LC-MS를 이용한 감귤류의 수확 후 처리 살균제 분석 (Determination of Post-harvest Fungicide in Citrus Fruits Using LC-MS)

  • 국내 유통되는 감귤류 총 100건을 대상으로 LC-MS을 이용하여 감귤류에 대한 수확 후 처리 농약 잔류량을 조사하였다. 보다 효율적인 농약 분석을 위해 LC-MS를 이용하여 직선성, 정확성, 정밀성, 검출한계 및 정량한계로 분석법의 유효성 검증을 하였다. 상관계수($R^2$)는 0.999이상의 우수한 직선성을 보였고, 농약의 회수율은 80.2-98.3%였고, RSD는 0.48-3.93%로 우수한 재현성을 나타냈다. 검출한계는 0.002-0.008 mg/kg였고, 정량한계는 0.006-0.027 mg/kg였다. 확립된 분석법에 의해 분석한 결과 최고 잔류 수준은 carbendazim은 0.541 mg/kg, thiabendazole은 0.958 mg/kg, imazalil은 0.721 mg/kg, azoxystrobin은 0.052 mg/kg였다. 본 연구결과에서 나타난 수확 후 처리 살균제는 잔류허용기준(MRL) 이내에서 안전하게 관리되고 있으나, 농약의 잔류성을 줄이기 위한 지속적인 모니터링이 필요하다.

인구감소에 따른 농촌지역 거점형 중심지 공공시설 입지 및 운영방안 개선에 관한 실증적 연구 (An Empirical Study on the Location and Management Plan of Public Facilities in the Rural Center Place by Declining Population)

  • This study investigated the public use of Myeon Location in Jeollanam - do where the population is at risk of declining. And discussed the rational provision and coordination of public facilities as a guarantee of fundamental rights. As a result of the study, the rational supply and adjustment directions of public facilities are as follows. First, there are problems such as high availability of management, finance, shopping, and health facilities, lack of culture, leisure, community facilities, and weakening of center function. Therefore, in the continuing declining population situation, face management, safety and health facilities should be upgraded to multifunctional facilities, and utilization efficiency and integration should be promoted. In addition, town center rehabilitation should be centered on multifunctional facilities. In addition, elementary schools will have to utilize and preserve them to take advantage of local cultural values as nostalgia storing place that provide incentives for young people and elementary school children in the future. Second, in order to cope with population decline, public facilities need to strengthen public services through complexity and multifunctionality, wide-area operation with neighboring areas, and user-centered operation management.

  • This survey was conducted to evaluate not only the present status of health center directors-their personal histories, their will to private practice in the future, their responses to governmental policies, -but also the distribution of doctorless myons, budget and subsidy, and director's opinions to the enhancement of health center activities. This survey questioned 116 health center directors and 16 health personnel from August to October of 1970 and obtained the following results; 1) The average ages of directors of kun, city, and total health centers were $43.2{\pm}7.8,\;42.1{\pm}7.7,\;and\;42.9{\pm}10.3$ respectively. 2) The average family sizes of directors of kun, city, and total health centers were $5.6{\pm}2.7,\;5.6{\pm}2.1,\;and\;5.6{\pm}2.6$ respectively. 3) Directors holding M. D. degrees were 79.3%, those holding qualified M. D. degrees ('approved director') were 20.7%. 4) M. P. H., M. S., and Ph. D. holders were 6.0%, 6.1%, and 4.3% respectively. 5) The average duration of present directorship in kun and city were 30.2 months and 20.4 months respectively. 6) The majority of directors had been employed in related fields before assuming current position : directorship at other health center 26.7%, army 22.4%, health subcenter 21.6%, private practice 19.0%. 7) Average length of directorship is 41.8 months. Average length of public health career, including health subcenter and present position, is 56.5 months. 8) Both rural and urban experience in health centers for regular directors is 16.3% and for approved directors, 12,5%. A total of 15.5% of all survey directors had experience in both rural and urban health center. 9) A total of 70.7% of health center directorships were staffed by local doctors. 10) Nearly 40% wanted to quit the directorships within 3 years and 60.3% had already experienced private practice. 11) Of the regular directors 17.4% felt strongly about devoting their lives to public health fields, but only 4.1% of the approved approved directors felt so. 12) There wire 432 doctorless myons among 996 respondent myons and 4.5 doctorless myons per kun. 13) The percentage of doctorless myon by Province are as follows, Cholla buk-do 57.2%, Cholla nam-de 55.0%, Kyungsang nam-do 52.0%, Kyungsang buk-do 49.7%, Chungchong but-do 42.4%, Kyonggi-do 32.9%. Cheju-do 30.8%, Kangwon-do 25.8%. 14) Two thirds of health critters have experienced the abscence of the director for a certain period since 1966 and the average span of the abscence was 18.2 months. 15) The percentage of doctorless myons increased proportionally with the span of the director's abscence. 16) The average budgets of health centers, kun, city and ku, were $W15.03\;million{\pm}W4.5\;million,\;W22.03\;million{\pm}W17.80\;million,\;W13.10\;million{\pm}W7.9\;million$ respectively. 17) Chunju city had the highest health budget per capita(W344) while Pusan Seo ku had the lowest(W19). 18) Director's medical subsidies are W30,000-50,000 in kun, and roughly W20,000 in city. 19) The older of priority in health center activities is T.B. control(31.1%), Family Planning and M. C. H.(28.0%), prevention of acute communicable disease and endemic disease (18.2%) and clinical care of patients(14.3%). 20) Nearly 32% opposed in principle the governmental policy of prohibiting medical doctors from going abroad. 21) Suggestions for immediate enhancing the position of director of health centers and subcenters: (1) Raise the base subsidy (48.2%), (2) Provide more opportunities for promotion (20.7%), (3) Exemption from army services(12.1%), (4) Full scholarship to medical students for this purpose only (7.8%). 22) A newly established medical school was opposed by 56.9% of the directors, however 33.6% of them approved. 23) Pertaining to the division of labor in Medicine and Pharmacy, the largest portion (31.9%) urged the immediate partial division of antibiotics and some addictive drugs to be given only by prescription. 24) More than half wanted a W70,000 level for the director's medical subsidies, white 36.2% stated W50,000. 25) Urgently needed skills in the kun are clinical pathologist (38.6%) and doctor (health center director) (25.5%); while in the city nurse (37.1%), doctors(clinical)(31.4%) and health educators(14.4%) are needed. 26) Essential treatment for the better health center administration; raising the base subsidy (22.7%), obtaining the power of personal management (19.3%) and the establishment of a Board of Health (14.3%). etc.

독립형 호스피스 센터 모델 개발에 관한 연구 (A Study on the Development of an Independent Hospice Center Model)

  • The study was aimed at developing an independent hospice center model that would be best suited for Korea based on a literature review and the current status of local and international hospices. For the study, five local and six international hospice organizations were surveyed. Components of the hospice center model include philosophy, purpose, resources (workers, facilities, and equipment), allocation of resources, management, financial support and hospice team service. The following is a summary of the developed model: Philosophies for the hospice center were set as follows: based on the dignity of human life and humanism, help patients spend the rest of their days in a meaningful way and accept life positively. On the staff side, to pursue a team-oriented holistic approach to improve comfort and quality of life for terminally ill persons and their families. The hospice center should have 20 beds with single, two, and four bed rooms. The center should employ, either on a part-time or full-time basis, a center director, nurses, doctors, chaplains, social workers, pharmacists, dieticians, therapists, and volunteers. In addition, it will need an administrative staff, facility managers and nurses aides. The hospice should also be equipped with facilities for patients, their families, and team members, furnished with equipment and goods at the same level of a hospital. represented by a center director who reports to a board and an advisory committee. Also, the center director administers a steering committee and five departments, namely, Administration, Nursing Service, Social Welfare, Religious Services, and Medical Service. Furthermore, the center should be able to utilize a direct and support delivery systems. The direct delivery system allows the hospice center to receive requests from, or transfer patients to, hospitals, clinics, other hospice organizations (by type), public health centers, religious organizations, social welfare organizations, patients, and their guardians. On the other hand, the support delivery system provides a link to outside facilities of various medical suppliers. In terms of management, details were made with regards to personnel management, records, infection control, safety, supplies and quality management. For financial support, some form of medical insurance coverage for hospice services, ways to promote a donation system and fund raising were examined. Hospice team service to be provided by the hospice center was categorized into assessment, physical care, emotional care, spiritual care, bereavement service, medication, education and demonstrations, medical supplies rental, request service, volunteer service, and respite service. Based on the results, the study has drawn up the following suggestions: 1. The proposed model for a hospice center as presented in the study needs to be tested with a pilot project. 2. Studies on criteria for legal approval and license for a hospice center need to be conducted to develop policies. 3. Studies on developing a hospice charge system and hospice standards that meet local conditions in Korea need to be conducted.

일본의 노인건강관리체계 검토를 통한 한국 지역사회노인 예방 건강관리 방안모색 연구 (Development of a Community-based Preventive Health Care Model for the Elderly in Korea through the Evaluation of a Japanese Counterpart)

  • Purpose: Through a thorough examination of the CCSC (Community Comprehensive Support Center) system in Japan, this study suggests a scheme to provide community-based preventive health care services for the elderly in Korea. Methods: The study inquired into the applicability of the Japanese model by reviewing the data related to the CCSC project, aided by both in-depth interviews with staff in the field and consultations with specialists. Results: Rearrangement of the Visiting Health Management Project system is needed to manage the collective or individual visiting care management for frailty prevention of the elderly in communities. The delegated service system for preventive care in the community, including direct management by one of the public health centers, also needs to be reviewed and the application of stricter standards for the selection of the agency or corporation to run the delegated service is necessary. Long-Term Care Insurance, along with national and local grants, is to be considered as a financial resource for the community-based preventive health care model for the elderly. By making active use of education rooms at district offices, senior citizen centers in neighborhoods for the elderly with easy access can be created. The project needs to raise active supports from communities, develop programs which can be absorbed into particular local cultures, and promote the understanding of the preventive project in local communities. The preventive program should focus on first solving the problems of depression, seclusion, and lack of mobility of the elderly. Second, the program should instruct physical self-management for exercise-nutrition-dental maintenance, and third, the program should strengthen the cognitive abilities of the elderly. In addition, it is necessary to systematize and implement counter-plans of the family and community to protect the elderly who has mental and cognitive problems. Finally, by establishing a network of public health welfare resources based upon research on a community level, assessment and planning for the health of the elderly should be one with their family, and comprehensive consultation and recommendations should be provided to the family. Conclusion: Taking into consideration the experience Japan has had with respect to a similar project, it is appropriate to develop and implement a service system which would combine the Visiting Health Management Project system which has already been established and a preventive health care model for the elderly on a community level.

Anterior Cranial Base Reconstruction in Complex Craniomaxillofacial Trauma: An Algorithmic Approach and Single-Surgeon's Experience

  • Management of traumatic skull base fractures and associated complications pose a unique reconstructive challenge. The goals of skull base reconstruction include structural support for the brain and orbit, separation of the central nervous system from the aerodigestive tract, volume to decrease dead space, and restoration of the three-dimensional appearance of the face and cranium with bone and soft tissues. An open bicoronal approach is the most commonly used technique for craniofacial disassembly of the bifrontal region, with evacuation of intracranial hemorrhage and dural repair performed prior to reconstruction. Depending on the defect size and underlying patient and operative factors, reconstruction may involve bony reconstruction using autografts, allografts, or prosthetics in addition to soft tissue reconstruction using vascularized local or distant tissues. The vast majority of traumatic anterior cranial fossa (ACF) injuries resulting in smaller defects of the cranial base itself can be successfully reconstructed using local pedicled pericranial or galeal flaps. Compared with historical nonvascularized ACF reconstructive options, vascularized reconstruction using pericranial and/or galeal flaps has decreased the rate of cerebrospinal fluid (CSF) leak from 25 to 6.5%. We review the existing literature on this uncommon entity and present our case series of n = 6 patients undergoing traumatic reconstruction of the ACF at an urban Level 1 trauma center from 2016 to 2018. There were no postoperative CSF leaks, mucoceles, episodes of meningitis, or deaths during the study follow-up period. In conclusion, use of pericranial, galeal, and free flaps, as indicated, can provide reliable and durable reconstruction of a wide variety of injuries.

제조업 남자 근로자의 심혈관질환 위험요인에 대한 모형 구축 (A Structural Model Development on the Cardiovascular Disease Risk Factors among Male Manufacturing Workers)

  • Purpose: The purpose of this study was to develop and test a structural model on cardiovascular disease risk factors among male manufacturing workers. Methods: Data were collected through questionnaires and health exams from 201 workers in a local electronic company during September 2004. Data analysis was done with SAS 9.1 for descriptive statistics and PC-LISREL 8.54 for covariance structural analysis Results: The overall fit of the hypothetical model to the data was moderate, it was modified by deleting five paths. The modified model had a better fit to the data($x^2=504.23$(p<001, df: 180), $x^2/df=2.80$, GFI=.95, RMR=.07, NFI=.90, PGFI=.64). Health behaviors and psychosocial distress were found to have significant direct effects on the cardiovascular disease risk factors. Self-concept had direct effect on psychosocial distress or health behaviors. Self-concept, work environment, and work condition had direct effect on social support. Work environment had indirect effect on psychosocial distress. Social support had indirect effect on health behaviors. But work environment and work condition were found to have little direct effect on health behaviors, psychosocial distress or cardiovascular disease risk factors. Conclusion: A cardiovascular health promotion program should therefore include psycho-social factors as well as health behavioral determinants in worksites.

여성 독거노인의 사회적 지지가 자기효능감의 매개효과를 통하여 건강증진행위에 미치는 영향 (Effects of Social Support on Health Promotion Behavior through the Mediating Effect of Self-efficacy in Older Women Living Alone)

  • Purpose: Self-efficacy encompass one's belief in one's ability to organize and achieve goals. Previous studies have not adequately examined the mediating role of self-efficacy between social support and health promotion behavior. Therefore, this study explored the mediating role of self-efficacy in the relationship between social support and health promotion behavior among older women living alone. Methods: Participants were 145 older women living alone attending a local welfare center for seniors. They completed the Self-efficacy Scale, Medical Outcome Study Social Support Survey Scale, and Health Promoting Lifestyle Profile II. Data were analyzed using Descriptive statistics, Pearson correlation coefficients, Baron and Kenny's regression analysis and the Sobel test with the SPSS program. Results: The average social support, health promotion behavior, and self-efficacy were not high. Self-efficacy was a partial mediating role in the relationship between social support and health promotion behavior. Social support was positively correlated with self-efficacy (r=.31, p<.001) and with health promotion behavior (r=.43, p<.001), and self-efficacy was positively related with health promotion behavior (r=.39, p<.001). Conclusion: To enhance health promotion behavior in older women who live alone, intervention strategies to increase social support and self-efficacy for these women should be developed.

Forecasting the flap: predictors for pediatric lower extremity trauma reconstruction

  • Background Predicting the need for post-traumatic reconstruction of lower extremity injuries remains a challenge. Due to the larger volume of cases in adults than in children, the majority of the medical literature has focused on adult lower extremity reconstruction. This study evaluates predictive risk factors associated with the need for free flap reconstruction in pediatric patients following lower extremity trauma. Methods An IRB-approved retrospective chart analysis over a 5-year period (January 1, 2012 to December 31, 2017) was performed, including all pediatric patients (<18 years old) diagnosed with one or more lower extremity wounds. Patient demographics, trauma information, and operative information were reviewed. The statistical analysis consisted of univariate and multivariate regression models to identify predictor variables associated with free flap reconstruction. Results In total, 1,821 patients were identified who fit our search criteria, of whom 41 patients (2.25%) required free flap reconstruction, 65 patients (3.57%) required local flap reconstruction, and 19 patients (1.04%) required skin graft reconstruction. We determined that older age (odds ratio [OR], 1.134; P =0.002), all-terrain vehicle accidents (OR, 6.698; P<0.001), and trauma team activation (OR, 2.443; P=0.034) were associated with the need for free flap reconstruction following lower extremity trauma in our pediatric population. Conclusions Our study demonstrates a higher likelihood of free flap reconstruction in older pediatric patients, those involved in all-terrain vehicle accidents, and cases involving activation of the trauma team. This information can be implemented to help develop an early risk calculator that defines the need for complex lower extremity reconstruction in the pediatric population.

양.한방협진에 대한 지역주민의 인식 - 일부 보건소 이용자를 중심으로 - (Perceptions of Residents Visiting Local Health Centers on the Collaborating Care of Korean Traditional Medicine and Western Medicines)

  • Objective : The purpose of this research was to provide basic data for developing the collaborating care of Korean traditional medicine and western medicines by analyzing the perceptions of residents visiting local health centers on the collaborating care. Method : To this end, a self-administrated questionnaire was surveyed to 417 participants from March 10 to March 19, 2005. The questionnaires were regarding medical preferences, effectiveness, co-operative treatment types, and the demographic characteristics of the study population. The main statistical methods employed for analysis were frequency chi-square test analysis, using SPSS system 12.0 software for Windows. Result : First, the perceptions of collaborating care, such as preference and effectiveness, were better for residents who had experienced Korean traditional medicine(p < 0.05). Second, the most favorable collaborating care type was the neuromuscular disease and rehabilitation, and in particular, the preference of the patient who had experienced Korean traditional medicine was much higher than those who had not experienced it(p < 0.05). Third, as for recognizing the future of collaborating care, respondents insisted that collaboration care has to be conducted under evidence-based research. The reasons why collaborating care has not been active were reported as "difference in solving disease problems between oriental medicine and western medicine." The most important role of the Korean traditional medicines in the public sector was to provide specialized service for the elderly and low income households. Conclusion : Most respondents expected the positive effects of the collaborating care and wanted it to develop, particularly for neuromuscular diseases. As for the health promotion program in health centers, it was more popular than the home visiting program for the elderly and preventive rehabilitation for stroke. Now we must plan to balance between the need of the community and the medical provider on collaborating care.
