중의미용(中醫美容)의 생리병리, 임상응용에 관한 문헌고찰 (A Literature Study On Physiology, Pathology and Clinical Applications for TCM Cosmetology)

  • Objectives : This study was performed to investigate physiology, pathology and clinical applications for cosmetology in Traditional Chinese Medicine. We hope most clinicians practicing cosmetic acupuncture to understand theoretical background well and to treat cosmetic diseases more diversely by this paper. Methods : We collected useful informations from 35 papers in Chinese at website www.cnki.net about TCM treatments including acupuncture, moxibustion and herbal medicine. And we could see important factors connected with physiology, pathology and clinical applications within this field. Conclusions : Liver, lung, stomach, mental disorders and yin-deficiency constitution were connected each other in cosmetic physiology. Blood stasis and congestion were thought highly in cosmetic pathology. In clinical applications, wet cupping therapy on the acupoints located in the back line of Governor Vessel(GV), ear-acupuncture, acupuncture using local facial acupoints, pharmacopuncture, moxibustion were used diversely for each case with cosmetic disorders. Especially, local acupoints and remote acupoints were selected at the same time for wrinkle care. And studies on plastic surgery sequelae, dark skin, acne and melasma were more published than those on wrinkle care.

The Characteristics and Outcomes of Abernethy Syndrome in Korean Children: A Single Center Study

  • Purpose: Abernethy malformation is a rare condition, which was first described in 1793 as a congenital extrahepatic porto-systemic shunt (CEPS) directing splanchnic blood flow into the inferior vena cava. Eighty cases have been published so far that reported CEPS, while in Korea, very few cases have been reported. Through this study, we present 6 cases of patients diagnosed with CEPS at Samsung Medical Center and compare these with other such cases published in France and China. Methods: We reviewed clinical, laboratory, and imaging data of 6 children with CEPS in our pediatric clinic between 2004 and 2017. Results: A total of 6 children with CEPS was included in this study, namely, one with type 1a, two with type 1b, and three with type 2 CEPS. The most common presenting symptom was gastrointestinal bleeding (50.0%). Therapeutic interventions included shunting vessel ligation (16.7%) in type 2 CEPS and liver transplantation (16.7%) in type 2 CEPS patient with suddenly developed hepatic encephalopathy. Conclusion: There is no consensus guideline for the optimal management of patients with CEPS. Large-sample studies regarding CEPS are needed to evaluate the characteristics of patients with CEPS and determine the treatment guideline for CEPS.

튜머로이드-혈관신생 상호작용의 가시화를 위한 개방형 구조 미세유체 칩 개발 (Development of open-top microfluidic chip for visualization of interactions between tumoroids and angiogenic sprouting)

  • Cancer cells secrete angiogenic factors, and nearby vasculatures make new blood vessels essential for cancer development and metastasis in response to these soluble factors. Many efforts have been made to elucidate cancer-endothelial cell interactions in vitro. However, not much is known due to the lack of a suitable co-culture platform. Here, we introduce a 3D printing-based microfluidic system that mimics the in vivo-like cancer-endothelial cell interactions. The tumoroids and endothelial cells are co-cultured, physically separated by porous fibrin gel, allowing communication between two cell types through soluble factors. Using this microfluidic system, we were able to visualize new vessel formation induced by tumoroids of different origins, including liver, breast, and ovary. We confirmed that the ovarian tumoroids most induced angiogenesis while the other two cancer types suppressed it. Utilization of the proposed co-culture platform will help the researchers unveil the underlying mechanisms of the dynamic interplay between tumor and angiogenesis.

Scalp metastasis of advanced gastric cancer

  • Head and neck cutaneous metastasis of advanced gastric cancer is uncommon, and scalp metastasis is particularly rare. We present the case of a 60-year-old man who was diagnosed with cutaneous metastasis on the scalp originating from advanced gastric cancer. The patient was referred to the plastic surgery department for a scalp mass near the hairline. He had been diagnosed with advanced gastric cancer and undergone total gastrectomy and Roux esophagojejunostomy 3 years previously. The differential diagnosis for a single flesh-colored nodule on the scalp included benign tumors such as epidermal cyst or lipoma; therefore, the patient underwent excision and biopsy. In the operative field, the mass was found to be located in the frontalis muscle. The biopsy result showed that the mass was a metastatic lesion of advanced gastric cancer. Whole-body computed tomography revealed a gastric tumor with blood vessel infiltration, peritoneal carcinomatosis, liver metastasis, and multiple disseminated subcutaneous metastases. Although scalp metastasis originating from an internal organ is extremely rare, plastic surgeons should always consider a metastatic lesion in the differential diagnosis if a patient with a scalp lesion has a history of malignant cancer.

LKB1/STK11 Tumor Suppressor Reduces Angiogenesis by Directly Interacting with VEGFR2 in Tumorigenesis

  • Cervical tumors represent a prevalent form of cancer affecting women worldwide; current treatment options involve surgery, radiotherapy, and chemotherapy. Angiogenesis, the process of new blood vessel formation, is a crucial factor in cervical tumor growth. The molecular mechanisms underlying the effects of the liver kinase B1 (LKB1/STK11) tumor suppressor protein on tumor angiogenesis have not been elucidated. Therefore, we investigated the role of LKB1 in cervical tumor angiogenesis both in vitro and in vivo in this study. Our results demonstrated that LKB1 inhibited cervical tumor angiogenesis by suppressing the expression of angiogenesis-related factors such as vascular endothelial growth factor (VEGF) and hypoxia inducible factor-1α. LKB1 directly affected both carcinoma and vascular endothelial cells, resulting in a significant reduction in tumor growth and angiogenesis. Furthermore, LKB1 was found to bind to VEGF receptor 2 (VEGFR-2) and target the VEGFR-2-mediated protein kinase B/mechanistic target of rapamycin signaling pathway in endothelial cells, thereby reducing cervical tumor growth and angiogenesis. Our study provides new insights into the molecular mechanisms underlying the anti-tumor and anti-angiogenic effects of LKB1 in cervical cancer. These findings will help develop new therapeutic strategies for cervical cancer.

영상 분석 프로그램을 이용한 관상동맥 혈관 조영상의 화질 측정 방법 고안 (Development of Image Quality Measurement Method of Coronary Angiography Using Image Analysis Program)

  • 병원에서 사용하는 X선 장비는 X선 장비의 능력 연구와 더불어 출력되는 영상의 화질까지도 분석하는 연구가 활발히 진행되어야 한다. 영상의학과 장비의 X-Ray기계나 CT, MRI와 같은 장비의 후향적 이미지 분석은 활발히 진행되었으나 혈관조영실의 혈관조영장비를 이용하여 획득한 영상의 화질 측정은 팬텀을 이용하여 측정하는 방법이 대부분이고 실제 환자에게 촬영된 후의 영상에 관한 화질 측정은 미비한 실정이며 정확한 화질 측정 방법에 관한 연구가 현저히 부족한 상태이다. 따라서 본 연구를 통해 관상동맥 조영술 후 획득된 영상의 화질측정 방법을 고안한 연구를 진행해 시술자에게 질 높은 영상을 제공하고자 연구를 진행하게 되었다. 사용된 장비 및 프로그램은 혈관 조영 검사 장비(Axiom Artis Zee Ceiling)와 Image J프로그램을 사용하였다. 대상으로는 관상동맥 조영술 후 PACS프로그램에 자동 저장된 영상들을 대상으로 하였다. 화질 측정 방법으로는 Image J프로그램을 이용했을 때 관상동맥 조영술 중 좌 관상동맥의 주요 혈관들이 가장 잘 펼쳐져 보이는 각도의 영상인 AP Caudal 30°영상과 우 관상동맥의 혈관이 가장 잘 펼쳐져 보이는 LAO 30° 영상을 선택하여 선택된 영상의 배경영역(Background)과 관심영역(ROI)을 측정하기 위해 혈관과 간, 폐 등의 음영 중복이 가장 최소화 되는 구간을 찾아 측정하는 방법에 관한 기준을 제시하였다. 결론적으로 혈관 조영 영상의 화질측정 분석 방법은 정확히 제시되어 있는 기준이 없기 때문에 시술자에게 질 좋은 영상을 제공하기 위해선 그 장비의 기술자들뿐 아니라 실제로 사용하는 사용자들이 연구자가 되어 다양한 방법으로 화질측정에 관한 연구를 진행해야만 환자에게도 우수한 영상을 제공할 수 있을 것으로 사료된다.

고지혈증 유발 흰쥐에 있어서 생식의 건강개선효과 (Effects of Saengshik Supplementation on Health Improvement in Diet-Induced Hypercholesterolemic Rats)

  • 4주간의 고지혈 유도식이를 섭취시킨 후 고지혈증 유도가 이루어진 실험 동물을 이용하여 생식 섭취로 인한 고지혈증 개선 효과를 확인하고자 하였다. 고지혈 유도가 확인된 실험 동물에게 고지혈사료에 30% 생식을 혼합한 식이를 투여하였을 경우 혈중 콜레스테롤량을 약 37% 정도 감소시킨 것으로 나타났으며, 간조직중의 콜레스테롤 역시 약 28% 감소한 것으로 나타나 생식의 섭취가 체내의 콜레스테롤 농도를 낮추어 주는 효과가 있는 것으로 확인되었다. 또한, 체내 콜레스테롤 구성에 있어서도 관상동맥 질환을 억제하는 것으로 알려진 HDL-C의 함량은 대조군에 대비하여 생식 섭취군이 약 28%정도 증가되었으며, 동맥벽에 축적되어 관상동맥질환 발병과 연관관계가 높은 것으로 알려진 LDL-C의 함량을 약 39%정도 감소시킨 것으로 나타나 체내 콜레스테롤 수치의 저하와 더불어 콜레스테롤의 구성 양상에도 이로운 점을 제공한 것으로 나타났다. 이러한 결과로부터 순환기계 질환 발병율을 나타내는 지표로 널리 이용되는 동맥경화지수와 심혈관위험지수를 산출한 결과 생식 섭취군은 대조군에 대비하여 동맥경화지수의 약 60%를 저하시켰으며, 심혈관위험지수 역시 약 56% 정도 저하시킨 것으로 나타났다. 이상의 결과를 종합하여 볼 때 생식 섭취는 체내 콜레스테롤의 농도를 저하시킴으로서 순환기계 질환의 위험도를 낮추는데 효과가 있는 것으로 생각된다.

  • Background and Objective: Atherosclerosis is a diffuse, systemic disease that affects the coronary, cerebral, and peripheral arterial trees. Clopidogrel is widely used antiplatelet agent and its efficacy has been proven in cardiac and extracardiac vascular diseases, but it has several side effects. Therefore we investigated whether Sopunghwalhyeoltang, which is widely used for treating the blood stasis syndrome in traditional medicine, could decrease the side effect of antiplatelets and have a synergic effect. Methods & Materials: Male $ApoE^{(-/-)}$ mice were randomly divided into three different experimental groups, non-treated group (Control group), clopidogrel-treated group (CP group) and clopidogrel with Sopunghwalhyeol-tang treated group (CPS group). The control group was fed with only an atherogenic diet, the CP group an atherogenic diet plus clopidogrel 25mg/kg and the CPS group an atherogenic diet plus clopidogrel 25mg/kg with Sopunghwalhyeol-tang 100 mg/kg. We investigated plasma lipids with liver function test, and performed a histological investigation of liver and abdominal aorta. Results: 1. Photomicrographs of liver and abdominal aorta tissue showed lower histological injury and lipid accumulation in the CP and CPS groups than those in the Control group. 2. In the CPS group, plasma triglyceride level was significantly lower than in the Control and CP groups. 3. In the CPS group, the plasma aspartate aminotransferase (AST) level was significantly lower than in the CP group. Conclusions: The above results shows that a combined treatment of Sopunghwalhyeol-tang and clopidogrel have a synergic effect through inhibiting vessel injury and decrease the side effects of clopidogrel alone.

섬어(語語)와 정성(鄭聲)에 대한 동서의학적(東西醫學的) 고찰(考察) (The oriental-western literatural study of Delirious speech and Fading murmuring)

  • Literatural study for Delirious speech and Fading murmuring, the results were as follows. 1. Delirious speech and Fading murmuring are given at the speech impediment. Derious speech to be out of language's order and slur the end of his words, and Fading murmuring is to repeat in losing conscious. 2. In constrast with Delirious speech and Fading murmuring, Maniac speech is induced by a general term for manic-depressive psychosis. Luoyan is to say in a feeble voice and mumble in a sleeping condition, and Paraphasia and Solioquy are appeared in a clear mental condition. The speech impediment is caused by damages of the nervous system and speech organ, and Yuyancuoluan is appeared in a feverless condition. 3. The symptoms of Delirious speech are to utter ravings and have a loud and heavy voice, and these resemble the delirium which specially has a speech impediment and muddle in the western medical world. The symptoms of Fading murmuring are to speak ambigously, repeatedly, and illogically and so are similar to the Wernicke dysphasia which is caused by a incomprehensible conversation. 4. The causes of Delirious speech are to spread a stomach heat and the lungs pathogenic qi into heart, not to sweat in cold damage, the Three Yang Combination of syndrome, stomach repletion, yang collapse due to excessive sweat, diarrhea, after diarrhea, heat to enter the blood chamer, feces to remain in the stomach, stasis blood to enter the viscera, to carry anger to extremity, and to be constipated. the cause of Fading murmuring is to despair vacuity desertion of vital essence and energy after a serious illness. 5. The causes of delirium are general infection, postoperative states, and metabolism disorders and those of Wernicke dysphasia are disorders of the blood vessel, brain tumors and traumas. 6. Delirious speech is cured with the discrimination of vacuity and repletion. Baitong Tang(白通湯), Chaihu Guizhi Tang(柴胡桂枝湯), Chaihu Jia Longgu Muli Tang(柴胡加龍骨牡蠣湯) are prescribed in case of vacuity, while Chengqi Tang(承氣湯), Baihu Tang(白虎湯), Liangge San(凉膈散) are in case of repletion. Fading murmuring is treated with Xiao Chaihu Tang(小柴胡湯), Fuzi Tang Jiawei(附子湯加味), Shengmai San(生脈散), and Renshen Sanbai Tang(人蔘三白湯). 7. To acupunture Qimen-Xue(期門穴) is required when it is late to prescribe a medical decoction or the hyperactive liver qi attacking the spleen.

이중에너지 방법을 이용한 가상 단색 영상의 화질 시뮬레이션 연구 (A Simulation Study on Image Quality of Virtual Monochromatic Image using Dual-energy Method)

  • 본 연구의 목적은 혈관의 가상단색성 영상 화질을 단색성 영상과 비교하여 평가한 시뮬레이션 연구이다. 이중에너지 영상은 저에너지와 고에너지 각각 50 keV, 80 keV에서 다섯 가지 물질의 선형감약계수를 바탕으로 획득되었다. 가상단색성 영상을 합성하기 위해 가중계수가 필요하며, 가중계수를 획득하기 위해 간과 뼈를 기저 물질로 사용하였다. 가상단색성 영상은 30 keV에서 100 keV 범위의 에너지에서 합성되었다. 영상 평가는 칼슘과 조영제를 신호로, 혈액을 백그라운드로 설정하여 CNR과 noise로 평가되었다. 연구결과에 따르면, 최대 CNR을 가지는 에너지는 칼슘과 조영제에서 각각 50 keV, 60 keV였다. 최소 noise를 보이는 에너지는 칼슘, 요오드 조영제, 혈액에서 각각 70 keV, 70 keV, 60 keV였다. VM 영상은 noise를 최소화하고 CNR을 최대화하는 최적의 에너지에서의 영상을 구현할 수 있으므로 CT 검사에서 진단능 향상에 기여할 수 있다.
