Clinical and biological analysis in graftless maxillary sinus lift

  • Maxillary sinus lift for dental implant installation is a well-known and versatile technique; new techniques are presented based on the physiology of intrasinus bone repair. The aim of this review was to determine the status of graftless maxillary sinus lift and analyze its foundations and results. A search was conducted of the literature between 1995 and 2015 in the Medline, ScienceDirect, and SciELO databases using the keywords "maxillary sinus lift," "blood clot," "graftless maxillary sinus augmentation," and "dental implant placement." Ten articles were selected for our analysis of this technique and its results. Despite the limited information, cases that were followed for at least six months and up to four years had a 90% success rate. Published techniques included a lateral window, elevation of the sinus membrane, drilling and dental implant installation, descent of the membrane with variations in the installation of the lateral wall access and suturing. The physiology behind this new bone formation response and the results of the present research were also discussed. We concluded that this is a promising and viable technique under certain inclusion criteria.

Rudder Gap Cavitation Suppression Using Gap Flow Blocking Devices

  • Development of rudder gap flow blocking device for lift augmentation and cavitation suppression is presented. In order to verify the performance of this device, cavitation visualization and surface pressure measurements were carried out in a cavitation tunnel. Numerical simulations were conducted using a computational fluid dynamics code for more rigorous verification. The new rudder system is equipped with cam devices, which effectively close the gap between the horn/pintle and movable wing parts. The experimental and computational results show that the proposed rudder system is superior to the conventional rudder systems in terms of the lift augmentation and cavitation suppression.

A radiographic evaluation of graft height changes after maxillary sinus augmentation

  • Purpose: The aims of the present study were to quantitatively assess graft height changes after sinus lift procedures and to analyze the factors that influenced graft height changes, including the residual bone height before surgery, surgical approach, and tooth type. Methods: A total of 39 maxillary posterior implants placed during a simultaneous sinus lift procedure were evaluated. Panoramic radiographs of all patients were taken immediately after implant installation and at 3 months, 6 months, 1 year, 2 years, and 3 years. To analyze graft height changes over time, we measured the distance between the implant platform and the base of the grafted sinus floor at 3 locations. The radiographs were analyzed by a single examiner. Results: Graft height tended to decrease over time, and a statistically significant difference was observed at 2 years compared to baseline (P<0.05). There was no statistically significant difference in graft height change according to the surgical approach or tooth type. For residual bone height, a statistically significant difference in graft height change was found between those with 4-7 mm of residual bone height and those with ${\geq}7mm$ (P<0.05). Conclusions: Graft height after sinus lift procedures significantly decreased at 2 years compared to baseline after sinus augmentation. Further studies should be done with controlled variables, and prospective studies with 3-dimensional images are needed to clarify the factors that influence graft height changes.

Lateral approach for maxillary sinus membrane elevation without bone materials in maxillary mucous retention cyst with immediate or delayed implant rehabilitation: case reports

  • This case series study demonstrates the possibility of successful implant rehabilitation without bone augmentation in the atrophic posterior maxilla with cystic lesion in the sinus. Sinus lift without bone graft using the lateral approach was performed. In one patient, the cyst was aspirated and simultaneous implantation under local anesthesia was performed, whereas the other cyst was removed under general anesthesia, and the sinus membrane was elevated in a second process, followed by implantation. In both cases, tapered 11.5-mm-long implants were utilized. With all of the implants, good stability and appropriate bone height were achieved. The mean bone level gain was 5.73 mm; adequate bone augmentation around the implants was shown, the sinus floor was moved apically, and the cyst was no longer radiologically detected. Completion of all of the treatments required an average of 12.5 months. The present study showed that sufficient bone formation and stable implantation in a maxilla of insufficient bone volume are possible through sinus lift without bone materials. The results serve to demonstrate, moreover, that surgical treatment of mucous retention cyst can facilitate rehabilitation. These techniques can reduce the risk of complications related to bone grafts, save money, and successfully treat antral cyst.

Perception of upper lip augmentation utilizing simulated photography

  • Background No head to head comparison is available between surgical lip lifting and upper lip filler injections to decide which technique yields the best results in patients. Despite the growing popularity of upper lip augmentation, its effect on societal perceptions of attractiveness, successfulness and overall health in woman is unknown. Methods Blinded casual observers viewed three versions of independent images of 15 unique patient lower faces for a total of 45 images. Observers rated the attractiveness, perceived success, and perceived overall health for each patient image. Facial perception questions were answered on a visual analog scale from 0 to 100, where higher scores corresponded to more positive responses. Results Two hundred and seventeen random observers with an average age of 47 years (standard deviation, 15.9) rated the images. The majority of observers were females (n=183, 84%) of white race (n=174, 80%) and had at least some college education (n=202, 93%). The marginal mean score for perceived attractiveness from the natural condition was 1.5 points (95% confidence interval [CI], 0.9-2.18) higher than perceived attractiveness from the simulated upper lip filler injection condition, and 2.6 points higher (95% CI, 1.95-3.24) than the simulated upper lip lift condition. There was a moderate to strong correlation between the scores of the same observer. Conclusions Simulated upper lip augmentation is amenable to social perception analysis. Scores of the same observer for attractiveness, successfulness, and overall health are strongly correlated. Overall, the natural condition had the highest scores in all categories, followed by simulated upper lip filler, and lastly simulated upper lip lift.

Survival rate of Astra Tech implants with maxillary sinus lift

  • Objectives: The aim of this study was to retrospectively evaluate the clinical survival rate of Astra Tech implants in the maxillary molar region performed with sinus lift and bone graft. Materials and Methods: Ninety-nine Astra Tech implants (Osseospeed) placed in the maxillary molar region using sinus lift from September 2009 to February 2012 were selected with a minimum follow-up period of 1 year. The height of alveolar bone, sinus approach technique, bone material and implant survival rate were evaluated. Results: Of the 99 implants, the survival rate was 90.9%; 8 implants failed within 1 year after implant placement, and 1 implant failed 1 year after implant loading. All failed implants were placed with sinus lift simultaneously. The average height of alveolar bone before implant placement was 6.9 mm, while the height of alveolar bone of failed implants was 2.1 mm, on average. Conclusion: Astra Tech implants placed in the maxillary molar region had generally good survival rates, but the relationship between reduced pre-implant alveolar bone height and implant failure requires further attention.

Aerodynamic characteristics of NACA 4412 airfoil section with flap in extreme ground effect

  • Wing-in-Ground vehicles and aerodynamically assisted boats take advantage of increased lift and reduced drag of wing sections in the ground proximity. At relatively low speeds or heavy payloads of these craft, a flap at the wing trailing-edge can be applied to boost the aerodynamic lift. The influence of a flap on the two-dimensional NACA 4412 airfoil in viscous ground-effect flow is numerically investigated in this study. The computational method consists of a steady-state, incompressible, finite volume method utilizing the Spalart-Allmaras turbulence model. Grid generation and solution of the Navier-Stokes equations are completed using computer program Fluent. The code is validated against published experimental and numerical results of unbounded flow with a flap, as well as ground-effect motion without a flap. Aerodynamic forces are calculated, and the effects of angle of attack, Reynolds number, ground height, and flap deflection are presented for a split and plain flap. Changes in the flow introduced with the flap addition are also discussed. Overall, the use of a flap on wings with small attack angles is found to be beneficial for small flap deflections up to 5% of the chord, where the contribution of lift augmentation exceeds the drag increase, yielding an augmented lift-to-drag ratio.

  • DUP(direct underside pressurization)는 위그선이 이륙하기 위하여 수면 위를 항주할때 낮은 속도에서 이륙할 수 있도록 양력을 증가시키는 역할을 한다. 이러한 DUP가 위그선의 안정성과 공기역학적인 힘에 미치는 영향에 관하여 3차원 수치해석을 수행하였다. 해석에 사용된 위그선인 "Aircat"은 동체의 중심에 프로펠러가 있고 동체하부에 공기부양실이 있으며 Lippisch형의 날개와 큰 T자형의 수평꼬리날개를 가지고 있다. 저속에서 대부분의 양력은 프로펠러에 의해 가속된 공기가 동체의 가운데 통로를 통하여 공기부양실로 들어가 정체됨으로써 발생한다. 그러나 DUP에 의해 가속된 공기는 동압의 증가로 인하여 항력을 증가 시킬 뿐만 아니라 안정성에 영향을 주게 된다.

콴다 효과를 이용한 혼-타의 양력성능 개선에 관한 연구 (A Study for Improvement of Lift Performance of a Horn-type Rudder with the Coanda Effect)

  • The Coanda effect is noticeable when a jet stream is applied tangential to a curved surface since then the jet stream remains attached to the surface beyond the point where flow separates otherwise and results in augmentation of circulation and lift. Numerous experimental and numerical studies have been performed in various fields of aerodynamics to exploit the Coanda effect and many of them found to be useful. It can be speculated that the Coanda effect may have practical application to the field of marine hydrodynamics as well since various control surfaces are being used to control behaviors of ships and offshore structures. In the present study, the Coanda effect has been applied to a horn type rudder and a series of numerical computations and model experiments are performed to find the practical applicability. The results indicate that the Coanda jet increases the lift coefficient of the rudder as much as 52% at the momentum coefficient $C_j$ = 0.1 and the rudder angle ${\alpha}=10^{\circ}$.

임프란트 식립을 위한 상악동 거상술의 임상적 연구 (A CLINICAL STUDY OF MAXILLARY SINUS LIFT FOR DENTAL IMPLANT)

  • A variety of materials and procedures such as sinus floor elevation, sinus-lift graft, inlay bone graft using Le Fort I osteotomy, and onlay graft have been used to create adequate bone volume in the maxillary sinus for placement of endosseous implants in the posterior atrophic maxilla. Because of the frequent lack of bone in the posterior maxilla, sinus lift procedure has become a commonly practiced treatment modality. The 138 endosseous implants of 36 patients with sinus augmentation procedures performed in Korea University Hospital from January 1991 to December 1998 were summarized and analysed. The result of this study were as follows: 1. Age ranged from 39 to 57, with a mean of 50.7. 2. The mean survival rate for 138 implants with maxillary sinus lift procedure was 80.4%. 3. There was no corelationship between the fixture length, width and the survival rate. 4. The result showed that the healing period for 8-12 months was necessary if the residual alveolar bone height was less than 5mm. 5. Autogenous iliac corticocancellous block graft showed the most favorable survival rate(95%).
