두경부에 국한된 Stage I, II, Non-Hodgkin's Lymphoma의 방사선치료 (Radiotherapy of Stage I and B localized Bead and Heck Non-Hodgkin's Lymphoma)

  • 53 patients of previously untreated Stage I&II Non-Hodgkin's Lymphoma in head and neck treated with irradiation at Yonsei Cancer Center from January, 1970 to December, 1978 were retrospectively analysed. 5 year survival rate and 5 year disease free survival rate were $51.5\%\;and\;42\%$. Local control rate by irradiation was $92.4\%$ with mainly $4,000\~6,000$ rads. 21 patients suffered relapses after radiotherapy, 4 cases recurred within irradiated area, 4 cases at contiguous site of irradiation field, and 13 cases recurred at distant area, more commonly below diaphragm. Most cases relapsed within 1 year 6 months after treatment. Optimum irradiation field for head and neck localized lymphoma, prognostic factors ana usefulness of chemotherapy are also discussed.

감마선 조사가 검정콩의 수분흡수 특성에 미치는 영향 (Effects of Gamma-Irradiation on the Water Absorption Property of Black Soybeans)

  • Effects of gamma irradiation(2.5∼20kGy) on water absorption property was studied for a local variety of black soybeans. In water absorption patterns of black soybeans, the time to reach a fixed moisture content was reduced depending on the increment of water soaking temperature and irradiation dose. Irradiation at 2.5∼10kGy resulted in the reduction of soaking time of black soybeans by about 1∼3 hours and the increase of hydration capacity by 10∼20%, respectively, compared to the nonirradiated control black soybean. The water uptake rate constant of the irradiated black soybean difinitely increased with the increase of dose levels and water soaking temperature. The activation energy for water absorption and z-value were lower in the irradiated black soybeans than in the nonirradiated control black soybean. The efficacy of water absorption property in the irradiated black soybeans was also recognized after one year of storage at room temperature.

원발성위장관임파종의 방사선치료 (Localized Primary Gastrointestinal Lymphomas)

  • Among 238 patients with Non-Hodgkin's lymphoma received radiotherapy at Yonsei Cancer center, Yonsei University Medical College, from 1970 to 1981, 30 patients presented with localized(Stage I&II ) gastrointestinal lymphomas. Retrospective analysis of these 30 cases in an attempt to evaluate the influence of various prognostic factors and the effectiveness of therapy is presented. Overall 5 year survival rate of 30 cases of primary gastrointestinal lymphoma was $48\%$. Bulk of residual disease after initial surgery and stage were significant prognostic factors. Stage I with small residual disease treated with post-op irradiation achieved $100\%$ 5 year survival rate. So above group is considered curable with surgery and post-op irradiation. $80\%$ of Stage II with large residual disease were died with intra-abdominal local tumor control failure. Stage II with small residual disease showed $31.5\%$ 5 year survival rate. Non of them died with local failure. So, we suggest that complete surgical resection of tumor mass should be attempted initially in the management of localized gastrointestinal lypmhomas and systemic chemotherapy is needed in addition to post-op irradiation in the cases of Stage II and large residual disease after initial surgery.

중앙성육아종의 방사선치료 (Radiation Therapy (RT) of Midline Granuloma)

  • 중앙성육아종 환자 7예에 대하여 가톨릭의대 강남성모병원 방사선치료실에서는 1983년 3월 부터 1986년 6월까지, 사이에 외부방사선치료를 시행하였다. 임상증상으로서, 모든 환자에서 침습부위에 궤사를 동반한 부비동염 증세를 보였고, 1예에선 대장(결장)병변도 나타냈다. 방사선치료는 6MV선형가속기를 사용하여 비강 및 부비동 부위를 포함하는 상기도 부위에 $2\~3$문 조사를 실시하여, 총선량 $4,000\~5,000cGy/5\~6$주를 조사하였다. 방사선치료 반응으로서 방사선치료 종료 후 $1.5\~20$개월 추적관찰 한 결과 완전 및 부분관해가 각각 3예, 대장부위에 병변을 보였던 1예에선 병이 진행되고 있음이 관찰되었다.

355nm UV 레이저를 이용한 마이크로 렌즈 어레이 쾌속 제작에 관한 연구 (A Study on Rapid Fabrication of Micro Lens Array using 355nm UV Laser Irradiation)

  • Micro lens array(MLA) is widely used in information technology(IT) industry fields for various applications such as a projection display, an optical power regulator, a micro mass spectrometer and for medical appliances. Recently, MLA have been fabricated and developed by using a reflow method having the processes of micro etching, electroplating, micro machining and laser local heating. Laser thermal relaxation method is introduced in marking of microdots on the surface of densified glass. In this paper, we have proposed a new direct fabrication process using UV laser local thermal-expansion(UV-LLTE) and investigated the optimal processing conditions of MLA on the surface of negative photo-resist material. We have also studied the 3D shape of the micro lens obtained by UV laser irradiation and the optimal process conditions. And then, we made chrome mold by electroplating. After that, we made MLA using chrome mold by hot embossing processing. Finally, we have measured the opto-physical properties of micro lens and then have also tested the possibility of MLA applications.

  • Micro lens array (MLA) is widely used in information technology (IT) industry fields, for examples such as a projection display, an optical power regulator, a micro mass spectrometer and for medical appliances. Recently, MLA have been fabricated and developed by using a reflow method, micro etching, electroplating, micromachining and laser local heating. Laser local thermal-expansion (LLTE) technology demonstrates the formation of microdots on the surface of polymer substrate, in this paper. We have also investigated the new direct fabrication method of placing the MLA on the surface of a SU-8 photoresist layer. We have obtained the 3D shape of the micro lens processed by UV laser irradiation and have experimentally verified the optimal process conditions.

직장암의 방사선 치료결과 및 실패양상 (The Result and Failure after Adjuvant Postoperative Irradiation in Carcinoma of Recum)

  • From Jan.1982 to Dec.1990, 77 patients with rectal cancer were treated with curative surgical resection followed by postoperative adjuvant irradiation alone or combined with chemotherapy at the Department of Radiation Oncology, Korea University Hospital (KUH). Fifty-four (54/77, $70.1{\%}$) patients underwent abdominoperineal resection , 20 (20/77, $26{\%}$) patients underwent low anterior resection, and 3 (3/77, $3.9{\%}$,) patients had wide excision only. Thirty-nine (39/77, $50.5{\%}$) received sequential chemotherapy (2 cycles to 12 cycles). The 5-year survival rate for the entire group was $43{\%}:\;78.2{\%}$ in B2+B3, $39.4{\%}$ in stage C1+C2+C3. Survival rates decreased with increasing penetration of the bowel wall by tumor and the presence of regional lymph node metastasis. Those patients survival who underwent an abdominoperineal resection also experienced a significant decrease in compared to low anterior resection ($23.1{\%}$ vs. $63.8{\%}$ in 5-year survival, p <0.05). Local failure occurred in 15 ($19.5{\%}$) out of the 77 patients overall, 1($5.3{\%}$) of 19 in stage B2+B3, and 14 ($24.1{\%}$,) of 58 in stage C1+C2+C3. Presacral area was most common site of local failufre (8/17, $47.1{\%}$). Distant failure occurred in 13 ($16.9{\%}$) of 77 patients. The most frequent site of distant failure was the lung followed by the liver, the bone, and the brain. Combined locoregional and distant failure occurred in 2 ($2.6{\%}$) of 77 patients. Pathological confirmation of perirectal fat and/or regional lymph node involvement resulted in a singificant decrease in survival and local control.

단일 국소방사선 조사 후 백서 대뇌 피질의 초급성기 변화에 대한 연구 (Hyperacute Radiation Effect on Cerebral Cortex after Local Gamma-irradiation in the Rat Brain)

  • Objective: We investigated the morphologic changes within 24 hours after a single ${\gamma}$-irradiation in the rat brain. Methods: Forty Sprague-Dawley rats were used. After a burr hole trephination on right parietal area, cerebral hemisphere was irradiated with 2Gy and 5Gy using iridium-192($^{192}Ir$), respectively. The effect was assessed at 4, 8, 12 and 24 hours after irradiation. The histological changes were scored following the detection of edema or disarray severity. TUNEL-positive cells exhibiting apoptotic morphology were counted in irradiated region. Results: Cortical edema and disarray were initially showed at 4 or 8 hour and almost all defined at 24 hour after irradiation. And the injury was wedge shape. TUNEL-positive cells were minimal at 8 hour after irradiation as the number of positive cells were $2.6{\pm}5.27$(n=5) after 2Gy, and $0.8{\pm}0.84$(n=5) after 5Gy. But, the number of apoptotic cells were increased markedly to $60{\pm}6.24$ at 12 hour after 2Gy and to $104{\pm}19.7$ at 24 hour after 5Gy. Conclusion: There were prominent morphologic changes immediately after ${\gamma}$-irradiation. And, apoptosis was increased according to the time period. These findings implicate that brain irradiation induces rapid apoptotic change, which may play an important role in the pathogenesis of radiation-induced pathologic conditions.

국소적으로 진행된 갑상선 유두암에서의 방사선치료의 역할 (The Role of External Irradiation for the Locally Advanced Papillary Thyroid Cancers)

  • 목적: 국소적으로 진행된 갑상선 유두암환자에서 수술 후 방사선치료의 성적을 분석하고, 수술과 방사선 동위원소치료를 받은 환자의 치료성적과 비교하여 방사선치료의 효과를 알아보고자 하였다. 대상 및 방법: 본 연구는 1981년 8월부터 1997년 9월까지 국소적으로 진행된 갑상선 유두암(capsular Invasion, extracapsular Invasion or LN metastasis) 환자 91명을 대상으로 하였다. 방사선치료군(23명)의 연령분포는 18세부터 71세였고, 평균연령은 44세였으며, 여자 21명, 남자 2명였다. 원발병소는 주변조직의 침습이 있는 경우(capsular invasion, extracapsular extension)가 22명, 림프절 전이는 16명에서 있었고, 원격전이는 2명에서 있었다. 모든 환자에서 근치적 수술를 시행하였고, 1-125치료는 12명에서 시행하였다. 방사선치료는 Co-60 과 4 MV X-ray를 이용하여 일일선량 $1.8\~2.0$ Gy씩 주 5회 치료하였고 원발병소에 $50\~70$ Gy의 방사선을 조사하였다. 수술과 방사선 동위원소치료군(68명)의 연령분포는 10세부터 74세였고, 평균연령은 43세였으며, 여자 51명, 남자 17명였다. 원발병소는 주변조직의 침습이 있는 경우 51명, 림프절 전이는 45명에서 있었고, 원격전이는 없었다. 모든 환자에서 근치적 수술과 방사선 동위원소치료를 시행하였다. 전체환자의 평균 추적기간은 55.7개월이었다. 결과: 방사선치료군은 국소재발이 2명, 원격전이가 2명(폐전이, 다발성 골전이)에서 있었고, 5년 국소제어율은 $95.2\%$였고, 수술과 동위원소 치료군은 국소재발이 16명, 원격전이가 4명(폐전이) 에서 있었으며, 5년 국소제어율은 $67.5\%$였다. 결론: 국소적으로 진행된 갑상선 유두암 환자에서 방사선치료는 국소재발을 감소시킬 수 있는 효과적인 치료 방법이 될 수 있음을 확인할 수 있었다.

비인강암의 강내조사법 (Intracavitary Irradiation of Carcinoma of the Nasopharynx)

  • 비인강암의 강내조사법은 외부조사후 추가조사법으로서, 또한 재발병소에 비교적 용이하게 재조사할 수 있는 장점이 있으며 비인강암의 국소치유율을 상승시킬 수 있다. 저자는 비인강암 환자에게 추가조사법으로서 또한 재발된 예에서 비인강암의 강내조사를 시행하여 보고하는 바이다.
