Local Anesthetics for Dental Procedure (치과시술에 사용되는 국소마취제)

  • Local pain management is the most critical aspect of patient care in dentistry. Local anesthesia is a reversible blockade of nerve conduction in an applied area that produces loss of sensation. The chemical agents used to produce local anesthesia stabilize neuronal membranes by inhibiting the ionic fluxes required for the propagation of neural impulses. Proper local anesthesia permits the dental surgeon to perform the necessary surgical procedure in a careful, gentle fashion that will be less stressful for both the operator and the patient. The improvements in agents for local anesthesia are probably the most significant advances that have occurred in dental science. Today's anesthetics are safe, effective, and can be administered with insignificant soft tissue damage and minimal concerns for allergic reactions. This article reviews the widely used local anesthetic agents for obtaining local anesthesia, and also discusses some frequently seen complications.

Efficacy of dental local anesthetics: A review

  • The objective of this review was to investigate the efficacy of dental local anesthetics, as it is well known among clinicians that local anesthesia may be challenging in some circumstances. Therefore, the focus of this review was on the efficacy of the products used in dental local anesthesia. In a Pubmed database literature search conducted, a total of 8646 articles were found to be related to dental local anesthetics. After having applied the inclusion criteria (human research, performed in the last 10 years, written in English language, and focus on dental local anesthetics) and having assessed the quality of the papers, 30 were deemed eligible for inclusion in this review. The conclusion of this review is that none of the dental local anesthetic amides provide 100% anesthesia. The problem appears to be more pronounced when mandibular teeth are attempted to be anaesthetized and especially if there is irreversible pulpitis involved. The authors conclude that this finding suggest exploration of more efficient techniques to administer dental local anesthesia, especially in the mandible, to establish a 100% efficacy, is needed.

Analysis of Importance and Satisfaction of Quality Attribute, Popularization and Tourism Commercialization Strategy of Pohang Local Food (포항 향토음식의 품질속성과 대중화 및 관광상품화 방안의 중요도와 만족도 분석연구)

  • This study was conducted to examine the importance and satisfaction of quality attribute, popularization and tourism commercialization of Pohang local food. The purpose of this study was to provide useful information for establishing efficient marketing direction that serves the popularization and the activation of the Pohang local food. On the prospects and improvement of Pohang local foods and its use, 86.1% of subjects stated that the status quo was being maintained or only a portion of the foods was being transmitted. On the dissemination of local foods as world foods, subjects stated 'cooking methods' and 'simplified cooking methods' were required. In the IPA result of Pohang local food quality attributes, it is important to continuously keep 'health', 'nutrition', 'hygiene', 'preference (taste)', 'freshness', and 'credibility toward the ingredients', while 'service' needs to do intensive care and operation. The importance and satisfaction of attributes regarding plans for popularizing local foods and commercializing tourism differed, with importance and satisfaction on average being high, although attributes on plans for popularizing local foods and commercializing tourism was lower than the expectation of customers. Such results mean that residents have high expectations for Pohang local foods, on the other hand, the quality of local foods fails to satisfy the expectations of residents. Therefore, it is important to improve the quality of local foods overall.

The Effects of Local Anesthetic Agent and Epinephrine on Blood Flow of Femoral Artery in Rabbit (가토에서 국소 마취제와 Epinephrine이 대퇴동맥이 혈류에 미치는 영향)

  • Many surgeons and anesthesiologists prefer using vasoconstrictor mixed with local anesthetic agent to reduce the incidence of side effects and prolong the duration of analgesia because most local anesthetic agents, except cocaine, were believed to possess vasodilating effect. However, some investigators recently reported vasoconstricting effect of local anesthetic agents. There is still controversy on the vasoactive effect of local anesthetic agents. So this study is aimed to clarify the vasoactive effect of local anesthetics in the animal model resembling clinical settings. Rabbits were anesthesized with ketamine and haloghane, and respirations were controlled with Harvard animal ventilator. Lidocaine (0.5%, 1.0%, 1.5%) and bupivacaine (0.125%, 0.25% and 0.5%) with or without 1:100,000 epinephrine were subdermaly injected on the femoral bupivacaine of the femoral artery were measured with Doppler flow meter in vivo. The mean arterial pressure, pulse rate, arterial blood gases, pH and level of serum electrolytes were measured at every 2 minute interval for 30 minutes. Results were as follows: 1) There was no significant vasoconstriction with 0.5% lidocaine and 0.125% bupivacaine. 2) Statistically significant (p<0.05) vasodilations were observed with lidocaine (1.0~2.0%) and bupivacaine (0.25~0.5%). 3) There were no changes on the duration of vasodilation induced by local anesthetic agents of various concentrations. 4) Onset of vasodilation induced by local anesthetic agents of high concentration were faster than that of lower concentrations. 5) In the mixed injection group of epinephrine and local anesthetic agent, the vasoconstriction induced by epinephrine was completely reversed by local anesthetics, approximately 5 minutes later. In conclusion, local anesthetic agents at dose exceeding 1.0% lidocaine and 0.25% bupivacaine increase local blood flow significantly in animal study in vivo which is applicable in human clinical settings. The increase blood flow may be due to dilatation of blood vessel. Further study on the analysis of association between amount of absorbed local anesthetics in blood vessels and dilatation of blood vessels is needed.

A Study on the Rerationships Between the Local Autonomy System and the Environmental Management by the Local Government in Korea - Especially on the Upper-local Governments - (지방자치제도와 지방자치단체 환경관리체계와의 관계에 관한 연구 - 광역자치단체의 환경조례를 중심으로 -)

  • This study has focused 302 environmental ordinances enacted from 1974 to 2008 by 16 upper-local governments (7 deignated cities and 9 provinces) and the relationships between them and the development of the Korean local autonomy system. The ordinances have been categorized into three types, the voluntary ones without any obligatory stipulations in laws, the optional ones based on laws, and the law based obligatory ones, which have been examined the enactment years and the contents of them. The local ordinances have been enacted in four periods with a few characteristics, which are "the pre-local-autonomy period (1974-1991)", "the introductory-local-autonomy period (1991-1995)", "the local-autonomy-developing period (1995-1999)", and "the mature-local-autonomy period (after 2000)", along the local autonomy development. 57 ordinances were enacted in the first period, 20 enacted in the second period, 46 enacted in the third period and 179 ordinances in the fourth period. The obligatory ordinances were the most in the first period and in the second period, while the voluntary ordinances were enacted most in the third period. 7 designated cities have pretty more ordinances, 160 in all, than 9 provinces, 142. The tendency to enact earlier in the metropolitan area, SeoulCity, IncheonCity and Gyeonggi province, than the others can be seen. It can be said that the establishment of the local autonomy in Korea had accelerated the development of the local environmental management.

  • Purpose: This study aims to explore the significance and potential strategies for revitalizing local currency systems. Designed for use within specific regions or communities, local currencies play a crucial role in supporting and revitalizing local economies. By promoting local consumption and supporting local businesses, local currencies contribute to economic growth and job creation at the community level. Research design, data and methodology: The research methodology involves a comprehensive review of existing literature and various case studies to analyze the effectiveness and sustainability of local currencies. The literature review investigates the theoretical background and diverse implementation cases of local currencies, while the case studies identify the success factors and challenges faced by actual local currency initiatives. Results: This paper confirms that local currencies contribute to economic growth and job creation at the community level by promoting local consumption and supporting local businesses. Additionally, this study investigates the economic, social, and environmental impacts of local currencies. Conclusion: In conclusion, this study analyzes the challenges faced by local currency systems and proposes strategies to enhance their effectiveness and sustainability. These strategies are expected to provide practical assistance in the development and implementation of local currencies, contributing to the revitalization of local economies and the sustainable development of local communities.

An Efficient Fault Tolerance Protocol with Backup Foreign Agents in a Hierarchical Local Registration Mobile IP

  • A Mobile IP allows IP hosts to move between different networks without changing their IP addresses. Mobile IP systems supporting local registration were introduced to reduce the number of times a home registration with the remotely located home agent was needed. The local registration Mobile IP scheme enhanced performance by processing registration requests of mobile nodes at a local agent. The local registration approach may affect other aspects of the Mobile IP systems such as fault tolerance. In this paper, we briefly review previous solutions for supporting fault tolerance in local registration Mobile IP systems and propose a fault tolerance protocol with a backup foreign agent in a hierarchical local registration mobile IP to enhance the efficiency of such systems against foreign agent failures. We also describe the specification of the proposed protocol using LOTOS and perform its validation using MiniLite. Finally, we analyze the performance of our proposed fault tolerance protocol through simulation.

Distributions of Local Supply and Exhaust Effectiveness according to Room Airflow Patterns

  • A pulsed tracer gas technique is applied to measure the distributions of local mean age and local mean residual-life-time of air in a half-scale experimental chamber, The airflow patterns in the chamber are visualized by a Helium bubble generator for three different exhaust locations. A supply slot is located at the top of a right wall, and an exhaust slot is at either bottom-left (Case 1), bottom-right (Case 2), or top-left (Case 3) location. Results show that the distributions of local mean age and local mean residual-life-time are different from each other, but both of them are closely related to the airflow pattern in the space. Included are discussions on explaining the variations of overall room ventilation effectiveness depending upon airflow rates for three different supply-exhaust configurations.

  • This paper proposes an improved image segmentation algorithm by the watershed algorithm based on the local adaptive threshold on local minima search and the fixing threshold on label allocation. The previous watershed algorithm generates the problem of over-segmentation. The over-segmentation makes the boundary in the inaccuracy region by occurring around the object. In order to solve those problems we quantize the input color image by the vector quantization, remove noise and find the gradient image. We sorted local minima applying the local adaptive threshold on local minima search of the input color image. The simulation results show that the proposed algorithm controls over-segmentation and makes the fine boundary around segmented region applying the fixing threshold based on sorted local minima on label allocation.

LOCAL COLLISION SIMULATION OF AN SC WALL USING ENERGY ABSORBING STEEL

  • This study evaluates the local damage of a turbine in an auxiliary building of a nuclear power plant due to an external impact by using the LS-DYNA finite element program. The wall of the auxiliary building is SC structure and the material of the SC wall plate is high manganese steel, which has superior ductility and energy absorbance compared to the ordinary steel used for other SC wall plates. The effects of the material of the wall, collision speed, and angle on the magnitude of the local damage were evaluated by local collision analysis. The analysis revealed that the SC wall made of manganese steel had significantly less damage than the SC wall made of ordinary steel. In conclusion, an SC wall made of manganese steel can have higher effective resistance than an SC wall made of ordinary steel against the local collision of an airplane engine or against a turbine impact.
