  • PURPOSE : The purpose of this study was to examine the oral health behaviors of child users of local children's centers before and after receiving oral health education in an effort to develop a well-organized oral health education program. METHOD : The subjects in this study were the elementary school students who used 13 different local children's centers in the city of Busan. The questionnaire was intended for children and consisted of 31 items, which were four about eating food, three about washing hands, two about smoking, five about toothbrushing and dental clinic visit, five about tooth damage and dental pain, seven about knowledge and awareness of dental health and six about dental health attitude. RESULT : 1. The rate of the children who replied they didn't brush their teeth on the previous day stood at 5.8 percent before the program, and this rate rose to 13.0 percent after that. The differences were significant(p=0.026). 2. The rate of the former after the program stood at 61 percent, and that of the latter stood at 39 percent. The differences were statistically significant(p=0.019). CONCLUSION : The oral health behaviors were investigated before and after oral health education was provided, and this education was found not to be sufficient enough to change their oral health care. So it seems necessary to increase the frequency of oral health education by providing it twice a year. In addition, oral health education programs geared toward teachers in local children's centers who spend time with children should additionally be strengthened to offer more education to these children.

The survey was conducted from September 10 to October 5, 2015 using a structured questionnaire. Each of the 254 public health centers were sent two questionnaires but I only received 348 and 339 were used for the final analysis. As a result of this study, the all subjects were showed the average level of efficiency, autonomy, accountability, and the support of the Ministry of Health and Welfare, and the related organizations. Also, the evaluation of all the variables were positive when there was a dedicated department for integrated health promotion program within the health center and when the proportion of essential program was high. However, after the Integrated Health Promotion Program was included in the Health centers, the workload has increased and the implementation of the essential program does not fully reflect the regional health problems or local conditions. Based on these results, it is necessary to find a plan for the successful operation of the Integrated Health Promotion Program.

The results of this study are as follows. 1) The health education system in Korea has only the head. But it does not have the trunk and the limbs that it can move with. 2) Health educator should have the essential work that is the planning and coordinating work of intersectoral health education programs. They should also have the trust works from other sectors. 3) The proposition in the health education policy is as follows: First, the department or section of health education should be made newly in the public health organization. Second, at the level of province(Do) and county(Gun), the health educator should be stationed. Third, most training courses of health care members should involve health education subjects. Fourth, the health center at the level of county(Gun) should have a minimum material and audio-visual equipment of health education. Fifth, regular health education should be put into practice through local broadcast or CATV etc.. Sixth, school health education should be consolidated. Seventh, village health worker(nurse) should be stationed at the level of health center, so that he(she) can work as health educator. 4) The ultimate model of health education system is that of Fig. 5. But it is impossible to change the system synoptically. At first health educator should be stationed at health center. And then the system should be gradually organized.

  • Objectives: The distribution of hospitals in Korea is unbalanced in terms of accessibility. Many local public health centers (PHCs) exempt out-of-pocket payments (OOPs) based on local government laws to increase coverage. However, this varies across administrative regions, as many make this exemption for the elderly, while others do not. This study aimed to evaluate the effects of the OOP exemption at local PHCs among elderly individuals. Methods: This study used online data on Korean national law to gather information on individual local governments' regulations regarding OOP exemptions. Individual-level data were gathered from the 2018 Community Health Survey and regional-level data from public online sources. Results: The study analyzed 132 regions and 44 918 elderly people. A statistical analysis of rate differences and 2-level multiple logistic regression were carried out. The rate difference according to whether elderly individuals resided in areas with the OOP exemption was 1.97%p (95% confidence interval [CI], 1.07 to 2.88) for PHC utilization, 1.37%p (95% CI, 0.67 to 2.08) for hypertension treatment, and 2.19%p (95% CI, 0.63 to 3.74) for diabetes treatment. The regression analysis showed that OOP exemption had an effect on hypertension treatment, with a fixed-effect odds ratio of 1.25 (95% CI, 1.05 to 1.48). Conclusions: The OOP exemption at PHCs can affect medical utilization in Korea, especially for hypertension treatment. The OOP exemption should be expanded to improve healthcare utilization in Korea.
