Heterotopic bone formation in normal gastric cardiac mucosa (정상 위 분문부 점막에서 발견된 이소성 골 형성)

  • Heterotopic bone formation in the gastrointestinal tract is a rare phenomenon. Most reported cases were associated with benign and malignant neoplasms, except for a case in which heterotopic bone formation was found in a patient with Barrett's esophagus. The exact pathogenesis of the disease has not yet been established. However, most heterotopic bones found in the gastrointestinal tract were associated with mucinproducing tumors of the appendix, colon, and rectum. Inflammation may also play a role in osseous metaplasia in a case with bone formation at the base of an ulcer in Barrett's esophagus. Here, we report on a patient with heterotopic bone formation in normal gastric cardiac mucosa. A 50-year-old female visited our hospital for a routine health examination. She had no gastrointestinal symptoms, and her physical examination, blood test, X-ray, urine, and stool examination results were normal. A 0.3 cm sized polypoid lesion located just below the squamocolumnar junction was observed on upper gastrointestinal endoscopy. A piece of biopsy was taken. Histologically, a lamella bone trabecula and chronic inflammatory cells were observed in the gastric cardiac mucosa. The follow-up endoscopy performed one month later showed no residual lesion.

New-Onset Malignant Pleural Effusion after Abscess Formation of a Subcarinal Lymph Node Associated with Endobronchial Ultrasound-Guided Transbronchial Needle Aspiration

  • We present a case of an unusual infectious complication of a ruptured mediastinal abscess after endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA), which led to malignant pleural effusion in a patient with stage IIIA non-small-cell lung cancer. EBUS-TBNA was performed in a 48-year-old previously healthy male, and a mediastinal abscess developed at 4 days post-procedure. Video-assisted thoracoscopic surgery was performed for debridement and drainage, and the intraoperative findings revealed a large volume pleural effusion that was not detected on the initial radiographic evaluation. Malignant cells were unexpectedly detected in the aspirated pleural fluid, which was possibly due to increased pleural permeability and transport of malignant cells originating in a ruptured subcarinal lymph node from the mediastinum to the pleural space. Hence, the patient was confirmed to have squamous cell lung carcinoma with malignant pleural effusion and his TNM staging was changed from stage IIIA to IV.

An Epidemiological Survey of Helminthic Infections Among Inhabitants in Boeun-Gun, Chungbuk Province (충북 보은군 일부 농촌지역 주민의 윤충류 기생충 감염상)

  • Helminthological parasites for inhabitants in Boeun-Gun, Chungbuk province were survyed. Total infection rate was 21.4% and helminth ova of-seven species were observed: Clonorchis sinensis 11.7%, Metagonimus sp. 7.0%, Echinostoma hortense 2.7%, Taenia sp. 2.3%, Ascaris lumbri-coides 0.8%, Trichuris trichiura 1.6% and Enterobius vermicularis 3.5%. Infection rate of food-transmitted helminths (C. sinensis, Metagonimus sp. E.hortense, Taenia sp.) was 76.4% (42/55), soil-transmitted helminths (A.lumbricoides, T. trichiura) was 7.3% (4/55), contageous helminths(E. vermicularis) was 16.3% (9/55). Multiple infection rate was 30.9% and most of them were limited in snail-transmitted helminths(C.sinensis, Melagonimas sp., E. hortense). Infection rate of male (18. 3%) was higher than those of female (3.1%). Intensities of C.sinensis and Metagonimus sp. were light or moderate, showed 530 and 444 by mean E.P.G. respectively. In this survey, we newly found the small endemic areas of E.hortense, which was previously reported sporadically in Korea other than in Chungbuk province.

  • Hominis Placenta has a broad array of clinical applications in Korean medicine, including treatment of inflammatory conditions such as rheumatoid arthritis. The purpose of this study is to explore the global gene expression profiles in human RAW 264.7 cell lines treated with Hominis Placenta herbal-acupuncture solution (HPHAS) using microarray analysis. The RAW 264.7 cells were treated with lipopolysaccharide (LPS), HPHAS, or both. Of the 8,170 genes profiled in this study, with a cut-off level of two-fold change in the expression, 72 genes (CTD1, regulating synaptic membrane exocytosis 2, etc.) were upregulated and 135 genes(splicing factor, arginine/serine-rich 1, actinin, alpha 1, etc.) downregulated following LPS treatment. One gene (acrosin) was upregulated and 12 genes (phospholipase A2, group IB, neurofilament, heavy polypeptide 200kDa, etc.) were downregulated following HPHAS treatment. Eleven genes (RAB27A, member RAS oncogene family, eosinophil peroxidase, etc.) were upregulated and 16 genes (V-maf musculoaponeurotic fibrosarcoma oncogene homolog G (avian), RW1 protein, etc.) were downregulated following co-stimulation of HPHAS and LPS. It is thought that microarrays will play an ever-growing role in the advance of our understanding of the pharmacological actions of HPHAS in the treatment of arthritis. Further studies, however, are required to concretely prove the effectiveness of HPHAS.

  • In this paper, we investigated the maximum ankle joint plantarflexion moment(MPM) of young and elderly subjects during various modes of isokinetic contractions with special interest in the elderly females who were reported to have higher fall rate than the elderly males. Subjects include the young subjects(14 males: $22.7{\pm}2.5yrs$, 13 females: $20.5{\pm}1.3yrs$) and the elderly subjects(17 males: $65.8{\pm}10.5yrs$, 26 females: $71.4{\pm}5.2yrs$). The MPM was normalized by each subject's body weight. The MPM of elderly females was the weakest among the groups, particularly, at the eccentric contractions. The male-to-female ratio of MPM was greater in the elderly than in the young and the young-to-elderly ratio of MPM was greater in female than in male, both during the eccentric contractions. The result suggests that the decreased muscle force per body weight, especially at the eccentric contraction, may be one reason for the more frequent fall of the elderly female than the elderly male.

  • Apocrine carcinoma of the breast is a very rare subtype. Although it has no clinical differences from usual ductal carcinoma of the breast, it should be categorized as a subtype of breast carcinoma because the cells of apocrine carcinoma reveal characteristic abundant eosinophillic cytoplasms with intraductal apical snouting as well as round or oval nuclei and central macronucleoli. On fine needle aspiration cytology, the cells of apocrine carcinoma have a lot of similarity to benign or reactive apocrine cells of the breast. Therefore, it is difficult to make a differential diagnosis of apocrine carcinoma from mammary neoplasms with similar cytologic findings unless the subtle cytologic differences are recognized. We report the cytologic and histologic findings of a case of apocrine carcinoma in the breast of a 40-year-old female patient. After the fine needle aspiration cytology, she received the lumpectomy and lymph node dissection. The cellularity was moderate to high. The cytoplasmic borders of tumor cells of three-dimensional clusters were relatively distinctive, and the cytoplasm was abundant, eosinophilic, and granular. Although the nuclear/cytoplasmic ratio was low, the nuclei of the cells were variable in size and shape with prominent macronucleoli. Histologically, it was a typical invasive apocrine carcinoma, showing numerous cytoplasmic lysosomes and mitochondriae on electron microscopy.

Cloning of Dextransucrase Gene from Leuconostoc citreum HJ-P4 and Its High-Level Expression in E. coli by Low Temperature Induction

  • A dextransucrase (LcDS) gene from Leuconostoc citreum HJ-P4 has been amplified and cloned in E. coli. The LcDS gene consists of 4,431 nucleotides encoding 1,477 amino acid residues sharing 63-98% of amino acid sequence identities with other known dextransucrases from Leuc. mesenteroides. Interestingly, 0.1 mM of IPTG induction at $15^{\circ}C$ remarkably increased the LcDS productivity to 19,187 U/I culture broth, which was over 330-fold higher than that induced at $37^{\circ}C$. Optimal reaction temperature and pH of LcDS were determined as $35^{\circ}C$ and pH 5.5 in 20 mM sodium acetate buffer, respectively. Meanwhile, 0.1 mM $CaCl_2$ increased its activity to the maximum of 686 U/mg, which was 2.1-fold higher than that in the absence of calcium ion. Similar to the native Leuconostoc dextransucrase, recombinant LcDS could successfully produce a series of isomaltooligosaccharides from sucrose and maltose, on the basis of its transglycosylation activity.

  • Today, A matter of concern of home network technology increase. The standard of communication between home network devices are required. IEEE 802.1 AVB(Audio/Video Bridging) specifies transmission method for time-sensitive data between these devices using Ethernet in bridged local area networks. IEEE 802.1 AVB and IEEE 1588 PTP(Precision Time Protocol) have various message type for grandmaster selection and synchronize the devices. These messages bring on complexity protocol. We propose a method that uses Single TimeSync frame in order to the problem. Our proposal is appropriate process complexity and low transmission delay for home network by using the TimeSync frame. Furthermore, after all devices are adjusted to the single TimeSync frame, a resource reservation, a forwarding and queueing rule are needed for a time-sensitive application.

Measurement and Comparison of Finger Tapping Movement in Patients with Idiopathic Parkinson's Disease and Normal Subjects using Gyrosensor (자이로센서를 이용한 특발성 파킨슨병 환자와 정상인의 손가락 벌렸다 오므리기 동작의 측정과 비교)

  • The purpose of this study is to compare finger tapping (FT) movement of patients with Parkinson's disease (PD) with normal subjects. A gyrosensor system was used for the measurement of FT movement, because it provides angular velocity free from the gravitational artifact and it can be used during clinical FT test listed in unified PD rating scale (UPDRS). Forty PD patients (age: 65.7 ${\pm}$ 11.1 yrs, H&Y stage:2.3 ${\pm}$ 0.5), 14 age-matched elderly subjects (65${\pm}$3.9 yrs) and 17 healthy young subjects (24${\pm}$2.1yrs) participated in this study. Angular velocity of finger tapping movement was measured in both right and left index finger. As quantitative measures, root-mean-squared (RMS) angular velocity, RMS angle, peak power and total power were used. ANOVA showed that all measures were significantly different among three groups (p<0.001) in all quantitative measures. Post-hoc test revealed that all quantitative measures except peak power in patients with PD were significantly smaller than in both healthy elderly and young subjects (p<0.01). This suggests that the measures developed in this study can distinguish patients with PD from normal subjects.

Effect of Carbon and Nickel on Microstructure and Low Temperature Charpy Impact Properties of HSLA Steels (HSLA 강의 미세조직과 저온 샤르피 충격 특성에 미치는 탄소와 니켈의 영향)

  • In this study, effects of carbon and nickel on microstructure and low temperature Charpy impact properties of HSLA (high strength low alloy) steels are investigated. To understand the complex phase transformation behavior of HSLA steels with high strength and toughness before and after welding processes, three kinds of HSLA steels are fabricated by varying the carbon and nickel content. Microstructure analysis, low temperature Charpy impact test, and Vickers hardness test are performed for the base metals and CGHAZ (coarse-grain heat affected zone) specimens. The specimens with the lowest carbon and nickel content have the highest volume fraction of AF, the lowest volume fraction of GB, and the smallest GB packet size. So, the low temperature Charpy absorbed energy of the CGHAZ specimen is the highest. The specimens with increased carbon and nickel content have the lowest volume fraction of AF, the highest volume fraction of GB, and the largest GB packet size. So, the low temperature Charpy absorbed energy of the CGHAZ specimen is the lowest.
