  • The authors report a case of epidural and extraforaminal calcification caused by repetitive triamcinolone acetonide injections. A 66-year-old woman was admitted presenting with lower extremity weakness and radiating pain in her left leg. Ten months before admission, the patient was diagnosed as having an L4-5 spinal stenosis and underwent anterior lumbar interbody fusion followed by posterior fixation. Her symptoms had been sustained and she did not respond to transforaminal steroid injections. Repetitive injections (10 times) had been performed on the L4-5 level for six months. She had been taking bisphosphonate as an antiresorptive agent for ten months after surgery. Calcification in the ventral epidural and extraforaminal space was detected. The gritty particles were removed during decompressive surgery and these were proven to be a dystrophic calcification. The patient recovered from weakness and radiating leg pain. Repetitive triamcinolone acetonide injections after discectomy may be the cause of dystrophic calcification not only in the degenerated residual disc, but also in the posterior longitudinal ligament. Possible mechanisms may include the toxicity of preservatives and the insolubility of triamcinolone acetonide. We should consider that repetitive triamcinolone injections in the postdisectomy state may cause intraspinal ossification and calcification.

  • X-linked Charcot-Marie-Tooth disease type 1 (CMTX1) is caused by the mutation in GJB1 gene, characterized by the transient central nervous system involvement and long standing peripheral polyneuropathy which does not fulfill the criteria of demyelination or axonopathy. We describe a 37-year-old man with progressive bilateral leg weakness since his early teen. He suffered transient right hemiparesis, followed by quadriparesis at 14 years of age. When we examined him at 37 years of age, he presented a distal muscle weakness on lower extremities with a sensory symptom. The nerve conduction study demonstrated a motor conduction velocity between 26 and 49 m/s. The whole exome sequencing revealed a novel variant c.136 G>A in GJB1. This report will raise awareness in this rare disease, which is frequently misdiagnosed early in its course.

  • Plantar fasciitis is most common cause of heel pain which starts anterior tubercle of calcaneus. It is chronic inflamation of plantar fascia, reduces collagen and water content of heel pain which incur the degenerative changes with elastic fiber weakness. We treated one patient after extracorporeal shock wave treatment. We diagnosed him with X-ray and treated her conservative maneuver as to oriental medical method. We measured Vas(Visual analogue scale) and thermographic picture of both leg. Visual analogue scale is from 10 to 4 and thermographic picture of both leg were improved in cases.

  • Herpes zoster can involve the variable peripheral nervous system but there have been few reports about plexopathy by the herpes zoster. We report a 54-year-old man with left leg weakness soon after herpes zoster in the left leg. His electrophysiological findings were consistent with the left lower lumbosacral plexopathy. It was concluded that herpes zoster can be considered to be one of the rare causes of the lumbosacral plexopathy.

  • Hemiplegia cruciata (HC) manifests as paralysis of the ipsilateral arm and contralateral leg. Herein, we report a 64-year-old man with weakness of the right leg and of the left arm after multiple sclerosis (MS). His brain and spine magnetic resonance imaging show a lower medulla lesion, which is extended to posterior part of C1 spine through cervicomedullary junction. HC usually results from stroke or trauma, but it is rare as presenting symptom of MS.

  • Background: The gluteus maximus (GM) muscle comprise the lumbo-pelvic complex and is an important stabilizing muscle during leg extension. In patients with low back pain (LBP) with weakness of the GM, spine leads to compensatory muscle activities such as instantaneous increase of the erector spinae (ES) muscle activity. Four-point kneeling arm and leg lift (FKALL) is most common types of lumbopelvic and GM muscles strengthening exercise. We assumed that altered hip position during FKALL may increase thoraco-lumbar stabilizer like GM activity more effectively method. Objects: The purpose of this study was investigated that effects of the three exercise postures on the right-sided GM, internal oblique (IO), external oblique (EO), and multifidus (MF) muscle activities and pelvic kinematic during FKALL. Methods: Twenty eight healthy individuals participated in this study. The exercises were performed three conditions of FKALL (pure FKALL, FKALL with 120° hip flexion of the supporting leg, FKALL with 30° hip abduction of the lifted leg). Participants performed FKALL exercises three times each condition, and motion sensor used to measure pelvic tilt and rotation angle. Results: This study demonstrated that no significant change in pelvic angle during hip movement in the FKALL (p > 0.05). However, the MF and GM muscle activities in FKALL with hip flexion and hip abduction is greater than pure FKALL position (p < 0.001). Conclusion: Our finding suggests that change the posture of the hip joint to facilitate GM muscle activation during trunk stabilization exercises such as the FKALL.

  • The chicks from 6 field broiler farms revealed peripheral neuropathy including leg weakness, curled toes and drooped wings. Grossly distinctive enlargements of sciatic nerve, branchial nerve and lumbar nerve were observed in the chicks. Histologically nerve lesions consisted of demyelination of myelin sheaths, Schwann cell proliferation and swelling, and interstitial edema in the peripheral nerves of all birds examined. Axonal swelling and infiltration of small lymphocytes were observed, but not a primary lesion. After treatment of riboflavin, neurological disorder was markedly recovered. From these results, it is suggested that the peripheral nerve lesions in these cases were caused by dietary riboflavin deficiency.

  • The right hip abductor musculature has been reported to demonstrate "stretch weakness" attributable to chronic elongation imposed by standing posture common to right-handed healthy persons. Kendall and associates have described the concept of "stretch weakness",. The purpose of this study was to assess isometric hip abduction torque and surface electromyographic activity (using MYOMED 432) in a sample of 40 healthy right-handed persons (20 male, 20 female), all of whom agreed to participate in the study, and compare side difference in the hip abductor musculature. In order to assure the statistical significance of the results, the paired t test was applied at the .05 level of significance. The results were as follows: 1) The difference in apparent leg length of right and left legs was significant at the .05 level. 2) There was a significant difference between right and left pelvic height (standing position) at the 05 level measurements, and scapula height at the .05 level. 3) Power measurements and action potentials of right hip abductor were greater than the left hip abductor regardless of the range of joint motion (inner range, outer range) 4) The difference in muscle power and action potentials according to inner or outer range of both hip abductor were significant at the .05 level. 5) In supine during active left hip abduction, the appearance of action potentials in the right hip abductors is indicative of contra-lateral effect (p<.05) These results suggest: In healthy right-handed persons, the apparent leg length on the right is longer than on the left, and pelvic height is elevated on the right side. Muscle those and muscle action potentials of the right hip abductor are higher than those of the left hip abductor in the lengthened position. Therefore, the results in this study are contrary to Kendall's. This type of study should be carried out in many physical therapy departments.

  • Objectives : This study aimed to investigate the associations between physical disorders and prevalent/incident suicidal ideation in a community dwelling older population aged 65 years or over. Methods : 1204 people aged 65 years or over evaluated at baseline. Suicidal ideation was identified using the questions from the community version of the Geriatric Mental State Schedule ("GMS B3"). Reported physical disorders covering 11 common and generally chronic health problems were ascertained. Covariates included were depression, age, gender, years of education, accommodation status, past occupation, and current occupation. Of 1066 without suicidal ideation at baseline, 805 (76%) were followed 2 years later, and incident suicidal ideation was evaluated. Results : Prevalent suicidal ideation was significantly associated with 4 of 11 physical disorders: eyesight problems, persistent cough, heart disease and paralysis or weakness in one leg or arm. Incident suicidal ideation was associated with 3 physical disorders: asthma, high blood pressure and paralysis or weakness in one leg or arm. Both prevalent and incident suicidal ideation were significantly associated with increased number of physical disorders. Conclusions : Certain physical disorders were comorbid and precipitating factors of suicidal ideation in elders. And appropriate intervention and treatment of physical disorders might prevent suicidal ideation in elderly.
