Liver ischemia-reperfusion injury (IRI) is the main cause of organ dysfunction and failure after liver surgeries including organ transplantation. The mechanism of liver IRI is complex and numerous signals are involved but cellular metabolic disturbances, oxidative stress, and inflammation are considered the major contributors to liver IRI. In addition, the activation of inflammatory signals exacerbates liver IRI by recruiting macrophages, dendritic cells, and neutrophils, and activating NK cells, NKT cells, and cytotoxic T cells. Technological advances enable us to understand the role of specific immune cells during liver IRI. Accordingly, therapeutic strategies to prevent or treat liver IRI have been proposed but no definitive and effective therapies exist yet. This review summarizes the current update on the immune cell functions and discusses therapeutic potentials in liver IRI. A better understanding of this complex and highly dynamic process may allow for the development of innovative therapeutic approaches and optimize patient outcomes.
Purpose: The purpose of the study was to evaluate the effect of the immediate postoperative nutritional status and calorie adequacy on clinical outcomes in liver transplant recipients. Methods: A total number of 99 patients who received liver transplants were recruited from a tertiary university hospital. Demography, subjective global assessment, clinical outcomes and calorie adequacy were evaluated through personal interviews, electronic medical records and dietary records. Anthropometric measures, body mass index and percent of ideal body weight were also obtained. Results: At admission to the Surgical Intensive Care Unit (SICU), the triceps skinfold thickness and mid-arm muscle circumference were significantly lower in the malnourished group than in the well-nourished group (p<.05, respectively). In the clinical outcomes, transfusion of red blood cells, mechanically ventilated hours, length of stay in the Intensive Care Unit (ICU), length of stay in the hospital, and prothrombin time were significantly higher in the malnourished group than in the well-nourished group (p<.05, respectively). The mechanically ventilated hour was significantly higher in the group with less than 50% of their required energy intake (p<.05). Conclusion: Therefore, it is important to assess the nutritional status of immediate postoperative patients. Furthermore, studies on nutritional interventions are urgently needed to provide adequate nutritional care for patients in ICUs.
본 논문은 급성 간 부전 환자나 간이식 대기상태에 있는 환자에 이용하기 위한 체외 간 보조장치를 개발하기 위한 기초 실험에 관한 것이다. 밀도 33kg/m, 평균 직경 500um, 60-70%의 window를 가진 폴리우레탄 폼을 15% NCO-프리폴리머로 제조하였다. 폴리우레탄 폼에 접종된 간세포는 3일 후에 거의 모두 100-150um 크기의 구상체를 형성하였다. BSA 코팅을 한 폴리우레탄 폼은 BSA로 코팅하지 않은 폴리우레탄 폼보다 간세포 구상체 형성 시간이 더 짧았다. 폴리우레탄 폼에 형성된 간세포 구상체의 간 기능을 증명하기 위하여 암모니아 제거율과 요소 및 알부민 분비율을 측정하였다. 폴리우레탄 폼은 간세포 구상체 배양에 적합하며, 폴리우레탄 폼 내에서의 간세포 구상체 배양은 체외 간 보조장치로의 개발 가능성이 높다.
Liver transplantation is recommended for patients with liver failure, but liver donors are limited. This necessitates the development of artificial livers, and hepatocytes are necessary to develop such artificial livers. Although induced hepatocyte-like cells are used in artificial livers, the characteristics of mouse induced hepatocyte-like cells (miHeps) reprogrammed with embryonic fibroblasts have not yet been clarified. Therefore, this study investigated the mechanisms underlying the survival, function, and death of miHeps. miHeps showed decreased cell viability, increased cytotoxicity, decreased hepatic function, and albumin and urea secretion at passage 14. Addition of necrostatin-1 (NEC-1) to miHeps inhibited necrosome formation and reactive oxygen species generation and increased cell survival. However, NEC-1 did not affect the hepatic function of miHeps. These results provide a basis for development of artificial livers using hepatocytes.

Korean traditional fermented soy paste (doenjang) prolonged the life span of Balb/c mice injected with the sarcoma-180 cells. The activities of liver enzymes, such as xanthine oxidase, aminopyrine N-demethylase, aniline hydroxylase, ${\gamma}$-glutamylcysteine synthetase, glutathione reductase and glutathione S-transferase (GST), and the contents of lipid peroxide and glutathione were determined from the sarcoma-180 cell injected mice that were treated with methanol extracts from doenjang, miso and soybean. The content of lipid peroxide and the activity of xanthine oxidase in the liver of Balb/c mice which were increased by the transplantation of the sarcoma-180 cells were decreased by treatment with the methanol extract from doenjang. But the activities of aminopyrine N-dementhylase and aniline hydroxylase were not affected by the treatment of methanol extracts from doenjang to the mice injected with the sarcoma-180 cells. The content of glutathione, the activities of glutamylcysteine synthetase, glutathione reductase and glutathione S-transferase decreased by the injection of the sarcoma-180 were recovered considerably by the treatment of the methanol extract from doenjang.
The paradigm of chronic liver diseases has been shifting. Although hepatitis B and C viral infections are still the main causes of liver cirrhosis and hepatocellular carcinoma (HCC), the introduction of effective antiviral drugs may control or cure them in the near future. In contrast, the burden of nonalcoholic fatty liver disease (NAFLD) has been increasing for decades, and 25 to 30% of the general population in Korea is estimated to have NAFLD. Over 10% of NAFLD patients may have nonalcoholic steatohepatitis (NASH), a severe form of NAFLD. NASH can progress to cirrhosis and HCC. NASH is currently the second leading cause to be placed on the liver transplantation list in the United States. NAFLD is associated with obesity, type 2 diabetes, dyslipidemia, and metabolic syndrome. The pathophysiology is complex and associated with lipotoxicity, inflammatory cytokines, apoptosis, and insulin resistance. The only proven effective treatment is weight reduction by diet and exercise. However, this may not be effective for advanced fibrosis or cirrhosis. Therefore, effective drugs are urgently needed for treating these conditions. Unfortunately, no drugs have been approved for the treatment of NASH. Many pharmaceutical companies are trying to develop new drugs for the treatment of NASH. Some of them are in phase 2 or 3 clinical trials. Here, pharmacologic therapies in clinical trials, as well as the basic principles of drug therapy, will be reviewed, focusing on pathophysiology.
목적: 원발성 간암의 치료를 위해 간이식이 시행되고 있으나, 8-54%의 환자에서는 이식 후 원격 전이로 인한 재발이 보고 되고 있다. 이 연구는 간이식 예정인 원발성 간암 환자들에서 고식적인 검사로 진단하지 못한 간외 전이 진단에 있어서 FDG-PET의 유용성을 평가하고자 시행되었다. 대상 및 방법: 2001년 8월 이후 본원에서 간이식을 계획하고 전신 FDG-PET을 시행한 환자 중, FDG-PET 검사 이전에 복부 초음파와 CT, 흉부 x선 검사와 CT, 골스캔 등의 고식적인 검사를 시행하여 전이의 증거가 없거나(n=22), 애매한 이상 소견이 있었던(n=4) 원발성 간암 환자 26명(남:여=23:3, 평균 나이 55.7세)을 조사하였다. FDG-PET에서 양성을 보인 결과는 조직검사나 임상적인 추적 검사로 전이 여부를 확인하였다. 결과: 고식적 검사에서 애매한 이상 소견이 있었던 환자 4명 중 3명의 6개 병변이 FDG-PET에서 국소 대사 항진을 보였고, 전이로 확인되어 간이식을 시행하지 않았다. 이 중 5 병변들은 의심되지 않았던 부위에서 새로 발견된 것이었다. 다른 1명은 PET에서 음성 소견으로 간이식을 시행하였다. 고식적인 검사에서 전이를 의심할 만한 소견이 없었던 22명 중에서는 5명의 환자에서 7개의 간외 국소 대사항진 병변들이 FDG-PET에서 발견되었는데, 이들중 1명은 2개의 전이 병변이 확인되어 간이식을 시행하지 않았다. 다른 4명의 환자들에서 보인 5개의 국소 대사항진 병변들은 양성병변으로 확인되었고 이 중 3명은 간이식을 시행하였다. 요약하면, FDG-PET으로 4명의 환자에서 고식적인 검사로 찾지 못하던 전이 병변을 찾아 불필요한 간이식을 피할 수 있었다. 모두 17명에서 간이식이 시행되었다. 간의 조직 검사 소견과 비교하였을 때 생존 원발성 간암을 진단하는 FDG-PET의 민감도는 55.6% (5/9)이었고, 특이도는 87.5% (7/8)이었다. 결론: FDG-PET 전신 스캔은 간이식 예정인 원발성 간암 환자들에서 이전에 시행한 고식적인 검사들로 진단하지 못한 전이 병소들을 추가로 진단하여 불필요한 간이식 수술을 줄이는데 기여할 수 있었다.
Regulatory T cells (Treg) naturally rein in immune attacks, and they can inhibit rejection of transplanted organs and even reverse the progression of autoimmune diseases in mice. The initial safety trials of Treg against graft-versus-host disease (GVHD) provided evidence that the adoptive transfer of Treg is safe and capable of limiting disease progression. Supported by such evidence, numerous clinical trials have been actively investigating the efficacy of Treg targeting autoimmune diseases, type I diabetes, and organ transplant rejection, including kidney and liver. The limited quantity of Treg cells harvested from peripheral blood and subsequent in vitro culture have posed a great challenge to large-scale clinical application of Treg; nevertheless, the concept of CAR (chimeric antigen receptor)-Treg has emerged as a potential resolution to the problem. Recently, two CAR-T therapies, tisagenlecleucel and axicabtagene ciloleucel, were approved by the US FDA for the treatment of refractory or recurrent acute lymhoblastic leukemia. This approval could serve as a guideline for the production protocols for other genetically engineered T cells for clinical use as well. The phase I and II clinical trials of these agents has demonstrated that genetically engineered and antigen-targeting T cells are safe and efficacious in humans. In conclusion, both the promising results of Treg cell therapy from the clinical studies and the recent FDA approval of CAR-T therapies are paving the way for CAR-Treg therapy in clinical use.
지난 30년동안 장기이식분야의 괄목할만한 발전과 더불어 국내에서도 신장, 간 등은 물론 심장이식도 활 발히 이루어지고 있다. 그러나 폐 이식만은 많은 제약으로 인하여 답보상태를 면하지 못하고 있으며 그중에 서도 장기공여자의 부족과 상대적으로 허혈-재관류손상에 예민한 폐 자체의 보존시간의 문제가 가장 먼저 해결해야 할 과제로 여겨지고 있다. 일반적으로 폐는 허혈에 견딜수 있는 시간이 4-6시간으로 알려져 있으 며 많은 연구자들이 더 좋은 폐보존방법을 연구하고 평가하기 위한 실험모델을 개발하여 왔다. 그러나 많 은 실험모델의 등장으로 같은 실험도 서로 상반되는 결과를 낳고 있으며 각각의 모델이 가진 단점들로 인 하여 실험모형의 표준화와 결과분석의 기준을 정하는데 있어 완벽한 모델은 정립된 것이 없는 상태이다. 폐 보존후 기능에 영향을 미칠 수 있는 요인들 즉 허혈성 보존기간, 보존 온도, 폐관류액치 구성성분 등 을 분석하는데는 비교적 간단하고, 경비가 적게 들며 실험결과의 신뢰도가 높은 새로운 모형의 개발이 요 구 되었다. 이에 서울대학교병원 흉부외과에서는 빠르게 변화하는 폐 보존 방법의 실험 및 한국에서는 거 \ulcorner이루어지지 않고 있는 폐 보존 방법에 대한 통계 자료를 확보하여 폐 이식 수술에 이를 적용하고자, 연 구노력 끝에 토끼 폐장으로 상기 목적에 적합한 폐장 분리관류 모형을 완성하였다.
