  • 오늘날 CT나 MR등을 통한 의학 영상 기술과 컴퓨터 성능의 향상으로 인체 내부 장기의 영상을 비교적 용이하게 얻을 수 있으며 얻어진 영상 정보는 컴퓨터로 수치와 되므로 데이터의 조작 및 가공이 용이하다. 그러나, 이 데이터는 2D 슬라이스들의 연속으로 표현되므로 이것을 보다 편리하게 가시화. 조작, 분석이 용이한 상태로 바꾸기 위해서는 3차원 구조로의 재구성이 필요하게 된다. 이것을 위하여 무엇보다도 먼저 CT나 MR을 통하여 얻어진 영상을 분석하여 특정 장기의 영상 부분를 다른 조직의 영상부분으로부터 분리(segmentation)할 필요가 있다. 이러한 Segmentation방법에는 여러가지가 있는데, 수작업의 결합 등으로 인해서 비효율적인 문제점을 가지고 있다. 이에 본 논문은 보다 효율적인 segmentation의 처리를 위하여 region-based 기법을 응용하여 새로운 segmentation 방법을 개발하였다. 그리하여, 본 논문이 제안한 알고리즘을 슬라이스 간격이 큰 2차원 복부 CT 영상에 적용시켜 간(liver)의 추출을 시도하였고 향상된 성능을 확인할 수 있었다.

A Case of Epithelioid Hemangioendothelioma Involving the Lung, Pleura and Liver (흉막과 간을 동시에 침범한 폐의 유상피 혈관내피종 1예)

  • Epithelioid hemangioendothelioma (EH) is a rare borderline malignant tumor which originating from vascular endothelial cells and occurs in many organs such as soft tissues, lung, liver and bone. But, pulmonary EH which simultaneously involves pleura and liver is very rare. In the present report, we describe an uncommon case of EH involving the lung, pleura and liver in a 26-year-old woman. Chest CT showed single nodule in right upper lobe with large amount of pleural effusion at ipsilateral side and abdominal CT multiple round low densities in liver. The EH was confirmed by microscopic analysis and immunohistochemical staining of CD34+ from open lung biopsy specimen.

  • Epithelioid Hemangioendothelioma(EH) is a rare vascular tumor, originating from endothelial cells. The principal locations are lung, soft tissue, bone and liver. This tumor is of borderline malignancy, relatively benign course. In the lung, the tumor is often multifocal, bilateral and frequently lead to the mistaken diagnosis of metastatic carcinoma. Although EH of the lung is relatively slow growing tumor, extensive pulmonary involvement, systemic metastasis, mainly to the liver have been documented. A 26-year-old man with EH involving the lung and liver was reported. Chest X-ray and chest CT showed multiple nodules in both lung fields and Abdominal CT multiple round low densities in liver. Transbronchial lung biopsy was performed. The patient diagnosed as EH by light microscopic finding and immunohistochemical study for Factor VIII-related antigen.

Long-term Survival of Recurrent Pancreatic Cancer Treated with Tumorectomy and Stereotactic Body Radiation Therapy (수술 후 재발한 췌장암에서 종양절제술과 정위적 체부 방사선치료로 장기간 생존을 보인 환자)

  • A 70-year-old female diagnosed with pancreatic ductal adenocarcinoma was treated by pylorus-preserving pancreaticoduodenectomy (PPPD) and adjuvant concurrent chemoradiotherapy with 5-fluorouracil. Pancreatic ductal adenocarcinoma pT3N0 (stage IIA) was pathologically confirmed. Abdominal computed tomography (CT) findings 14 months after PPPD showed 10 mm sized solitary liver metastasis in segment 3. After 12 cycles of gemcitabine and 9 cycles of capecitabine plus oxaliplatin, the metastatic nodule increased in size to 27 mm. Tumorectomy at segment 3 of liver was done. 25 months after tumorectomy, chest CT showed 23 mm sized cavitary nodule in right upper lobe of lung. The result of percutaneous biopsy favored metastatic adenocarcinoma. Two sets of stereotactic body radiation therapy were done and the patient has survived without further disease progression for 6 years after initial diagnosis. This case suggests that selected population of recurrent pancreatic cancer patients with solitary liver or pulmonary metastasis can be treated by resection of metastatic site and ablative therapies.

Hierarchical Organ Segmentation using Location Information based on Multi-atlas in Abdominal CT Images (복부 컴퓨터단층촬영 영상에서 다중 아틀라스 기반 위치적 정보를 사용한 계층적 장기 분할)

  • In this paper, we propose an automatic hierarchical organ segmentation method on abdominal CT images. First, similar atlases are selected using bone-based similarity registration and similarity of liver, kidney, and pancreas area. Second, each abdominal organ is roughly segmented using image-based similarity registration and intensity-based locally weighted voting. Finally, the segmented abdominal organ is refined using mask-based affine registration and intensity-based locally weighted voting. Especially, gallbladder and pancreas are hierarchically refined using location information of neighbor organs such as liver, left kidney and spleen. Our method was tested on a dataset of 12 portal-venous phase CT data. The average DSC of total organs was $90.47{\pm}1.70%$. Our method can be used for patient-specific abdominal organ segmentation for rehearsal of laparoscopic surgery.

Computer Tomography and Magnetic Resonance Image Manifestations of Primary Hepatic Neuroendocrine Cell Carcinomas

  • Aim: This study aims to investigate the manifestation of CT, MRI and dynamic enhanced scans for primary hepatic neuroendocrine cell carcinoma. Methods: CT or MRI arterial and venous phase scan images of 19 cases of pathologically confirmed PHNEC were retrospectively analyzed. Results: 14 cases (73.68%) with single lesion, 5 cases (26.3%) with multiple lesions, with an average diameter of 13.2 cm. Some 12 cases (63.16%) showed inhomogeneous enhancement, seven cases (36.8%) showed homogeneous enhancement, 13 cases (68.4%) demonstrated significant enhancement in the arterial phase, 13 cases (68.4%) had significantly enhanced portal venous phase including 7 cases (36.8 %) with portal venous phase density or signal above the arterial phase and 5 cases (26.3%) with the portal vein density or signal below the arterial phase. Seven cases (36.8%) had continued strengthened separate shadows in the center of the lesion. Thrombosis were not seen in portal veins. Conclusion: CT and MRI images of liver cell neuroendocrine carcinoma have certain characteristics that can provide valuable information for diagnosis and differential diagnosis.

Imaging Features of Hepatic Adenoma in a Dog with Atypical Computed Tomographic Findings

  • Computed tomography (CT) findings of hepatic adenoma in veterinary medicine are variable and unlike in human medicine, not defined clearly. A 12-year-old neutered male Shih Tzu presented after a seizure, with weight loss, salivation, and cachexia. An abdominal mass was identified on radiography, and ultrasonographic images showed a mixed echo pattern with marked vascularity. CT showed that the mass originated from caudate lobe, was heterogeneously hypoattenuated compared with the hepatic parenchyma, and had irregular margins. Contrast enhanced CT showed that the mass enhanced like the surrounding liver parenchyma. However, it contained unenhanced areas and enhanced vessels were observed in the arterial phase at the periphery of the mass. The margins of mass were more enhanced in the venous phase than the arterial phase and the hypoattenuating regions within the mass were not enhanced. Greater enhancing in the venous phase is seen with adenomas; however, the heterogeneous enhancement pattern, especially the marginal vascular enhancement and internal hypoattenuating regions, is seen with malignancy. Although this is a single case of hepatic adenoma, the atypical enhanced pattern of this case can provide useful information to predict the malignancy of primary liver tumor.

  • Heterogeneity assessment of tumor in oncology is important for diagnosis of cancer and therapy. The aim of this study was performed assess heterogeneity tumor region in PET image using texture analysis. For assessment of heterogeneity tumor in PET image, we inserted sphere phantom in torso phantom. Cu-64 labeled radioisotope was administrated by 156.84 MBq in torso phantom. PET/CT image was acquired by PET/CT scanner (Discovery 710, GE Healthcare, Milwaukee, WI). The texture analysis of PET images was calculated using occurrence probability of gray level co-occurrence matrix. Energy and entropy is one of results of texture analysis. We performed the texture analysis in tumor, liver, and background. Assessment textural features of region-of-interest (ROI) in torso phantom used in-house software. We calculated the textural features of torso phantom in PET image using texture analysis. Calculated entropy in tumor, liver, and background were 5.322, 7.639, and 7.818. The further study will perform assessment of heterogeneity using clinical tumor PET image.

Preoperative estimation of hemi-liver volume using standard liver volume and portal vein diameter ratio in living donor liver transplantation

  • Backgrounds/Aims: Although body surface area (BSA)-based standard liver volume (SLV) formulae have been used for living donor liver transplantation and hepatic resection, hemi-liver volume (HLV) is needed more frequently. HLV can be assessed using right or left portal vein diameter (RPVD or LPVD). The aim of this study was to validate the reliability of using portal vein diameter ratio (PVDR) for assessing HLV in living liver donors. Methods: This study included 92 living liver donors (59 males and 33 females) who underwent surgery between January 2020 and December 2020. Computed tomography (CT) images were used for measurements. Results: Mean age of donors was 35.5 ± 7.2 years. CT volumetry-measured total liver volume (TLV), right HLV, left HLV, and percentage of right HLV in TLV were 1,442.9 ± 314.2 mL, 931.5 ± 206.4 mL, 551.4 ± 126.5 mL, and 64.6% ± 3.6%, respectively. RPVD, LPVD, and main portal vein diameter were 12.2 ± 1.5 mm, 10.0 ± 1.3 mm, and 15.3 ± 1.7 mm, respectively (corresponding square values: 149.9 ± 36.9 mm2, 101.5 ± 25.2 mm2, and 237.2 ± 52.2 mm2, respectively). The sum of RPVD2 and LPVD2 was 251.1 ± 56.9 mm2. BSA-based SLV was 1,279.5 ± 188.7 mL (error rate: 9.1% ± 14.4%). SLV formula- and PVDR-based right HLV was 760.0 ± 130.7 mL (error rate: 16.2% ± 13.3%). Conclusions: Combining BSA-based SLV and PVDR appears to be a simple method to predict right or left HLV in living donors or split liver transplantation.

  • One of my administrational patient had the disease of liver cirrhosis with cerebrovascular hemorrhage simultaneously. so we investigated the process of cureness in detail. In the investigational method, at the day of onset the patient was administered the sunghyangjunggisan(星香正氣散) for 4 days. hyelbuchugatanggamibang(血府逐瘀湯加味方) for 7 days, the day after saengganggunbitang(生肝建脾湯) and the extracts of Artemisia capillaris(茵蔯), Alisma plantago(澤瀉) were took for 17 days. So we searched the attentioanable results in the Biochemistry. Brain CT, Sonogram. Abdominal CT, and the decrease of self symptom. In the results we recognized that the patient was improved in the LFT, the smaller portal vein, the resolving of hemorrhage, the decrease of self symptom etc.
