주관절 원발성 골성 관절염의 관절경적 전방 변연 절제술 및 최소 절개 후방 절제술 (Arthroscopic Anterior Debridement and Mini-Open Posterior Resection for Primary Osteoarthritis of the Elbow)

  • 목적: 주관절의 원발성 골성 관절염의 치료로 관절경적 전방 변연 절제술 및 최소 절개를 이용한 후방 골극 절제술을 시행하여 이 술식의 유용성을 알아보고자 하였다. 대상 및 방법: 2003년 3월부터 2010년 2월까지 불응성 주관절 골성 관절염의 치료로 관절경적 전방 변연 절제술 및 최소 절개 하 후방 골극 절제술을 시행한 19예를 대상으로 하였다. 추시 기간은 평균 19개월이었으며, 평균 연령은 49세였다. 결과는 Andrew-Carson Rating Scale (ACRS)과 Mayo Elbow Performance Score (MEPS)를 이용하여 평가하였다. 결과: 운동 범위는 굴곡 구축이 수술 전 $28.7^{\circ}$에서 수술 후 $17.9^{\circ}$, 후속 굴곡은 $105.1^{\circ}$에서 $121.8^{\circ}$로 증가하였다. MEPS는 51.1점에서 수술 후 87.9점으로 호전되어 우수 3예, 양호 13 예, 보통 3예를 보였다. ACRS는 92.9점에서 수술 후 168.2점으로 호전되어 우수 3예, 양호 14예, 보통 2예를 보였다. 1예를 제외한 전 예에서 일상생활에 지장이 없이 이전 직업으로 복귀하였다. 결론: 불응성 주관절 골성 관절염의 치료로 관절경적 전방 변연 절제술 및 최소 절개 하 후방 골극 절제술은 동통 완화 및 기능 회복에 도움이 되는 술식이라 생각된다. 그러나 골극의 재발이나 관절염의 진행 여부는 장기적 추시가 필요할 것으로 사료된다.

Clavicle Nonunion: Matchstick Bone-grafting and Osteosynthesis

  • Background: This study was conducted to evaluate the surgical outcomes of plate fixation using autologous iliac bone graft shaped in the form of a matchstick in clavicle nonunion resulting from prior surgical or non-surgical management. Methods: From May 2005 to February 2013, 17 patients underwent surgery for clavicle nonunion. The mean age at the time of surgery was 48.8 years. The iliac bone was harvested and shaped into slivers approximately 3 cm long, which resembled matchsticks. After opening of the medullary canal, the plate and screws were applied and cancellous bone was placed at the nonunion site. Matchstick bone grafts which were longitudinally placed and encircled on the nonunion site were tied with periosteum using 3-4 stitches. Union was determined via postoperative plane X-ray. Clinical status was evaluated using the visual analogue scale score for pain, and Constant and American Shoulder Elbow Surgeon score. Results: All patients had a stable radiological union at the follow-up. The mean duration from index operation to nonunion operation was 13.2 months (7-32 months). The mean follow-up period was 20.1 months (8-56 months), and the mean duration until union was 11.2 weeks (8-16 weeks). All clinical scores were improved at the final follow-up (p<0.001). The mean segmental defect was $3.3{\pm}2.6mm$ (1-18 mm); and the difference in clavicular length between operative and non-operative site was $5.9{\pm}6.9mm$. Conclusions: Matchstick shaped autologous iliac bone grafting technique in clavicle nonunion is acceptable with a high union rate.

중등학생 골프선수의 퍼팅 스트로크 동작에 대한 운동학적 분석 (Kinematic Analysis of Secondary School Golf Player's Putting Stroke Motion)

  • The objective of this study was to analyze the difference in kinematic variables for successful and unsuccessful golf putting strokes. The study population included 8 male secondary school golf players who had played golf for over 3 years and whose handicap was 4 or lower. A hole was made on a 5-m-long artificial flat mat for practice, and an environment similar to that of a real green was created. The participants' motions were analyzed through 3D image analysis, and the difference in kinematic variables for successful and unsuccessful putting strokes in the same direction was determined. Data analysis revealed the following findings: The time spent for a segment of putting was the greatest for the backswing segment for both successful and unsuccessful strokes. During address and impact, the both changed to a larger extent. For successful putting strokes, the change in the elbow angle during the downswing was greater for the right elbow than for the left elbow. For both successful and unsuccessful putting strokes, the left shoulder angle increased during the segment from address to the turning point and decreased during the segment from the turning point to impact. In contrast, the right shoulder angle significantly differed between successful and unsuccessful putting strokes only during address. During successful and unsuccessful motions, the swing was executed with the moving displacement of the X-axis of the club head maintained almost constant along a straight light without back and forth movement. In the backswing segment, moving displacement of the Y- and Z-axes was greater in successful strokes than in unsuccessful strokes; however, this difference was very small for the Y-axis. The velocity of the club head for successful and unsuccessful motions significantly differed during address and at the turning point. The highest velocity of the ball was greater for successful than for unsuccessful putting strokes.

동적 초음파를 이용하여 진단한 견관절 상완 이두건 장두의 아탈구 (Subluxation of the Long Head Biceps Tendon Diagnosed by Dynamic Ultrasound)

  • 목적: 이학적 검사나 단순 방사선, 정적 초음파, 자기공명영상 등의 검사를 통해 진단하기 어려운 상완 이두건 장두 아탈구의 진단 시 동적 초음파의 유용성에 대해 보고하고자 한다. 대상 및 방법: 동적 초음파를 이용해 상완 이두건 장두 아탈구로 진단된 2명의 남자환자에 대해 관절경을 이용한 연부조직 상완 이두건 장두 고정술을 시행하고 증상 및 기능적 회복 정도를 평가 하였다. 결과: 동적 초음파를 이용해 상완 이두건 아탈구를 쉽게 진단할 수 있었으며 수술 후 증상 및 기능적 평가에서도 향상을 나타내었다. 수술 후 특이한 합병증은 발생하지 않았다. 결론: 진단이 어려운 상완 이두건 장두 아탈구의 진단 시 동적 초음파의 이용은 유용한 방법으로 판단된다.

  • Medial dislocation of the long head of the biceps brachii is a rare condition that usually occurs in association with tears of the subscapularis, chronic impingement, capsular defects or a fracture of the lesser tuberosity. Less commonly, a biceps tendon dislocation may occur after an acute traumatic event. Following a dislocation, the biceps tendon will assume either an intra- or extra-articular position depending on whether or not the subscapularis tendon detaches from its humeral insertion. Magnetic resonance imaging has been found to provide valuable information concerning the location of the biceps tendon and the integrity of the subscapularis tendon. We present a patient with a traumatic dislocation of the biceps brachii tendon in which the diagnosis remained elusive for an extended period of time. In this case, he was evaluated using MRI and reconstruction was performed by restoring the tendon to its anatomical position.

Long-term outcomes of initially conservatively treated midshaft clavicle fractures

  • Background: Recent studies about completely displaced midshaft clavicle fractures have reported that their nonunion/malunion rates were significantly higher in conservatively treated patients compared to surgically treated patients. The purpose of this study was to evaluate the factors associated with treatment decisions for midshaft clavicle fractures and also the factors that affect patient satisfaction with their treatment choice. Methods: We retrospectively reviewed the records of 75 patients who had been diagnosed with a midshaft clavicle fracture and were treated conservatively at a single institution between March 1, 2013, and December 31, 2014. Their medical records were reviewed to investigate the severity of the initial vertical displacement. A telephone survey was carried out to identify the presence of any patient-perceived deformity and determine if the patient eventually underwent surgery and whether the patient would prefer surgery if the injury recurred. Results: Significantly more patients with vertical displacement ≥100% (9/28) eventually underwent surgery compared to patients with vertical displacement <100% (3/32, p=0.028). Patients with vertical displacement ≥100% (13/28) were significantly more likely to prefer surgery compared to patients with vertical displacement <100% (7/32, p=0.044). Among the conservatively treated patients, nine of 32 participants with a patient-perceived deformity and one of 16 without a patient-perceived deformity responded that they would prefer to receive surgery in same situation in the future (p=0.079). Conclusions: Patients with a midshaft clavicle fracture with vertical displacement of ≥100% may eventually require surgical treatment. When conservative treatment is carried out, the long-term patient results may be unsatisfactory due to perceived residual deformities.

Rotator cuff repair with or without proximal end detachment for long head of the biceps tendon tenodesis

  • Background: Rotator cuff tears cause pathologies of the long head of the biceps tendon (LHBT). One of the surgical treatments for such a tear is LHBT tenodesis to the humerus. This study aims to compare simultaneous rotator cuff repair and LHBT tenodesis with or without detachment of the proximal end of the LHBT (PELHBT) from its site of adhesion to the glenoid. Methods: This retrospective study involved patients affected by LHBT pathology with rotator cuff tear. The patients were divided into two groups, with or without PELHBT detachment from the glenoid. Therapeutic outcomes were investigated by evaluation of patient satisfaction, pain based on visual analog scale, shoulder function based on Constant score and simple shoulder test, and biceps muscle strength based on the manual muscle testing grading system before surgery, at 6 months, and at the final visit after surgery. Results: Groups 1 and 2 comprised 23 and 26 patients, respectively, who showed no significant differences in demographic characteristics (p>0.05). Shoulder function, biceps muscle strength, pain, and satisfaction rate improved over time (p<0.05) but were not significantly different between the two groups (p>0.05). No post-surgical complication was found in either group. Conclusions: There was no difference in final outcomes of tenodesis with or without detachment of the PELHBT from the supraglenoid tubercle. Such tendon detachment is not necessary.

혈우병 (Hemophilia)

  • Hemophilia is the most common coagulation disorder. It has a long history. Hemophilia A is caused by FVIII gene mutation, and hemophilia B by FIX gene mutation. Those genes are located on X chromosome long arm. Bleedings in hemophiliacs predominantly occur in joints and muscles. Because those site are insufficient in tissue factor to induce hemostasis. Among joints knee, ankle and elbow are most frequently affected because their synovial structure is vulnerable to injury compared to other joints. Hemophilia is diagnosed with factor assay. Severe hemophilia is below 1% of FVIII : C, moderate between 1% and 5%, mild over 5%. Carrier detection and prenatal diagnosis have been conducted with RFLP-based linkage analysis and DNA sequencing. Mainstay of treatment is factor replacement therapy so far. Bleedings can be controlled by infusion of factor concentrates. Hemophilc arthropathy and muscle contracture are representative sequelae. Complications of facotor replacement therapy are inhibitor development and infections. Hemophiliacs with inhibitor should be managed with large dose factor concentrate, bypassing agent, ITI and immunosuppression. Ultimately, hemophilia could be cured by gene therapy.

Short Humeral Stems in Shoulder Arthroplasty

  • Since the introduction of shoulder arthroplasty by Neer in 1974, the design of not only the glenoid component but also the humeral component used in shoulder arthroplasty has continually evolved. Changes to the design of the humeral component include a gradually disappearing proximal fin; diversified surface finishes (such as smooth, grit-blasted, and porous coating); a more contoured stem from the originally straight and cylindrical shape; and the use of press-fit uncemented fixation as opposed to cemented fixation. Despite the evolution of the humeral component for shoulder arthroplasty, however, stem-related complications are not uncommon. Examples of stem-related complications include intraoperative humeral fractures, stem loosening, periprosthetic fractures, and stress shielding. These become much more common in revision arthroplasty, where patients are associated with further complications such as surgical difficulty in extracting the humeral component, proximal metaphyseal bone loss due to stress shielding, intraoperative humeral shaft fractures, and incomplete cement removal. Physicians have made many attempts to reduce these complications by shortening the stem of the humeral component. In this review, we will discuss some of the limitations of long-stem humeral components, the feasibility of replacing them with short-stem humeral components, and the clinical outcomes associated with short-stemmed humeral components in shoulder arthroplasty.

변형된 Y형 해부학적 금속판을 이용한 상완골 원위부 골절의 치료 (Treatment for Distal Humeral Fractures by Modified Anatomical Y-Plate)

  • Comminuted fracture of the distal end of the humerus in adults is very rare and difficult to treat. In operative treatment, an implant which can achieve accurate anatoraical reduction and rigid fixation is needed. But the pre­existing Y -shaped plate had wide and thick limbs and some problems in fixation for most distal humeral fractures. So we devised a modified plate which is more narrow and th1n and has a different hole distance and wing length (long lateral wing). The aim of the study was to evaluate the clinical result of this modified anatomical Y -plate. From 1991 to 1997, we treated 23 cases of distal humeral fractures using modified anatomical plate and the results were as follows. 1. Fracture type in 18 patients(78%) was C-type(C1,C2,C3) which were intraarticular and mostly displaced or comminuted. 2. Bony union was obtained in 22 patients(96%) through rigid fixation and observed radiologically at 3.5 months(2-6months) on the average. 3. In 23 patients, 19 patients(82%) showed satisfactory results after 34 months(6-73months) follow-up. So if the modified anatomical Y-plate is used in the treatment of distal humeral fracture, a satisfactory result can be obtained through a more accurate, easy, and rigid fixation than preexisting plate.
