응급실 간호사의 감염노출 예방행위 수행정도와 영향요인 (Performance of Preventive Actions to be Exposed to Infection in Emergency Nurses and its Influencing Factors)

  • Purpose: The purpose of this study was to identify factors influencing on performance of preventive actions to be exposed to infection in emergency nurses. Methods: Participants were 200 emergency nurses working in a regional emergency medical center, 4 local emergency medical centers, and 5 local emergency medical facilities in B city. The data were analyzed with descriptive statistics, independent t-test, ANOVA, Scheff$\acute{e}$'s test, Pearson's correlation coefficients, and stepwise multiple regression analysis using SPSS/WIN 21.0 programs. Results: There was no difference in performance of preventive actions to be exposed to infection by socio-demographic and job-related characteristics. The results of stepwise multiple regression analysis showed that levels of perception of preventive actions to be exposed to infection (${\beta}$=.40, p<.001) and protective environment on exposure to infection (${\beta}$=.22, p<.001) were significantly associated with performance of preventive actions, explaining 26.3% of the variance. Conclusion: In conclusion, improving performance of preventive actions to be exposed to infection is important to protect emergency nurses from exposure to infection. Thus, efforts to enhance protective environment on exposure to infection and to improve perceptions of preventive actions to be exposed to infection are necessary to improve the performance of preventive actions in emergency nurses.

Antimicrobial effect of topical local anesthetic spray on oral microflora

  • Background: To evaluate the antimicrobial activity of lidocaine (LD) topical anesthetic spray against oral microflora. Methods: Antimicrobial effects of 10% LD spray were assessed against six bacterial cultures obtained from volunteers: Escherichia coli, Enterococcus faecalis, Staphylococcus aureus, Streptococcus salivarius, Streptococcus pyogenes, and Streptococcus sanguinis. The filter papers contained $50-{\mu}l$ LD, brain heart infusion (BHI) broth, or 0.2% chlorhexidine. Papers were placed on the cultured blood plates for 1-3 min. After the papers were removed, plates were incubated for 24 h. Bacterial growth on the contact areas was recorded as the antimicrobial score. The split mouth technique was use in for sample collection in clinical study. Filter papers soaked with either BHI broth or LD were placed on the right or left buccal mucosa for 1 min, and replaced with other papers to imprint biofilms onto the contact areas. Papers were placed on blood plates, incubated for 24 h, and antimicrobial scores were determined. Experiments were conducted for 2- and 3-min exposure times with a 1-day washout period. Results: LD exhibited bactericidal effects against E. coli, S. sanguinis, and S. salivarius within 1 min but displayed no effect against S. aureus, E. faecalis, and S. pyogenes. The antimicrobial effect of LD on oral microflora depended upon exposure time, similar to the results obtained from the clinical study (P < 0.05). LD showed 60-95% biofilm reduction on buccal mucosa. Conclusions: Antimicrobial activity of 10% LD topical anesthetic spray was increased by exposure time. The 3 min application reduced oral microflora in the buccal mucosa.

An Off-site Screening Process for the Public in Radiation Emergencies and Disasters

  • Background: A contamination screening process for the local population in radiation emergencies is discussed. Materials and Methods: We present an overview of the relevant Korean governmental regulations that underpin the development of an effective response system. Moreover, case studies of foreign countries responding to mass casualties are presented, and indicate that responses should be able to handle a large demand for contamination screening of the local public as well as screening of the immediate victims of the incident. Results and Discussion: We propose operating procedures for an off-site contamination screening post operated by the local government for members of the public who have not been directly harmed in the accident. In order to devise screening categories, sorting strategies assessing contamination and exposure are discussed, as well as a psychological response system. Conclusion: This study will lead to the effective operation of contamination screening clinics if an accident occurs. Furthermore, the role of contamination screening clinics in the overall context of the radiation emergency treatment system should be clearly established.

중소기업 도금공정에서의 트리클로로에틸렌 폭로와 발산량에 관한 연구 (A Study on Worker Exposure to Trichlorethylene and Emission Factor for Degreasers in Plating Plants)

  • This study was conducted at seven degreasing processes in plating plants located in Seoul, Incheon, Ansan, and Taejeon areas from July 21 to august 27, 1992. This study was performed to assess the TWA exposures to trichloroethylene (TCE) and evaluate factors affecting TCE concentrations in degreasing process. Two-Point Eddy Diffusional Model suggested by Wadden et al. was employed to calculalte emission factors according to degreaser type. Results are summarized as follows. 1. The TWA exposures of the degreasing operators ranged from 1.4 ppm to 123 ppm, and those of three plants out of seven were exceeding 50 ppm of both the Korean and U.S. OSHA standards. Degreasing assistant of Plant B, was exposed to 59 ppm. 2. The average concentrations at the distance 0, 1.5, 3.0 m from the degreasers were 1.014, 24, and 18 ppm, respectively, and showed a signifficant difference by distance (p<0.01). 3. The emission of TCE was reduced by installing local exhaust systems, condensers, and refrigeration lines at the degreasers (p<0.01). 4. The major factors related to exposure of operators were workload (r=.9621. p<0.01) and dimensions of degreansing room(r=-.8667, p<0.05). 5. If the air in degreasing room is mixed violently by other factors in addition to diffusion, the emission factors can not be evaluated because the important hypothesis of the Two-Point Eddy Diffusional Model can not be accepted. 6. The ultrasonic degreaser without the local exhaust ststem, condenser, and refrigeration lines emitted TCE three times greater than the ultrasonic degreaser with condenser and refrigeration 1ines only.

울산지역 초등학생 알레르기 질환 유병률: 지역적 차이와 환경위험인자 (Prevalence of Children's Allergic Diseases in Ulsan: Local Differences and Environmental Risk Factors)

  • Objectives: This study aims to investigate the AD (allergic diseases: asthma, allergic rhinitis and atopic dermatitis) prevalence among elementary schoolchildren in an industrial city, Ulsan, and identify major environmental risk factors associated with AD prevalence. Methods: Data on the physician-diagnosed prevalence over the past 12 months and potential risk factors of AD were collected through a questionnaire from a 2009-2010 survey of 4,067 schoolchildren living in different urban environments. The logistic regression analysis was performed to assess differences in AD prevalence among the areas and to determine which environmental factors impacted AD. Results: Our survey results showed that the AD prevalence rate ranged between 26.2% and 35.9%. Children living in polluted areas (near industrial and central urban areas) had about a 10% higher prevalence of AD than did those living in coastal or suburban residences. The Chi-Square test demonstrated that this local difference was statistically significant before and after adjustment of major confounders such as parental AD history and parental education. The results of the logistic regression analysis showed a statistically significant association between several environmental factors (ventilation in winter, odor conditions and exposure to traffic smoke, and outdoor $PM_{10}/O_3$ pollution) and the prevalence of AD found by multivariate model after adjusting confounders. Conclusion: These results suggest that local differences in AD prevalence are significantly associated with outdoor environmental factors. Although there are likely to be other risk factors for AD, living in a polluted area and exposure to high levels of air pollutants can contribute to an increased risk of childhood AD.

소규모 지역단위 교통사고예측모형 개발 - 서울시 행정동을 대상으로 (Development of Traffic Accident frequency Prediction Model by Administrative zone - A Case of Seoul)

  • 우리나라는 교통안전법에 의해 지역교통안전기본계획을 수립 시행하고 있다. 지자체별 맞춤형 교통안전시책 수립을 통해 교통안전 향상을 위한 대책 수립 및 역할분담이 강조되고 있으며, 이는 곧 지자체가 지역실정에 맞는 내실 있는 지역교통안전 정책을 실질적으로 주도하여 추진하는 것을 의미한다. 지자체들이 가지고 있는 다면적인 특성에 부합되는 효율적인 교통안전정책이 시행되기 위해서는 지역특성을 고려한 지역단위 교통사고를 예측하고 사고에 미치는 영향요인 분석이 선행되어야 하지만 이에 대한 연구는 미흡한 실정이다. 지역을 기반으로 하는 교통사고 예측에 관한 기존 연구들은 자료 수집의 한계로 대부분 국가 또는 도시를 분석단위로 사고노출환경과 관련되는 사회경제변수들을 활용한 연구가 대부분이었다. 교통사고 예측모형을 개발하는 이유는 교통사고 발생특성을 파악하여 교통사고를 줄일 수 있는 효율적인 대책을 발굴하는 것이 주요 목적이다. 이에 본 연구에서는 기존연구에서 다루지 못한 보다 작은 지역단위인 행정동을 단위로 지역교통사고 예측모형을 개발하였으며, 사고 노출환경 측면의 사회경제적 변수 외 교통안전정책가가 제어할 수 있는 교통안전시설 및 정책변수를 모형에 반영하여 지역교통안전 정책 수립시 활용할 수 있는 방안을 제시하였다. 모형개발 결과 사고노출환경 측면에서는 도로연장, 건축물 총 연면적, 버스전용차로 설치율, 교차로 및 횡단보도 개소수는 사고와 양(+)의 관계를 보이고 있으며, 횡단보도예고 설치율, 과속방지턱 개소수 및 경찰인력에 의한 단속실적은 사고와 음(-)의 관계에 있는 것으로 나타났다.

  • 현재 지역 방송사들은 수입원 확보의 어려움으로 인한 재정 부족과 경영 악화를 겪고 있다. 이는 해당 방송사들의 컨텐츠가 지역민들의 관심과 참여를 이끌어내지 못해 시청률이 지속적으로 감소하는데 기인한다. 이러한 문제를 해결하기 위해, 본 논문에서는 지역 방송과 밀결합된 소셜 네트워크 서비스를 제안하였다. 제안 방법에서는 작성글의 노출 범위를 지역으로 한정하여 기자들이 손쉽게 지역민들의 관심사를 찾을 수 있도록 구성하였다. 또한 지역민들의 작성글 중 기자가 취재 요청한 글들은 메인 화면의 상단에 배치함으로써, 지역민들의 의견이 방송과 직접적으로 연결될 수 있음을 한 눈에 알아볼 수 있도록 사용자 인터페이스(UI)를 구성하였다. 이는 지역민들의 참여를 활성화하고자 위함이다. 제안한 서비스는 울산 지역 방송사인 JCN과 함께 프로토타입 시스템의 형태로 구현되었으며, 본 논문에서는 해당 시스템의 UI 및 시스템 구조, 작성글의 우선순위 산정 알고리즘 등에 대해 소개한다.

Risk Management on Radiation Under Prolonged Exposure Situation - Focusing on the Tokyo Metropolitan Area in Japan Under the TEPCO Fukushima dai-ich NPP Accident -

  • Examples and experiences of risk management on radiation under prolonged exposure situation are shown. The accident of the Fukushima dai-ichi nuclear power plant after the great east Japan earthquake (11 March, 2011) elevates background level of environmental radiation around the east Japan. For example, ambient dose equivalent rate around Tohkatsu area next to Tokyo located about 200 km-south from the plant, is about 0.1-0.6 micro-Sv $h^{-1}$ mainly due to $^{134}Cs$ and $^{137}Cs$ falling on the ground soil. This level is about double or up to ten times higher than the genuine natural level around the area. International Commission on Radiological Protection (ICRP) recommends how to face the existing exposure situation; that is the prolonged exposure situation. Referring to ICRP's reports and/or related international/domestic documents, we have been discussing and acting to gain public's safety and relief, who have a possibility to be exposed to prolonged lower-dose radiation. Here, we introduce our several experiences on risk management, especially focusing on risk communication, radiation education to public, and stakeholder involvements into making decision in local governments on radiation protection, relating to the accident.

일부 종합병원 중앙공급실에서의 Ethylene Oxide 노출에 관한 연구 (A Study on Worker Exposure to Ethylene Oxide in Central Supply Sterilizing Room of Hospital)

  • This study was conducted at five Ethylene Oxide(EO) sterilizing processes in hospitals located in Seoul from August 8 to August 30, 1994. The main purposes of this study were to assess the TWA(Time Weighted Average) and short term exposures to EO and to evaluate factors affecting EO concentrations in sterilizing room. Results are summarized as follows. 1. The TWA concentrations of the sterilizing operators ranged from <0.005ppm to 3.04ppm and those of two sterilizing rooms out of five exceeded 1ppm, the Korean and ACGIH standards. 2. When the door of the sterilizer is opened at the end of the sterilization cycle, the short term concentrations of operators ranged from <0.005ppm to 11.4ppm, and those of three sterilizing room out of five exceeded 5ppm, the ACGIH short term exposure limit(STEL). The short term concentrations of area samples ranged from 0.24ppm to 49.2ppm and those of four sterilizing room out of five exceeded 5ppm. 3. Factors affecting EO exposure level were aeration type, the location of storage site for sterilized item, amount of gas, use period of sterilizer(p<0.005). 4. Following recommendations are suggested to minimize exposure to EO. The use of EO gas should be reduced by using another available sterilization methods, and the sterilizers and gas tank storage site should be isolated from, other work areas. Combination of local and general ventilation system should be installed. Metal carts or baskets for sterilization load should be used, and work environment and medical monitoring should be performed regularly.
