Usefulness assessment of secondary shield for the lens exposure dose reduction during radiation treatment of peripheral orbit (안와 주변 방사선 치료 시 수정체 피폭선량 감소를 위한 2차 차폐의 유용성 평가)

  • Purpose : This study presents the usefulness assessment of secondary shield for the lens exposure dose reduction during radiation treatment of peripheral orbit. Materials and Methods : We accomplished IMRT treatment plan similar with a real one through the computed treatment planning system after CT simulation using human phantom. For the secondary shield, we used Pb plate (thickness 3mm, diameter 25mm) and 3 mm tungsten eye-shield block. And we compared lens dose using OSLD between on TPS and on simulation. Also, we irradiated 200 MU(6 MV, SPD(Source to Phantom Distance)=100 cm, $F{\cdot}S\;5{\times}5cm$) on a 5cm acrylic phantom using the secondary shielding material of same condition, 3mm Pb and tungsten eye-shield block. And we carried out the same experiment using 8cm Pb block to limit effect of leakage & transmitted radiation out of irradiation field. We attached OSLD with a 1cm away from the field at the side of phantom and applied a 3mm bolus equivalent to the thickness of eyelid. Results : Using human phantom, the Lens dose on IMRT treatment plan is 315.9cGy and the real measurement value is 216.7cGy. And after secondary shield using 3mm Pb plate and tungsten eye-shield block, each lens dose is 234.3, 224.1 cGy. The result of a experiment using acrylic phantom, each value is 5.24, 5.42 and 5.39 cGy in case of no block, 3mm Pb plate and tungsten eye-shield block. Applying O.S.B out of the field, each value is 1.79, 2.00 and 2.02 cGy in case of no block, 3mm Pb plate and tungsten eye-shield block. Conclusion : When secondary shielding material is used to protect critical organ while irradiating photon, high atomic number material (like metal) that is near by critical organ can be cause of dose increase according to treatment region and beam direction because head leakage and collimator & MLC transmitted radiation are exist even if it's out of the field. The attempt of secondary shield for the decrease of exposure dose was meaningful, but untested attempt can have a reverse effect. So, a preliminary inspection through Q.A must be necessary.

A Study of Small Radiation Dosimeter by Using Microfilm and Carbon Elecrtode (마이크로필름과 탄소막 전극을 이용한 소형방사선측정기 개발에 관한 연구)

  • We developed very small parallel plate radiation detector by using our existing experience of mating radiation dosimeter and capability of analyzing characteristics of dosimeter. The radiation detector was consisted of microfilm and carbon electrode. The detector was parallel plate type of all-filled ionization chamber. The ionization chamber had been fabricated using an acrylic plate for the air cavity and carbon coated microfilm for electrical configuration. The alr gap between two electrodes was 0.48 mm. The diameters of collect electrode and guard electrode were 3.3 mm, 5 mm respectively. The diameter of high voltage electrode was 5 mm. Nominal sensitive volume of the chamber was 0.016 ㎤. The major parameters of the chamber characteristics such as leakage current, reproducibility, dose rate effect, and polarity effect were measured. The experimental results were as followings. Leakage current was 0.1 pA. Standard deviation of reproducibility was less than 0.1%. Dose rate effect was less than 1.5%. Polarity effect was less than 2.4%. These data were comparable to those of commercially available dosimetric system for QA-purpose. As the result, we found that the radiation detector consisting of the ionization chamber, microfilm and carbon electrode, was satisfactory for the purpose of the small field dosimetry in size and characteristics. In the future, We will try to refine the dosimeter for use in very small volume.

The Radiation Safety Management in the Animal Hospital Using Inspection Standard of Diagnosis Radiation System (진단용 방사선발생장치의 검사기준을 적용한 동물병원의 방사선 안전관리)

  • The purpose of this study was to investigate the actual conditions of radiation safety supervision in animal clinics using inspection standard of X-ray generator for diagnosis. The surveys for inspection standard system, equipment condition, and safety supervision were carried out in 18 animal clinics randomly. The inspection standard included reproducibility of dose exposure, kVp, mAs, collimator accuracy test, collimator luminance test, X-ray view box luminance test, grounding system equipment test and external leakage current test. The surveys of equipment condition and safety supervision used one-on-one interview with 5 points measurement. As a result, 44.44% of reproducibility of dose exposure was proper, 81.25% of kVp test was good, and 100% of mAs test was appropriate. Also, 66.66% of collimator accuracy test was proper, 61.11% of collimator luminance test was good, 53.13% of X-ray view box luminance test was suitable. In addition, only 5.55% of grounding system equipment and ground resistance was proper, 63.64% of external leakage current test was appropriate in grounding system equipment test. The 100mA electric capacity of X-ray generator for diagnosis was popular with 44.44%, and its 55.56% was purchased used equipment. Monthly average of less than 50 times (61.11%) was top frequency in use, and no animal clinic had a thermo-luminescence dosimeter(TLD). The 16 animal clinics with radiation safety zone and 2 without radiation safety zone were appeared.

A Study on Development of a PIN Semiconductor Detector for Measuring Individual Dose (개인 선량 측정용 PIN 반도체 검출기 개발에 관한 연구)

  • The fabrication process and the structure of PIN semiconductor detectors have been designed optimally by simulation for doping concentration and width of p+ layer, impurities re-contribution due to annealing and the current distribution due to guard ring at the sliced edges. The characteristics to radiation response has been also simulated in terms of Monte Carlo Method. The device has been fabricated on n type, $400\;{\Omega}cm$, orientation <100>, Floating-Zone silicon wafer using the simulation results. The leakage current density of $0.7nA/cm^2/100{\mu}m$ is achieved by this process. The good linearity of radiation response to Cs-137 was kept within the exposure ranges between 5 mR/h and 25 R/h. This proposed process could be applied for fabricating a PIN semiconductor detector for measuring individual dose.

Clinical assessment of the efficacy of supraglottic airway devices compared with endotracheal tubes in cats during volume-controlled ventilation

  • The efficacies of a supraglottic airway device (SGAD) and an endotracheal tube (ETT) in cats under general anesthesia with volume-controlled ventilation (VCV) were compared. Thirty healthy cats were randomly allocated for airway control using either an SGAD or an ETT. Five tidal volumes (6, 8, 10, 12, and 14 mL/kg) were randomly tested, and respiratory rates were adjusted to achieve a minute ventilation of 100 mL/kg/min. The dose of propofol necessary to insert the SGAD or ETT, the static respiratory pressure, leakage during VCV, and end tidal CO2 (ETCO2) were recorded. Dosages of propofol and static respiratory measurements for the SGAD and ETT groups were compared using a t-test. The distribution of leakages and hypercapnia (ETCO2 > 45 mmHg) were compared using Fisher's exact test. A significance level of p < 0.05 was established. No significant difference in dose of propofol was observed between the SGAD and ETT groups (7.1 ± 1.0, 7.3 ± 1.7 mg/kg; p = 0.55). Static resistance pressure of the SGAD (22.0 ± 8.1 cmH2O/L/sec) was significantly lower than that of the ETT (36.6 ± 12.9 cmH2O/L/sec; p < 0.01). Of the 75 trials, leakage was more frequent when using an SGAD (8 events) than when using an ETT (1 event; p = 0.03). Hypercapnia occurred more frequently with SGAD (18 events) than with ETT (3 events; p < 0.01). Although intubation with an ETT is the gold standard in small animal anesthesia, the use of an SGAD can reduce airway resistance and the work of breathing. Nonetheless, SGAD had more dead space and the tidal volume for VCV needs adjustment.

Feasibility Study of Parallel- Plate Detector Using Dielectric film for 6 MV X-ray (6MV X-선 검출특성 조사를 위하여 유전체 필름을 이용하여 제작한 평행판 검출기의 유용성)

  • The parallel plate detector with dielectric film for dosimetry was designed to measure detection characteristic of 6 MV X-ray with medical linear accelerator. PTFE film was inserted into FEP films that are made by two one-side metal coated materials for ion source. The thicknesses of PTFE dielectric film was 100 ${\mu}{\textrm}{m}$ and the thickness of FEP dielectric film was 100 ${\mu}{\textrm}{m}$, respectively. This detector was fixed by two acrylic plate for physical hardness ad geometrical consistency. The geometrical condition for measurement with parallel-plate for detector was below; SSD=100 cm and the 5 cm depth between detector and phantom surface The major parameter of detector characteristics such as zero drift current, leakage current, charge response by applied voltage, reproducibility, linearity, TMR measurement, dose rate effect were measured. The zero drift currents are 8.3 pA and leakage currents are 10 pA. The charge response of applied voltage is showing linearity in 414 voltage. The measurement deviation of reproducibility in this detector is within 1% for dose and the linearity of applied dose shows in this detector. The TMR curves in phantom between this parallel plate detector and reference detector are matched within 3% deviation from maximum dose depth to 7.5 cm depth. It is considered that this dosimetric system is satisfactory for the purpose of the constancy check of the 6 MV x-ray from medical linear accelerator.

Effects of Low Dose Gamma-irradiation on Alleviation of Salt Stress in Rice (Oryza sativa L.) Seedling (저선량 감마선 조사가 벼 유묘의 염 스트레스 경감에 미치는 영향)

  • To investigate alleviation effects of salt stress by low dose gamma-irradiation on seedling growth of rice (Oryza shrive L), in plants of two rice cultivars (Ilpumbyeo and Gancheokbyeo), seeds were irradiated with the different doses $(0{\sim}32\;Gy)$ of gamma-ray by irradiator ($^{60}Co$, ca.150 TBq of capacity, AECL). The salt stress caused a remarkable decrease of seedling growth, particularly for dry weight. In addition, relative growth rate was decreased over 50% by the salt stress. However, the salt stress-induced inhibition of growth was alleviated by gamma-irradiation. Especially, the relative growth rate of shoot showed the reduction effect over 30% at 4 Gy-irradiated groups as compared with that of control. Also, electrolyte leakage and relative water content were damaged, respectively 4 times and 30% by the salt stress but the damages were alleviated by the gamma-irradiation. These alleviation effects were observed similarly in both the cultivars tested. These results suggest that the low dose gamma-irradiation may have a better protection or the salt stress.

  • The extract of Mawhang-tang showed considerable inhibition of 70.7 and 57.2% at oral doses of 200 and 400mg/kg, respectively, on carrageenin edema in rat paw. In case of combined oral administration of the extract at each dose of 100, 200 and 400mg/kg with 100mg/kg of aspirin, the inhibition percentages were 78.2, 87.2 and 72.5%, respectively. The combined oral administration of 200mg/kg of the extract with 200mg/kg of aspirin exhibited 87.6% inhibition of the edema. On the writhing syndrome induced by 0.7% acetic acid solution, the oral administration of 200mg/kg of the extract with 200mg/kg of aspirin showed remarkable inhibition of 90.3%. In the inhibitory effect of the leakage of dye into peritoneal cavity were shown to be 51.9, 56.6 and 58.1% at the combined administration of the extract at the doses of 100, 200 and 400mg/kg with 100mg/kg of aspirin, respectively.

  • In vitro cell culture system has been proposed as a promising alternative model to in vivo skin irritation test. These studies were performed to screen the cytotoxicity effects of surfactants using normal human skin fibroblasts. Cell membrane integrity assessed by the leakage of lactate dehydrogenase (LDH) and mitochondrial integrity by MTT [3-(4, 5-dimethylthiazol-2-yl)-2, 5-diphenyl tetrazolium bromides reduction test were affected in a dose dependent manner. The irritation potential of surfactants to human skin patch test, and the changes of capillary permeability by rabbit intradermal safety test were assessed as in vivo methods. Our results suggest that LDH leakage assay and MTT reduction test using cultured human fibroblasts could be predictive for the irritancy of various surfactants in human, and LDH assay is superior correlated with in vivo test (r=0.886) to MTT test with in vivotest (r=0.757).

Estimation of Secondary Scattered Dose from Intensity-modulated Radiotherapy for Liver Cancer Cases (간암환자에 대한 세기조절방사선치료에서의 2차 산란선량평가)

  • We estimated secondary scattered and leakage doses for intensity-modulated radiotherapy (IMRT), volumetric arc therapy (VMAT) and tomotherapy (TOMO) in patients with liver cancer. Five liver patients were planned by IMRT, VMAT and TOMO. Secondary scatter (and leakage) dose and organ equivalent doses (OEDs) are measured and estimated at various points 20 to 80 cm from the iso-center by using radiophotoluminescence glass dosimeter (RPLGD). The secondary dose per Gy from IMRT, VMAT and TOMO for liver cancer, measured 20 to 80 cm from the iso-center, are 0.01~3.13, 0.03~2.34 and 0.04~1.29 cGy, respectively. The mean values of relative OED of secondary dose of VMAT and TOMO for five patients, which is normalized by IMRT, measured as 75.24% and 50.92% for thyroid, 75.14% and 40.61% for bowel, 72.30% and 47.77% for rectum, 76.21% and 49.93% for prostate. The secondary dose and OED from TOMO is relatively low to those from IMRT and VMAT. OED based estimation suggests that the secondary cancer risk from TOMO is less than or comparable to the risks from conventional IMRT and VMAT.
