Tumors of the nasal cavity are rare and are usually classified and reported jointly with tumors of the paranasal sinuses. Neverthless their frequency, diagnostic problems, management, and prognosis differ considerably from the latter and justify their study as a separate entity in order to define clearly and evaluate their characteristics. Males outnumbered females in a ratio of 2 to 1. Regional lymph node metases is infrequent; only 10% of the patients presented nodes on admission. Radiation therapy is more favorable method of treatment for nasal cavity tumor secondary to good cosmetic results and high local control rate. The tumor doses are in the range of 5,000 to 6,000 rads in 5 to 6 weeks and an additional dose of 500 to 1,000 rads were given through reduced field to residual disease area. The results of two cases of nasal cavity tumor treated by radiation alone were reasonably good, and review of literature indicated that the radiation alone can give high local control rate and good cosmetics. Therefore, at present time, we recomend radiation therapy for the nasal cavity tumor.
Surgery has been known to be traditional treatment modality for the malignant salivary gland tumor, the tumors being considered as radioresistant. However, reviews of the literature have shown a high recurrence rate, especially in advanced and/or high grade tumors. The retrospective data suggests that conservative surgery with adjunctive radiation therapy is justified in view of the enhanced locoregional control. In inoperable and recurrent tumor, definitive radiotherapy can achieve $30{\sim}40%$ of average 5 year survival rate, but in early stage, local control and survival rate could be raised more than 80%. This results shown to be aggressive radiotherapy can replace surgical management for the selected cases of small sized tumor. Neutron therapy is another option for curative treatment of malignant salivary gland tumors.
Purpose: To evaluate long-term local control rate and toxicity in patients treated with external beam radiotherapy (EBRT) for pituitary adenomas. Materials and Methods: We retrospectively reviewed the medical records of 60 patients treated with EBRT for pituitary adenoma at Korea University Medical Center from 1996 and 2006. Thirty-five patients had hormone secreting tumors, 25 patients had non-secreting tumors. Fifty-seven patients had received postoperative radiotherapy (RT), and 3 had received RT alone. Median total dose was 54 Gy (range, 36 to 61.2 Gy). The definition of tumor progression were as follows: evidence of tumor progression on computed tomography or magnetic resonance imaging, worsening of clinical sign requiring additional operation or others, rising serum hormone level against a previously stable or falling value, and failure of controlling serum hormone level so that the hormone level had been far from optimal range until last follow-up. Age, sex, hormone secretion, tumor extension, tumor size, and radiation dose were analyzed for prognostic significance in tumor control. Results: Median follow-up was 5.7 years (range, 2 to 14.4 years). The 10-year actuarial local control rates for non-secreting and secreting adenomas were 96% and 66%, respectively. In univariate analysis, hormone secretion was significant prognostic factor (p = 0.042) and cavernous sinus extension was marginally significant factor (p = 0.054) for adverse local control. All other factors were not significant. In multivariate analysis, hormone secretion and gender were significant. Fifty-three patients had mass-effect symptoms (headache, dizziness, visual disturbance, hypopituitarism, loss of consciousness, and cranial nerve palsy). A total of 17 of 23 patients with headache and 27 of 34 patients with visual impairment were improved. Twenty-seven patients experienced symptoms of endocrine hypersecretion (galactorrhea, amenorrhea, irregular menstruation, decreased libido, gynecomastia, acromegaly, and Cushing's disease). Amenorrhea was abated in 7 of 10 patients, galactorrhea in 8 of 8 patients, acromegaly in 7 of 11 patients, Cushing's disease in 4 of 4 patients. Long-term complication was observed in 4 patients; 3 patients with cerebrovascular accident, 1 patient developed dementia. Of these patients, 3 of 4 received more than 60 Gy of irradiation. Conclusion: EBRT is highly effective in preventing recurrence and reducing mass effect of non-secreting adenoma. Effort to improve tumor control of secreting adenoma is required. Careful long-term follow-up is required when relatively high dose is applied. Modern radiosurgery or proton RT may be options to decrease late complications.

Purpose: To evaluate the benefit of adjuvant radiation therapy (RT) for retroperitoneal liposarcoma (RPLS) following gross tumor removal. Materials and Methods: We reviewed 77 patients with primary RPLS surgically treated between January 2000 and December 2013. Cases with gross residual disease were excluded. Tumor grade was evaluated according to the French Federation of Cancer Centers Sarcoma Group (FNCLCC) system. Adjuvant RT was delivered to 32 patients (42%) using external beam RT alone. Median follow-up time was 36 months (range, 5 to 169). Results: Among 77 patients, 33 (43%) presented with well-differentiated, 31 (40%) with de-differentiated, 8 (10%) with myxoid/round and 4 (5%) with pleomorphic morphology. The RT group included less well-differentiated subtype than surgery group (28% vs. 53%). During follow up, 34 patients (44%) showed local recurrence. Local recurrence rate was lower in the RT group (38%) compared to the surgery group (49%). The 3-year local control rate (LC) was 55.6%, and the 3-year overall survival (OS) was 82.1%. Tumor histology and FNCLCC grade were significantly associated with local recurrence. There was no statistical significance of adding adjuvant RT in LC (p = 0.312). However, patients with tumor histology other than well-differentiated subtype showed marginally decreased local recurrence rate after adjuvant RT (3-year LC, RT 43.9% vs. no RT 35.3%; p = 0.087). Conclusion: RPLS patients receiving RT experienced less local recurrence. We suggest that the addition of adjuvant RT may be related to improvement of LCs, especially in patients with non-favorable histologic subtypes.
목적 : 수술과 방사선치료를 받은 이하선 악성종양에서 국소 종양 제어율에 영향을 주는 요인을 알기 위하여 본 연구를 수행하였다. 대상 및 방법 : 1986년부터 1995년까지 전남대학교병원에서 이하선 악성 종양으로 수술과 방사선 치료를 함께 받은 26명을 대상으로 하였다. 대상 환자의 연령 분포는 14세부터 72세까지였고 중앙간은 55세였다. 조직학적 유형별 분포는 10명이 점액표피양암종, 7명이 편평상피세포암, 4명이 포상세포암, 4명이 선양성낭종암, 1명이 선암이었다. 수술은 15명이 이하선 전절제술, 7명이 표재성 절제술, 4명이 아전절제술을 받았다. 안면신경이 절제된 경우는 5명 있었다. 수술 후 병리학적으로 잔존 병변이 있었던 경우가 4명, 절제연 양성이 4명이었다. 방사선치료는 선형가속기의 광자선과 전자선속을 사용하였다. 26명 중 11명은 광자선만으로 치료하였고 전자선이 병용된 경우는 15명이었으며 전자선량은 900 cGy부터 3800 cGy (중앙값: 1760 cGy)까지였다. 이하선종양 부위에 조사된 총방사선량은 5000 cGy부터 7560 cGy (중앙값 : 6020 cGy)까지였다. 대상 환자들의 최소추적기간은 2년이었다. 국소종양제어율의 산출은 Kaplan-Meier법을 이용하였고 단변량분석에는 generalized Wilcoxon test, 다변량분석에는 Cox 모델을 이용하였다. 결과 : 방사선 치료 후 국소종양의 재발은 25명 중 5명 (19$\%$)에서 관찰되었으며 국소종양제어율은 5년에 77$\%$였다. 환자의 5년 생존율은 70$\%$였다. 성별, 연령 (>60세), 종양크기 (>4 cm), 수술소견상 경부림프절 침범과 신경 침범, 절제연 침범 여부 및 총방사선량 (>60 Gy)의 요인이 국소 종양 제어율에 미치는 영향을 단변량 분석한 결과 종양 크기 (p=0.002), 절제연 침범 여부(p=0.011)에 따라 국소종양제어율에 있어서 통계학적으로 의의가 있는 차이를 보였다. 같은 요인들에 대한 다변량 분석에서는 종양 크기 (p=0.022)만이 국소 종양 제어율에 관련이 있는 것으로 나타났다. 결론 : 국소종양 제어율에 영향을 주는 여러가지 예후 인자들의 다변량 분석에서 종양 크기만이 의의가 있었으나 앞으로 더욱 많은 환자를 대상으로 연구가 요구된다.
Background: The pain caused by local anesthetic injection can lead to patient anxiety prior to surgery, potentially necessitating sedation or general anesthesia during the excision procedure. In this study, we aim to compare the pain relief efficacy and safety of using a digital automatic anesthetic injector for local anesthesia. Methods: Thirty-three patients undergoing excision of a benign soft tissue tumor under local anesthesia were prospectively enrolled from September 2021 to February 2022. A single-blind, randomized controlled study was conducted. Patients were divided into two groups by randomization: the experimental group with digital automatic anesthetic injector method (I-JECT group) and the control group with conventional injection method. Before surgery, the Amsterdam preoperative anxiety information scale was used to measure the patients' anxiety. After local anesthetic was administered, the Numeric Pain Rating Scale was used to measure the pain. The amount of anesthetic used was divided by the surface area of the lesion was recorded. Results: Seventeen were assigned to the conventional group and 16 to the I-JECT group. The mean Numeric Pain Rating Scale was 1.75 in the I-JECT group and 3.82 in conventional group. The injection pain was lower in the I-JECT group (p< 0.01). The mean Amsterdam preoperative anxiety information scale was 11.00 in the I-JECT group and 9.65 in conventional group. Patient's anxiety did not correlate to injection pain regardless of the method of injection (p= 0.47). The amount of local anesthetic used per 1 cm2 of tumor surface area was 0.74 mL/cm2 in the I-JECT group and 2.31 mL/cm2 in the conventional group. The normalization amount of local anesthetic was less in the I-JECT group (p< 0.01). There was no difference in the incidence of complications. Conclusion: The use of a digital automatic anesthetic injector has shown to reduce pain and the amount of local anesthetics without complication.
Purpose: To evaluate intracranial control after surgical resection according to the adjuvant treatment received in order to assess the optimal radiotherapy (RT) dose and volume. Materials and Methods: Between 2003 and 2015, a total of 53 patients with brain oligometastases from non-small cell lung cancer (NSCLC) underwent metastasectomy. The patients were divided into three groups according to the adjuvant treatment received: whole brain radiotherapy (WBRT) ${\pm}$ boost (WBRT ${\pm}$ boost group, n = 26), local RT/Gamma Knife surgery (local RT group, n = 14), and the observation group (n = 13). The most commonly used dose schedule was WBRT (25 Gy in 10 fractions, equivalent dose in 2 Gy fractions [EQD2] 26.04 Gy) with tumor bed boost (15 Gy in 5 fractions, EQD2 16.25 Gy). Results: The WBRT ${\pm}$ boost group showed the lowest 1-year intracranial recurrence rate of 30.4%, followed by the local RT and observation groups, at 66.7%, and 76.9%, respectively (p = 0.006). In the WBRT ${\pm}$ boost group, there was no significant increase in the 1-year new site recurrence rate of patients receiving a lower dose of WBRT (EQD2) <27 Gy compared to that in patients receiving a higher WBRT dose (p = 0.553). The 1-year initial tumor site recurrence rate was lower in patients receiving tumor bed dose (EQD2) of ${\geq}42.3Gy$ compared to those receiving <42.3 Gy, although the difference was not significant (p = 0.347). Conclusions: Adding WBRT after resection of brain oligometastases from NSCLC seems to enhance intracranial control. Furthermore, combining lower-dose WBRT with a tumor bed boost may be an attractive option.
Purpose: This study evaluated the treatment effectiveness and proper radiation dose of helical tomotherapy (HT) in spine oligometastases from gastrointestinal cancers. Materials and Methods: From 2006 to 2010, 20 gastrointestinal cancer patients were treated with HT for spine oligometastases (31 spine lesions). The gross tumor volume (GTV) was the tumor evident from magnetic resonance imaging images fused with simulation computed tomography images. Clinical target volume (CTV) encompassed involved vertebral bodies or dorsal elements. We assumed that the planning target volume was equal to the CTV. We assessed local control rate after HT for 31 spine metastases. Pain response was scored by using a numeric pain intensity scale (NPIS, from 0 to 10). Results: Spine metastatic lesions were treated with median dose of 40 Gy (range, 24 to 51 Gy) and median 5 Gy per fraction (range, 2.5 to 8 Gy) to GTV with median 8 fractions (range, 3 to 20 fraction). Median biologically equivalent dose (BED, ${\alpha}/{\beta}$ = 10 Gy) was 52 $Gy_{10}$ (range, 37.5 to 76.8 $Gy_{10}$) to GTV. Six month local control rate for spine metastasis was 90.3%. Overall infield failure rate was 15% and outfield failure rate was 75%. Most patients showed pain relief after HT (93.8%). Median local recurrence free survival was 3 months. BED over 57 $Gy_{10}$ and oligometastases were identified as prognostic factors associated with improved local progression free survival (p = 0.012, P = 0.041). Conclusion: HT was capable of delivering higher BED to metastatic lesions in close proximity of the spinal cord. Spine metastases from gastrointestinal tumors were sensitive to high dose radiation, and BED (${\alpha}/{\beta}$ = 10 Gy) higher than 57 $Gy_{10}$ could improve local control.

It is well known that malignant tumor have hypoxic cell fraction, which is radio resistant and is one of the most important cause of local recurrence after radiotherapy. One of the causes of hypoxia in tumor is blood flow decrease due to increase in blood flow resistance and one of the causes of increased blood flow resistance could be attributed to the increase in blood viscosity. For the evaluation of the change of blood viscosity in oral cancer, experiments were carried out to test the change of blood viscosity among the normal control and xenografted oral cancer nude mice. Relative viscosity measured against distilled water was $3.30{\pm}0.14$ for normal control, and $3.67{\pm}0.62$ for tumor bearing mice at the first time of blood sampling in experimental period ($100mm^3$$200mm^3$). There was no statistically significant difference between the control group and experimental group (p>0.05). However, as the tumor grew, significant difference of blood viscosity was detected at the third time of blood sampling (control group:$3.37{\pm}0.59$, and experimental group: $4.31{\pm}0.41\;300mm^3$
Objective : Brain metastases in primary breast cancer patients are considerable sources of morbidity and mortality. Gamma knife radiosurgery (GKRS) has gained popularity as an up-front therapy in treating such metastases over traditional radiation therapy due to better neurocognitive function preservation. The aim of this study was to clarify the prognostic factors for local tumor control and survival in radiosurgery for brain metastases from primary breast cancer. Methods : From March 2001 to May 2011, 124 women with metastatic brain lesions originating from a primary breast cancer underwent GKRS at a tertiary medical center in Seoul, Korea. All patients had radiosurgery as a primary treatment or salvage therapy. We retrospectively reviewed their clinical outcomes and radiological responses. The end point of this study was the date of patient's death or the last follow-up examination. Results : In total, 106 patients (268 lesions) were available for follow-up imaging. The median follow-up time was 7.5 months. The mean treated tumor volume at the time of GKRS was 6273 $mm^3$ (range, 4.5-27745 mm3) and the median dose delivered to the tumor margin was 22 Gy (range, 20-25 Gy). Local recurrence was assessed in 86 patients (216 lesions) and found to have occurred in 36 patients (83 lesions, 38.6%) with a median time of 6 months (range, 4-16 months). A treated tumor volume >5000 $mm^3$ was significantly correlated with poor local tumor control through a multivariate analysis (hazard risk=7.091, p=0.01). Overall survival was 79.9%, 48.3%, and 15.3% at 6, 12, and 24 months, respectively. The median overall survival was 11 months after GKRS (range, 6 days-113 months). Multivariate analysis showed that the pre-GKRS Karnofsky performance status, leptomeningeal seeding prior to initial GKRS, and multiple metastatic lesions were significant prognostic factors for reduced overall survival (hazard risk=1.94, p=0.001, hazard risk=7.13, p<0.001, and hazard risk=1.46, p=0.046, respectively). Conclusion : GKRS has shown to be an effective and safe treatment modality for treating brain metastases of primary breast cancer. Most metastatic brain lesions initially respond to GKRS, though, many patients have further CNS progression in subsequent periods. Patients with poor Karnofsky performance status and multiple metastatic lesions are at risk of CNS progression and poor survival, and a more frequent and strict surveillance protocol is suggested in such high-risk groups.

제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
