A Study of Applied Pressure According to Knee variable During Lachman Test for Anterior Cruciate Ligament using Telos Device (Telos Device를 이용한 전방십자인대 Lachman 검사 시 무릎두께와 활성도에 따른 인가압력에 대한 고찰)

  • The purpose of study is to investigate the difference of the daN force applied during the examination using Telos Device and the degree of fluctuation of anterior cruciate ligament according to the individual differences of the muscles involved in knee stability. The examination was done by the Lachman test using Telos Device and the changes of anterior cruciate ligament were measured by varying the force of 0. 15, 30 daN on each right and left side and the force of 30 daN after the bruce protocol. Computed tomography (CT) was used to measure muscle mass. As a result of measuring the degree of fluctuation of the anterior cruciate ligament according to the change of the force applied to the Telos Device, there was a statistically significant difference in the knee fluctuation when 15 daN and 30 daN were applied on both right and left. Also, it is analyzed that the degree of fluctuation of anterior cruciate ligament varies according to the difference of individual's muscle mass. Therefore, it is considered that the force applied to the Telos Device should be changed according to the difference of individual's muscle mass.

  • The purpose of this case study is to show a case of acute posterior cruciate ligament(PCL) complete injury improved by conservative acupuncture therapy. The patient was treated for 100 days. The patient received deep interarticular electroacupuncture(EA) stimulation at S35, LE200, LE211 and treated by other acupoint stimulation, herb medication, cupping, physical treatments and exercise. We evaluated through MRI and Lysholm knee score. We suggest to treat acute posterior cruciate ligament(PCL) injury with acupuncture therapy meaning conservative therapy and would like to research acupuncture treatment plan for the further treatments.

  • The anterior cruciate ligament(ACL) is an important stabilizer of knee joint. The ACL injury of knee is common and a serious ACL injury leads to ligament reconstruction surgery. Gait analysis is essential to identify knee condition of patients who display abnormal gait. The purpose of this study is to evaluate and classify knee condition of ACL deficient patients using a nonlinear dynamic method. The nonlinear method focuses on understanding how variations in the gait pattern change over time. The experiments were carried out for 17 subjects(l2 healthy subjects and five subjects with unilateral deficiency) walking on a motorized treadmill for 100 seconds. Three dimensional kinematics of the lower extremity were collected by using four cameras and KWON 3D motion analysis system. The largest Lyapunov exponent calculated from knee joint flexion-extension time series was used to quantify knee stability. The results revealed the difference between healthy subjects and patients. The deficient knee was significantly unstable compared with the contralateral knee. This study suggests an evaluation scheme of the severity of injury and the level of recovery. The proposed Lyapunov exponent can be used in rehabilitation and diagnosis of recoverable patients.

  • A ganglion cyst is a soft tissue mass that is surrounded by a dense connective-tissue capsule. The capsule is filled with a viscous fluid that is rich in hyaluronic acid and other mucopolysaccharides. But, Ganglion cysts in the knee joint are rare. There are very few case reports of ganglion cysts related to the surface of the anterior cruciate ligament, Posterior cruciate ligament and medial meniscus. We are reporting a case of a ganglion cyst in the anterior aspect of the anterior cruciate ligament accompanying with discoid lateral meniscus in the right knee of a 46-year-old woman without any history of trauma. The cyst and discoid lateral meniscus were treated successfully with arthroscopic excision and partial meniscectomy.

  • Forefoot injuries are common in runners or martial art athletes, but due to the anatomical stability, collateral ligament injury of the hallucal interphalangeal joint has been rarely reported. We report a college Taekwondo athlete with chronic varus instability of the hallucal interphalangeal joint due to chronic lateral collateral ligament rupture. The patient had been treated with lateral ligament reconstruction and achieved good clinical outcome.

The Effects of Korean Medicine Treatment on Posterior Cruciate Ligament Avulsion Fracture: A Case Report

  • The purpose of this case study was to use Korean medicine to treat posterior cruciate ligament avulsion fracture. The patient suffered left knee pain resulting from an avulsion fracture and was hospitalized for 59 days and treated with acupuncture and traditional Korean medicine. The effect of the treatment was evaluated using the Numerical Rate Scale, the Western Ontario and McMaster universities Osteoarthritis Index, and the European Quality of Life Five Dimensions. Following treatment of the pain in the left knee, scores for Numerical Rate Scale, Western Ontario and McMaster universities Osteoarthritis Index, and European Quality of Life Five Dimensions were improved. This study suggested that Korean Medicine could be an effective treatment for posterior cruciate ligament avulsion fractures. Further studies are needed to substantiate this conclusion.

  • Reports of localized giant cell tumor arising from the posterior cruciate ligament are rare, and its recurrence rate after incomplete excision. We experienced a case of localized giant cell tumor arising from the posterior cruciate ligament that were excised arthroscopically with transseptal approach and pathologically confirmed. The patient showed good clinical result for one year after operation. So we report a case with a review of the literatures.

  • Pretibial ganglion cyst after arthroscopic anterior cruciate ligament reconstruction is a very rare complication, and a few are reported in and outside the country. Some authors report that it may break out with a variety of causes at any time. We experienced a case of pretibial ganglion cyst after arthroscopic anterior cruciate ligament reconstruction using Achilles tendon allograft and bioabsorbable interference screw in 34-year-old male. So we report this case including the cause and the treatment with a review of the literature.

  • 45% of the sports accidents is the knee damage and the representative case is the damage of an Anterior Cruciate Ligament (ACL) and the Posterior Cruciate Ligament(PCL). Although the past different views of ACL reconstruction comes to an agreement, the disputes of PCL is remained yet. The most important engineering approach for these various surgery techniques is accurately to understand and to evaluate the fatigue behavior depending on the stress flow and the stress distribution under the allotted load and the cyclic load, which are caused by the graft fixing device, the proximal tibia of the PCL reconstructing structure. Therefore, this study is the basic research of these above facts. The current transtibial tunnel surgery using the cadaveric Achilles tendon grafts is chosen for the various PCL reconstruction. The relationships between the slippage, the extension ratio, and the slippage ratio by the heel bone fixing method and the soft tissue fixing method of the Achilles tendon were also defined. This research will be the essential data to help the resonable operating techniques for the next PCL reconstruction.

  • A torn anterior cruciate ligament(ACL) is the most common serious ligamentous injury to the knee joint. The incidence of ACL tears seems to be increasing, at least partly as a result of the increasing participation of individuals of all ages in high-risk sports. The most commonly used graft source for ACL reconstruction is the autogenous bone-patellar tendon-bone graft unit. Despite a good success record. postoperative complications, such as infection, patellar contracture/patellar baja, patellar fracture, rupture of the patellar tendon, graft failure without reinjury, can occur following ACL surgery. The purpose of this paper is to provide guidelines regarding the key points of the reconstructive procedure in a sequence and how to prevent or minimize the complications that can follow ACL reconstructive surgery. We want this knowledge can help orthopaedic surgeons to understand the reasons for previous and current successes and failures of reconstruction of the ACL, and it can help them to plan the care of patients who have an injury of the ligament.
