health Insurance.

Impact of Nurse, Nurses' Aid Staffing and Turnover Rate on Inpatient Health Outcomes in Long Term Care Hospitals (요양병원의 간호사와 간호조무사 확보수준과 이직률이 입원환자의 건강결과에 미치는 영향)

  • Purpose: This study was conducted to explore the impact of registered nurse/nurses' aid (RN/NA) staffing and turnover rate on inpatient health outcomes in long term care hospitals. Methods: A secondary analysis was done of national data from the Health Insurance Review and Assessment Services including evaluation of long term care hospitals in October-December 2010 and hospital general characteristics in July-September 2010. Final analysis of data from 610 hospitals included RN/NA staffing, turnover rate of nursing staff and 5 patient health outcome indicators. Results: Finding showed that, when variables of organization and community level were controlled, patients per RN was a significant indicator of decline in ADL for patients with dementia, and new pressure ulcer development in the high risk group and worsening of pressure ulcers. Patients per NA was a significant indicator for new pressure ulcer development in the low risk group. Turnover rate was not significant for any variable. Conclusion: To maintain and improve patient health outcomes of ADL and pressure ulcers, policies should be developed to increase the staffing level of RN. Studies are also needed to examine causal relation of NA staffing level, RN staffing level and patient health outcomes with consideration of the details of nursing practice.

Design of Pressure Injury Management Mobile Application Structure and User Interface (욕창관리 모바일 어플리케이션 구조 설계 및 사용자 인터페이스 구현)

  • Purpose: This study aimed to design user interfaces of a mobile application for managing pressure injury patients in a long-term care hospital based on the user's needs. Methods: To reflect users' needs in the mobile application, the user interfaces in this study were designed in five steps: brainstorming and mind mapping, persona and scenario, needs list and priority, a draft version of flow chart and user interfaces and expert review. These steps were conducted with a step nurse at a long-term care hospital, a professor who majored in nursing informatics, a professor who had lots of research experiences about pressure injury and a wound ostomy continence nurse. Results: Two personas, scenarios and needs' lists were derived. Listed Needs included the followings; Accurate staging of pressure injury; Appropriate management by staging; Acquisition of professional knowledge about pressure injury; Acquisition of easy pressure injury information through text, picture and video; and Sharing pressure injury information in unit. The structure, menus and features of the pressure injury mobile application were visualized with user flow based on two personas' scenarios and needs' lists. Conclusion: Our study suggests and visualizes the key features of the 'Pressure Injury Guide', a pressure injury management mobile application for nurses in a long-term care hospital, which can be utilized by nurses, application developers, and related researchers.

Factors Related to Quality of Life in the Rural Elderly People Affiliated with Long-term Care Insurance Services (일부 농촌지역 장기요양급여노인들의 삶의 질과 관련 요인)

  • This study was performed to determine the levels of quality of life(QOL) according to the grade of long-term care service in the elderly people who affiliated from long-term care insurance, and to reveal its association with socio-demographic characteristics, health status and health related behaviors. The interviews were performed, during the period from March 1st, to May 31th, 2009, to 410 elderlies in rural areas. As a results, the levels of QOL were lower in the group of higher grade of long-term care insurance. In hierarchial multiple regression analysis, the factors influencing on the level QOL were age, Living status, Subjective health status, Bed ridden status, Disability of body, Urinary incontinence, Amnesia and Frequency of going out. Especially, the health status variables were higher related with QOL than other variables.

A Study on the Effects of Psychological Adaptation of Social Service Users on Family Relations

  • Social services are divided into care services and other support services for the elderly, children and the disabled. These social services are the subject of great policy interest in that they can create two effects at the same time: increase in labor demand for service providers and increase the possibility of long-term accumulation of human capital for service recipients. Therefore, this study aims to confirm the hypothesis according to whether the use of social services affects family relationships and the effects of social service users' psychological adaptation on marital and parent-child relationships even when other related variables are controlled. The final result of this thesis is a regression analysis to find out the effect of psychological adaptation on the family relationship, spouse relationship, and relationship with children of subjects who have experience using social services. -.661, p=0.001), the higher the psychological adaptation, the higher the spouse satisfaction (β=.465, p=0.001) and relationship satisfaction with children (β=.360, p=0.001). In other words, it was found that the more depressed the psychologically, the more negative the relationship with spouse and children.

  • Japanese state has continuously tried to adapt itself to the social demands coming from rapidly greying population. Japanese government introduced the new social insurance system of elderly care, i.e. Long term Care Insurance with epoch-making changes in the Japanese welfare system. The most important aspects of the new system can be summarised as follows: 1) social insurance system of obligatory entry with paying premiums and co-payment 2) emphasis on the customer choice, competition, flexibility, free-market, relaxation of the regulation. This characteristics brought unexpected results of the emergence of ethnicity-centred contents of welfare services. As a selling point in freemarket and as a countermeasures against expected ethnic disadvantages, the Korean ethnic organisations brought the ethnic elements resulting in the diversified Japanese welfare services.

  • With the prolonged spread of the new coronavirus infection worldwide and the entry of the super-aged society, smart health care, which combines IT technology for senior health care and the health care industry, is emerging as a solution to the aging problem. The development of non-face-to-face care services using Ai is on a global trend, not in some countries, and the form of care services for the elderly using AI artificial intelligence technology is changing rapidly. The convenience of AI-based care services for the elderly is expected to be highlighted, and the technology and market are expected to develop significantly. As the number of single-person households is increasing, the shortage of welfare workers for the elderly is emerging as a social issue. It is presented as a vision to solve long-term social problems such as the labor shortage of elderly care workers as well as the advantages of convenient care services using IT technology. Therefore, we would like to propose the development direction of care services for the elderly as a case study of care services for the elderly and a countermeasure against the super-aging age.

The Necessary Conditions and Deterring Factors of Good Care (좋은 돌봄의 필요조건과 저해요인에 관한 연구 - 노인돌봄을 중심으로 -)

  • Social care service has grown much with the introduction of Long-Term Care Insurance, but it remains difficult to answer in the affirmative to the question: are we providing sufficiently good care? This study has its purpose in figuring out what conditions are necessary to realize good care in our society. The study has used focus group interview (FGI) as a way to acquire realistic knowledge on the conditions that create good care and its deterring factors. The focus group interview, which targeted long-term care workers and facility heads, was conducted three times from January through March 2014, with each session taking about 3 hours. The analysis showed that the components of good care were mutual understanding and recognition through active interaction, making a good relationship based on mutual trust and respect, professionalism of care worker with flexible judgment that provides customized services, professionalism of center manager with appropriate intermediation, and the tripartite partnership among family, elderly, and care worker. Meanwhile, the deterring factors of good care were identified as devaluation of care labor, ambiguous job description and abuse, unprofessionalism of care worker and manager, inappropriate interference and indifference of family, and the structural constraints on long-term care environment.

The Survey of Awareness and Necessity on Introduce Home Physical Therapy in the Long-term Care Insurance (노인장기요양보험제도 내 가정방문물리치료 도입에 관한 인식도 및 필요성 조사)

  • This study aims to investigate the awareness and needs of patients as consumers and therapists as providers on the introduction of home physical therapy in the long-term care insurance. The survey participants were 96 patients and 132 physical therapists to a questionnaires, we analyzed for frequency of each questionnaire about awareness and necessity. As a result of the test, they considered that physical activity support services provided by nurse and nurses aid ware not specialized enough(pt 60,4%, PT 75,4%) and quality therapy should be available through a home physical therapy(pt 47.9%, PT 59.2%). Also, patients responded that the priorities to improve long-term care insurance were given to lowering expenses(35,4%) and expanding coverage of subjects(32.3%) while the physical therapists responded that professionals should be expanded(73.8%) and diverse rehabilitation services(20.2%) and a rehabilitation team should be introduced. They responded that the reasons of home physical therapy were needed that it was hard for patients to visit medical centers(PT 30.0%) themselves and it would be improvable for them to get physical function in elderly(pt 47.1%) through the service. In conclusion, as most patients as well as physical therapists responded that home visit therapy services should be introduced and their awareness of it was very high, it is suggested that the service should be quickly introduced as soon as possible.

An Exploratory Study on the Experience of the female Elderly using a Long-Term Care: Centering on Users of Home-Visit Bath (장기요양보호를 이용하는 여성노인의 경험에 관한 탐색적 연구: 방문목욕 이용자를 중심으로)

  • This research, with the participants of the female elderly using a home-visit bath among long-term care services, made an in-depth analysis of what they experience while getting a home-visit bath. We conducted in-depth interviews with 8 elderly people. According to the result, the female elderly experienced the absence of a caregiver, difficulty in carrying out daily life due to physical diseases, getting what they need by themselves, getting comfortable in body and mind, accepting their given situation though feeling shame at getting a bath, and expressing their desires. In addition, they had a close relationship with a care helper. On the basis of the results, a systematic training system which could intensify the professionalism of care helpers was suggested. For the enhancement of the elderly's emotional stability in a long-term care, an integrated case management system was also suggested, which supports the family by organizing an integrated network by region between a long-term care service, home-visit care service, welfare center, and the National Health Insurance Corporation.
