Hospital-Acquired Pressure Injury: Clinical Characteristics and Outcomes in Critical Care

  • Electronic health records (EHRs) enable us to use and re-use electronic data for various multiple purposes, such as public reporting, quality improvement, and patient outcomes research. Current hospital-acquired pressure injury (HAPI) risk assessment instruments have not been specifically developed for intensive care unit (ICU) patients and showed false positive rates in this specific populations. Previous research studies report a number of risk factors; however, it is still not clear what factors influence ICU HAPI in this population. As part of a larger research study, we performed an exploratory analysis by using a large electronic health record data. The aims of this study were to compare characteristics of patients who developed HAPIs during their ICU stay with those who did not, and to determine whether the two groups were different in the aspects of length of ICU stay, discharge disposition, and discharge destinations. We conducted chi-square test and t-test for group comparison. Association was examined by using bivariate analyses. Pearson correlation coefficients were used to examine correlation between LOS and number of medications. Our findings suggest a number of consistent and potentially modifiable risk factors, such as sedation, feeding tubes, and the number of medications administered. The mortality of the HAPI group was significantly higher than the non-HAPI group in our data. Discharge disposition was significantly different between the groups. 67% of the HAPI group transferred to intermediate or long-term care hospitals whereas 57.7% of the non-HAPI group went home after discharge. Awareness of these risk factors can lead to clinical interventions that can be preventative in the ICU setting.

  • It depends on the joint configuration, dimensions and constraints of the joint whether the residual stress at the root of single-sided butt weld is tensile or not. Therefore, recommendation is generally made that high R ratio should be used in the fatigue test of welded joint in order to prevent excessively long life caused by compressive residual stress. In this research, the residual stress profile in butt weld joint was obtained through compliance method, using successive extension of a slot and measurement of the variation of strain during the slot extension. The residual stress profile was firstly assumed to be the linear summation of Legendre polynomials up to 9th order excluding 0th and 1st order. Strain variation on the surface was measured while the slot was being extended by cutting to find out the 8 unknown coefficients of each polynomial term. The cut was made by the electric discharge machine. It was concluded that the residual stress near the surface is positive valued, however, it turned into the negative value as soon as it passed through 2 or 3 mm of the depth.

  • As the issue of microplastics (MPs) detection in tap water was raised in other countries in 2017, monitoring of MPs in drinking and source water, and sewage treatment plant (STP) effluents was initiated. This study intends to look into other studies on MPs in STPs at home and abroad, and review the characteristics of MPs and their removal efficiencies in the STPs, the risk and effect of MPs on watersheds, and management practices in order to help better understand MPs in STPs. To manage MPs effectively in STPs, it is necessary to investigate the detection of MPs discharged from STPs, do research on human health risk and control measures, and build a monitoring system including standardized analytical methods.

환경영향평가와 사후환경조사의 비교에 관한 연구 - 태안화력발전소 사례분석 - (The Study on the Improvement of Environmental Assessment Method through Analysis of Environmental Investigation Results of Taean Thermal Power Plant)

  • This study compares the results of environmental impact assessment with the results of post-environmental investigation, using the case of Taean thermal power plant construction. The atmospheric and water qualities were not greatly changed before and after the construction of the power plant. However, the site of the highest concentration predicted by the atmospheric quality modeling in environmental impact assessment was different from that after operation of 4 power plants. There was also a difference in the diffusion range of thermal discharge water between the measured result(1km) and the predicted value(1.5km) with the model. Thus, environmental impact evaluation should be based on long-term (more than a year) environmental monitoring data. For the modeling of atmospheric quality and numerical thermal discharge water diffusion, appropriate models for each plant should be selected and the numerical modeling should be accompanied by computer simulation, wind tunnel test, etc. Moreover, environmental evaluation should focus more on the degree of impact on surroundings than the prediction of changes in surroundings caused by operation of plants.

Investigations on PD Characteristics of Thermal aged Palm and Corn Oil for Power Transformer Insulation Applications

  • Partial discharge (PD) detection plays a major role in the life time assessment of liquid insulation in power transformers. Many research works are being carried out to replace conventional mineral oil insulation in transformers by vegetable oils. It is necessary to understand the PD characteristics of vegetable oils before recommending them as an alternate for mineral oil. In this paper, the breakdown strength and PD characteristics of palm and corn oil were investigated in both unaged and thermally aged conditions. Laboratory experiments were performed as per IEC test procedures. PD signals were measured using wide band detection system. Phase resolved PD pattern of vegetable oils and mineral oil were compared. Effect of increase in voltage stress on the PD pattern of palm and corn oil were studied. Time and frequency domain analysis of PD pulses at needle-plane electrode configuration was carried out. Statistical analysis of PD pattern i.e. skewness and shape parameter variations with respect to applied thermal stress were also carried out. From the results, it is observed that palm and corn oils have better breakdown strength and PD characteristics even under long-term thermal stress and hence they can be used for power transformer applications.

데이터 마이닝의 Model Tree를 활용한 내성천의 유사량 산정 및 안정하도 평가 (Sediment discharge assessment and stable channel analysis using Model Tree of data mining for Naesung Stream)

  • 본 연구에서는 국내 하천에서 실측한 유사량 자료를 기초로 데이터 마이닝의 Model Tree 기법을 통해 유사량 산정 공식들을 도출하였으며, 이를 활용하여 내성천에서의 안정하도 단면을 평가하였다. 본 연구에서 도출한 유사량 공식은 국내 모래하상 전체를 대상으로 한 경우, 하폭, 유속, 수심, 경사, 하상토 중앙입경을 선택하였을때 적합도가 가장 높은 것으로 나타났다. 또한 내성천으로 자료의 범위를 한정한 경우에는 경사를 제외한 하폭, 유속, 수심, 하상토 중앙입경을 선택한 유사량 공식의 적합도가 가장 높게 나타났다. 각각의 Model Tree 공식들은 내성천 영주댐 하류 용혈지점에서의 안정하도 단면 평가를 수행하는데 적용되었으며, 현재 내성천의 단면과 비교했을 때 향후 안정하도 단면으로의 변화를 위해 하상의 침식이 발생할 것으로 예측되었다. 또한 현재 하상보다 완만한 경사가 유지될 경우 장기적으로 평형상태에 도달할 것으로 예측되었다.

영산강 하구둑 담수 방류에 따른 하구 건강성 평가 (Assessment of Ecosystem Health during the Freshwater Discharge in the Youngsan River Estuary)

  • 영산강 하구는 연안지역에 하구언이 축조되어 인공적으로 환경이 변화된 시스템이다. 또한 최근에는 담수역에 보가 건설되었고 담수 조류가 대증식하는 사례가 빈번하게 보고되고 있다. 이러한 담수가 간헐적으로 하구언 수문을 통해 해수역으로 방류되면서 하구의 해수역 수질에 영향을 미치는 것으로 알려져 있지만 아직까지 해양 생태계의 건강성 변화에 대한 평가는 부족한 실정이다. 따라서 본 연구에서는 3개 정점을 대상으로 담수 방류 이전과 이후 4일 동안 시행된 해양환경과 식물플랑크톤에 대한 현장조사 자료를 활용하여 수질 및 건강성을 평가하고자 하였다. 건강성 평가를 위해 WQI, TRIX, P-IBI 등의 기법을 활용하였고 P-IBI 기준 설정을 위해 장기간 수집된 자료를 이용하였다. 평가 결과 공간적으로 하구언의 수문과 가까운 정점에서 건강성 등급이 낮았고 외해로 갈수록 등급이 높아지는 것으로 나타났지만 시간적으로는 평가기법에 따라 건강성 지수의 차이를 보였다. WQI와 TRIX는 방류 중에 지수가 증가하였다가 방류 후에 전 상태로 회복되는 경향을 보인 반면 P-IBI는 방류 후에 오히려 증가함으로써 건강성이 악화되는 결과를 보였다. 따라서 하구와 같이 급변하는 환경에서는 변화에 민감하게 반응하는 부유생물인 식물플랑크톤 군집이 지표로 포함된 P-IBI와 같은 기법이 더 적합할 것으로 사료된다.

유역토양수분 추적에 의한 실시간 홍수예측모형 (Real-time Flood Forecasting Model Based on the Condition of Soil Moisture in the Watershed)

  • One of the most difficult problem to estimate the flood inflow is how to understand the effective rainfall. The effective rainfall is absolutely influenced by the condition of soil moisture in the watershed just before the storm event. DAWAST model developed to simulate the daily streamflow considering the meteologic and geographic characteristics in the Korean watersheds was applied to understand the soil moisture and estimate the effective rainfall rather accurately through the daily water balance in the watershed. From this soil moisture and effective rainfall, concentration time, dimensionless hydrograph, and addition of baseflow, the rainfall-runoff model for flood flow was developed by converting the concept of long-term runoff into short-term runoff. And, real-time flood forecasting model was also developed to forecast the flood-inflow hydrograph to the river and reservoir, and called RETFLO model. According to the model verification, RETFLO model can be practically applied to the medium and small river and reservoir to forecast the flood hydrograph with peak discharge, peak time, and volume. Consequently, flood forecasting and warning system in the river and the reservoir can be greatly improved by using personal computer.

Comparison of Psychological Influence on Breast Cancer Patients Between Breast-conserving Surgery and Modified Radical Mastectomy

  • Objective: To compare the influence of breast-conserving surgery (BCS) and modified radical mastectomy (MRM) on the psychological state of breast cancer patients. Methods: Patients receiving MRM or BCS, and fulfilling the study criteria, were recruited. Patients were required to complete a self-reporting inventory (SCL-90) on admission and 6 months after surgery and a self-rating depression scale (SDS) when discharged from hospital and 6 months after surgery. Results: A total of 70 patients received MRM and 50 BCS. Compared with the national standard, patients suffered to some extent psychological problems on admission, at discharge from hospital and at 6 months after surgery. Patients received BCS had a higher score of SDS compared with those with MRM when discharged from hospital. However, 6 months after surgery, SDS score increased in MRM and decreased in the BCS group, so the difference was significant. Conclusion: The short-term psychological state of patients receiving BCS is worse than that with MRM but superior to MRM 6 months postoperatively. BCS imposed less influence on long term psychological state of breast cancer patients compared with MRM.

만삭 크기 거대 난소 낭종의 복강경 적출술 (Laparoscopic Extirpation of the Term Sized Huge Ovarian Cyst)

  • A 23 years old single nulligravida woman underwent laparoscopic removal of a huge cystic adnexal mass that occupied her entire abdomen, giving the appearance of a full term pregnancy. After anesthesia, a vertical infra-umbilical incision, 1 cm long, was made and a telescope was introduced through the port to determine the status of the intra-abdomen and the surface contour of the mass. A needle tipped with a laparoscopic suction apparatus was inserted into the cyst through the infra-umbilical port, directly under the mass. Subsequently, 3,200 ml of cystic fluid was aspirated without spillage. A huge cyst, reaching to the level of the xyphoid process was effectively excised through the operative laparoscopy after prelaparoscopic drainage. Operation time was 140 minutes and hospital stay was 2 days. There were no complications during hospital stay and after discharge. It seems the size of the cyst is not a criteria for the contraindication of laparoscopic surgery.
