Arthroscopic Anterior Cruciate Ligament Reconstruction Using Autogenous Hamstring Tendon Graft Without Detachment of the Tibial Insertion (경골 종지부를 분리하지 않은 자가슬괵건을 이용한 관절경적 전방십자인대 재건술)

  • This article describes a modified arthroscopic technique of anterior cruciate ligament (ACL) reconstruction using quadrupled hamstring tendon graft. The autogenous semitendinosus and gracilis grafts are harvested without detachment of the tibial insertion. To obtain longer graft, the accessory tibial insertions of the hamstring tendons are dissected. The EndoButton(Acupex Microsurgical, Andover, MA) is used for femoral fixation and two spiked staples are used for tibial fixation in a belt buckle fashion. Then the residual anterior laxity is restored by additional absorbable interference screw fixations. In this technique. more viable graft is obtained and firmer distal fixation is achieved by preservation of the tibial insertion of hamstring tendons.

Effect of nano-composite materials on repair of ligament injury in sports detoxification

  • Extraordinary properties of nanocomposites make them a primary replacement for many conventional materials. Anterior cruciate ligament (ACL) reconstruction, which is a frequent surgery in sport activities, is one of the fields in which nanocomposites could be utilized. In the present study, the mechanical properties of different porous scaffolds made from graphene nano-composites are presented ad load bearing capacity of these materials is calculated using finite element method. The numerical results are further compared with experimental published data. In addition, several geometrical and material parameters are analyzed to find the best configuration of nanocomposite scaffolds in reconstruction of ACL. Moreover, coating of detoxification chemicals are extremely easier on the nano-structured materials than conventional one. Detoxification potential of nano-composites in the injured body are also discussed in detail. The results indicated that nano-composite could be successfully used in place of auto- and allografts and also instead of conventional metallic screws in reconstruction of ACL.

  • Ulnar collateral ligament injuries of the elbow are frequent among overhead athletes. The incidence of ulnar collateral ligament reconstructions (UCLRs) in high-level players has increased dramatically over the past decade, but the optimal technique of UCLR is controversial. Surgeons need to manage the patients' expectations appropriately when considering the mode of treatment. This article reviews current studies on the management of ulnar collateral ligament injuries, particularly in overhead athletes.

  • Three cases who had medial compartment osteoarthritis of the knee (Kellgrene-Laurence grade 3 and Outerbridge grade 4) and anterior instability of the knee due to rupture of the anterior cruciate ligament in relative young ages underwent staged anterior cruciate ligament reconstruction followed by medial unicondylar arthroplasty in 2 cases and simultaneous anterior cruciate ligament reconstruction and unicondylar arthroplasty. We evaluated clinical results some kinds of preoperative and postoperative International Knee Documentation Committee (IKDC), Lysholm score and last follow-up hospital for special surgery (HSS), knee society score (KSS). We consider that medial unicondylar arthroplasty with staged or simultaneous anterior cruciate ligament reconstruction is very good option of the treatment for the anterior instability and pain from advanced arthritis.

  • The most recent concept in anterior cruciate ligament reconstruction is an anatomical single bundle anterior cruciate ligament reconstruction. For an anatomical anterior cruciate ligament reconstruction, the tibial tunnel is made anterior than before, and the femoral tunnel is made in a lower and oblique direction compared to the classical method using the transtibial technique. The anteromedial portal technique, outside-in technique, and modified transtibial technique have been performed to produce femoral tunnels with anatomical positions. Each method has different advantages and disadvantages and is chosen based on the operator's preferences, experience, instruments, and implants.

  • The isometric position of the anterior cruciate ligament was calculated during flexion-extension. Flexion-extension motion data of the knee joint were obtained by Fastrak, a three-dimensional motion measurement system. A subject was seated on a flat table and the tibia sensor position was measured with the femur fixed at the table. A three-dimensional knee model was constructed using a graphic tool to simulate the knee motion. Three surgical positions of the femoral tunnel were selected and the distances between the determined tibial tunnel and each femoral tunnel were calculated. The maximum elongation position was found to be in the ten thirty direction of clock.

  • Recurrent dislocation of elbow joint occurs relatively rarely by the injury of the collateral ligament which contributes elbow joint stability. Among them, posterolateral rotatory instability occurs by the injury to the lateral ulnar collateral ligament. We experienced a case of recurrent dislocation of elbow joint due to posterolateral rotatory instability. We treated operatively with lateral ulnar collateral ligament reconstruction using the palmaris longus tendon by technique of Nestor et al. We report it with literature analysis.

  • The anterior cruciate ligament(ACL) is an important stabilizer of knee joint. The ACL injury of knee is common and a serious ACL injury leads to ligament reconstruction surgery. Gait analysis is essential to identify knee condition of patients who display abnormal gait. The purpose of this study is to evaluate and classify knee condition of ACL deficient patients using a nonlinear dynamic method. The nonlinear method focuses on understanding how variations in the gait pattern change over time. The experiments were carried out for 17 subjects(l2 healthy subjects and five subjects with unilateral deficiency) walking on a motorized treadmill for 100 seconds. Three dimensional kinematics of the lower extremity were collected by using four cameras and KWON 3D motion analysis system. The largest Lyapunov exponent calculated from knee joint flexion-extension time series was used to quantify knee stability. The results revealed the difference between healthy subjects and patients. The deficient knee was significantly unstable compared with the contralateral knee. This study suggests an evaluation scheme of the severity of injury and the level of recovery. The proposed Lyapunov exponent can be used in rehabilitation and diagnosis of recoverable patients.

  • Anterior cruciate ligament(ACL) injury of the knee is common and a serious ACL injury leads to ligament reconstruction surgery. Gait analysis is used to identify the result of surgery. The purpose of this study is to numerically evaluate and classify knee condition of patients through the chaos analysis. Experiments were carried out for 13 subjects (8 healthy subjects, 5 ACL deficient patients) walking on a treadmill. Sagittal kinematic data of the right lower extremity were collected by using a 3D motion analysis system. The recorded gait patterns were digitized and then coordinated by KWON3D. The largest Lyapunov exponent from the measured knee angular displacement time series was calculated to quantify local stability. It was found that the Lyapunov exponent becomes larger as the knee condition becomes worse. This study suggested a method of the severity of injury and the level of recovery. The proposed method discerns difference between healthy subjects and patients.
