뇌졸중 재발예방을 위한 생활양식조절 코칭 프로그램의 효과 (The Lifestyle Modification Coaching Program for Secondary Stroke Prevention)

  • Purpose: The purpose of this study was to investigate the effects of the lifestyle modification coaching program on self efficacy, lifestyle and physiologic indexes related to the recurrence of stroke in patients with stroke. Methods: Sixty-one patients with stroke registered with a stroke center participated in this study. Of the participants, 32 were assigned to the experimental group and 29 to the control group. Self efficacy, alcohol drinking, smoking, physical activity, BMI (body mass index), WHR (waist-hip ratio), blood lipid level, and blood pressure were measured both for the baseline, as well as after intervention. The lifestyle modification coaching program consisted of an 8-week telecoaching session following face-to-face education. The control group received only the face-to-face education. Results: There were significant differences in physical activities, WHR, blood pressure within and between groups after intervention. The lifestyle modification coaching program had significant influences on blood pressure even after gender, age, and physical activity had been adjusted. Conclusion: The results of the study indicate that lifestyle modification coaching program is effective for physical activity, abdominal obesity and blood pressure. Therefore it can be used by nurses in hospitals and communities as one of the secondary stroke prevention programs for patients with stroke.

  • Objectives: This study was conducted to expand evidence-base for the effectiveness of lifestyle intervention programs to prevent the onset of diabetes and cardiovascular diseases. Methods: Seven intervention studies between 2006 and 2014 at Korea Association of Health Promotion were analyzed. All the studies were randomized controlled trials(RCTs) and included multi-component behavioral interventions. The participants of the programs were 2,172 adults with risk factors regarding metabolic syndrome criteria. The proportions of normalized participants were compared within and across the studies using odds ratio effect sizes. Results: The reductions in the prevalence of metabolic syndrome were from 49.6% to 65.1% in intervention groups, and from 38.7% to 52.3% in comparison groups. Significant differences in effectiveness between groups were found in two studies, one in 2006 with odds ratio of 1.69(p<0.01) and another in 2009 with odds ratio of 2.36(p<0.001). Proportions of normalized participants were higher in blood pressure(31.9% to 52.5% in the intervention groups and 23.0% to 43.3% in comparison groups) than other risk factors. Abdominal obesity showed weakest improvement after the intervention in both groups. Conclusions: Lifestyle modification program is an effective method to reduce diabetes and cardiovascular risks in adults by decreasing the prevalence of metabolic syndrome and its components.

심뇌혈관질환 예방을 위한 생활습관 개선 프로그램에 관한 문헌분석 연구 (The Literature Review on Life style Intervention Program for the Prevention of Cardiovascular Disease)

  • 본 연구는 심뇌혈관질환 예방을 위한 생활습관 중재 프로그램의 특성을 파악하기 위하여 프로그램 중재 연구논문을 분석한 문헌분석 연구이다. 연구대상 논문은 Pubmed 등 전자 데이터베이스를 통해 "심뇌혈관질환", "중재" 등의 검색어를 적용하여 최종 18편의 논문이 선정되었다. 대상 문헌에서의 프로그램 참여 대상자 선정기준은 주로 신체생리학적 기준이 이용되었고, 신체활동과 식습관 생활습관 영역을 중재한 프로그램이 많았다. 중재유형은 교육이 가장 많이 이용되었고, 여러 중재를 병합하여 적용하는 연구가 많은 것으로 나타났다. 중재기간은 평균 22주 이상의 장기개입이 많았고, 중재효과를 측정하는 효과지표는 신체생리학적 지표가 가장 많이 활용되었다. 본 연구를 통해 여러 생활습관의 상호관련성을 고려한 중재 프로그램의 개발과 건강한 생활습관을 유지하기 위한 환경 및 사회적 지원이 필요함을 제언한다.

Wheel of Wellness Counseling in Community Dwelling, Korean Elders: A Randomized, Controlled Trial

  • Purpose: The purpose of this study was to investigate the effects of Wheel of Wellness counseling on wellness lifestyle, depression, and health-related quality of life in community dwelling elderly people. Methods: A parallel, randomized controlled, open label, trial was conducted. Ninety-three elderly people in a senior welfare center were randomly assigned to two groups: 1) A Wheel of Wellness counseling intervention group (n=49) and 2) a no-treatment control group (n=44). Wheel of Wellness counseling consisted of structured, individual counseling based on the Wheel of Wellness model and provided once a week for four weeks. Wellness lifestyle, depression, and health-related quality of life were assessed pre-and post-test in both groups. Results: Data from 89 participants were analyzed. For participants in the experimental group, there was a significant improvement on all of the wellness-lifestyle subtasks except realistic beliefs. Perceived wellness and depression significantly improved after the in the experimental group (n=43) compared to the control group (n=46) from pre- to post-test in the areas of sense of control (p =.033), nutrition (p =.017), exercise (p =.039), self-care (p <.001), stress management (p =.017), work (p =.011), perceived wellness (p =.019), and depression (p =.031). One participant in the intervention group discontinued the intervention due to hospitalization and three in the control group discontinued the sessions. Conclusions: Wheel of Wellness counseling was beneficial in enhancing wellness for the community-dwelling elderly people. Research into long-term effects of the intervention and health outcomes is recommended.

Lifestyle intervention might easily improve blood pressure in hypertensive men with the C genotype of angiotensin II type 2 receptor gene

  • BACKGROUND/OBJECTIVES: Recent studies have reported an association of the angiotensin II type 2 receptor (AT2R) 3123Cytosine/Adenine (3123C/A) polymorphism with essential hypertension and cardiovascular diseases. The purpose of the study was to investigate whether the AT2R 3123C/A polymorphism affects blood pressure for free-living hypertensive men during a 5-month intervention period. SUBJECTS/METHODS: The subjects were free-living hypertensive Japanese men aged 40 to 75 years who agreed to intervention in the period from 2004 to 2011. Detection of the AT2R 3123C/A polymorphism was determined by polymerase chain reaction. The dietary intervention was designed to decrease salt level and to increase potassium level through cooking instructions and self-monitoring of the diet. The exercise session consisted of activities such as stretching, resistance training, and walking. Blood pressure, urinary sodium and potassium excretion, dietary and lifestyle data, and non-fasting venous blood sample were collected at baseline and after the intervention period. RESULTS: Thirty nine subjects were eligible for participation and the follow-up rate was 97.4%. The C allele proportion was 57.9%. AT2R 3123C/A polymorphism was X-chromosome-linked, therefore we analyzed the C and A genotypes. At baseline, no significant differences were observed between the genotype groups. After the intervention, there were no significant differences in lifestyle habit between the groups. Nevertheless, the estimated salt excretion (g/day) was significantly decreased only in the C genotype (13.0-10.3, P = 0.031). No significant change was observed in systolic blood pressure (SBP) (mmHg) in the A genotype, but a significant decrease was observed in the C genotype (150.0-141.5, P = 0.024). CONCLUSTIONS: In the C genotype, it might be easy to improve SBP through lifestyle intervention in free-living hypertensive Japanese men, however generalization could not be achieved by the small sample size.

중.고등학교 교사의 자기효능감이 건강증진 생활양식에 미치는 영향 (Effects of Self-Efficacy on Health Promotion Lifestyle in Teachers)

  • Purpose: The purpose of this study was to examine the relationships between self-efficacy and health promotion lifestyle in middle and high school teachers. Methods: This study used survey data from 26 middle and high schools The study included 181 teachers who completed questionnaires. The questionnaires were consisted of demographic and occupational characteristics, self-efficacy on health behaviors, and Health Promotion Lifestyle Profile (HPLP). Analyses were done using frequency, percentage, correlation, and multiple regression analysis with dummy variables. SAS 8.2 was used. Results: Mean self-efficacy score on health behaviors was 4.1${\pm}$0.5. Mean health promotion lifestyle scores were healthy diet (2.4${\pm}$0.5), physical activity (2.0${\pm}$0.8), stress management (2.3${\pm}$0.5), self-fulfillment (2.9${\pm}$0.5), responsibility of health (2.3${\pm}$0.6), and personal relationship (2.7${\pm}$0.6). Self-efficacy was significantly related to all health promotion lifestyle scores (healthy diet, physical activity, stress management, self-fulfillment, responsibility of health, and personal relationship). Among demographic and occupational characteristics, sex and school level was significantly related to healthy diet. Sex was significantly associated with physical activity. Marital status was significantly related to responsibility of health. Conclusions: The results showed that intervention programs for middle and high school teachers targeting health promotion lifestyle are needed. These intervention programs would be effective when sex, age, marital status, and school level are considered. In addition, given that higher self-efficacy was related to higher health promotion lifestyle scores, it strengthens the need for further investigations aimed at how to change self-efficacy in teachers.

Effects of Energy Conservation Strategies on Cancer Related Fatigue and Health Promotion Lifestyle in Breast Cancer Survivors: a Randomized Control Trial

  • Purpose: Cancer-related fatigue is frequently experienced by patients during and after therapy. The present study was conducted to assess the impact of energy conservation strategies and health promotion in breast cancer survivors. Methods: A randomized controlled trial was carried out to compare the intervention effect (n=69.0) with controls (n=66.0) based on routine oncology ward care. The intervention was five weekly sessions for groups of 6-8 breast cancer survivors. Data on fatigue and health promotion lifestyle were obtained before and after completion the intervention and then 8 weeks later for analysis of variance (ANOVA) with repeated measures. Results: Our findings showed cancer-related fatigue to be reduced in the intervention group from pre- to post-intervention, and this persisted over the 8-weeks follow-up period (F = 69.8, p<0.001). All subscales of the cancer fatigue scale demonstrated statistically significant effects with partial eta-squared values ranging from 0.15 (the smallest effect in cognitive fatigue) to 0.21 (the largest for affective fatigue). Changes in the health promotion life style indicated a significant promotion from pre- to post-intervention, and this again continued after 8-weeks follow-up (F = 41.6, p < 0.001). All six domains of a health promoting life style featured significantly elevated values, the largest effect being seen in the interpersonal relations subscale (F=57.7, partial ${\eta}^2=0.21$, p<0.001) followed by physical activity (F=51.9, partial ${\eta}^2=0.18$, p<0.001). Conclusions: The program was effective in decreasing cancer related fatigue and promoting a healthy lifestyle.

성인의 생활양식과 건강관련 삶의 질에 대한 연구 (Lifestyle Characteristics and Health Related Quality of Life in Korean Adult)

  • Objectives: The purpose of this study was to investigate the relationship between Korean lifestyle characteristics and health status and to identify the variables influencing health in Korea. Methods: A cross-sectional descriptive correlational design was used to explore the lifestyle characteristics and health status of 397 Korean adults. Correlational analysis calculated the correlation between lifestyle and health status. To examine the relationship among demographic characteristics, lifestyle, and health status we used the t-test and one-way ANOVA. Stepwise multiple regression was conducted to examine the significant predictors of general health among subjects. Results: Positive correlations were seen between general health (GH) and the overall score and subscales of the Lifestyle. The stepwise regression model showed that vitality (VA), body pain (BP), nutrition, and occupation were significant variables influencing general health (GH). Conclusions: These findings provide evidence regarding the lifestyle patterns and healthstatus among Koreans. When planning intervention strategies for this population, exercise and physical activity should be principal focus areas.

지역사회 거주 노인의 건강관리를 위한 작업기반 라이프스타일 중재 프로그램의 효과 (Effectiveness of Occupation Based Lifestyle Intervention Program for Health Management of Community Dwelling Older Adults)

  • 목적 : 본 연구는 작업기반 라이프스타일 중재 프로그램이 지역사회 노인에게 미치는 효과를 알아보고 적용가능성을 알아보기 위해 진행되었다. 연구방법 : 지역사회에 거주하고 있는 노인 9명을 대상으로 진행되었으며 단일집단 사전-사후 검사 설계를 사용하여 연구를 진행하였다. 참가자들은 해외 연구를 통해 효과가 확인된 라이프스타일 리디자인 프로그램(Lifestyle redesign program)을 기반으로 한 작업기반 라이프스타일 중재 프로그램을 그룹회기 12회, 개인회기 1회로 총 7주에 걸쳐 제공받았다. 효과를 알아보기 위해 작업균형, 시간사용, 활동참여 수준, 활동 수행도 및 만족도, 우울, 건강관련 삶의 질에 대한 평가를 프로그램 전, 후로 진행하고 평가 결과를 분석하였다. 또한 후기평가 시 프로그램에 대한 설문을 진행하여 참가자들이 생각하는 주관적인 프로그램의 타당성, 필요성, 효과성에 대해 알아보았다. 결과 : 초기평가와 후기평가를 통해 얻은 자료를 분석한 결과 전반적인 작업균형(p=.012)과 하위영역인 건강(p=.017), 관계(p=.012), 도전 및 흥미(p=.012), 자아정체성(p=.012)에 대한 작업균형 그리고 활동참여수준(p=.008), 활동 수행도(p=.012) 및 만족도(p=.008), 전반적인 건강관련 삶의 질(p=.034) 및 하위영역 중 신체적 건강(p=.041)이 향상되었으며, 여가(p=.008)와 휴식(p=.008)에 대한 시간사용에 변화가 있었고, 마지막으로 우울(p=.012)이 감소한 것을 확인할 수 있었다. 또한 프로그램에 대한 설문 결과 참가자들이 생각하는 프로그램에 대한 주관적 타당성, 필요성, 효과성 모두 적절한 수준을 나타내는 것을 확인하였다. 결론 : 작업기반 라이프스타일 중재프로그램은 지역사회 노인에게 충분히 받아들여질 수 있으며 건강에 긍정적인 영향을 미칠 수 있는 중재법으로 사료된다.

중년기 성인의 자기 효능감 및 건강관련 강인성이 건강증진 생활양식에 미치는 영향 (Effects of Self Efficacy, Health Related Hardiness on a Health Promoting Lifestyle of Middle-aged People)

  • Purpose: The purpose of this study was to investigate the effects of self efficacy, and health related hardiness on a health promoting lifestyle of middle-aged people Method: The study was designed as a descriptive survey study. Data was collected using a structured questionnaire which included general characteristics, measure of self efficacy, health related hardiness, and health promoting lifestyle. Data collection was done between May and August, 2007 with 197 middle-aged participants. Result: The average score of the participants for self efficacy was 34.78, for health related hardiness 57.01 and for a health promoting lifestyle 127.53. There were positive correlations between self efficacy and a health promoting lifestyle, health related hardiness and a health promoting lifestyle, and self efficacy and health related hardiness. The correlations were statistically significant. Health related hardiness was a significant predictors of a health promoting lifestyle. Conclusion: A strategy for improving self efficacy and health related hardiness should be developed and a nursing intervention program should also be provided based on the developed strategy of middle-aged people.
