  • This study reports anti-hierarchical initial-syllable lengthening of an utterance-internal phrase in Korean. That is, the phrase-initial syllable (e.g., /a/ of "apa-do" or /ma/ of "mapa-do") starting with a voiced phoneme (i.e., vowels or voiced consonants) manifests itself as significantly longer when it is preceded by another phrase without a pause than when it leads an utterance or follows a pause utterance-internally. The phenomenon was examined with regard to two other factors: (1) tempo and (2) tenseness of the consonant (/p, $p^{\prime}$, $p^h$/) following the target syllable /a/. First, the effect of tempo on initial lengthening was not significant. Apart from the statistical significance, however, a tendency was observed, i.e., the slower the tempo is, the greater the lengthening. By contrast, the faster the tempo is, the higher the ratio (%) of lengthening. Second, contrary to our expectations, initial-syllable lengthening was even greater before tense stops /$p^{\prime}$, $p^h$/ than before lax stop /p/ regardless of tempo, and it was remarkable when it comes to the ratio (%), which means that initial lengthening is free of the pre-consonantal vowel shortening effect. Final-syllable lengthening is a pre-boundary marker, while the initial-syllable lengthening is regarded as a post-boundary marker of a phrase.

  • Brachymetatarsia is a rare congenital deformity which occurs most commonly in the fourth metatarsal. Most common purpose of surgery is to make a normal metatarsal parabola for better external appearance as well as physiological load bearing under the metatarsal heads. Common surgical treatment for brachymetatarsia is lengthening of short metatarsal either by one-stage lengthening with intercalary bone graft or gradual lengthening by distraction osteogenesis. Sometimes shortening of long metatarsal is combined with lengthening of the short metatarsal. The function of the foot is diminished due to diverse complication accompanying lengthening of the metatarsal, therefore preoperative consultation about the functional result is essential.

  • Purpose: We analysis the outcome and complications of treatment of lesser toe brachymetatarsia. Materials and Methods: We analysed 28 patients 35 cases of lesser toe brachymetatarsia. Mean post operative follow up period was 2 years 8 months. All of the patients were female and mean age at operation was 21 years old. 2 cases of third metatasal bone and 33 cases of fourth metatarsal bone were operated. 8 metatarsal bones were treated using one staged lengthening with tricortical bone graft and 27 metatarsal bones were treated using callotasis with monofixator. Results: The average amount of lengthening was 13.3mm(12mm-15mm) in one staged lengthening, while 14.4mm(4mm-23mm) in callotasis. Average percentile increase was 28.9%(26%-34%) in one staged lengthening and 32%(18%-46%) in callotasis. The average healing index of callotasis was 76 days/cm (41 days/cm-166 days/cm). Satisfied outcomes in 4 cases of 8 cases (50%) after one staged lengthening and 17 cases of 27 cases (63%) after callotasis. 6 complications in 4 cases were occurred after one staged lengthening; insufficient length gain in 3 cases, fracture on the junction of graft bone and metatarsal bone in 1 case, plantar bowing deformity in 1 case and bony fusion of metatarsophalangeal joint in 1 case. 17 complications in 10 patients were occurred after callotasis ; metatarsophalangeal joint stiffness in 8 cases, metatarsophalangeal joint subluxation in 2 cases, overlengthened metatarsal bone in 2 cases, tapering of callus in 1 case, fracture of callus in 1 case, premature consolidation of callus in 1 case, osteomyelitis of metataral head in 1 case and plantar bowing deformity in 1 case. Conclusion: Although one staged lengthening and gradual lengthening using callotasis are effective treatment for lesser toe brachymetatarsia, complications not rarely occured after lengthening. Insufficient lengthening are most common complication after one staged lengthening while metatarsophalangeal joint stiffness lire most common complication after callotasis.

  • Thirty rabbits underwent femoral lengthening using monolateral external fixator to evaluate results and complications of limb lengthening. Twenty rabbits survived until consolidation of callus formed at the lengthening site after finishing lengthening. Ten rabbits were sacrificed during femoral lengthening because of complications. Survived 20 rabbits were classified to two groups according the amount of lengthening: group I (10% lengthening of the femoral length); group II (20% lengthening). There was no significant difference of consolidation time between two groups (p=0.25). Varus angulation at the lengthening site occurred in 60% of two groups and the amount of varus angulation in group II was larger than that of group I. Degenerative change of articular cartilage at the medial condyle of the distal femur was found in 30% of group II. Of sacrificed ten rabbits, 5 had pin loosenings with pull an of pins from the femur, 2 had fermoral fractures around the pin-tract site, and 3 had severe osteomyelitis of the femur around the pin-tract site.

  • Ilizarov limb lengthening has been the new method to deal with a variety of orthopaedic problems. Ilizarov apparatus consists of stainless steel rings that surround the limb and are interconnected by threaded rods. Tensioned wires pierce the bone in the plane of each ring and are tightly attached to the ring. Ilizarov has reported distraction osteogenesis with the use of the typical fixator that allows functional loading. The purpose of this commentary is to introduce a four-stage rehabilitation protocol currently used by the authors in the physical therapy management of a post -limb lengthening. Each treament stage corresponds to a medical stage in the lengthening process. Treatment goals for each of the physical therapy treatment stage are presented to guide treatment planning. Two case examples are presented to illustrate the use of the management goals in treatment planning.

  • This paper aims to reexamine Homorganic Cluster Lengthening in Late Old English whereby OE short vowels became lengthened before specific consonant clusters such as /-ld, -nd, -mb, -rd, -rð, -ng, -rz/. As for the motivation for this apparently odd-looking sound change, I propose that it was the result of phonologization of the phonetic lengthening of syllables containing resonants homorganic with a following voiced obstruent. Adopting Luick's (1898) view of "resonant+voiced homorganic obstruent" phonologically as a single coda, I show that Homorganic Cluster Lengthening is in fact a natural sound change that can be explained with the proper postulation of a few quantity-related universal constraints within the framework of the Optimality Theory. The fact that the constraints and their ranking as posited in this paper can also account for Pre-Cluster Shortening points to the validity of my approach in the analysis of other quantity changes in Middle English.

  • This paper investigates the intonation patterns of Korean spontaneous speech through an analysis of four dialogues in the domain of travel planning. The speech corpus, which is a subset of spontaneous speech database recorded and distributed by ETRI, is labeled in APs and IPs based on K-ToBI system using Momel, an intonation stylization algorithm. It was found that unlike in English, a significant number of APs and IPs include hesitation lengthening, which is known to be a disfluency phenomenon due to speech planning. This paper also claims that the hesitation lengthening is different from the IP-final lengthening and that it should be categorized as a new category, as it greatly affects the intonation patterns of the language. Except for the fact that 19.09% of APs show hesitation lengthening, the spontaneous speech shows the same AP patterns as in read speech with higher frequency of falling patterns such as LHL in comparison with read speech which show more LH and LHLH patterns. The IP boundary tones of spontaneous speech, showing the same five patterns such as L%, HL%, LHL%, H%, LH% as in read speech, show higher frequency of rising patterns (H% and LH%) and contour tones (HL%, LH%, LHL%) while read speech on the contrary shows higher frequency of falling patterns and simple tones at the end of IPs.

  • Acute bone shortening and delayed lengthening by Ilizarov surgery have been used to treat a wide range of soft tissue injuries including open fracture, osteomyelitis of the tibia and lower leg amputation. It has advantages such as bone lengthening as well as minimizing the loss of damaged tissues via tissue expansion. Here, we report a case of 52-year-old male with satisfactory results through acute bone shortening, replantation, and gradual bone lengthening after complete amputation of the ankle with related literature reviews.

  • Rare complications of distal tibia fracture are claw-toe deformity, cavus deformity and checkrein deformity. These complications may be due to contracture of the muscles of the deep posterior compartment of the leg after a compartment syndrome. These 4 cases were treated by selective tendon lengthening of flexor tendon at the retromalleolar level.

  • The purpose of this paper is to analyze vowel lengthening triggered by resyllabification. In general, short vowels become long when a following consonant is deleted, as shown in stehli 'steel' > st$\bar{e}$li and *fimf 'five' > five. We can account for the phenomenon in a straightforward way within the framework of CV phonology (Clements & Keyser, 1983) : deletion of a coda consonant C slot and then the preceding vowel spreads onto it, resulting in a long vowel. The analysis, however, cannot hold for words like ealhes 'temple', where deletion of an onset segment eventually triggers vowel lengthening in the preceding syllable. In order to account for this, I propose resyllabification. That is, ealhes first becomes eales by /h/-deletion. Next, the /l/ in coda is resyllabified as the onset of the following syllable, leaving its C slot unoccupied. Finally, the vowel spreads onto the empty slot, producing a long vowel.
