Objective: We performed small-scale mutation and large genomic rearrangement (LGR) analysis of BRCA1/2 in ovarian cancer patients to determine the prevalence and the characteristics of the mutations. Methods: All ovarian cancer patients who visited a single institution between September 2015 and April 2017 were included. Sanger sequencing, multiplex ligation-dependent probe amplification (MLPA), and long-range polymerase chain reaction (PCR) were performed to comprehensively study BRCA1/2. The genetic risk models BRCAPRO, Myriad, and BOADICEA were used to evaluate the mutation analysis. Results: In total, 131 patients were enrolled. Of the 131 patients, Sanger sequencing identified 16 different BRCA1/2 small-scale mutations in 20 patients (15.3%). Two novel nonsense mutations were detected in 2 patients with a serous borderline tumor and a large-cell neuroendocrine carcinoma. MLPA analysis of BRCA1/2 in Sanger-negative patients revealed 2 LGRs. The LGRs accounted for 14.3% of all identified BRCA1 mutations, and the prevalence of LGRs identified in this study was 1.8% in 111 Sanger-negative patients. The genetic risk models showed statistically significant differences between mutation carriers and non-carriers. The 2 patients with LGRs had at least one blood relative with breast or ovarian cancer. Conclusion: Twenty-two (16.8%) of the unselected ovarian cancer patients had BRCA1/2 mutations that were detected through comprehensive BRCA1/2 genetic testing. Ovarian cancer patients with Sanger-negative results should be considered for LGR detection if they have one blood relative with breast or ovarian cancer. The detection of more BRCA1/2 mutations in patients is important for efforts to provide targeted therapy to ovarian cancer patients.
In this study, we have cloned a novel cDNA encoding for a papain-family cysteine protease from the Uni-ZAP XR cDNA library of the polychaete, Periserrula leucophryna. This gene was expressed in Escherichia coli using the T7 promoter system, and the protease was characterized after partial purification. First, the partial DNA fragment (498 bp) was amplified from the total RNA via RT-PCR using degenerated primers derived from the conserved region of cysteine protease. The full-length cDNA of cysteine protease (PLCP) was prepared via the screening of the Uni-ZAP XR cDNA library using the $^{32}P-labeled$ partial DNA fragment. As a result, the PLCP gene was determined to consist of a 2591 bp nucleotide sequence (CDS: 173-1024 bp) which encodes for a 283-amino acid polypeptide, which is itself composed of an 59-residue signal sequence, a 6-residue propeptide, a 218-residue mature protein, and a long 3'-noncoding region encompassing 1564 bp. The predicted molecular weights of the preproprotein and the mature protein were calculated as 31.8 kDa and 25 kDa, respectively. The results of sequence analysis and alignment revealed a significant degree of sequence similarity with other eukaryotic cysteine proteases, including the conserved catalytic triad of the $Cys^{90},\;His^{226},\;and\;Asn^{250}$ residues which characterize the C1 family of papain-like cysteine protease. The nucleotide and amino acid sequences of the novel gene were deposited into the GenBank database under the accession numbers, AY390282 and AAR27011, respectively. The results of Northern blot analysis revealed the 2.5 kb size of the transcript and ubiquitous expression throughout the entirety of the body, head, gut, and skin, which suggested that the PLCP may be grouped within the cathepsin F-like proteases. The region encoding for the mature form of the protease was then subcloned into the pT7-7 expression vector following PCR amplification using the designed primers, including the initiation and termination codons. The recombinant cysteine proteases were generated in a range of 6.3 % to 12.5 % of the total cell proteins in the E. coli BL21(DE3) strain for 8 transformants. The results of SDS-PAGE and Western blot analysis indicated that a cysteine protease of approximately 25 kDa (mature form) was generated. The optimal pH and temperature of the enzyme were determined to be approximately 9.5 and $35^{\circ}C$, respectively, thereby indicating that the cysteine protease is a member of the alkaline protease group. The evaluation of substrate specificity indicated that the purified protease was more active towards Arg-X or Lys-X and did not efficiently cleave the substrates with non-polar amino acids at the P1 site. The PLCP evidenced fibrinolytic activity on the plasminogen-free fibrin plate test.
목 적: HBeAg 음성 만성 B형 간염 환자에서 라미부딘의 적절한 치료 기간에 대한 연구는 국내외에 보고된 바 없다. 저자 등은 라미부딘의 장기 치료 효과를 분석하여 HBeAg 음성 소아 만성 B형 간염의 적절한 치료 기간을 결정하는데 예비자료로 제공하고자 하였다. 방 법: 1999년 7월부터 2006년 8월까지 경북대학교병원 소아청소년과에 내원하여 만성 B형 간염으로 진단받은 83명 중 HBeAg 음성 만성 B형 간염으로 진단되어 라미부딘 치료를 시작하였던 환자 7명 중 2년 이상 경과한 6명을 대상으로 하였다. 대상 환자들은 모두 혈청 ALT치가 정상 상한치의 2배 이상으로 증가되어 있었다. 라미부딘을 3 mg/kg (최대 100 mg)으로 매일 1회, 최소 2년 이상 경구 투여하였다. HBV DNA의 소실과 혈청 ALT 수치의 정상화를 1차 목표로 하였고 라미부딘 종료 후 관해 유지를 최종 목표로 하였다. 라미부딘은 HBV DNA의 음전화 및 ALT치의 정상화 후 2년 이상 추가 투여하기로 하였다. 치료 시작 후 매 2~3 개월 마다 HBV DNA, 혈청 ALT, HBeAg과 anti-HBe 역가의 변화 추이를 조사하였다. 결 과: 라미부딘 치료 기간은 평균 32.3개월(26~40개월)이었고 평균 추적 관찰 기간은 59.5개월(26~110개월)이었다. 라미부딘 치료를 받은 모든 환자에서 3개월 이내에 HBV DNA가 0.5 pg/mL 미만으로 감소하였다. 2005년 이후에 치료를 시작한 환자는 3명이었는데 3명 모두에서 3~23개월에 0.007 pg/mL (=357 IU/mL) 미만으로 감소하였다. 혈청 ALT치 정상화에 걸린 기간은 6명의 환자에서 평균 3.5개월(2~7개월)이었다. 라미부딘으로 치료한 환자 중 5명은 HBV DNA PCR에서 357 IU/mL 미만으로 유지되고 있으나 한 명의 환자에서 18개월에 생화학적 돌파현상(breakthrough)이 관찰 되어 28개월째에 투여를 중단하였다. 라미부딘 치료가 종료된 4명에서 평균 23.8개월(4~75개월)동안 추적 관찰하였지만 재발의 소견은 보이지 않았다. 결 론: HBeAg 음성 만성 B형 간염 환자에서 라미부딘은 효과적으로 HBV 증식을 저지할 뿐 아니라 혈청 ALT치를 정상화시켰다. 치료 종료 후의 재발률을 낮추기 위해 HBeAg 음성 만성 B형 간염 환자에서 라미부딘의 적절 치료 기간은 HBV DNA 음전 및 혈청 ALT치 정상화 후 2년 추가가 필요할 것으로 추정할 수 있으나 더 많은 환자를 대상으로 한 연구가 필요하다.
Glucose, fructose and sucrose being the main sugars that can be found in natural fruit juice. Many instrumental methods, such as GC, LC, electrochemical or spectrometric methods provide information about both the total content of sugars and the specific concentration of each carbohydrate[1]. The simplicity of sample handling and measurement in the near IR(NIR) wavelength region, which allows the use of long pathlength, optical glass cells and optical fibers, makes NIR a good alternative for sugar determination [2]. In the present study, six varieties of persian grapes were harvested at intervals through august to october and analysed for sugars by NIR. The results were processed by principal component regression (PCR) and partial least squares (PLS) analysis. Sample juice was prepared by squeezing through gauze from crashed grape. This solution was treated by zinc ferrocyanide prior to analysis in order to eliminate colored compounds and all optically active nonsugar substances. For glucose and fructose the most characteristic wavelengths were 1456nm corresponding to the first harmonic O-H stretching and the second at 2062nm corresponding to O-H stretching and deformation; secondary characteristic combination bands were also seen at 2265 nm (O-H and C-C stretching) and at 2240 nm (C-H and C-C stretching). However these spectra were taken over a wavelength range from 1100-2500nm at room temperature of 25-$30^{\circ}C$. To test the accuracy of the described procedure, samples of six varieties of grape were analysed by the proposed NIR and a standard method[2]. Good agreement were found between these two sets of the results. To perform the recovery studies , samples of grape juices previously analysed by the proposed method, were spiked with known amounts of each individual sugars and then analysed again. Relative standard deviations varied from 1.4 to 1.8% for six independent measurements of individual and total sugar concentration. In the analysis of real and synthetic samples, precise and accurate results were obtained , providing accuracy errors lower than 1.9% in all cases. Average recoveries of ${97}{\pm}{4%}$ for total sugar and between ${95}{\pm}{5%}$ and ${99}{\pm}{2%}$ for sing1e sugars demonstrate the applicability of the methodology developed to the direct analysis of grape Juice.
문주란(Crinum asiaticum Linne var. japonicum)은 한국, 말레이지아 등 동남아시아 지역에서 관절통, 해열, 궤양 치료, 국부의 소염 및 해열 등의 목적에 민간요법 등으로 오래 전부터 사용되어 오고 있다. 본 연구에서는 이러한 문주란의 화장료 조성물로서의 가능성을 확인하기 위하여 iNOS (inducible nitric oxide synthase) 억제 그리고 PGE2, IL-6, and IL-8 방출 억제에 의한 항염 효능을 측정하였다. pH를 3.5로 조절하고 95% ethanol을 사용하여 추출한 후, HPLC 실험으로 문주란의 주성분이 면역조절물질로 잘 알려진 lycorine이며 대략 1% 정도 함유한 것을 확인하였고 그 함량은 문주란의 추출 부위 및 추출방법에 따라 달랐다. 리포다당(lipopolysaccharide, LPS)에 의해 활성화된 생쥐 대식세포(RAW 264.7 cells)에서 생성되는 NO 형성의 억제능을 측정한 항염실험에서, 문주란 에탄을 추출물은 투여량에 비례하는 저해능을 가지고 있음을 확인할 수 있었다($IC_{50} = 83.5 {\mu}g/mL$). 또한 RT-PCR법을 이용하여 문주란 에탄을 추출물이 iNOS 유전자 발현도 억제함을 확인하였다. 또한, 문주란 추출물은 전혀 세포독성을 보이지 않았으며 오히려 LPS에 대하여 세포증식효과를 나타내었다(세포생존율 약 $10{\sim}60%$ 증가). 인간의 섬유아세포에서 과산화수소에 의해 활성화된 PGE2, IL-6 및 IL-8 방출 억제효능 측정 실험에서는 0.05%와 1% 농도 이상에서 (PGE2와 IL-6의 분비가) 거의 완전히 억제됨을 확인할 수 있었고, 나아가서 IL-8의 경우는 모든 실험농도 범위에서 완전히 억제되었다(>0.0025%). 이러한 결과로부터 문주란의 추출물이 충분한 항염 효과를 가지고 있음을 확인할 수 있었다.
목 적 : HSP 사구체 신염은 소아 만성 사구체신염의 흔한 원인이다. 본 연구에서는 HSP 환자들을 신장 침범이 있는 군과 없는 군으로 분류, 양 군 사이에 AGT 유전자의 235번 아미노산이 methionine에서 threonine으로 치환되는 AGT M235T 유전자 다형성의 분포에 차이가 있는지를 조사하고, HSP 신염 환자군을 대상으로 AGT M235T 유전자 다형성이 임상양상과 관련이 있는지를 조사하였다. 방 법 : 1996년 1월부터 2001년 6월까지 부산백병원 소아과를 방문하여 HSP로 진단된 61명의 환자를 대상으로 하였다. AGT M235T 유전자형은 PCR로 측정하였다. 결 과 : 1) AGT M235T 유전자형의 분포는 HSP군에서 MM형이 75%, MT형이 25%, 그리고 TT형이 0%이었다. HSP 신염군에서는 MM형이 64%, MT형이 36%, TT형이 0%으로 HSP 신염군과 HSP 군 사이에 유전자형 분포의 유의한 차이는 없었다. 2) HSP 신염군에서 각각의 유전자형에 따른 단백뇨의 동반, 사구체 여과율, 혈청 알부민, 혈청 크레아티닌치 등은 초기와 추적 관찰 후의 검사에서 유전자형에 따른 유의한 차이가 없었다. 추적기간 동안 말기 신부전으로 진행되어 신장이식을 받은 1명은 MT형이었으며 나머지 환자는 모두 정상 신기능을 유지하였고, 고혈압의 발생도 관찰되지 않았다. 3) 24시간 채집뇨의 단백량은 초기와 추적관찰 후 각각 MT형이 MM형에 비해 높은 경향을 나타내었으나, 통계적으로 유의하지 않았다(P=0.3529, P=0.8469). 중등도 이상의 단백뇨(≥500 $mg/m^2/day$)를 가진 경우도 초기와 추적 관찰 후 각각 MM형에서 29%, 20%, MT형에서 42%, 36%로 MT형이 MM형에 비해 높은 경향을 나타내었으나, 통계적으로 유의하지 않았다. 결 론 : 본 연구에서 소아 HSP 신염 환자에서 AGT M235T 유전자형의 분포는 HSP 환자군과 유의한 차이가 없었다. 본 연구의 경우 추적 관찰기간은 평균 25개월이므로 보다 정확한 HSP 신염에 대한 AGT 유전자 다형성의 영향을 확인하기 위해서는 보다 많은 증례를 대상으로 하여 장기간의 추적 관찰이 필요하리라 생각한다.
