Impact of Lifestyle Diseases on Postoperative Complications and Survival in Elderly Patients with Stage I Non-Small Cell Lung Cancer

  • Background: The influence of lifestyle diseases on postoperative complications and long-term survival in patients with non-small cell lung cancer (NSCLC) is unclear. The aim of this study was to determine whether lifestyle diseases were significant risk factors of perioperative and long-term surgical outcomes in elderly patients with stage I NSCLC. Methods: Between December 1995 and November 2013, 110 patients aged 65 years or older who underwent surgical resection of stage I NSCLC at Dong-A University Hospital were retrospectively studied. We assessed the presence of the following lifestyle diseases as risk factors for postoperative complications and long-term mortality: diabetes, hypertension, chronic obstructive pulmonary disease, stroke, and ischemic heart disease. Results: The mean age of the patients was 71 years (range, 65 to 82 years). Forty-six patients (41.8%) had hypertension, making it the most common lifestyle disease, followed by diabetes (n=23, 20.9%). The in-hospital mortality rate was 0.9% (n=1). The 3-year and 5-year survival rates were 78% and 64%, respectively. Postoperative complications developed in 32 patients (29.1%), including 7 (6.4%) with prolonged air leakage, 6 (5.5%) with atrial fibrillation, 5 (4.5%) with delirium and atelectasis, and 3 (2.7%) with acute kidney injury and pneumonia. Univariate and multivariate analyses showed that the presence of a lifestyle disease was the only independent risk factor for postoperative complications. In survival analysis, univariate analysis showed that age, smoking, body mass index, extent of resection, and pathologic stage were associated with impaired survival. Multivariate analysis revealed that resection type (hazard ratio [HR], 2.20; 95% confidence interval [CI], 1.08 to 4.49; p=0.030) and pathologic stage (HR, 1.89; 95% CI, 1.02 to 3.49; p=0.043) had independent adverse impacts on survival. Conclusion: This study demonstrated that the presence of a lifestyle disease was a significant prognostic factor for postoperative complications, but not of survival, in elderly patients with stage I NSCLC. Therefore, postoperative complications may be influenced by the presence of a lifestyle disease.

TNT Explosion Demonstration and Computational Fluid Dynamics for Safety Verification of Protection Wall in Hydrogen Refueling Station (수소충전소 방호벽 안전성 검증을 위한 TNT 폭발실증 및 전산유동 해석)

  • In realizing a hydrogen society, it is important to secure the safety of the hydrogen refueling station, which is the facility where consumers can easily meet hydrogen. The hydrogen refueling station consists of compressed gas facilities that store high-pressure hydrogen, and there is a risk that the high-pressure compressed gas facility will rupture due to a fire explosion due to hydrogen leakage in the facility or the influence of surrounding fires. Accordingly, the Korea Gas Safety Corporation is making every effort to find out risk factors from the installation stage, reflect them in the design, and secure safety through legal inspection. In this study, a TNT explosion demonstration test using a protection wall was conducted to confirm the safety effect of the protection wall installed at the hydrogen refueling station, and the empirical test results were compared and verified using FLACS-CFD, a CFD program. As a result of the empirical test and CFD analysis, it was confirmed that the effect of reducing the explosion over-pressure at the rear end of the protection wall decreased from 50% to up to 90% depending on the location, but the effect decreased when it exceeded a certain distance. The results of the empirical test and computer analysis for verifying the safety of the protection wall will be used in proposals for optimizing the protection wall standards in the future.

A Study on the Reliability Analysis and Risk Assessment of Liquefied Natural Gas Supply Utilities (천연가스 공급설비에 대한 기기신뢰도 분석 및 위험성 평가)

  • Natural gas has been supplied through underground pipelines and valve stations as a new city gas in Seoul. In contrast to its handiness the natural gas has very substantial hazards due to fires and explosions occurring from careless treatments or malfunctions of the transporting system. The main objectives of this study are to identify major hazards and to perform risk assessments after assessing reliabilities of the composing units in dealing with typical pipeline networks. there-fore two method, fault tree analysis ;1nd event tree analysis, are used here. Random valve stations are selected and considered its situation in location. The value of small leakage, large rupture, and no supply of liquefied natural gas is estimated as that of top event. By this calculation the values of small leakage are 3.29 in I)C valve station, 1.41 in DS valve station, those of large rup-lure are $1.90Times10_{-2}$ in DC valve station, $2.32$\times$10^{-2}$ in DS valve station, and those of no supply of LNG to civil gas company are $2.33$\times$10 ^{-2}$ , $2.89$\times$10^{-2}$ in each valve station. And through minimal cut set we can find the parts that is important and should be more important in overall system. In DC valve station one line must be added between basic event 26,27 because the potential hazard of these parts is the highest value. If it is added the failure rate of no supply of LNG is reduced to one fourth. In DS valve station the failure rate of basic event 4 is 92eye of no supply of LNG. Therefore if the portion of this part is reduced (one line added) the total failure rate can be decreased to one tenth. This analytical study on the risk assessment is very useful to prepare emergency actions or procedures in case of gas accidents around underground pipeline networks and to establish a resolute gas safety management system for loss prevention in Seoul metropolitan area.

Comparison of the Outcomes between Surgical Aortic Valve Replacement and Transcatheter Aortic Valve Replacement in Patients Aged above 80

  • Background: Transcatheter aortic valve replacement (TAVR) has been suggested as a less invasive treatment for high-risk patients with aortic valve disease. I n this study, we compared the outcomes of conventional surgical aortic valve replacement (AVR) and TAVR in elderly patients aged over 80. Methods: A total of 108 patients aged 80 years or older who underwent isolated AVR (n=35) or TAVR (n=73) from 2010 through 2015 at Asan Medical Center were identified. Early and late clinical outcomes, including echocardiographic findings, were evaluated in both groups. The mean follow-up duration was $766.4{\pm}528.7days$ in the AVR group and $755.2{\pm}546.6days$ in the TAVR group, and the average timing of the last follow-up echocardiography was at $492.6{\pm}512.5days$ in the AVR group and $515.7{\pm}526.8days$ in the TAVR group. Results: The overall early mortality was 2.8% (0 of 35, 0% in the AVR group vs. 3 of 73, 4.1% in the TAVR group). Permanent pacemaker insertion was significantly more common in the TAVR group (p=0.010). Renal failure requiring dialysis and new-onset atrial fibrillation was more frequent and the length of hospital stay was longer in the AVR group; however, this difference did not reach statistical significance. In the TAVR group, 14 patients (19.2%) were rehospitalized due to cardiac problems, and 13 patients (17.8%) had developed significant paravalvular leakage by the time of the last follow-up echocardiography. Conclusion: TAVR could be a good alternative to conventional surgical AVR in elderly patients. However, TAVR has several shortcomings, such as frequent significant paravalvular leakage or readmission, which should be considered in decision-making.

  • Financial firms strengthen to protect personal information from the leakage, introducing various security solutions such as print output security, internet network Isolation system, isolationg strorage of customer information, encrypting personal information, personal information detecting system, data loss prevention, personal information monitoring system, and so on. Financial companies are also entering the era of cutthroat competition due to accept of the new channels and the paradigm shift of financial instruments. Accordingly, The needs for security for customer information held by financial firms are keep growing. The large security accidents from the three card companies on January 2014 were happened, the case in which one of the outsourcing personnel seized customer personal information from the system of the thress card companies and sold them illegally to a loan publisher and lender. Three years after the large security accidents had been passed, nevertheless the security threat of the IT outsourcing workforce still exists. The governments including the regulatory agency realted to the financail firms are conducting a review efforts to prevent the leakage of personal information as well as strengthening the extent of the sanction. Through the analysis on the application of security policy for outsourcing personnel in case of large-scale Financial IT projects and the case study of appropriate security policies for security compliance, the theis is proposing a solution for both successfully completing large-scale financial IT Project and so far as possible minizing the risk from the security accidents by the outsouring personnel.

A model of Isolated Renal Hemoperfusion (허혈/재관류 손상연구를 위한 체외 신장 재관류 모델)

  • Ischemia-reperfusion (I/R) injury is associated with an increased risk of acute rejection, delayed graft function and long-term changes after kidney transplantation. The reperfusion models remain unsolved complications such as vascular obstruction and blood leakage. We developed an alternative model of isolated hemoperfusion in porcine kidneys. In the present study we introduced a newly developed reperfusion method. A connector was used instead of surgical suture for the vascular anastomosis on the inguinal region in which main femoral vessels are parallel and big enough to perfuse the kidney. To assess renal perfusion quality of the modified hemoreperfusion model, we analyzed both hemodynamic values and patterns of I/R injury following a renal reperfusion. Following unilateral nephrectomy, the kidneys were preserved for 0, 24 and 48 hours at $4^{\circ}C$ with histidine-tryptophan ketogluatarate (HTK) solution and reperfused for 3 hours by vascular anastomosis connected to the femoral artery and vein in inguinal region. Histolopathological examinations were assessed on kidney biopsy specimens, taken after each cold storage and reperfusion. No differences of hemodynamic values were observed between aorta and femoral artery. The average warm ischemia time before reperfusion start was $7.0{\pm}1.1$ minutes. There were no complications including vascular obstruction and blood leakage during the reperfusion. I/R injury of the perfused kidneys in this model was dependent upon the cold ischemia time. The results support that the modified perfusion model is simple and appropriate for the study of early renal I/R injury and transplant immunology.

Empirical Analysis of the Relationship between a Market and Technological Characteristics and Performance of the Activities of Technology Security (시장특성과 기술특성이 기술보호성과에 미치는 영향)

  • Technology Security, especially in knowledge-intensive society, is becoming the most important organizational activity for the long-term success of a firm. However, there is not sufficient empirical research of activities of technology securities and business performance. This is one of the reason why Korean firms are reluctant to invest their resources to the technology security. We have tried to empirically analyze the relationship between a market and technological characteristics, which is one of the important business environmental characteristics and the activities of technology securities of Korean business firm by taking three hypotheses related to this research and investigating 209 Korean firms. According to the statistical research, results of the analyses suggest that the market and technological characteristics have positive impact on the activities of technology securities. Also the activities of technology securities have positive impact on the performance of technology security. More specifically, the results suggest that the relationship between the competitiveness of market and technological excellency and the activities of technology securities is supported statistically. But, the reverse relationship between the easiness of market entrance and the activities of technology securities is supported statistically. Also, there is no moderating effect of firm size between the relationship between a market and technological characteristics and the activities of technology securities. Finally, the relationship between the activities of technology securities and the risk of technology leakage is supported statistically.

A numerical analysis study on the flammable volume by leakage of hydrogen fuel vehicles in parking lot (지하주차장 내 수소연료차의 수소 방출시 가연체적에 관한 수치해석적 연구)

  • The recent reduction in greenhouse gases, interest in environmental pollution such as low-carbon emission policies is increasing. Accordingly, the penetration rate of eco-friendly vehicles, including hydrogen battery vehicles capable of reducing carbon emission, is increasing, and thus it is required for disaster prevention and safety-related measures. In this study, the degree of risk for the concentration distribution of hydrogen when leaking hydrogen fuel vehicles according to ventilation conditions was analyzed through numerical analysis, limited to places in parking lots. As a result, when only one hydrogen tank was released, the combustible volume ratio of hydrogen in the underground parking lot was up to 8.6%, and as ventilation continued, the volume ratio of combustible hydrogen decreased to less than 1% after 150 seconds, indicating that mechanical ventilation is essential. In the case of simultaneous release or stage release of three hydrogen tanks, the final combustible volume ratio of hydrogen is similar, but the increase in the combustible volume ratio of hydrogen in the early stage of release is low, and further research is expected.

Relative Importance Analysis of Management Level Diagnosis for Consignee's Personal Information Protection (수탁사 개인정보 관리 수준 점검 항목의 상대적 중요도 분석)

  • Recently ICT, new technologies such as IoT, Cloud, and Artificial Intelligence are changing the information society explosively. But personal information leakage incidents of consignee's company are increasing more and more because of the expansion of consignment business and the latest threats such as Ransomware and APT. Therefore, in order to strengthen the security of consignee's company, this study derived the checklists through the analysis of the status such as the feature of consignment and the security standard management system and precedent research. It also analyzed laws related to consignment. Finally we found out the relative importance of checklists after it was applied to proposed AHP(Analytic Hierarchy Process) Model. Relative importance was ranked as establishment of an internal administration plan, privacy cryptography, life cycle, access authority management and so on. The purpose of this study is to reduce the risk of leakage of customer information and improve the level of personal information protection management of the consignee by deriving the check items required in handling personal information of consignee and demonstrating the model. If the inspection activities are performed considering the relative importance of the checklist items, the effectiveness of the input time and cost will be enhanced.

Risk Ranking Analysis for the City-Gas Pipelines in the Underground Laying Facilities (지하매설물 중 도시가스 지하배관에 대한 위험성 서열화 분석)

  • In this article, we are to suggest the hazard-assessing method for the underground pipelines, and find out the pipeline-maintenance schemes of high efficiency in cost. Three kinds of methods are applied in order to refer to the approaching methods of listing the hazards for the underground pipelines: the first is RBI(Risk Based Inspection), which firstly assess the effect of the neighboring population, the dimension, thickness of pipe, and working time. It enables us to estimate quantitatively the risk exposure. The second is the scoring system which is based on the environmental factors of the buried pipelines. Last we quantify the frequency of the releases using the present THOMAS' theory. In this work, as a result of assessing the hazard of it using SPC scheme, the hazard score related to how the gas pipelines erodes indicate the numbers from 30 to 70, which means that the assessing criteria define well the relative hazards of actual pipelines. Therefore. even if one pipeline region is relatively low score, it can have the high frequency of leakage due to its longer length. The acceptable limit of the release frequency of pipeline shows 2.50E-2 to 1.00E-l/yr, from which we must take the appropriate actions to have the consequence to be less than the acceptable region. The prediction of total frequency using regression analysis shows the limit operating time of pipeline is the range of 11 to 13 years, which is well consistent with that of the actual pipeline. Concludingly, the hazard-listing scheme suggested in this research will be very effectively applied to maintaining the underground pipelines.
