본 논문은 노인요양시설에서 생활하는 고령자를 대상으로 12주간 실시한 근력운동 프로그램(주 3회, 회당 60분)이 노인들의 건강관련 체력 및 대사증후군 요인에 미치는 영향을 살펴보기 위하여 실시되었다. 총 27명의 대상자(운동군 15명, 대조군 12명)가 참여하였으며 연구의 결과는 다음과 같다. 12주간의 근력운동 프로그램은 노인의 건강관련 체력요인을 유의하게 증가시켰으며, 특히 근력 및 평형성, 그리고 유연성의 증가가 보행능력을 크게 향상시켜 낙상을 예방하는데 도움이 될 수 있을 것으로 생각된다. 그리고 노인요양시설에서 근력운동이 노인들의 혈압과 혈당을 유의하게 감소시켜 대사증후군의 위험인자를 긍정적으로 개선시킴으로써 건강수준을 증가시키는 것으로 나타났다.
최근 한국사회의 고령화 및 노인장기요양 보험제도의 시작으로 노인 신체기능 및 장애에 대한 관심이 증가한 가운데, 이에 영향을 미치는 요인 중 하나로 근위축비만(Sarcopenic obesity) 즉, 근육양 감소와 지방량 증가가 동시에 나타나는 현상에 대한 관심이 증가하고 있다. 비만과 근 위축 각각이 노인의 신체기능에 미치는 영향에 대한 연구는 존재하나, 근위축 비만이 노인의 신체기능 및 체력에 미치는 영향에 관한 연구는 극히 제한적이다. 이에 본 연구는 경기도 Y시에 자립적으로 거주하는 60세 이상의 여성고령자 257명(평균연령 74.5세)의 신체조성을 측정한 후 Davison의 분류법을 이용하여 정상체지방군, 높은 체지방군, 근위축군, 근위축비만군의 4개 군으로 분류하고 7개의 노인신체기능측정(의자에서 앉았다 일어서기, 아령들기, 상완 유연성, 정적형성, 2분 제자리 걷기, 하지 유연성 및 8 feet 앉았다 일어서서 돌아오기(8ft TUG))검사항목과 측정장치를 이용한 4개의 기초체력항목(악력, 자세안정성, 무릎신근력 및 굴근력)에 미치는 영향을 조사하였다. SPSS 12.0을 이용하여 일원배치분산분석과 다중비교검정을 위한 LSD 사후검정을 실시하였으며 유의수준은 5%에서 검정하였다. 정상군의 의자에서 앉았다 일어서기, 아령들기, 상완유연성, 2분 제자리 걷기 결과는 근위축군(p<.05)과 근위축비만군(p<.05)보다 유의하게 높았다. 정적평형성은 정상군이 근위축비만군에 비하여 유의하게 높았으며(p<.01), 근위축군이 근위축비만군보다 유의하게 높았다(p<.01). 8ft TUG는 근위축비만군만이 정상군에 비하여 유의하게 낮은 결과를 보였다(p<.01). 악력(F=20.87,p<.001), 60도에서의 최대 신근력은 정상군이 근위축군(p<.01)과 근위축비만군(p<.01)보다 유의하게 높았으며 60도에서의 최대 굴극력역시 정상군이 근위축군(p<.01)과 근위축비만군(p<.01)보다 유의하게 높은 결과를 보였다. 본 연구의 결과는 노후의 자립적 삶을 영위하기 위한 기능유지의 위험인자로서 비만상태보다는 근위축비만군임을 입증하였으며, 노후 신체허약의 위험인자로 근위축비만에 대한 중재가 필요함을 보여주고 있다.
Purpose: The purpose of this study was to determine the effect of EPMLM(educational program of manual lymph massage) on the arm functioning and QOL(quality of life) in breast cancer patients with lymphedema. Method: Subjects in the experimental group(n=20) participated in EPMLM for 6 weeks from June to July, 2005. The EPMLM consisted of training of lymph massage for 2 weeks and encourage and support of self-care using lymph massage for 4 weeks. The arm functioning assessed at pre-treatment, 2weeks, and 6weeks using Arm functioning questionnaire. The QOL assessed at pre-treatment and 6 weeks using SF-36. The outcome data of experimental group was compared with control group(n=20). The collected data was analyzed by using SPSS 10.0 statistical program. Result: The arm functioning of experimental group was increased from 2 weeks after(W=.224, p=.011) and statistically differenced with control group at 2 weeks(Z=-2.241, p=.024) and 6 weeks(Z=-2.453, p=.013). Physical function of QOL domain increased in experimental group(Z=-1.162, p=.050), also statistically differenced with control group(Z=-2.182, p= .030) at 6weeks. Conclusion: The results suggest that the educational program of manual lymph massage can improve arm functioning and physical function of QOL domain in breast cancer patients with lymphedema.
Purpose: The purpose of this study was to understand and describe the experiences of exercise maintenance in elderly with osteoarthritis (OA). Methods: Three focus group interviews were conducted with a total of 19 older adults with OA who have been exercising for more than six months. Results: Two main themes emerged from the analysis: the changing exercise in life's journey (exercise maintained by self and family affection, exercise maintained by social networking expansion, exercise changing for function and body-image and exercise changing for economic efficiency) and the exercise pervaded into the life (Exercise used as a side effect-free treatment, exercise internalized by will and a sense of accomplishment and exercise become daily routine habits). Conclusion: Despite the various problems associated with aging and OA, participants made exercise a part of their lives by changing it to fit their own functions, abilities and circumstances. It is necessary to emphasize the positive aspects of the overall situation and to provide appropriate education and exercise based on the theory as well as the individual's overall fitness and limitations.
In order to improve the efficiency of the design process and the quality of the resulting design, this study proposes a design method for determining design variables of an automotive wheel-bearing unit of double-row angular-contact ball bearing type by using a genetic algorithm. The desired performance of the wheel-bearing unit is to maximize system life while satisfying geometrical and operational constraints without enlarging mounting spae. The use of gradient-based optimization methods for the design of the unit is restricted because this design problem is characterized by the presence of discrete design variables such as the number of balls and standard ball diameter. Therefore, the design problem of rolling element bearings is a constrained discrete optimization problem. A genetic algorithm using real coding and dynamic mutation rate is used to efficiently find the optimum discrete design values. To effectively deal with the design constraints, a ranking method is suggested for constructing a fitness function in the genetic algorithm. A computer program is developed and applied to the design of a real wheel-bearing unit model to evaluate the proposed design method. Optimum design results demonstrate the effectiveness of the design method suggested in this study by showing that the system life of an optimally designed wheel-bearing unit is enhanced in comparison with that of the current design without any constraint violations.

Constrains for manufacturing are applied in optimization scheme. Results obtained for several 63 series deep groove ball bearings demonstrated the effectiveness of the proposed design methodology by showing that the average basic dynamic capacities of optimally designed bearings increased about 9-34% compared with the standard ones.
In order to improve the efficiency of the design process and the quality of the resulting design for the application-based exclusive rolling element bearings, this study propose design methodologies by using a genetic-based combinatorial optimization. By the presence of discrete variables such as the number of rolling element (standard component) and by the engineering point of views, the design problem of the rolling element bearing can be characterized by the combinatorial optimization problem as a fully discrete optimization. A genetic algorithm is used to efficiently find a set of the optimum discrete design values from the pre-defined variable sets. To effectively deal with the design constraints and the multi-objective problem, a ranking penalty method is suggested for constructing a fitness function in the genetic-based combinatorial optimization. To evaluate the proposed design method, a robust performance analyzer of ball bearing based on quasi-static analysis is developed and the computer program is applied to some design problems, 1) maximize fatigue life, 2) maximize stiffness, 3) maximize fatigue life and stiffness, of a angular contact ball bearing. Optimum design results are demonstrate the effectiveness of the design method suggested in this study. It believed that the proposed methodologies can be effectively applied to other multi-objective discrete optimization problems.
Purpose: The purpose of this study is to identify health-related factors, especially for the elderly who are subject to visiting health care at vulnerable populations. Methods: Tools were Guide to Community Integrated Health Promotion Project 2016, Visit Health Care Health Interview Survey, measures of physical function, motor skills, composite mobility, BMI, and subjective fitness levels. Depression was measured with the Short Results: Older elders living alone were more vulnerable than those with living others. Elders with less education showed greater weakness but the difference was not significant. Average scores for frailty were 2.21 (healthy group), 7.66 (high-risk group) and 15.69 (frail group). Scores based on weakness level differed significantly with the exception of nutrition. Nine out of 10 elders in disadvantaged areas were in the frail group or at high risk. Conclusion: Results support the goal to maintain/improve physical/mental functions through individual management of high-risk/frail older adults at risk of becoming infirm. It is imperative to implement a public health care delivery system to ensure programs are operated effectively and personalized.
The purpose of this study is to compare the level of health-related quality of life and relating factor between institutional elderly and community living elderly. The subjects were 390 from Sanatorium or Nursing home and 467 from the community in Kwangju. The results are followed : 1) A comparison of ADL between two groups, institutional elderly and community living elderly, resulted in that community elderly were more significantly independent in the areas of bathing and transfer than institutional elderly. 2) A comparison of IADL between two groups resulted in that : Community elderly were more independent in the areas of using telephone and transportation, food preparation, house keeping, and doing laundry. Institutional elderly were more independent in the area of handling finances. 3) In the case of poor health-related quality of life, institutional elderly showed 2.4 times in the dimension of physical fitness, 1.8 times in daily activity, 2 times in social activity, 2 times in pain, 26.7 times in social support, and 0.4 times in subjective quality of life higher than community elderly There was no significant differences in the rest of dimensions. 4) In institutional elderly, the analysis of variables related to the health-related quality of life resulted in that; The relating factors were sex, education, and chronic illness in the dimension of physical function. Direct contact with family or significant others in the dimension of social activity. Chronic illness in the dimension of pain and perceived health status. Throughout general daily activity, community elderly showed more satisfactory results than institutional elderly, but in the subjective area of health-related quality of life, such as subjective quality of life, institutional elderly group showed more positive results. And community elderly had more relating factors than institutional elderly. For the health care of the elderly that focused on quality of life, new approaches considering the characteristics of both group, institutional and community living elderly, are needed.
Purpose: The purpose of this study was to exam me the effects of aquatic exercise on pain, fatigue, body composition, physical fitness and psychological variables in women with arthritis. Method: With a quasi-experimental design, 37 women who had arthritis were assigned into an experimental group (n=19) or a control group (n=18). Aquatic exercise consisted of one hour of exercise in water, two days per week, for six weeks and a self-help process. Measures included physiologic and psychological variables before and after the exercise. Data was analyzed with the SPSS WIN using descriptive statistics, t-test, and ANCOVA. Result: The mean age of the subjects was 59 years in the experimental group and 52 years in the control group. Pretest scores in outcome variables were similar in both groups except BMI and body fat(%). After controlling for age, shoulder flexibility scores in the experimental group improved more than the control group (Rt: F=10.58, p=.003; Lt: F=5.91, p= .02; Waist: F=4.95, p= .03). Depression scores in the experimental group decreased (F=12.96, p= .001), and self efficacy and quality of life improved more than the others (F=5.07, p= .03; F=6.9l, p= .01). Conclusion: Aquatic exercise can improve waist flexibility and shows consistent findings of improved shoulder flexibility and psychological function in women with arthritis.
