Cervical Tuberculous Lymphadenitis : Clinicopathological Features and AFB Positivity (결핵성 경부 림프절염의 병리소견과 항산균 검출 및 임상소견에 관한연구)

  • Background : Histological analysis of tuberculosis shows a spectrum of findings, from well formed granulomatous inflammation with few bacilli in patient with normal immune response to M. tuberculosis to poorly formed granulomatous inflammation with many bacilli in patient with defective immune response. To evaluate the degree of immune response to M. tuberculosis, we studied the histologic features, including the presence of acid fast bacilli(AFB) in lymph node of patients with cervical tuberculous lymphadenitis, and compared them with clinical characteristics. Methods : We reviewed the histologic features of 33 cases of cervical tuberculous lymphadenitis and processed the excised nodes for auramine-rhodamine staining to detect AFB. The AFB positivity in tissue was compared with the histologic features(degree of granuloma formation, presence of caseation necrosis, presence of neutrophilic infiltration) and clinical characteristics(lymph node size, duration of symptom, presence of local symptom or radiologic evidence of pulmonary tuberculosis). Results : 1) The mean age at diagnosis was 42.4 years, and male to female ratio was 1:4.5. 2) Histologically, all cases showed well formed granuloma and variable degrees of caseation necrosis, and 39% of the cases showed neutrophilic infiltration in the granulomatous inflammation. 3) AFB were confirmed in 52% of the cases, and they were found extracellularly and at the periphery of caseation necrosis. 4) There was no association between AFB positivity and histological features or clinical characteristics. Conclusion : Cervical tuberculous lymphadenitis showed well formed granulomatous inflammation with caseation necrosis, and there was no association between AFB positivity in the tissue and histological or clinical characteristics.

A Study on Hydraulic Characteristics of Rock Joints Dependant on JRC Ranges (JRC 등급에 따른 절리면 수리특성 연구)

  • In order to characterize hydraulic property dependant on join roughness in rock mass, this study computed permeability coefficients on each range of joint roughness coefficient (JRC) suggested by Barton(1976). For a quantitative analysis of roughness components spectral analysis using the fast fourier transform was performed to select effective frequencies on each PC range. The results of spectral analyses show that low ranges of the JRC are mainly composed of low frequency domain, while high ranges of the JRC have dominant components at high frequency domain. The inverse Fourier transform made it possible to generate joint models of each JRC range using the effective frequencies of roughness spectrum. The homogenization analysis was applied to calculate permeability coefficient at homogeneous microscale, and then, computes a homogenized permeability coefficient (C-permeability coefficient) at macro scale. Therefore, it is possible to analyze accurate characteristics of permeability reflected with local effect of facture geometry. According to the calculation results, permeability coefficients were distributed between $10^{-3}m/sec\;and\;10^{-4}/sec$. In cases of sheared joint models permeability coefficients were plotted between $10^{-4}m/sec\;and\;10^{-5}/sec$, showing irregular distribution of permeability coefficients on each IRC range. The differences of permeability coefficients for the same aperture models or for the sheared joint models indicate that changes of roughness pattern influence on permeability coefficients. Therefore, the effect of joint roughness should be considered to characterize hydraulic properties in rock joints.

Tidal and Sub-tidal Current Characteristics in the Central part of Chunsu Bay, Yellow Sea, Korea during the Summer Season (서해 천수만 중앙부의 하계 조류/비조류 특성)

  • This study analyzed the ADCP records along with wind by KMA and discharge records at Seosan A-, B-district tide embankment by KRC for 33 days obtained in the Chunsu Bay, Yellow Sea, Korea spanning from July 29 to August 30, 2010. Various analyses include descriptive statistics, harmonic analysis of tidal constituents, spectra and coherence, complex correlation, progressive vector diagram and cumulative curves to understand the tidal and sub-tidal current characteristics caused by local wind and discharge effect. Observed current speed ranges from -30 to 40 (cm/sec), with standard deviation from 1.7 (cm/sec) at bottom to 18.7 (cm/sec) at surface. According to the harmonic analysis results, the tidal current direction show NNW-SSE. The magnitudes of semi-major axes range from 9.4 to 14.8 (cm/sec) for M2 harmonic constituent and from 4.4 to 7.0 (cm/sec) for S2, respectively. And the magnitudes of semi-minor axes range from 0.1 to 0.5 (cm/sec) for M2 and from 0.4 to 1.4 (cm/sec) for S2, respectively. In the spectral analysis results in the frequency domain, we found 3~6 significant spectral peaks for band-passed wind and residual current of all depth. These peak periods represent various periodicities ranging from 2 to 8 (days). In the coherency analysis results between band-passed wind and residual current of all depth, several significant coherencies could be resolved in 3~5 periodicities within 2.8 (days). Highest coherency peak occurred at 4.6 (day) with 1.2-day phase lag of discharge to band-passed residual current. The progressive vector of wind and residual current travelled to northward at all layers, and the travel distance at middle layer was greater than surface layer distance. The Northward residual current was caused by a seasonal southern wind, and the density-driven current formed by fresh water input effected southward residual current. The sub-tidal current characteristics is determined by seasonal wind force and fresh water inflow in the Chunsu Bay, Yellow Sea, Korea.

Importance of Microtextural and Geochemical Characterizations of Soils on Landslide Sites (산사태지역 토층의 미세조직과 지화학적 특성의 중요성)

  • The purposes of this study are to evaluate and discuss the importance of geochemical properties of soil materials that play an important role in the occurrence of the landslide, using analyses of microtexture, particle size distribution, XRC, and FE-SEM equipped with energy dispersive spectrum on soils collected from landslide slopes of gneiss, granite and sedimentary rock areas. Soils from gneiss and granite areas where landslides took place have much clay content relative to those from non landslide areas, particularly pronounced in the granite area. Therefore the clay content is considered a sensitive factor on landslide. Clay minerals contained in soils are illite, chlorite, kaolinite and montmorillonite. Especially the content of clay minerals in soils from the Tertiary sedimentary rocks is highest, with abundant montmorillonite as expandable species. It is believed that this area was much vulnerable to landslide comparable to other areas because of its high content of monoorillonite, even though there might be weak precipitation. Since no conspicuous differentiation in mineralogy between the landslide area and non landslide area can be made, the occurrence of landslide may be influenced not by mineralogy, but by local geography and mechanical properties of soils. Geochemical information on weathering properties, mineralogy, and microtexture of soils is helpful to better understand the causes and patterns of landslide, together with engineering geological analyses.

Incidence of Escherichia coli and Its Susceptibility to Antimicrobials in Childhood Urinary Tract Infection (소아 요로감염에서 Escherichia coli의 빈도와 항생제 감수성에 대한 연구)

  • Purpose : Empirical antimicrobial treatment is indicated before bacteriological results are available for young children with febrile UTI to minimize renal scarring. To ensure appropriate therapy, knowledge of the prevalence of causative organisms and their susceptibility patterns to antimicrobials is mandatory. We performed a retrospective analysis investigating the local prevalence and resistance patterns of uropathogens, primarily E. coli, isolated from community-acquired UTIs. Methods : A total of 103 positive urine cultures from children with febrile UTI collected at Bundang CHA General Hospital from February 2004 to February 2005 were analyzed. Inclusion criteria were fever higher than $37.5^{\circ}C$, significant bacteriuria with single strain growth of at least 10s colony forming units/mL urine, and leukocyturia >5/HPF. Results : E. coli(89.3%) was the leading uropathogen followed by Enterococcus spp.(3.9%) Klebsiella spp.(2.9%), Citrobctcter spp.(1.9%) and Enterobacter spp.(1.9%). E. coli strains revealed a low proportion of antimicrobial susceptibility to ampicillin(AMP; 27.2%) ampicillinsulbactam(AMS; 34.8%) and trimethoprim-sulfamethoxazole(SXT; 65.2%). Susceptibility patterns to cephalosporins were as follows; cefazolin(1st generation; 91.3%), cefoxitin(2nd; 100%), ceftriaxone(3rd; 97.8%) and cefepime(4th; 97.8%). Three E. coli isolates produced ex tended - spectrum beta-lactamase(ESBL). Conclusion : Empirical treatment with AMP, AMS and SXT, which are commonly used in pediatric clinics, is not recommended for childhood UTI due to high incidence of resistance. The high level of susceptibility to cephalosporins makes these drugs reasonable alternatives. However the emergence of ESBL-producers, even though they are quite few, may have an impact on cephalosporin treatment in the future. (J Korean Soc Pediatr Nephrol 2006;10:18-26)

A Mid-late Maturing Rice Cultivar with High-Quality and Bacterial Blight Resistance "Jinbaek" (벼 중만생 고품질 흰잎마름병 신균계(K3a) 저항성 품종 "진백")

  • A new rice cultivar "Jinbaek" carrying Xa3 and xa5 was derived from the cross between 'HR15204-38-3' with xa5 gene resistant to bacterial blight K1, K2, K3 and K3a, and F1 plant derived from the cross between Junam and Sindongjin with Xa3 gene. "Jinbaek" has approximately 125 days of growth duration from transplanting to harvesting in the west-southern coastal and Honam plain of Korea. Culm length of "Jinbaek" is 71 cm. In reaction to biotic stresses, it shows moderate resistance to blast, and wide spectrum resistance to bacterial blight pathogen, K1, K2, K3, and K3a but susceptible to rice stripe virus and blast. The milled rice of "Jinbaek" exhibits translucent, relatively clear non-glutinous endosperm and midium short grain. It has lower amylose content (18.8%) and protein content (6.2%) compared with Nampyeong. The milled rice yield of this cultivar was 5.30 MT/ha in local adaptability test of three years from 2006 to 2008. This cultivar would be adaptable to the bacterial blight-prone area in the south-western coast and Honam plain of Korea.

Bacterial Blight Resistant Mid-late Maturing Rice 'Manbaek' with High Grain Quality (벼흰잎마름병 저항성 고품질 중만생 벼 '만백')

  • 'Manbaek' is a bacterial blight resistant mid-late maturing rice cultivar with high grain quality. 'Manbeak' was derived from anther culture using the backcross combination, $Hopum^*2/SR30075$. 'Hopum' is a mid-late maturing rice cultivar with high grain quality and 'SR30075' is a pyramid line carrying three bacterial blight resistance genes. 'Manbaek' was selected through the pedigree method, yield trials, and local adaptability tests. 'Manbeak' carrying two bacterial blight resistance gene Xa3 and xa5 showed high-level and broad-spectrum resistance against bacterial blight. 'Manbaek' was resistant to K3a, mostly virulent race in Korea, and exhibited resistance reaction against 16 Korean bacterial blight isolates. 'Manbaek' was a mid-late maturing rice. The heading date of 'Manbaek' was August 19th, which was 5 days later than that of 'Nampyeong'. Manbaek' was a lodging-tolerant rice with short culm and dark green leaf. Due to the low viviparous germination, 'Manbaek' could be a useful material to prevent pre-harvest sprouting. 'Mabeak' was resistant to bacterial blight and rice stripe virus, but susceptible to other virus diseases and insect pests. The yield of 'Manbaek' was similar to 'Nampyeong'. 'Manbaek' showed excellent grain appearance and good tastes of cooked rice, so that it could contribute to improving the quality of bacterial blight resistant cultivars. 'Manbaek', bacterial blight resistant cultivar with high grain quality, is suitable for the cultivation at bacterial blight prone area and has been utilized in the breeding programs for enhancing the resistance against bacterial blight (Registration No. 6069).

Clinical and Epidemiological Characteristics of Common Human Coronaviruses in Children: A Single Center Study, 2015-2019

  • Purpose: Common human coronaviruses (HCoVs) are relatively understudied due to the mild nature of HCoV infection. Given the lack of local epidemiology data on common HCoVs, we aimed to describe clinical and epidemiological characteristics of common HCoVs in children. Methods: Respiratory viral test results from 9,589 respiratory samples from Seoul National University Children's Hospital were analyzed from January 2015 to December 2019. Viral detection was done by the multiplex reverse transcription polymerase chain reaction. Demographics and clinical diagnosis were collected for previously healthy children tested positive for HCoVs. Results: Of the 9,589 samples tested, 1 or more respiratory viruses were detected from 5,017 (52.3%) samples and 463 (4.8%) samples were positive for HCoVs (OC43 2.8%, NL63 1.4%, 229E 0.7%). All 3 types co-circulated during winter months (November to February) with some variation by type. HCoV-OC43 was the most prevalent every winter season. HCoV-NL63 showed alternate peaks in late winter (January to March) and early winter (November to February). HCoV-229E had smaller peaks every other winter. Forty-one percent of HCoV-positive samples were co-detected with additional viruses; human rhinovirus 13.2%, respiratory syncytial virus 13.0%, influenza virus 4.3%. Common clinical diagnosis was upper respiratory tract infection (60.0%) followed by pneumonia (14.8%), croup (8.1%), and bronchiolitis (6.7%). Croup accounted for 17.0% of HCoV-NL63-positive children. Conclusions: This study described clinical and epidemiological characteristics of common HCoVs (OC43, NL63, 229E) in children. Continuing surveillance, perhaps by adding HKU1 in the diagnostic panel can further elucidate the spectrum of common HCoV infections in children.

Bacterial Blight-Resistant Medium Maturing Rice Cultivar 'Haepum' with High Grain Quality (벼흰잎마름병 저항성 고품질 중생 벼 '해품')

  • 'Haepum' is a bacterial blight-resistant, medium maturing rice cultivar with high grain quality. It was derived from a cross between 'Iksan493' (cultivar name 'Jinbaek') and the F1 cross between 'Iksan495' ('Dacheong') and 'Iksan496' ('Jungmo1005'). Of these three cultivars, 'Jinbaek' is a bacterial blight-resistant mid-late maturing rice cultivar with high grain quality, 'Dacheong' is a mid-late maturing rice cultivar with multiple resistance to disease and insects, and 'Jungmo1005' is a mid-late maturing rice cultivar with lodging tolerance. To develop fixed lines, the anther culture method was applied to F1 plants. The cultivar 'Haepum' was selected using the pedigree method, yield trials, and local adaptability tests. The heading date of 'Haepum' was August 11th, three days earlier than that of 'Nampyeong' cultivar. Haepum' is a cultivar tolerant to lodging and it has short culms. Due to its low rate of viviparous germination, 'Haepum' could be useful for preventing pre-harvest sprouting in cultivation of medium maturing rice in the southern plain area of Korea. 'Haepum' carries two bacterial blight resistance genes (Xa3 and xa5), and in our study, it exhibited high-level and broad-spectrum resistance against bacterial blight, including K3a, the most virulent race in Korea. 'Haepum' is also resistant to the rice stripe virus and moderately resistant to rice blast. The yield of 'Haepum' was similar to that of 'Nampyeong'. 'Haepum' showed excellent grain appearance and good taste of cooked rice, and therefore it could contribute to the development of bacterial blight-resistant rice cultivars of improved quality.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N.
