  • A 18-year-old man had a painless swelling in the right anterior portion of maxilla for 2 years. On radiographic examination, a radiolucent region that was not associated with an unerupted tooth was seen. Small scattered radiopaque foci were seen in the cystic lumen. At second case, a 16-year-old girl had a painless swelling in the anterior portion of maxilla for 3 years. On radiographic examination, a radiolucent region that associated with an unerupted tooth was seen. Multiple scattered radiopaque foci were seen in the radiolucent cystic lumen. With the patient under local anesthesia, well encapsulated tumors were enucleated. The diagnosis made in the pathologist's report was Adenomatoid Odontogenic Tumor, benign lesion often having distinct clinical and radiographic features.

  • Keratocystic odontogenic tumor (KCOT) is a benign odontogenic tumor with a potentially aggressive and infiltrative behavior. KCOT is most commonly occurred in mandible and demonstrate a unilocular, round, oval, scalloped radiolucent area, while large lesions may appear multilocular. An important characteristic of KCOT is its propensity to grow in an antero-posterior direction within medullary cavity of bone causing minimal expansion. Definitive diagnosis relies on histological examination. In this report, a KCOT that had an expansion both buccal and lingual cortical bone is described including its features in computed tomography and ultrasonographic exams. The lesion was removed surgically via an intraoral approach under local anesthesia and histologically reported as a KCOT.

  • Sialolithiasis is the formation of calcific concretions within the ductal system of major or minor salivary glands. The exact nature of sialolith evolution is not known. It may occur at any age but, it is most common in middle-aged adult and rare in childhood. In this paper, a case of sialolithiasis just beneath the mucosa in the anterior portion of the Wharton's duct observed in a 5-year old boy. It was approximately $2.5{\times}5$ mm in size and has no clinical symptoms. Under local anesthesia, it was removed by dilatation of orifice of the duct and pressure on the floor of oral cavity by finger.

  • Purpose: Cutis verticis gyrata(CVG) is a morphologic syndrome, typically occurring on the scalp, characterized by varing development of permanent folds and furrows conferring a corrugated or convoluted appearance which resembles the outer surface of the cerebrum. Cutis verticis gyrata can be categorized into primary and secondary types according to various etiologic factors and associated conditions which cause disease process. Methods: We report a case of essential primary cutis verticis gyrata in a 24-year-old male who did not have any other underlying or associated conditions. Results: After we made an incision to the galea aponeurotica of patient's scalp four times under local anesthesia, we dissected along the subapponeurotic layer and adjusted the flaps each other, and then sutured the flap. Conclusion: After the operation, the clear margin disappeared and it did not recur.

  • During the past 3 years, 15 patients who could not be anesthetized generally because of poor general condition or cachecic state, of 111 patients with empyema, have been treated with modified Eloesser`s operation under the local anesthesia with 2% procaine. There were 13 males and 2 females ranging from 21 years to 61 years of age. The etiology was tuberculosis [6 cases], pyogenic pneumonia [5 cases], lung abscess [1 case], post-trauma [2 cases] and malignancy[1 case]. " The over-all mortality rate was 6.7%[1 case] and cause of its death was poor oral feeding because of post-traumatic psychosis. Modified Eloesser`s operation was performed after closed tube thoracostomy and irrigation with 1% zephanon solution for over 2 weeks. And then the other operation was not performed and all patients except 1 case appeared good progression (Complete healing; 4 cases, Progressive healing 10 cases, death: 1 case).

  • We applied our technique of selective bronchial suctioning (SBS) for the treatment of atelectasis after resection surgery of lung in four patients with refractory atelectasis who were treated successfully. We considered that SBS using hydro-catheter insertion under local anesthesia above fourth tracheal ring is the effective technique for the treatment of refractory atelectasis when conventional respiratory therapy is not effective and a bronchoscopist is not available.

  • From July 1988 to January 1991 six patients, aged 29 to 70 years underwent transfemoral thrombectomy for the treatment of deep vein thrombosis. Preoperative venograms showed thrombi in the following locations: calf veins[five], superficial femoral or popliteal veins [four], common femoral veins[three], and iliac veins[three], Durations of symptoms before admission were from 1 day to 20days. Operations were performed under local anesthesia and all the patients were requested for doing Valsalva maneuver during thrombectomies. All patients were received heparin pre-and postoperatively, which was switched to Coumadin for preventing of rethrombosis. One patient was transferred to other hospital 4 months after operation due to regional reason, and the remained five patients were evaluated with a mean follow-up time of 20 months. There was no evidence of postoperative pulmonary embolism. Three of five patients were clinically asymptomatic. One complained of the heaviness of involved leg in the evening, and the other had discomfort on walking Even though our cases were a few in number, we concluded that thrombectomy is a valuable treatment modality of deep vein thrombosis.

  • Dermoid cysts are benign neoplasm that is derived from both ectoderm and mesoderm. They may be found at various sites of the body, but are extremely rare to occur in the lower neck. We present the case of a 22-year old woman with a midline cystic mass anterior to the cervical trachea. The mass was excised under local anesthesia. Histopathologically the diagnosis was confirmed as a dermoid cyst.

  • Office based injection laryngoplasty for voice and swallow disorders is increasing steadily. In recent years, the application of injection laryngoplasty is extended to inject medicine, such as cidofovir or botox. The reason why office based injection laryngoplasty is increasing in popularity is several potential advantages when compared with other procedures. It can avoid surgical scar and is easily performed with local anesthesia. However, injection can be performed in a variety of settings, depending on several factors, including the goal of the procedure, patient comfort and anatomy, and physician skill. In this article, we describe indication of injection laryngoplasty and discuss about various techniques for procedure.

  • The temporomandibular joint arthrocentesis is indicated the 'closed lock' due to anterior disc displacement without reduction. It can be easily carried out under local anesthesia with little complications. We performed arthrocentesis to the 24 patients, 9 patients who suffered from acute or chronic closed lock with anterior disc displacement, 2 patients from temporomandibular joint dysfunction related to systemic disease and 13 patients from joint dysfunction with pain. We present the common clinical aspect of arthrocentesis that operator can be easily faced with and possible modifications of this method.
