Primary Hepatic Tumors in Children (소아 원발성 간종양)

  • Primary liver tumors are uncommon in childhood, with a relative frequency of 3 % of all childhood tumors. Seventy-three cases of pediatric primary liver tumors operated on at single institution between 1986 and 2002 were reviewed. Malignant tumors included 37 cases of hepatoblastoma, 11 hepatocellular carcinomas, 6 undifferentiated (embryonal) sarcomas, and 1 mixed germ-cell tumor. Benign tumors constitute only 24.6 % of liver tumors, including 7 hemangioendotheliomas, 5 mesenchymal hamartomas, 3 congenital cysts, and one each with focal nodular hyperplasia and hemangioma. The common presenting clinical features were abdominal mass or abdominal distension. Anatomical hepatic resections were carried out in 38 cases, and nonanatomical resections in 34 cases. One patient died of a direct result of hepatic resection (1.4 %). The complication rate was 16.4 %.

Role of Endoscopic Ultrasound in Evaluation of Pancreatic Neuroendocrine Tumors - Report of 22 Cases from a Tertiary Center in Iran

  • Background: The pancreatic neuroendocrine tumor (pNET) is relatively rare and generally felt to follow an indolent course. EUS has an important role in detection of pNET. This is a review of clinical and radiological presentation and pathologic reports of 22 patients with pNET. Patients and methods: In this study we analyzed clinical and radiological presentations and pathologic reports of all relevant cases who were referred to Taleghani hospital for 3 years since 2008. Results: A total of 22 patients 28-74 years old (mean=49) were enrolled between 2008 and 2011. Among the total, 13 (59%) were male, 9 (41%) were female and 16 (72.7%) had functional tumors. The results of CT were negative in 12 (54%) cases but EUS was capable of detecting the lesions in these patients, cysts being found in 4 (19%) patients. Conclusion: EUS is a highly sensitive procedure for the localization of functional pNETs and especially insulinomas. Nonfunctional tumors were detected in more advanced and late stages and cystic lesions were more common in this group.

In Vitro Scolicidal Effects of Salvadora persica Root Extract against Protoscolices of Echinococcus granulosus

  • It has been known that Arak, Salvadora persica, has a number of medicinal properties. We tried to investigate in vitro scolicidal effect of root extracts of this plant against protoscolices from hydatid cysts of Echinococcus granulosus. Protoscolices were aseptically collected from sheep livers containing hydatid cysts. S. persica root extract was used in 10, 30, and 50 mg/ml concentration for 10, 20, and 30 min. The viability of protoscolices was ascertained by 0.1% eosin staining. Scolicidal activity of S. persica extract at a concentration of 10 mg/ml was 36.3%, 50.3%, and 70.8% after 10, 20, and 30 min of exposure, respectively. The scolicidal effect of this extract at a concentration of 30 mg/ml was 52.9%, 86.7%, and 100% after 10, 20, and 30 min of exposure, respectively. S. persica extract at a concentration of 50 mg/ml, meanwhile, killed 81.4%, 100%, and 100% of protoscolices after 10, 20, and 30 min, respectively. Also, the cytotoxic potential of S. persica was assessed on human liver cells (HepG2) using trypan blue exclusion test. No cytotoxic effect was observed on HepG2 cell line. The present study confirmed for the first time that the ethanolic extract of S. persica has high scolicidal power in vitro. However, in vivo effect of this material remains to be studied for treatment of echinococcosis in humans and herbivorous animals.

Sarcocystosis among Wild Captive and Zoo Animals in Malaysia

  • Sarcocystis sp. infection was investigated in 20 necropsied captive wild mammals and 20 birds in 2 petting zoos in Malaysia. The gross post-mortem lesions in mammals showed marbling of the liver with uniform congestion of the intestine, and for birds, there was atrophy of the sternal muscles with hemorrhage and edema of the lungs in 2 birds. Naked eye examination was used for detection of macroscopic sarcocysts, and muscle squash for microscopic type. Only microscopically visible cysts were detected in 8 animals and species identification was not possible. Histological examination of the sections of infected skeletal muscles showed more than 5 sarcocysts in each specimen. No leukocytic infiltration was seen in affected organs. The shape of the cysts was elongated or Circular, and the mean size reached $254{\times}24.5{\mu}m$ and the thickness of the wall up to $2.5{\mu}m$. Two stages were recognized in the cysts, the peripheral metrocytes and large numbers of crescent shaped merozoites. Out of 40 animals examined, 3 mammals and 5 birds were positive (20%). The infection rate was 15% and 25% in mammals and birds, respectively. Regarding the organs, the infection rate was 50% in the skeletal muscles followed by tongue and heart (37.5%), diaphragm (25%), and esophagus (12.5%). Further ultrastructural studies are required to identify the species of Sarcocystis that infect captive wild animals and their possible role in zoonosis.

Statistical Study on the Results of Daehan Life's Mediscreen(Human-dock) (당사(當社) 종합건강진단(綜合健康診斷)센터에서 실시(實施)한 수진자(受診者)의 통계학적관찰(統計學的觀察))

  • During the period from september, 1985 to september, 1986, 1,005 cases(475cases in male, 529 cases in female) of employees and family member were observed for the general physical examination(Human-dock) in Medical Department of Daehan Life Insurance Co. Ltd. The results were as follows. 1) The occurrence of obesity cases were observed as 130 cases(12.9%), and among the 130 cases, 34 cases(26.2%) were male and 96 cases(73.8%) were female. 2) Diabetes mellitus patients were detected as 93 cases(9.3%), and 53 cases(57.1%) were male and 40 cases(43.0%) were female. The most frequent age groups were demonstrated in age of 5th and 6th decades. 3) Hypertension patients were 85 cases(8.5%), and among the 85 cases, 42 cases (49.4%) were male and 43 cases(50.6%) were female. The most frequent age groups were 5th and 6th decades, and complication of hypertensive retinopathy revealed 54 cases(63.5%). 4) Hyperlipidemia cases were observed as 71(7.1%), and 42 cases(59.2%) were male and 29 cases(40.8%) were female. The most frequent age groups were 5th and 6th decades. 5) 69 cases(6.9%) of positive reaction of HBs Ag and 46 cases(4.6%) of abnormal erectrocardiography were detected in the total examination cases. 6) Abnormalities of liver function were observed as 58 cases(5.8%), and 46 cases(79.3%) were male and 12 cases(20.7%) were female. In ultrasonographic study, 25 cases of fatty liver were obtained in the abnormality cases of liver function. 7) Cholelithiasis and gastroduodenal ulcer patients were detected as 2 cases(2.0%) respectively. 8) In the total examination cases, pulmonary tuberculosis, positive reaction of syphilis and renal cysts(ultrasonography) were obtained as 9 cases(0.9%), 7 cases(0.7%) and 4 cases(0.4%) respectively. 9) In the total examination cases, gastric cancer and liver cancer patients were detected as 2 cases(0.2%) respectively.

Distribution and Abundance of Opisthorchis viverrini Metacercariae in Cyprinid Fish in Northeastern Thailand

  • To increase public health awareness for prevention of opisthorchiasis caused by eating raw freshwater fish, the distribution and abundance of Opisthorchis viverrini metacercariae (OV MC) was investigated in freshwater fish obtained from 20 provinces in northeastern Thailand between April 2011 and February 2012. A cross-sectional survey was conducted on 12,890 fish consisting of 13 species randomly caught from 26 rivers, 10 dams, and 38 ponds/lakes. Fish, were collected in each of the rainy and winter seasons from each province. Fish were identified, counted, weighed, and digested using pepsin-HCl. Samples were examined for OV MC by a sedimentation method, and metacercariae were identified under a stereomicroscope. OV MC were found in 6 species of fish; i.e., Cyclocheilichthys armatus, Puntius orphoides, Hampala dispar, Henicorhynchus siamensis, Osteochilus hasselti, and Puntioplites proctozysron from localities in 13 provinces. Among the sites where OV MC-infected fish were found, 70.0% were dams, 23.7% were ponds/lakes, and 7.7% were rivers. The mean intensity of OV MC ranged from 0.01 to 6.5 cysts per fish (or 1.3-287.5 cysts per kg of fish). A high mean intensity of OV MC per fish (>3 cysts) was found in 5 provinces: Amnat Charoen (6.5 cysts), Nakhon Phanom (4.3), Mukdahan (4.1), Khon Kaen, (3.5) and Si Sa Ket (3.4). In conclusion, OV MC are prevalent in natural cyprinid fish, with the infection rate varying according to fish species and habitats.

  • Cardiac involvement in hydatid disease is more seldom than the involvement of the liver and the lungs. Cardiac cyst hydatid disease is diagnosed incidentally or by means of symptoms such as dyspnea and angina pectoris. Here, we present the case of a 45-year-old male patient who underwent open heart surgery for a randomly detected cardiac cyst hydatid during investigations carried out in a healthcare institution after accidentally falling from height. On the other hand, this patient did not have any complaints associated with hydatid disease before this event.

Diagnosis of Cholangiocellular Carcinoma in a Dog (개에 발생한 간내 담관상피암의 진단예)

  • A 5-years-old female pointer dog was submittedto the hospital with the disease history of gradual distension of abdomen and emaciation for two months. An applesized hard mass was palpated on the right costal arch on physical examination. On blood chemical analysis, the marked elevation of SALT and decrease in serum albumin were detected. On paracentesis, a large volume of blood tinged fluid was detected. The peritoneal fluid contained tumor cells and the numerous blood cells. At autopsy the tumor mass located in the left medial lobe of liver was whitish and firm, and was consisted of many cysts. Histologically the tumor was identified as typical cholangiocellular carcinoma. Metastatic lesions of the tumor were detected in the lung, stomach, spleen, diaphragm, and lymph glands. This observation indicates that the cytological examination of the abdominal fluid have a diagnostic significance in the clinical examination of patients with abdominal tumors.

  • The mortality of wild mullet, Mugil cephalus was detected in Kwang- Yang bay on February, 2002. The mullet were infected with Myxobolus sp., the cysts of Myxobolus sp. were found in the mesentery, liver, gill and pharyngeal pocket. The histological findings suggested a systemic infection by the Myxobolus sp.. The spores were measured 10-12 (10.9)${\mu}m$ in length, 9-10 (9.4) ${\mu}m$ in width, 6.4-7.2 (6.8) ${\mu}m$ in thickness, with polar capsules of 4-5.2 (4.4) ${\mu}m$ in length and 2.5-3.3 (2.9) ${\mu}m$ in width.

Characterization of focal hepatic lesions with ferucarbotran (Resovist)-enhanced T2 and T2*-weighted MR imaging

  • The purpose of this study was to characterize focal hepatic lesions through pre and post ferucarbotran-enhanced T2 and T2*-weighted imaging and to help differentiate benign and malignant lesions 대상 및 방법: Consecutive 34 patients with 52 hepatic lesions underwent MRI before and after intravenous bolus injection of ferucarbotran (Resovist Sobering, Berlin, Germany) for evaluation of focal hepatic lesions. Lesions included hemangiomas (n=17), metastases (n=12), cysts (n=10), hepatocellular carcinomas (n=8), dysplastic nodules (n=4), and focal fat deposit (n=1). T2-weighted fast spin echo (TR/TE: 4060/138) and gradient echo T2*-weighted images(TR/TE: 140/5.3, FA = 90) were obtained according to the institutional routine imaging protocol. Lesional signal-intensity and lesion-to-liver contrast changes were measured by contrast-to-noise ratio (CNR) from region of interest.
