A new type of high precision electromagnetic suspension (EMS) which can support heavy tray along long stroke rail is proposed in this paper. Compared with the conventional EMS, the suggested moving-core typed EMS has the levitation electromagnets (EMs) on the fixed rail. This scheme has high load capability caused by iron-core and enables simple tray structure. Also it does not have precision degradation caused by heat generation from EMs, which is a drawback of conventional EMS. With these merits, the proposed EMS can be an optimal contactless linear bearing in next generation flat panel display (FPD) manufacturing process if the ability of long stroke movement is proved. So a special Section Switching Algorithm (SSA) is derived from the resultant force and moment equations of the levitated tray which enables long stroke movement of the tray. In order to verify the feasibility of the suggested SSA, a simple test-setup of the EMS with 2 Section-changes is made up and servo-controlled in the simulation and experiment. The simulation shows the perfect changeover the EMs, and the experiment shows overall control performance of under ${\pm}40{\mu}m$ gap deviations. These results reveal that the newly suggested contactless linear bearing can simultaneously achieve high load capability and precision gap control as well as long stroke.
Objective : Covered stenting is an optional strategy for traumatic carotid pseudoaneurysm, especially in malignant conditions of potential rupture, but the long-term outcomes are not clear. Our aim was to determine if covered stenting is an effective option for traumatic carotid pseudoaneurysm with promising long-term outcomes. Methods : Self-expanding Viabahn and balloon-expandable Willis covered stents were separately implanted for extra- and intracranial traumatic carotid pseudoaneurysm. The covered stent was placed across the distal and proximal pseudoaneurysm leakage under roadmap guidance. Procedural success was defined as technical success (complete exclusion of the pseudoaneurysm and patency of the parent artery) without a primary end point (any stroke or death within 30 days after the procedure). Long-term outcomes were evaluated as ischemic stroke in the territory of the qualifying artery by clinical follow-up through outpatient or telephone consultation and as the exclusion of the pseudoaneurysm and patency of the parent artery by imaging follow-up through angiography. Results : Five patients with traumatic carotid pseudoaneurysm who underwent covered stenting were enrolled. The procedural success rate was 100%. No ischemic stroke in the territory of the qualifying artery was recorded in any of the five patients during a mean clinical follow-up of 44±16 months. Complete exclusion of the pseudoaneurysm and patency of the parent artery were maintained in all five patients during a mean imaging follow-up of 39±16 months. Conclusion : Satisfactory procedural and long-term outcomes were obtained, suggesting that covered stenting is an effective option for traumatic carotid pseudoaneurysm.
Purpose: This study was to examine the effects of a footbath program on heart rate variability, blood pressure, body temperature and fatigue of stroke patients with stroke-induced hemiparesis. Methods: A non-equivalent control group pretest-posttest design was used. Participants were 40 stroke patients, twenty for the footbath program and twenty for the control group, who were hospitalized in a long-term rehabilitation hospital in G city of Korea, from February to April 2014. The twenty participants in the experimental group received the intervention of footbaths and an educational program focused on the prevention of stroke complications; Collected data were analyzed by the IBM SPSS WIN 20.0 program using a t-test, ${\chi}^2$ test, Mann-Whitney U test and repeated measures ANOVA. Results: Significant differences were found in heart rate variability, systolic blood pressure, hand and foot temperatures and fatigue between the two groups. But no significant differences were found in diastolic blood pressure, core temperatures, forehead temperatures, and hand temperatures between the two groups. Conclusion: The footbath program was an effective intervention for skin temperature change and fatigue reduction for stroke patients. Therefore, it is recommended that the footbath program can be utilized as an effective nursing intervention for stroke patients in long-term rehabilitation care hospitals.
Purpose: Myofascial release (MFR) is an effective treatment for improving muscle stiffness and balance in neurological patients. This study examined long-term effects of MFR on the balance ability of stroke patients. Methods: MFR using a tennis ball was applied to the lower extremity and sacroiliac joint of the affected side of four stroke patients. The four subjects performed the Berg balance scale (BBS) and Timed up and go (TUG) test at the beginning of an 8-week intervention, directly after the intervention, and 2 years later. Results: The BBS scores indicated a trend toward maintenance of balance (p=0.05), but there was no difference in the TUG time (p=0.47). Conclusion: MFR may be a clinically meaningful intervention to maintain balance in stroke patients over a long period. However, the sample size in this study was too small to draw general conclusions. A larger study with more participants is needed.
With a rapid development in the area of micro and ultra precision technology, the micro surface machining of small size parts are explosively increased. Especially, to improve efficiency of various beams in lens and reflector, non-rotational symmetric form and several mm level heights changeable surface can be machined at a time. These geometric complex 3D surface cannot be machined by general short stroke FTS. The long stroke FTS if firmly needed to move directly several mm and have nm level positioning accuracy for the complex surface form. The long stroke FTS used linear motors to drive moving unit long and fine, aero static bearings to decrease friction and moving errors in guide way, optical linear scale with nm level resolution to measure position of FTS. Furthermore, to increase the performance of acceleration of FTS, the light material, such as AL is used for the structure and the high stiffness box type structure is selected. In this paper, the genetic algorithm approach is described to determine a set of design parameters for auto tuning. The authors have attempted to model the design problem with the objective of minimizing the error, such as variable pattern change. This method can give the better alternative than existing other method.
Background: The method of measuring the walking function of patients with chronic stroke differs depending on patients walking capability and environmental conditions. Objects: This study aimed to demonstrate the influences of walking capacity and environmental conditions on the results of short- and long-distance walk tests in patients with chronic stroke. Methods: Forty patients with chronic stroke volunteered for this study, and allocated to group-1 (<.4m/s, household walking, $n_1=13$), group-2 (.4~.8m/s, limited community ambulation, $n_2=16$), and group-3 (>.8m/s, community ambulation, $n_3=11$) according to their walking capacity. The 10-meter walk test (10MWT) and 6-min walk tests, (6MWT) were used to compare the short- and long-distance walk tests results, which were randomly performed under indoor and outdoor environmental conditions. Results: The comparison of the results obtained under the indoor and outdoor conditions revealed statistically significant differences between the groups in the 6MWT and 10MWT (p<.05). Post-hoc tests' results showed significant differences between groups-1 and -2 and between groups-1 and -3 in the 10MWT, and between group-1 and -3 in the 6MWT. Furthermore, in group-2 the 10MWT and 6MWT results significantly differed between the indoor and outdoor conditions, and the values measured under the indoor and outdoor conditions significantly differed between 10MWT and 6MWT (p<.05). Group-3 showed a significant difference in 10MWT results between the indoor and outdoor conditions (p<.05). Conclusion: These findings suggest that the results of the short- and long-distance walk tests may differ depending on the walking capacity of patients with chronic stroke and the environmental condition under which the measurement is made, and these effects were greatest for the patients with the limited community ambulation capacity.
Purpose: The cane is one of the most popular assistive devices for stroke patients. Clinical complaints of sensorimotor functions on the ipsilateral upper limb were appealed in stroke patients who had used a cane for a long period. Therefore, we investigated whether cane usage for a long-term period affected sensoriomotor dysfunctions on the non-affected upper limb, in terms of pain presence, shoulder joint sense, a nine-hole pegboard test, and a tracking task. Methods: We recruited 12 stroke patients, who were divided into the cane-using (CU) group or the non-cane using (NCU) group, according to cane usage experience. We evaluated joint position sense for the integrity of proprioceptive reposition sense in the shoulder joint, used a nine-hole pegboard test for upper limb dexterity evaluation, and a tracking task for visuomotor coordination. Results: Four patients in the CU group had complained of shoulder pain none did in the NCU group. In addition, the CU group showed more reposition errors on the shoulder joint than the NCU group did. In addition, the CU group had more difficulty in proprioceptive sense perception and in performance of the nine-hole pegboard teat and tracking task, compared with the NCU group. Conclusion: Our findings suggest that cane usage for a long period in stroke patients could give rise to trigger joint pain and decrease proprioceptive sense. In addition, complex motor performance in the ipsilateral upper limb could deteriorate. In stroke patients who had used acane for long period, careful observation and proper intervention will be necessary.
본 연구는 장기간 노인(전문)병원 및 요양시설에서 생활하고 있는 뇌졸중 생존 노인들의 건강관련 삶의 질에 미치는 요인들이 무엇인지 규명함으로써 대상자들의 삶의 질 증진을 위한 사회복지 실천 방안들을 제언하고자 시행되었다. 연구 대상자는 충청지역 및 D 광역시 소재 노인(전문)병원 18곳과 요양시설 13곳에서 장기 입원(입소)하고 있는 55세 이상의 뇌졸중 생존 노인들이며 서베이 방법을 사용하여 직접면담조사를 실시하였다. 조사는 2008년 12월 16일부터 2009년 1월 17일까지 실시되었고, 총 328부의 설문지가 통계분석에 이용되었다. 연구결과 대상자들이 인지하고 있는 건강관련 삶의 질은 비교적 낮게 나타났고, 특히 건강관련 삶의 질을 구성하고 있는 하위 영역 중 사회적 관계 영역이 가장 낮게 나타났다. 투입된 독립변수들이 건강관련 삶의 질에 미치는 상대적 영향력은 '우울', '향후 경제적 부담감', '일상생활수행능력', '장애수용', '중복질환', '경제적 주부양자가 배우자인 경우'순으로 나타났다. 본 연구를 통해 장기재활 및 요양서비스를 이용하고 있는 뇌졸중 생존 노인들의 건강관련 삶의 질에 질병관련 요인뿐만 아니라 심리사회적 요인, 환경적 요인 모두 중요한 영향을 미치고 있다는 것이 규명되었고, 그 가운데서도 심리사회적 요인의 영향력이 제일 큰 것으로 밝혀졌다. 이를 바탕으로 노인의료복지시설에서 생활하고 있는 뇌졸중 생존 노인들의 삶의 질을 향상시키고 더 나아가 사회복지사들의 역량을 고취시킬 수 있는 사회복지 방안들이 논의되었다.
Purpose: The purpose of this study was to measure the functional status of stroke patients cared for in different long-term care settings. Method: We assessed all stroke patients in two home health care agencies, four nursing homes and one geriatric hospital in Korea (n=171) using the Resident Assessment Instrument (RAI), which comprises Activity of Daily Living (ADL), urine incontinence, bowel incontinence, a Cognitive Performance Scale (CPS),and being understood and understanding others. Data was collected by face-to-face surveys with patients. Results: The mean ADL score, urine incontinence score, bowel incontinence score, CPS, and being understood score and understanding others score were lowest for the patients receiving home health care, and highest for the patients in nursing homes. Low scores described poor and high scores good functional status. The results showed significant differences in physical and cognitive function scores between the three groups of patients. Conclusion: This study suggests that there may be large differences between the patients in these three types of long-term care settings. These findings can be used to help develop and implement efficient long-term care programs.
