Objectives: A large number of patients require psychiatric therapy. We attempted to determine the present situation regarding psychotherapy and neuropsychological tests for dementia in Korean medicine for the benefit of the Health Insurance Review and Assessment Service (HIRAS). The aim of this study was to aware of the current status about psychotherapy and neuropsychological test for dementia in Korean medicine. Methods: We searched the medical practice records for psychotherapy and neuropsychological tests in oriental neuropsychiatry between 2009 and 2013 using the Health Insurance Review and Assessment Service (HIRAS) database. The search categories were: IJeongByunGi (Medical practice code:59001), JiUnGoRoen (59002), Kyungjapyungji (59003), OhJiSangSeung (59004), neuropsychological test for dementia (29005). Results: 1. The number of patients treated with Korean Medical Psychotherapy increased annually by 151%. The total number of patients treated with Korean Medical Psychotherapy was 4,289 in 2013. 2. The total cost for patients treated with Korean medical Psychotherapy in the public health medical insurance budget was 268,032,000 won in 2013. The average medical cost for one therapy was 17,000 won in 2013. 3. The number of patients in local clinics is increasing faster than the number in Korean medical hospitals. 4. The age group between 20~30 years of age, for both men and women, is the group with the greatest density in Korean Medical Psychotherapy. 5. Neuropsychological Testing for Dementia in Korean Medicine is slowly decreasing. Conclusions: The prevalence of mental illness in Korea is increasing, therefore, the demand for Korean Medical Psychotherapy has increased recently. Authorizing Korean Medical psychiatrists to utilize Korean mental Health resources is essential. This study could be helpful in understanding the current status for the purpose of expanding Korean Medical Psychotherapy.
Events detected by the KIER microearthquake network operated in the Southern Part of Korea for 265 days in 1982~1984 were reviewed, and some of them were identified to be a dynamite explosion from several construction sites. The purpose of the present work is to determine the crustal structure of the Southern Korea using the time-destance data obtained from such explosion seismic records. The time·distance data can be well explained by a crustal model composed of four horizontal layers of which thickness, p and s-wave velocity ($V_p$ and $V_s$) are characterized as follows. 1st layer (surface) ; 0~2km, $V_p=5.5km/sec$, $V_s=3.3km/sec$ 2nd layer (upper crust) ; 2~15km, $V_p=6.0km/sec$, $V_s=3.5km/sec$ 3rd layer (lower crust) ; 15~29km, $V_p=6.6km/sec$, $V_s=3.7km/sec$ 4th layer (upper mantle) ; 29km~ , $V_p=7.7km/sec$, $V_s=4.3km/sec$ The relatively shallow crust·mantle boundary and low $P_n$ velocity compared with the mean values for stable intraplate region are noteworthy. Supposedely, it is responsible for the high heat flow in the South-eastern Korea or an anomalous subterranean mantle. The mean $V_p/V_s$ ratio calculated from the relation between p-wave arrival and s-p arrival times appears to be 1.735 which is nearly equivalent to the elastic medium of ${\lambda}={\mu}$. However, the ratio tends to be slightly larger with the depth. The ratio is rather high compared with that of the adjacent Japanese Island, and the fact suggests that the underlying crust and upper mantle in this region are more ductile and hence the earthquake occurrences are apt to be interrupted. As an alternative curstal model, a seismic velocity structure in which velocities are successively increased with the depth is also proposed by the inversion of the time·distance data. With the velocity profile, it is possible to calculate a travel time table which is appropriate to determine the earthquake parameters for the local events.

Objective : To evaluate the incidence of postsurgical sensory complications in patients with scalp masses and classify the locations of them from a surgical standpoint according to anatomical considerations. Methods : A total of 121 patients who underwent surgery for scalp mass were included in this study. The authors reviewed medical records and preoperative radiologic images. We investigated the complications related to sensory changes after procedure. Enrolled patients have been divided into three groups. Group A included patients with tumors above the superior nuchal line (SNL), Group B with tumors within the trapezius muscle area and patients who had tumors on the lateral trapezius muscle area were assigned to Group C. We compared the incidence related to postoperative sensory complications and summarized their additional treatments for these with clinical outcome. Results : There were 12 patients (10%) with sensory complications related on the mass excision site (Group A : 1 patient, Group B : 2 patients, Group C : 9 patients). Six patients were affected with lesser occipital nerve (LON), 2 patients on greater occipital nerve (GON) and 4 patients on GON and LON. Over 6 months after surgery, two of the twelve patients with sensory complications did not have complete recovered pain in spite of proper medications and local chemical neurolysis with 1.0% lidocaine and dexamethasone. Conclusion : Occipital neuropathy should be considered as a complication related excision of scalp mass. The sensory complications are more frequent in Group C because of the anatomical characteristics of the occipital nerves and there were no statistical difference for other variables.

Purpose: The living standards vary between the urban and rural areas in Korea. This study aims to compare the characteristics of acute poisoning victims in urban and rural areas. Methods: A retrospective study was conducted over a period of 2 years from 2008 to 2009. The study group included adults over 19 year old with acute poisoning and who were later were admitted to the local emergency medical center located in Daegu, Korea. The exclusion criteria were 1) the victims of adverse effects of therapeutic doses of drugs, 2) the victims with chronic exposure and 3) the victims who were missing data in their emergency medical records. We divided the victims into the adult group (19-64 years old) and the old group (over 65 years old). Results: There were 569 acute poisoning victims during the study period, and they constituted 1.11% of the total ED visits (51,199). Four hundred seventy six patients were enrolled in this study. Out of the 359 acute poisoning victims, 252 victims were from urban areas and 107 victims were from rural areas. They showed statistical differences for gender, ED access, transport, toxins and the time to the ED. In the old group, 61 victims out of 117 were from urban areas and the remaining 56 victims were from rural areas. They showed statistical differences for gender, ED access, toxins and transport. Conclusion: Through the clinical comparison between the acute poisoning victims of urban and rural areas, we exposed the clinical differences between the urban and rural areas, and we concluded that prevention and education for acute poisoning should be generated differently between the two groups.
Earthquake records written in the history documents such as “SeungJeongWon IlKi” and “Chosun Wangjo Shilok” for the earthquakes occurred around the offshore near Yangyang and Samcheok area in June, 1681, have been analyzed to estimate the intensity distribution and epicenters as well as magnitudes. Three consecutive earthquakes were analysed, which occurred near those area on June 12, 17, and 26, 1681. The Main-shock occurred on June 12, 1681 was felt in almost every provinces of Korea while after-shocks occurred on June 17 and 26, 1681, were felt in several provinces restrictively. Although the main-shock was felt in the entire provinces, the real damage had been concentrated in Kangwon province. A block of rock collapsed to fall due to main-shock in the mountain near Samcheok. No other man casualties were reported except a death due to being thrown off a horse by ground shaking in Seoul. Local micro-tsunami seems to be generated at the offshore of Samcheok due to the main-shock. The main-shock seems to be occurred in the offshore between Yangyang and Samcheok (38.0oN and 129.0oE) , the epicenter intensity and magnitude of which is over 8 in MMI and 6½ in Ms, respectively.

Background: Prophylactic antibiotics are commonly used in craniofacial surgeries. Despite the low risk of surgical site infection after nasal surgery, a lack of consensus regarding the use of antibiotic prophylaxis in the closed reduction of nasal bone fractures has led to inappropriate prescribing patterns. Through this study, we aimed to investigate the status of prophylactic antibiotic use in closed reductions of nasal bone fractures in Korea. Methods: This retrospective cohort study was conducted using data from the National Health Insurance Service-National Sample Cohort of Korea from 2005 to 2015. We analyzed the medical records of patients who underwent closed reduction of nasal bone fractures. The sex, age, region of residence, comorbidities, and socioeconomic variables of the patients were collected from the database. Factors that affect the prescription of perioperative antibiotics were evaluated using multivariate logistic regression analysis. Results: A total of 3,678 patients (mean±standard deviation of age, 28.7±14.9 years; 2,850 men [77.5%]; 828 women [22.5%]) were included in this study. The rate of antibiotic prescription during the perioperative period was 51.4%. Approximately 68.8% of prescriptions were written for patients who had received general anesthesia. The odds of perioperative prophylactic antibiotic use were significantly higher in patients who received general anesthesia than who received local anesthesia (odds ratio, 1.59). No difference was found in terms of patient age and physician specialty. Second-generation cephalosporins were the most commonly prescribed antibiotic (45.3%), followed by third- and first-generation cephalosporins (20.3% and 18.8%, respectively). In contrast, lincomycin derivatives and aminoglycosides were not prescribed. Conclusion: The findings of this study showed that there was a wide variety of perioperative antibiotic prescription patterns used in nasal bone surgeries. Evidence-based guidance regarding the prescribing of antimicrobial agents for the closed reduction of nasal bone fractures should be considered in future research.
Purpose: Ingrown toenails are a common disease in the adolescent period and the treatments could be more conservative for this early stage of the disease. This study is a case series on the results of a gutter splint for an adolescent ingrown toe nail as a simple, comfortable treatment method. Materials and Methods: From 2011 to 2018, 22 patients (mean age, 12.2 years; range, 8~15 years) with ingrown toenails were treated with a gutter splint. There were 16 boys and 6 girls with 7 patients on both great toes, and additional 4 both corners of a nail, giving a total of 33 splints. Flat, plastic straws and glue or suture were used to protect the nail corners under local anesthesia. The medical records were reviewed retrospectively and phone calls were made to obtain the long-term results. Results: Fifteen splints were fixed with a suture and the other 18 splints were fixed with glue. There were 9 cases of recurrence out of 33 gutter splints, 8 out of 15 sutured splints and 1 out of 18 glued splints (p=0.010). There was no gender (p=0.383) or age (p=0.305) difference in the number of recurrences. Conclusion: For growing people, ingrown nails can be cured easily by conservative treatment for a transiently shortened or broken toenail. The glued gutter splint had a reasonable success rate as a first line treatment.

Background Local symptoms and signs of snake envenomation mimic the clinical features of compartment syndrome. It is important to measure the intracompartmental pressure to diagnose compartment syndrome. In this study, we present our experiences of confirming compartment syndrome and performing fasciotomy in snakebite patients based on high intracompartmental pressure findings. Methods The medical records of patients who visited the trauma center of Wonju Severance Christian Hospital from January 2010 to December 2015 for the management of venomous snakebite were retrospectively reviewed. Starting in 2014, fasciotomy was performed in patients with an intracompartmental pressure of more than 40 mmHg in addition to the clinical symptoms of compartment syndrome. Results A total of 158 patients with snakebite came to the hospital within 48 hours for treatment. Most patients (110 patients) were bitten at the upper extremities (69.6%). Since 2014, 33 out of 59 patients were suspected to have compartment syndrome, and their intracompartmental pressures were measured. Seventeen of those patients had a high intracompartmental pressure (average, 49.6 mmHg; range, 37-88 mmHg), and fasciotomy was performed. Conclusions In this study, as many as 10.8% of all cases were in need of fasciotomy when compartment syndrome was diagnosed by measuring the intracompartmental pressure. Previously, it was reported that fasciotomy was not required in many cases of compartment syndrome originating from snakebite. However, some patients may develop very severe compartment syndrome, requiring fasciotomy.
Purpose: This study aims to design and verify an onsite EEWS that extracts the P-wave from a single seismic station and deduce the PGV. Method: The P-wave properties of Pd, Pv, and Pa were calculated by using 12 seismic waveform data extracted from historic seismic records in Korea, and the PGVs were computed using empirical equation on the P properties - PGV relationship and compared with the observed values. Results: Comparison of the observed and estimated PGVs within the alarm level shows the error rate of 86.7% as minimum. By reducing the PTW to 2 seconds, the alarm time can be shortened by 1 second and the seismic blind zone near the epicenter can be shortened by 6 Km. Conclusion: Through this study, we confirmed the availability of the on-site EEWS in Korea. For practical use, it is necessary to develop regression formula and algorithm reflect local effect in Korea by increasing the number of seismic waveform data through continuous observation, and to eliminate the noise from the site.
