Evaluation of Electrohydraulic Left Ventricular Assist Device through Animal Experiment (동물실험을 통한 전기유압식 좌심실 보조장치의 성능 평가)

  • We have been developed electrohydraulic left ventricular assist device and done various in vivo evaluation on the device. Through the in vivo experiment conducted from Jan. 23, 1996 to Feb. 8, we could have experience of long-term evaluation fur the first time. The sheep used in this experiment had survived for 16 days. We used new actuator with reduced size and linear motion guide replacing oil box and ball bearings. Also, we used improved blood chamber with reduced size, reduced weight facilitating fixing the chamber to animal's body, and polymer sac having improved folding pattern. Against suction problem, we used absolute pressure limiter only. Motor current for driving this new actuator was not much higher than older one. Effective stroke volume was about 48 cc. Thrombosis was found around top area and peripheral boundary of the sac and valves. There was no sign of damage from suction problem in the atrium observed at autopsy. Main cause of death was presumed to be progressive formation of thrombosis in the cannulae. In this paper, the results of this experiment are documented.

  • Some cardio-vascular assist systems need more inexpensive artificial heart valves for short-term use. To meet with this need, we have developed a new polymer valve that is very simple to manufacture and of which its dimensions are easy to change, depending on its application. We have tested the hydrodynamic performance of the new polymer valve using a mock circulatory testing system and studied the flow through the valve using high-speed camera combined with image processing techniques. The results show that this valve is superior in its performances to the other valves (Bjork-Shiley mechanical valve and trileaflet polymer valve) and have no stagnation points. We also have tested the hemolytic potential of the valve. The valve is less hemolytic than the Bjork-shiley mechanical valve finally, we have applied this valve to a left ventricular assist device that we are developing.

  • Ventricular assist device(VAD) has been clinically applied as a temporary circulatory sup- porting system in the patients with severe heart failure, but small sized VAD for infant is not available. The purpose of this paper is to introduce small sized VAD and presents the result of in vitro test. Sejong VAD is diaphragmatic type of pneumatic pump and stroke volume is 11cc. Cardiac outputs of the Sejong VAD were measured by overflow tank under variable conditions of driving parameters. The cardiac output was 1.3 1/min at the heart rate of 120 per minute, left atrial pressure of 15cmH2O, percent systole of 43%, driving pressure of 240 mmHg, vacuum pressure of -40 mmHg, and mean aortic pressure of 70 mmHg. No mechanical problem was developed during the continuous in vitro test for 3 months.

  • In this paper, we describe about control system for eletro-hydraulic LVAD(Left Ventricular Assist Device) and control algorithm for two operation mode. One is asynchronous operation mode and the other is synchronous operation mode synchronized with natural heart's R pulse. We also present implementation method of software PI algorithm for velocity control of motor used in LVAD system and its response.

An Investigation of the Pump Operating Characteristics as a Novel Control Index for LVAD Control

  • This work presents a novel control index to regulate the pump speed of an axial flow blood pump for the left ventricular assist device (LVAD). The control index is based on the characterization of pump operating conditions such as normal or suction status. The pump operating characteristics reveal that a certain pulsatility relationship between the pump pressure difference and the pump flow is a unique index to identify the pump operating status under the diverse pump operating environments.

  • Heart failure (HF) is an important cardiovascular disease because of its increasing prevalence, significant morbidity, high mortality, and rapidly expanding health care cost. The number of HF patients is increasing worldwide, and Korea is no exception. There have been marked advances in definition, diagnostic modalities, and treatment of HF over the past four decades. There is continuing effort to improve risk stratification of HF using biomarkers, imaging and genetic testing. Newly developed medications and devices for HF have been widely adopted in clinical practice. Furthermore, definitive treatment for end-stage heart failure including left ventricular assist device and heart transplantation are rapidly evolving as well. This review summarizes the current state-of-the-art management for HF and the emerging diagnostic and therapeutic modalities to improve the outcome of HF patients.

Changes of Ventricular Function and Mitral Regurgitation after Repair of Anomalous Origin of Coronary Artery from the Pulmonary Artery (관상동맥 폐동맥 이상 기시증의 외과적 치료 후 좌심실 및 승모판 기능의 변화)

  • Background: Investigation of the change of ventricular function and mitral regurgitation after surgical repair of patient with anomalous origin of left coronary artery from the pulmonary artery (ALCAPA) is key issue for the better surgical outcome. Material and Method: From April 1986 to July 2002, 12 patients presented with ALCAPA. The median age at repair was 4 months. Surgical methods included left coronary artery transfer to the aorta (10), Takeuchi procedure (1), saphenous vein free graft bypass (1). Mitral valve was repaired in 1. Result: There were 2 hospital death $(16.7\%)$. The mean follow-up period was $7.1\pm4.1$ years (range, 7 months to 13 years). Four patients required postoperative circulatory assist for $2.2\pm1.1$ days and one needed left ventricular assist device (LVAD) for 1day. Postoperative echocardiography demonstrated significant improvements in mean fractional shortening $(33.4\pm9.1\%\;vs\;17.7\pm9.6\%,\;n=10,\;p<0.05);$ left ventricular end diastolic dimension $(33.4\pm7.3\;mm\;vs\;44.8\pm7.0\;mm,\;n=10,\;p<0.05)$ and systolic dimension $(22.2\pm7.5\;mm\;vs\;33.4\pm7.9\;mm,\;n=10,\;p<0.05)$. Severities of mitral regurgitation decreased in all survivors at 1st and 4th year follow-up echocardiography. There were 2 reoperation due to residual MR and right ventricular outflow obstruction (Takeuchi case). Conclusion: Anatomic repair of anomalous left coronary artery from the pulmonary artery offered an excellent surgical results, especially in terms of the recovery of left ventricle function and mitral regurgitation. However, preoperative indications for mitral procedure is to be evaluated.

  • This paper outlines the development of a non-pulsatile axial flow type blood pump control system. By utilizing blood pressure and heart rate, this system can assist the left ventricle in controlling blood pressure and blood volume. The system is comprised of a blood pump, signal sensor, signal interface, and signal-processing component. A control algorithm is also proposed which can control non-pulsatile, continuous blood flow in the human circulatory system. To facilitate the control required for non-pulsatile blood pump in a physiological system, an experimental control rule was developed utilizing ECG and blood pressure data, both of which are easily detectable variables in the body. The system was then tested using a mock-up circulation system and we found that it is possible that this systems could be temporarily used in clinic.
