노인장기요양보험 시설서비스 본인부담금에 대한 가족수발자의 경제적 부담감 영향요인 (Factors Related to Family Caregiver Financial Burden of Out-Of Pocket Expenses for the Nursing home service under Long-term Care Insurance System)

  • The purpose of this study is to investigate the factors affecting family caregiver financial burden of out-of pocket expenses for the nursing home service under Long-term Care Insurance System. We conducted a national cross-sectional descriptive survey from July to September 2010 to collect data based on the long-term care benefits cost specification. Total 1,016 family caregivers completed questionnaires. 185 subjects of total were excluded from the data analysis due to being answered by user(18 cases), or caregivers not to pay for services expenditures(122 cases), having a missing data on family caregivers characteristics(45 cases). Finally, 831 subjects were included in the study. The average financial burden was 3.18(${\pm}0.71$). We divided subjects into two groups by level of burden, high-burden group and low-burden group. In the result of the multiple logistic regression analysis, family caregiver financial burden was significantly higher in family caregivers with ages 40 to 49 compared to less than 40, lower educational level, unsatisfaction for long-term care service, high percentage(more than 50%) of cost-sharing and high total out-of pocket expenses(more than 300,000 won) for long-term care services. Also, Family caregivers who are spouse felt higher financial burden compared to son. This study is meaningful as the first attempt to measure family caregiver financial burden for long-term care service and to identify factors affecting the financial burden. Family caregivers felt financial burden of out-of pocket expenses for the nursing home service. The policy makers, the insurer, and the providers need to pay attention to ease family caregiver financial burden.

유료노인장기요양보호서비스 이용의사 결정요인 (Determining Factors of Intention to Actual Use of Charged Long-term Care Services for the Aged)

  • Objectives : To help develop strategies to cope with the changes arising from the rapid aging process by predicting the determining factors of intention to actual use of the charged long-term care services for elderly as perceived by the middle aged who play the major role of supports. Methods : Subjects were the parents (men 177, women 507) in their 40s of the students selected from a university of Busan city. A questionnaire survey was conducted for 4 weeks in October 2003 about the knowledge for long-term care service, the intention of actual use, and the preferences about the type of service suppliers. Data analysis was performed with frequency, chi-square test, and t-test using SPSS program (ver 10.0K), along with data mining using decision tree of Enterprise Miner V8.2 by SAS. Results : About half of the subjects (53.7%) had the actual experiences of elderly supports. Intentions to use the charged services were relatively high in home visiting nursing care service (40.1%) and long-term care facilities service (40.4%), and were influenced by previous knowledge about the services. The intentions were stronger in women, those with higher education, and those with greater income levels. Actual elderly supports were mostly (80%) done by women, and the perceived burdens for the supports were bigger in women and those of lower socioeconomic level. Desired charges were about 10,000 won for the bath service, 20,000 won for the rests services per day, and about 500,000 won for the long-term care facilities service per month. From the result of decision tree analysis, the job professionalism was the most important determining factor of intention to actual use of the services with validation as $63{\sim}71%$. Health and welfare mixed type facilities were preferred, and the most important consideration was the level of professionalism. Conclusions : Intention to actual use of the charged services was largely determined by the aspects of time and cost. Polices to increase the number of service suppliers and to decrease the burdens perceived by actual supporters were strongly recommended.

재활요양병원의 재활군 환자의 만족도 요인에 관한 연구 (Satisfaction Determinants of Rehabilitation Care Group Inpatients in Rehabilitation Long-term Care Hospitals)

  • Objectives : The aim of this study was to investigate the satisfaction determinants of rehabilitation care group inpatients in rehabilitation long-term care hospitals. Methods : A survey was conducted with 262 patients from 6 rehabilitation long-term care hospitals located in Busan Kyungnam. Data were collected from June 2016 to July 2016 with a structured questionnaire and analyzed with SPSS 23.0. Results : The significant factors affecting the overall satisfaction score were disease group and 5 quality indices of medical services(Medical service quality, Rehabilitation services quality, Hospital reputation, External physical environment, and Internal physical environment). Additionally, the significant factors affecting intention to revisit were age, disease group, length of hospital stay and the 5 quality indices of medical services. Conclusions : The results of this study indicate that rehabilitation long-term care hospitals should try to improve the overall satisfaction of inpatients, especially focusing on the internal physical environment and the quality of the rehabilitation services and medical services, which will lead to a high level of intention for patients to revisit.

노인요양시설 사례관리자가 인지하는 조직 내 의사소통이 서비스 질에 미치는 영향 (The Effects of Communication Satisfaction of the Case Managers on Service Quality in the Long-term Care Institutions)

  • 본 연구는 노인요양시설의 사례관리자가 인지하는 조직 내 의사소통 만족과 서비스 질과의 관련성을 규명함으로써 사례관리에 있어 다양한 전문가로 구성된 팀원들간의 원활한 의사소통이 양질의 서비스의 전제조건임을 살펴보고자 하였다. 이를 위해 서울시와 경기도의 30인 이상 규모의 48개 법인시설에 종사하는 사례관리자 104명을 대상으로 설문조사를 진행하였으며, 기술분석 및 다중회귀분석을 통해 두 변인간의 관련성을 규명하고자 하였다. 연구결과 사례관리자가 인지하는 조직 내 의사소통의 만족도가 높을수록 제공하는 서비스의 질이 높은 것으로 평가하는 것으로 나타났다. 이러한 연구결과를 바탕으로 사례관리팀내의 의사소통을 원활히 하기 위한 방안과 다양한 사례관리모델 중 노인요양기관에서는'팀접근'모델이 보다 적합할 수 있음을 제언하였다.

일본의 개호보험체제상의 개호지원전문원제도의 구조와 특징 (The Structure and Characteristics of the Care Manager Systems in the Long-term Care Insurance of Japan)

  • 일본은 1990년 이후 사회복지제도를 개혁하는 과정에서 이용자본위의 서비스지원, 자기결정에 따른 서비스이용, 자립생활을 지원하는 서비스 등을 새로운 개혁이념으로 제시하면서, 이를 실천하는 제도적 장치의 하나로써 서비스에 대안 이용지원제도를 제시하였으며, 개호보험제도의 개호지원전문원이 당해 역할을 담당하게 되었다. 따라서 개호지원전문원의 도입은 사회복지서비스의 이용과정의 따른 패러다임의 변화를 가져왔다. 즉, 조치제도에 따른 서비스이용의 타율성과 서비스의 이용과정의 복잡성을 고려할 때, 개호지원전문원이라는 단일창구를 통한 서비스의 종합적 상담 지원 그리고 시장기능에 따라서 움직이는 개호지원전문원의 역할을 고려할 때, 요원조고령자의 서비스 선택폭은 크게 확대 강화되었다. 한편, 개호지원전문원제도는 구조적 취약점도 지니고 있다. 즉, 개호지원사업자(소)를 중심으로 한 개호지원기능 개호지원사업자와 서비스사업자간의 병행 운영, 개호지원전문원의 업무내용과 범위의 모호, 개호지원전문원의 겸무허용에 따른 업무과다, 처리역량을 벗어난 사례담당에 따른 어려움, 사회적 지원제도의 부족, 역할과 신분을 초과하는 의무와 처벌 등이 이에 해당된다. 한편, 일본의 개호보험제도와 비슷한 노인수발보장제도와 평가관리원의 도입을 추진하고 있는 한국의 입장에서 보면 이와 같은 개호지원전문원제도의 구조와 특징은 많은 시사점을 줄 수 있을 것으로 본다.

산재장해인의 장기요양서비스 요구 분석 (Long-Term Care Needs Assessment of the Disabled Workers After an Industrial Injury)

  • Purpose: The objective of this study was to evaluate long-term care needs using RAI MDS-HC and MI-CHOICE among the disabled workers. Methods: Data were obtained from 45 personal care recipients with the disability of mental and nervous system, and analyzed using SAS 9.1 by applying t-test, ${\chi}^2$ test, or fisher's exact test. Results: Only 'bed mobility' and 'indoor ambulation' items of ADL and problem activity were statistically significant factors by the level of personal care benefit. By MICHOICE grouping, 20.0 percent of subjects belonged to nursing home group, 51.5 percent were home care service, 28.9 percent were intermittent personal care. Conclusion: Personal care services in industrial accident compensation insurance have been categorized with two groups according to level of disability. But our results could contribute to provide personal care service according to the long term care needs.

  • Government came to enforce long-term care insurance system in preparation for the rapid aging society. Whether this system is successful or not depends on the professionalism of long-term care-givers who are professional population in charge of care service. Currently in the early stage of enforcement, such problems as a race cutting fee resulted from numerical increase of educational facilities, insolvent operation, degradation of education level resulted from unprofessional instructor, are pointed out. As a mean of manpower-training on long-term care insurance system, this study is to research public-private-university network model of the Academy of Continuing Education attached to Ko Sin University which is the case of Yong Do Gu Busan city. Networking between the vision and development strategy of Yong Do Gu on continuing education city, education system on community manpower-training supported by Ko Sin University, and service field of welfare for the elderly can not only contribute to the professionalism of long-term care-givers but also play an ideal role in manpower-utilization and arrangement of community. Through this networking, high quality of education level and circumstance, using the existing infra, manpower-training and utilization for continuing education of Yong Do Gu can be accomplished. Additionally, the connection with facilities related with welfare for the elderly can contribute to professionalism and accountability of manpower-networking.

장기요양서비스 전.후 가족의 수발 부담감에 영향을 미치는 요인 (Caregiver Burden in Caring for Elders Before and After Long-term Care Service in Korea)

  • Purpose: Purposes of this study were: evaluation of family burden of caring for elders who receive long term care services, and examination of differences in burden before and after the introduction of long term care service in Korea. Methods: Data were collected by questionnaires from 416 caregivers of elders who were registered with the Long Term Care Insurance Corporation in six cities. Data were collected in September, 2010 and analyzed using descriptive statistics, paired t-test, and ANOVA with the Scheffe test, and stepwise multiple regression. Results: Family burden decreased significantly after longterm care service was initiated. Subjective burden decreased from 2.93 to 2.69 (t=11.78, $p$<.001), and objective burden, from 3.40 to 3.10 (t=12.73, $p$<.001). Stepwise multiple regression analysis revealed that factors affecting subjective burden were family relations (F=13.60, $p$=.003), age (F=5.47, $p$=.019), job (F=6.98, $p$=.008), and education (F=4.59, $p$=.032), and that factors affecting objective burden were living together (F=17.66, $p$<.001), job (F=13.34, $p$=.003), monthly income (F=6.61, $p$=.010), and type of service (F=6.62, $p$=.010). Conclusion: The results of this first study to investigate caregiver burden after the Korean Long-term Care Insurance System was begun provide positive information for the development of strategies to decrease family burden in long term care.

요양보호사의 기관 및 서비스 평가를 통한 재가노인서비스 품질 향상 방안 (A Study on the Improvement of In-Home Care Service Quality through Evaluation of Services and Agency by Long-term Care Workers)

  • 본 연구의 목적은 서비스 공급자로서 재가요양서비스를 제공하고 있는 요양보호사의 서비스에 대한 평가를 통해 서비스 품질 개선 방안을 찾는 것이다. 이를 위해 부산에서 활동 중인 요양보호사 223명을 대상으로 직무 환경, 서비스 기관에 대한 평가, 재교육 등에 대한 내용을 중심으로 설문조사를 실시하였다. 조사결과 요양보호사는 학력은 낮지 않으나 연령대가 높고 불안정한 고용형태를 가진 이가 대다수였다. 더 나은 요양서비스를 위해 희망하는 슈퍼비전 내용은 서비스 이용자와의 관계성, 이용자 욕구 변화에 따른 대처 등이었다. 우선적으로 필요한 재교육 내용은 노년기 및 노인 가족에 대한 이해, 노인성 질환에 대한 보건 의료적 지식, 이용자 및 가족과의 상담기법 등으로 나타났다. 그리고 건강보험공단 등에 바라는 것은 이용자와 보호자의 인식 개선, 급여 인상, 그리고 전문성 강화 위한 교육 기회 확대 등이었다. 이상의 연구결과를 통해 요양보호서비스 향상을 위한 방안으로 서비스 공급구조적 측면에서의 개선과 요양보호사와 업무에 대한 인식개선, 적절한 슈퍼비전과 재교육 기회 제공 등을 제안하였다.

장기요양시설 노인에 대한 간호서비스 제공시간 분석 (Care Time of Elderly in Long-Term Care Facilities)

  • Purpose: This study was to define the care time of elderly in long-term care facilities and to compare and analysis the care time by staff and facility types. Method: The data were collected from 530 elderly over sixty, residing in two long-term care hospitals for dementia, three long-term care hospitals for the elderly and two skilled nursing facilities. Care time for individual residents was measured the total time spent by nurses, aides. Result: The average care time measured by nursing staff was 158.6 minutes a day. The average care time for one resident by nurse was 40.4 minutes, and by aides, 118.2 minutes. The difference of the care time showed statistical significance between long-term care facility types(p<0.001) : average care time for dementia hospital(199.1min) was twice as long as that of skilled nursing facility(94.1min). Conclusion: The results of the study showed that the care time differentiates care time provided to elderly in long-term care facilities in Korea. The study suggests the need to emphasize the importance of standardization of level of staff and service programs by the long-term facilities.
