On the application of artificial intelligence in acute myeloid leukemia therapy

  • This study is a randomized pretest-posttest design that aims to investigate the effect of early entrepreneurship education on cognitive and non- early entrepreneurship education, non-cognitive skills, creativity, self-efficacy, Bizworld cognitive skills of male sixth-grade primary school students. A total of 45 students were selected by multi-stage random sampling method and were assigned randomly to experimental, active-control and control groups. The experimental group received entrepreneurship education based on the Bizworld entrepreneurship program. The results indicate that early entrepreneurship education had an effect on non-cognitive skills (such as risk taking propensity, creativity, self-efficacy, persistence and need for achievement. It seems that early entrepreneurship education is a proper strategy to develop children's non-cognitive skills in late years of primary school. These skills will affect children's individual, educational, social and occupational future and can have long term benefits for students, families and society.

Aberrant Expression of HOXA5 and HOXA9 in AML

  • Background: Aberrant expression of HOX gene expression has been observed in cancer. The purpose of this study was to investigate the alteration of HOXA5 and HOXA9 expression and their clinical significance in acute meloid leukemia (AML). Materials and Methods: The expression of HOXA5 and HOXA9 genes of bone marrow samples from 75 newly diagnosed AML patients and 22 healthy controls for comparison were examined by Real-time quantitative PCR (RQ-PCR) assay. Statistical analysis was conducted to evaluate HOXA5 and HOXA9 expression as possible biomarkers for AML. Results: The results showed that the complete remission rate (52.6%) of the patients who highly expressed HOXA5 and HOXA9 was significantly lower than that (88.9%) in patients who lowly express the genes (P=0.015). Spearmann correlation coefficients indicated that the expression levels for HOXA5 and HOXA9 genes were highly interrelated (r=0.657, P<0.001). Meanwhile, we detected significant correlations between HOXA9 expression and age in this limited set of patients (P=0.009). Conclusions: The results suggest a prognostic impact of increased expression of HOXA5 and HOXA9 in AML patients.

Association of an LMP2 Polymorphism with Acute Myeloid Leukemia and Multiple Myeloma

  • Hematological malignancies (HM) are a group of neoplasms derived from the cells of the bone marrow and lymphatic system. Genetic factors leading to susceptibility to HM have been investigated for years but little is known yet. Low molecular weight polypeptide (LMP) 2 and LMP7 genes are important subunits of the immunoproteasome and play significant role in antigen presentation. The polymorphisms of LMP genes have been reported to be risk factors for various types of diseases. The aim of this study was to investigate the association of LMP2 and LMP7 polymorphisms with the occurrence of particular types of HM. A total of 132 patients with HM and 130 control subjects were investigated. No significant difference was obtained in the distribution of genotype and allele frequencies of LMP7 gene in HM patients and the control group. On the other hand, the prevalence of LMP2-AA genotype was found to be higher in acute myeolid leukemia (AML) patients while it was significantly lower in multiple myeloma (MM) cases than in the control subjects. Our results suggested that LMP7 could not be a risk factor for susceptibility to HM, whereas LMP2 polymorphisms could play a role in the development of AML and MM.

소아 종양 환아의 혈중 Ghrelin과 Leptin의 농도: 정상 소아와 비교 (Serum ghrelin and leptin concentrations in children with cancer : comparisons with normal children)

  • 목 적 : Ghrelin은 위에서 주로 분비되는 펩타이드로, 성장호르몬의 분비를 촉진시키고, 식욕을 증가시켜 에너지 균형을 조절한다고 알려져 있으며, leptin은 지방세포에서 분비되어 식욕을 억제시키는 작용을 통해 체중 및 에너지 대사에 관여한다. 저자들은 소아 종양으로 진단받고 항암 요법중인 환아의 ghrelin과 leptin 농도를 정상아와 비교해 보았다. 방 법 : 2003년 12월부터 2004년 12월까지 강남 성모병원과 성모병원에서 소아 종양을 진단 받고 방사선 조사나 항암 화학요법을 받은 환아들 중 43명(남아 31명, 여아 12명)을 대상으로 하였으며, 환아군과 성별, 나이, 체중 및 신장이 비슷한 건강한 소아 45명(남아 26명, 여아 19명)을 대조군으로 하였다. 대상아들의 혈중 leptin과 ghrelin 농도를 방사면역측정법으로 측정하였다. 결 과 : 환아군과 대조군에서 BMI와 leptin 농도는 환아군에서 더 높았으며, ghrelin은 대조군에서 더 높았다. 또한 leptin 농도는 환아군(r=0.61, P<0.05)과 대조군(r=0.40, P<0.05) 모두에서 BMI와 양의 상관관계가 있었다. ghrelin 농도는 정상군에서는 연령, 신장, 체중 및 BMI와 음의 상관관계를 보였으나 환아군에서는 의미있는 상관관계는 없었다. 한편, 방사선요법을 받은 군과 화학요법만을 받은 군으로 나누어 비교했을 때에는 두 군간의 의미있는 차이는 없었으며, 급성 골수성 백혈병군과 급성 림프구성 백혈병군으로 나누었을 때 ghrelin의 농도가 골수성 백혈병군에서 훨씬 높았다. 결 론 : 소아 종양으로 항암 요법을 받고 있는 환아는 정상아에 비해 BMI가 더 높고, leptin 농도도 더 높았으나, ghrelin 농도는 더 낮았다. 그러나 본 연구가 한 시점에서의 단기적이고 단편적인 연구이므로 추후 오랜 기간에 걸친 전향적인 연구가 더 필요하다고 생각된다.

소아 백혈병 환자의 동종 조혈모세포이식 전처치로서 전신방사선 조사 포함군과 비포함군의 비교 (Comparison of total body irradiation-based or non-total body irradiation-based conditioning regimens for allogeneic stem cell transplantation in pediatric leukemia patients)

  • 목 적 : 본 연구의 목적은 소아백혈병 환자에서 이식 전처치로서 전신방사선조사(total body irradiation, TBI)군과 비방사선조사(non-TBI)군과의 이식 성적 및 예후를 비교하고자 하였다. 방 법: 1996년 1월에서 2007년 12월까지 전남대학교병원에서 조혈모세포이식을 시행 받은 소아백혈병 환자 77명을 TBI군(n=40)과 non-TBI군(n=37)으로 나누어 각 군 간의 이식 유형, 이식 시 질병상태, 전처치 방법, 이식 세포 수, 생착 속도, 이식편대숙주반응(graft-versus-host disease, GVHD)의 발생빈도, 이식 합병증, 사망원인, 전체생존율(overall survival, OS)과 무사건생존율(event free survival, EFS) 및 후기 합병증을 비교 분석하였다. 결 과 : 급성림프구성백혈병(acute lymphoblastic leukemia, ALL) 환자의 82.4% (28/34)는 TBI를 받았고, 골수계열 백혈병 환자의 72.7% (24/33)는 non-TBI 군이었다. 전체 환자를 대상으로 TBI 여부에 따른 5년 EFS은 두 군간 차이는 없었으나 (62% vs. 63%), ALL 환자에서는 TBI군이 non-TBI군에 비해 통계적으로 유의하게 우수한 5년 EFS을 보였다(65% vs. 17%; $P$=0.005). AML (acute myelogenous leukemia) 환자에서는 non-TBI군이 TBI군보다 더 높은 5년 EFS을 보였으나 통계적인 유의성은 없었다(73% vs. 38 %; $P$=0.089). GVHD 발생률, 생착, 사망원인과 후기 합병증은 두 군간 차이는 없었다. 결 론 : 전처치로서 TBI군과 non-TBI군은 비슷한 결과를 보였으나, ALL환자에서는 TBI군이 non-TBI군에 비하여 유의하게 우수한 5년 EFS을 보였다. 두 군간 후기 합병증의 발생 및 사회경제적 삶의 질을 비교하기 위하여는 많은 환자를 대상으로한 전향적 무작위 연구가 필요하리라 사료된다.

Molecular Target Therapy of AKT and NF-kB Signaling Pathways and Multidrug Resistance by Specific Cell Penetrating Inhibitor Peptides in HL-60 Cells

  • Background: PI3/AKT and NF-kB signaling pathways are constitutively active in acute myeloid leukemia and cross-talk between the two has been shown in various cancers. However, their role in acute myeloid leukemia has not been completely explored. We therefore used cell penetrating inhibitor peptides to define the contributions of AKT and NF-kB to survival and multi drug resistance (MDR) in HL-60 cells. Materials and Methods: Inhibition of AKT and NF-kB activity by AKT inhibitor peptide and NBD inhibitor peptide, respectively, resulted in decreased expression of mRNA for the MDR1 gene as assessed by real time PCR. In addition, treatment of HL-60 cells with AKT and NBD inhibitor peptides led to inhibition of cell viability and induction of apoptosis in a dose dependent manner as detected by flow cytometer. Results: Finally, co-treatment of HL-60 cells with sub-optimal doses of AKT and NBD inhibitor peptides led to synergistic apoptotic responses in AML cells. Conclusions: These data support a strong biological link between NF-kB and PI3-kinase/AKT pathways in the modulation of antiapoptotic and multi drug resistant effects in AML cells. Synergistic targeting of these pathways using NF-kB and PI3-kinase/AK inhibitor peptides may have a therapeutic potential for AML and possibly other malignancies with constitutive activation of these pathways.

Posttransplant Malignancies in Renal Transplant Recipients: 22-years Experience from a Single Center in Pakistan

  • Objective: To study the incidence, types and distribution pattern of malignant tumors in renal transplant recipients at a single center in Pakistan. Materials and methods: This retrospective study was conducted at Sindh Institute of Urology and Transplantation (SIUT) and included all transplant patients on regular follow-up from November 1986 to December 2008. The original biopsy reports and case files of all patients who developed posttransplant malignancies were reviewed and relevant demographic, clinical, radiological, and histopathological data were retrieved and analyzed. SPSS version 10.0 was used for statistical analysis. Results: Over 22 years of study period, 1816 renal transplants were carried out at our center. Among these, 44 patients developed malignancies constituting an overall incidence rate of 2.4%. All patients in this study were males with a mean age of $34.9{\pm}9.5$ years (range: 9 to 60 years). The most common type of malignancy was lymphoma (27 patients, 61.4%), followed by Kaposi's sarcoma (11 patients, 25%) and skin malignancies (3 patients, 6.8%). One case each of adenocarcinoma of the gallbladder, acute myeloid leukemia (AML), conjunctival carcinoma-in-situ and seminoma were also diagnosed. Conclusion: Posttransplant malignancies occurring in our renal transplant recipients show different incidence rates and patterns as compared with western studies.

Successful Lung Transplantation in a Patient with Myasthenia Gravis

  • A 47-year-old man with myasthenia gravis (MG) was admitted for a lung transplant. He had bronchiolitis obliterans after allogeneic hematopoietic stem cell transplantation due to acute myeloid leukemia. MG developed after stem cell transplantation. Bilateral sequential lung transplantations and a total thymectomy were performed. The patient underwent right diaphragmatic plication simultaneously due to preoperatively diagnosed right diaphragmatic paralysis. A tracheostomy was performed and bilevel positive airway pressure (BiPAP) was applied on postoperative days 8 and 9, respectively. The patient was transferred to the general ward on postoperative day 12, successfully weaned off BiPAP on postoperative day 18, and finally discharged on postoperative day 62.

소음인 A형 인플루엔자 감염 치험례 (Treatment of Influenza A Virus Infection in a Soeumin Patient; a Single Case)

  • Objective This study aimed to report an improvement of influenza virus infection managed with Sasang Constitutional Medicine in a Soeumin patient with thrombocytopenia and histories of acute myeloid leukemia and interstitial pulmonary disease. Methods The medical records of patient including vital signs, laboratory test, simple x-ray, and symptoms were reviewed retrospectively. Patient was treated with Gunggwihyangso-san gamibang without any antiviral agents under the diagnosis of Congestive Hyperpsychotic symptomatology of Kidney Heat-based Exterior Heat disease. Results At admission, symptoms of shivering, fever, cough and myalgia were presented and her skin temperature was over $39^{\circ}C$. Influenza A virus was detected and inflammation biomarkers were elevated. After administration of herb medication, her vital signs and symptoms were improved and elevated inflammation biomarkers were decreased. There was no secondary complication during the treatment period. Conclusions This case treated with Gunggwihyangso-san gamibang showed a significant improvement from viral infection without any complication.

Revisiting Use of Growth Factors in Myelodysplastic Syndromes

  • Myelodysplastic syndromes (MDS) represent a heterogeneous group of clonal hematologic neoplasms characterized by morphologic dysplasia, aberrant hematopoiesis and peripheral blood refractory cytopenias. MDS is recognized to be associated with an increased risk of symptomatic anemia, infectious complications and bleeding diathesis, as well as a risk of progression to acute myeloid leukemia, particularly in patients with a high IPSS score. The advent of use of hematopoietic growth factors such as granulocyte colony-stimulating factor (G-CSF) and recombinant erythropoietin (EPO) has improved symptoms in MDS patients in addition to some data that suggest there might be an improvement in survival. G-CSF is an effective therapeutic option in MDS patients, and it should be considered for the management of refractory symptomatic cytopenias. G-CSF and EPO in combination can improve outcomes in appropriate MDS patients such as those with lower-risk MDS and refractory anemia with ring sideroblasts (RARS). This article reviews use of growth factors for lower-risk MDS patients, and examines the data for G-CSF, EPO and thrombopietic growth factors (TPO) that are available or being developed as therapeutic modalities for this challenging disease.
