Neural Injuries in Ankle Sprain (족근관절 염좌시 동반된 신경 손상)

  • Purpose: The neural injuries by the sprain around the ankle joint may contribute the chronic pain. Authors analyzed the incidence and the contributing factor of the neural injuries in ankle sprain. Materials and Methods: 52 patients (54 cases) were involved in this retrospective study. Patient with diabetes or spinal disease were excluded. Plain radiograph and MR image were evaluated. Treatments were consisted of cast immobilization for 4 weeks with weight bearing ambulation following bracing for 8 weeks. Neurologic evaluation were performed at 3 months, 6 months, 12 months post-injury period and each neural injury were confirmed by electromyography or lidocaine block test. Results: The average age was 39 years old and 34 cases were male and 20 cases were female. Rupture of the anterior talofibular ligament was observed in 48 cases, distal anterior tibiofibular ligament in 37 cases, calcaneofibular ligament in 6 cases. One cases revealed no injury of the ligament. Neural injuries around ankle was observed in 13 cases ; superficial peroneal nerve in 9 cases, sural nerve in 5 cases, saphanous nerve in 1 case. Neural injury was not influenced by the degree of ligament injuries but by the incidence numbers of ankle sprain. All cases were treated conservatively and symptom was subsided in all but 2 cases. Conclusion : Although the incidence is relatively low, the neural injuries in ankle sprain may occur in the recurrent ankle sprain and conservative treatment for neural injuries is satisfactory.

A Case Report of a Postoperative State Patient with Cervical Spinal Cord Injury due to Ossification of Posterior Longitudinal Ligament Treated by Korean Medicine (후종인대골화증으로 발생한 경추 척수손상 환자에 대한 수술 후 한방치료 증례보고 1례)

  • Objectives: The purpose of this study is to report the effectiveness of Korean medicine treatment on postoperative state patients with cervical spinal cord injury due to ossification of the posterior longitudinal ligament. Methods: A 55-year-old Korean male patient diagnosed with cervical spinal cord injury due to ossification of the posterior longitudinal ligament was treated in an inpatient setting with Korean medicine treatment consisting of herbal medicine, acupuncture, and others from May 22 to August 21. The outcome was evaluated with the American Spinal Injury Association Impairment Scale (AIS), Spinal Cord Independence Measure-III (SCIM-III), modified Japanese Orthopaedic Association scale (mJOA), Numeric Rating Scale (NRS) before and after, or during the treatment period. Results: After 92 days of treatment, the patient's scores on all scales improved. Conclusion: The results suggest that Korean medicine treatment may be effective for postoperative patients with cervical spinal cord injury due to ossification of the posterior longitudinal ligament. Further studies should be conducted on a larger number of patients.

The Relationship between Bone Bruise in MR-Imaging and the Degrees of Acute Anterior Cruciate Ligament Injury (급성 전방십자인대 손상 시 자기공명영상에서 나타나는 골멍과 전방십자인대 손상 정도의 관계)

  • Purpose: The aim of this retrospective study was to evaluate the relationship between appearance and size of bone bruise and severity of anterior cruciate ligament (ACL) injury with magnetic resonance imaging (MRI), furthermore, to identify the mechanism of ACL injury by analyzing the footprint of bone bruise. Materials and Methods: Sixty-five subjects who was taken an MRI within 6 weeks after acute ACL injury were studied. All MRI showed ACL injury and associated bone bruises. Bone bruises were analyzed using Costa-Paz classification based on appearance and location and also scored using Kornaat bone bruise grading system based on size and location. The severity of the ACL injury was graded using a 4-point system (ie, grade 0-3) with oblique coronal MRI. Results: There was statistically significant correlation between Costa-Paz classification of bone bruises in lateral femoral condyle and grade of ACL injury. There were more type 2 lesions in partial tear group (grade 1, 2), however, type 3 lesions of Costa-Paz classification in complete tear group (P=0.037). Significant positive correlations were observed between Kornaat total bone bruise score and grade of ACL injury (P=0.014). Most common ACL injury mechanism was pivot shift injury. Other mechanisms were clip valgus injury, phantom foot injury, and hyperextension injury. Conclusion: It is highly suggested that appearance and size of bone bruise is related to severity of ACL injury after acute ACL injury. The foot print of bone bruise provides valuable clues to identify ACL injury mechanism.

Treatment of Ruptured Posterior Cruciate Ligament using Augmentation of the Synthetic Polyester Prosthetic Ligament (Synthetic Polyester Prosthetic Ligament 보강술을 이용한 후방 십자 인대 손상의 치료)

  • Purpose : The purpose of this study was to assess the clinical outcomes and radiological changes of arthroscopic augmentation with synthetic polyester ligament for posterior cruciate ligament injury Materials and Methods : The assessment was made among 60 patients who underwent arthroscopic augmentation with synthetic polyester ligament (ABC ligament, Surgicraft, U.K.) into the substance of ruptured PCL proper under the arthroscopic control from January, 1990 to January, 1996 and whose follow-up period was more than 5 years (average 7.8 years). The results were analyzed by using the posterior stress radiographs by Telos stress device, KT-2000 arthrometer and the clinical assessment by using Lysholm knee score. Results : The average difference of posterior displacement on stress radiographs were 13.2 mm preperatively and 3.6 mm at last follow up. Both knee showed minimal difference on KT-2000 arthrometer (0.7 mm on 20 lb) at the last follow-up. The mean Lysholm knee score was 49.2 preoperatively and improved to 84.3 post-operatively. Conclusion : Authors observed that long-term results of arthroscopic augmentation with synthetic polyester ligament for PCL injury were similar with those using human allograft. Taking these results into consideration, the synthetic polyester ligament is assumed to be a method of treatment of the PCL injury.

Effects of Landing Tasks on the Anterior Cruciate Ligament Injury Risk Factors in Female Basketball Players (여자 농구 선수들의 착지 유형이 전방십자인대 손상위험 요인에 미치는 영향)

  • The purpose of this study was to investigate the effects of landing tasks on the anterior cruciate ligament (ACL) injury risk factors in female basketball players. Fifteen female basketball players performed a drop landing and a drop landing with a vertical jump on the 40 cm height box. Three-dimensional motion analysis system and ground reaction force system was used for calculate the ACL injury risk factors. Paired samples t-test with Bonfferoni correction were performed. The drop landing with a vertical jump had the higher knee flexion angle, peak knee varus moment, trunk flexion angle than a drop landing. However, the drop landing had the higher trunk rotation angle than a drop landing with a vertical jump. These results indicate that seemingly minor variations between drop landing and drop landing with a vertical jump may influence the ACL injury risk factors. Caution should be used when comparing studies using different landing tasks.

  • Recurrent dislocation of elbow joint occurs relatively rarely by the injury of the collateral ligament which contributes elbow joint stability. Among them, posterolateral rotatory instability occurs by the injury to the lateral ulnar collateral ligament. We experienced a case of recurrent dislocation of elbow joint due to posterolateral rotatory instability. We treated operatively with lateral ulnar collateral ligament reconstruction using the palmaris longus tendon by technique of Nestor et al. We report it with literature analysis.

Distal Clavicle Tunnel Widening after Coracoclavicular Ligament Reconstruction with Semitendinous Tendon: A Case Report (반건양 건을 이용한 오구쇄골 인대 재건술후 발생한 원위부 쇄골 터널 확장: 증례 보고)

  • Distal clavicle tunnel widening was observed in coracoclavicular ligament reconstruction with semitendinous tendon autografts in a patient with acromioclavicular joint injury. Acromioclavicular joint separation, in a 44 years-old man was treated by coracoclavicular ligament reconstruction. We have performed x-ray evaluation on 2years 10months after surgery. The immediate postoperative tunnel size was measured 4.5mm in diameter. At postoperative 2years 10month the tunnel diameter was from 9.3 to 11.4mm. But the weightbearing clavicle view showed no significant acromioclavicular joint separation. Moreover the patient complained only minor intermittent shoulder discomfort.

  • Evidence-based papers on the treatment of some chronic tendinopathy or ligament lesions using an injection in foot and ankle disorders have been reported, but there are few reports on the treatment of acute ankle ligament injury. On the other hand, some papers have reported a faster return to play for injection therapy that combines RICE (Rest, Icing, Compression, and Elevation) treatment and rehabilitation. Injection therapy can be used as an additional treatment for ankle ligament injury. Rather than having narrow-minded thinking about these treatments, it is important to make efforts to verify the safety and precautions of treatment and recognize them as a category of normal treatment. Continuous analysis and monitoring of these treatments can satisfy patients whose needs are changing rapidly.

Gait Study on the Normal and ACL Deficient Patients After Ligament Reconstruction Surgery Using Chaos Analysis Method (전방십자인대 재건수술 환자와 정상인의 보행 연구)

  • The anterior cruciate ligament(ACL) is an important stabilizer of knee joint. The ACL injury of knee is common and a serious ACL injury leads to ligament reconstruction surgery. Gait analysis is essential to identify knee condition of patients who display abnormal gait. The purpose of this study is to evaluate and classify knee condition of ACL deficient patients using a nonlinear dynamic method. The nonlinear method focuses on understanding how variations in the gait pattern change over time. The experiments were carried out for 17 subjects(l2 healthy subjects and five subjects with unilateral deficiency) walking on a motorized treadmill for 100 seconds. Three dimensional kinematics of the lower extremity were collected by using four cameras and KWON 3D motion analysis system. The largest Lyapunov exponent calculated from knee joint flexion-extension time series was used to quantify knee stability. The results revealed the difference between healthy subjects and patients. The deficient knee was significantly unstable compared with the contralateral knee. This study suggests an evaluation scheme of the severity of injury and the level of recovery. The proposed Lyapunov exponent can be used in rehabilitation and diagnosis of recoverable patients.

  • Anterior cruciate ligament(ACL) injury of the knee is common and a serious ACL injury leads to ligament reconstruction surgery. Gait analysis is used to identify the result of surgery. The purpose of this study is to numerically evaluate and classify knee condition of patients through the chaos analysis. Experiments were carried out for 13 subjects (8 healthy subjects, 5 ACL deficient patients) walking on a treadmill. Sagittal kinematic data of the right lower extremity were collected by using a 3D motion analysis system. The recorded gait patterns were digitized and then coordinated by KWON3D. The largest Lyapunov exponent from the measured knee angular displacement time series was calculated to quantify local stability. It was found that the Lyapunov exponent becomes larger as the knee condition becomes worse. This study suggested a method of the severity of injury and the level of recovery. The proposed method discerns difference between healthy subjects and patients.
