The Relationship between Explanation and Patient Compliance in Hirudotherapy

  • Background: The use of leeches can effectively increase the salvage rate of flap congestion. However, the first reaction from patients and carers in using leeches in clinical fields is strong aversion. This can be due to the fact that development of our culture from agriculture to industrial society, coming across leeches became fairly rare. Also because of the biological traits that leeches carry; staying attached to a leg or other body parts of the host, sucking blood, and leaving wounds. Methods: This study was conducted through questionnaires, divided into many subgroups. We scaled the compliance of the two therapies, with or without leech. Maximum scale of 10 showing no rejective response to the therapy and minimum scale of 0 showing the greatest rejective response. Results: Overall subjects' compliance was improved after explaining the benefits of hirudotherapy. Irrelevant to the explanation, there was no significant difference in general compliance between male and female. Young-aged group and medical personnel or people studying medicine showed higher compliance over older-aged group and the general public. Conclusion: In the terms of general social cognition, recognizing leech as a therapeutic material may not be welcomed at first, but provided with proper information and explanations, overall compliance of patients and carers can be improved and consequently result in superior outcomes in flap salvage.

A New Record of Roeselia triangulalis (Lepidoptera: Nolidae) from Korea

  • A nolid species, Roeselia triangulalis (Leech) is reported for the first time from Korea. Two males were collected from Jeju Island, South Korea. Roeselia triangulalis can be distinguished by the bipectinate male antennae, broad frons, long labial palpi, costally dark brownish spot and the rounded dark brownish postmedial line of forewing. In the male genitalia, the long saccus, the slender valve with distally expanded and spine-like harpe and long spinular cornutus of vesica are distinguishing characters. In the female genitalia, the simple sterigma, the long ductus bursae and the large ovate corpus bursae with a lip-shaped signum are distinguishing characters. Diagnosis and description of the species are given with the figures of the male genitalia.

  • Hirudin is a potent thrombin inhibitor originally derived from the medicinal leech. Leeches were commonly used at the turn of the century for bloodletting and are still used today in some countries. So, hirudin is being developed as a alternative to heparin which has several shortcomings. This study is intended to analyze R&D trends, potential market for hirudin and existing anticoagulant drug market. Anticoagulant drug market in Korea is estimated by over 1,200 billion won in 2010 and revenues in the world market are forecasted to reach 1.62 billion dollar in 1999.

  • Hirudin is a potent inhibitor of thrombin, which was originally obtained from the medicinal leech (Hirudo medicinalis) Now it is being produced through the recombinant technology on a large scale. Recombinant hirudin has been assayed for the anticoagulant activity by the measurement of clotting time and the inhibition of thrombin actvity using a chromogenic substrate. The assay range of partial thromboplastin time and thrombin time is within $0.2{\sim}1.0 {\mu}g/mι.$ Thrombin time is more sensitive to the measurement of clot. Ex vivo study showed the level of hirudin in rat plasma was highest in 10 min and then it was eliminated slowly. The half-life of r-hirudin was 80~110 min depending on the assay methods. Intraveneous injection of russel viper venom was used for thrombus induction combined with vents cava ligation. Inhibition of venous thrombosis was observed with i.v. hirudin. It was dependent on the concentration of hirudin.

  • Language is the method for communication. Semantician, G. Leech categorized the function of language as informational, expressive, directive, aesthetic and phatic one. Therefore, language can be defined as the social symbol system and the media for communication. Space, also, performs the role of symbol, information and communication. Space can be the media for the communication and the design can be logical construction process by it's arbitrary interpretation. The researcher considered the common points between verbal language and visual language as the tools for the mutual communication, tried to transit them into the 'space' as object language after analyzing 'image of poetry' as meta language, so that the old perspective of space design as function can be renewed as information system of the significant meaning delivery in the context of language's multiplicity, expandibility and changeability.

The Effects of Live Leech (Hirudo Medicinalis) Therapy on Diabetic Foot: a Clinical Case Report

  • Purpose : A clinical case of diabetic foot-wound repair using Hirudo medicinalis therapy and its application is reported and discussed. Methods : The subject was a sixty-six-year-old woman diagnosed with a non-healing wound indicating the amputation of her right diabetic foot. Instead, Hirudo medicinalis therapy was performed twelve times over forty days, twice a week. The transition of wound closure was observed with the naked eye, and the plethysmogram was measured at the beginning and end of the treatment period. Results : The wound contracted with decreased exudate, collagen-like connective tissue sprouted, and then the wound gradually closed. The initial value (-0.08) of the dicrotic elastic index rose to -0.03. Conclusion : Hirudo medicinalis therapy had a positive effect on the wound repair, without a negative effect on the circulation, allowing the foot to be saved. It is necessary to investigate its wound repair mechanism for the further development of the Sahyul method.

Gene Expression and Secretion of the Anticoagulant Hirudin in Saccharomyces cerevisiae

  • Hirudin, a 65-amino acid protein isolated from the salivary gland of the bloodsucking leech, Hirudo medicinalis, is a potent thrombin-specific inhibitor and blocks the thrombin-mediated conversion of fibrinogen to fibrin in clot formation. We have studied the gene expression and secretion of hirudin in yeast. Saccharomyces cerevisiae. A gene coding for hirudin was synthesized based on the amino acid sequence and cloned into a yeast expression vector $YEG{\alpha}-1$ containing the ${\alpha}-mating$ factor pre-pro leader sequence and galactose-inducible promoter, GALl0. Recombinant S. cerevisiae was found to secrete biologically active hirudin into the extracellular medium. The secreted recombinant hirudin was recovered from the culture medium and purified with ultrafiltration and reverse phase high performance liquid chromatography. Approximately 1 mg of hirudin per liter was produced under suboptimal culture conditions and brought to about 90% purity in two steps of purification.

  • A notodontid moth, Disparia diluta (Hampson, 1910) is reported for the first time from the Korean peninsula. At the same time, taxonomical note for D. nihonica (Wileman, 1911) is given. The male genitalia of D. nihonica (Wileman) were erroneously illustrated as those of Neodrymonia delia (Leech) by Tshisjakov and Kwon (1999), and Schintlmeister (2008) cited D. nihonica (Wileman) to be distributed in the Korean peninsula, based on the illustration.

Taxonomic Studies on Three Caeneressa Species (Lepidoptera: Syntominae) from India with Special Reference to Their External Genitalia

  • Caeneressa diaphana muirheadi (Felder) and Caeneressa brithyris (Druce) have been recorded for the first time from India. Further, a new combination viz. Caeneressa melaena (Walker) comb. nov., has also been proposed on the basis of external genitalic features. With these records, genus Caeneressa will be represented by five species from India, Caeneressa diaphana (Kollar), Caeneressa diaphana muirheadi (Felder), Caeneressa brithyris (Druce), Caeneressa melaena (Hampson) and Caeneressa swinhoei (Leech). The studied material was collected from North-Eastern states of India. Caeneressa brithyris (Druce) was collected near riverside at low altitude whereas the other two species were collected at high altitude.

  • Cochylidae(가는잎말이나방과 : 신칭)은 잎말이나방상과(Tortricoidea)에 속하며 우레 나라에서는 오늘날까지 알려지지 않았던 소형의 나방 그룹이다. 그러나 이미 1886년에 영국 곤충학자 J. H. Leech의 한반도 채집시에 2종(종)이 채집되어 외국 문헌상에는 기록되어 오고 있다. 이과에 슥하는 종들은 대개가 작물의 꽃, 신소, 줄기 등 비교적 연약한 조직의 내부를 갖아 작해를 주며 전세계적으로 분포하는 작물의 주요 해충들이다. 필자는 현재까지 채집된 재료 중에서 금번에 분류 등정된 한국 미기록 10종을 발표한다.
