Intracardiac a Aortic Foreign Body (심장 및 대동맥내 이물 치험 1례)

  • A 50-year-old male patient was admitted due to right ventricular & aortic foreign bodies with ascending aortic pseudoaneurysm. The patient had a history of Kirschner wire fixation of right sternoclavicular joint 3 months ago. Under cardiopulmonary bypass, two K-wires were removed and injured pulmonary valve leaflet and aortic wall were repaired successfully The postoperative course was uneventful and the patient was discharged on the 14th postoperative day.

Surgical Experience of Idiopathic Hypertrophic Subaortic Stenosis - Report of A case - (특발성 비후성 대동맥판하 협착증의 수술적 치험 1례 보)

  • A 23-y-o male patient was suffered from intermittent fainting and dyspnea on exertion [NYHA Class IIIIV]. 2-D - Echocardiogram and cardiac catheterization with cineangiogram showed typical IHSS findings those were asymmetrical septal hypertrophy [ASH], systolic anterior motion of anterior mitral leaflet [SAM] which induced mild mitral regurgitation [Seller Grade I/IV] and pressure gradient about 60 mmHg between left ventricle and the aorta. Medical treatment with 8-adrenergic blockade [propranolol] and Ca" channel antagonist [Verapamil] had no response. So, we performed trans-aortic ventricular septal myotomy and mymectomy. Resected rectangular muscle bar was 1 Cm x 1 Cm x 4.5 Cm. Post-operative pressure gradient between the left ventricle and the aorta was less than 10 mmHg and SAM. was disappeared with decreased mitral regurgitation grade. Post-operative course was smooth and his symptoms and signs were free without any medication during 12 months follow-up.w-up.

Surgical treatment of Ebstein`s Anomaly-Report of 6 cases- (Ebstein 기형의 외과적 치료-6례 보고-)

  • Increased clinical interest, stimulated by newer diagnostic techniques, has made it apparent that Ebstein`s anomaly represents a spectrum of right heart abnormalities. In the usual case, the septal and posterior leaflets of the tricuspid valve are not attached to the true annulus, but are displaced distally into the ventricle. Also the degree of malattachment and the structural deformities of the cusps may vary greatly. Recently we experienced 6 cases of Ebstein`s anomaly who underwent corrective operation in our hospital. Five patients were operated by plication and annuloplasty techniques based on the construction of a monocusp valve by the use of the anterior leaflet. Remained one was operated on by tricuspid valve replacement using No 31 mm C-E tissue valve. There was no operative mortality and the postoperative courses were relatively good with marked symptom improvement.

  • Atrioventricular canal is divided into incomplete, intermediate and complete types. In ostium primum ASD [incomplete type] mitral valve cleft is almost always present, but ostium primum ASD with tricuspid valve cleft is a rare congenital anomaly. The patient was a 7 year old female whose complains were palpitation, exertional dyspnea and growth retardation. The chest films showed moderate cardiomegaly [C-T ratio, 61%]. EKG, Echocardiography, cardiac catheterization and left ventriculography were performed. Open heart surgery was done under the impression of incomplete atrioventricular canal. At the time of operation, ostium primum ASD [2x2.5 cm in diameter], secundum type ASD [lxl.5 cm in diameter] and cleft in the septal leaflet of the tricuspid valve were noted. But mitral valve was normal without cleft and VSD was not noted. Each anomalous portion was repaired. The patient made an uneventful recovery and we report this case, review and discuss the literatures.

  • The mitral valve is not a simple but a complex structure, mitral valve complex. This complex is composed of mitral orifice, annulus, valve leaflet, chordae tendineae and papillary muscles. There were many controversies about this structures. We studied mitral valve complex in 63 cases of normal Korean adults, such as the circumference of mitral annulus, the intercommissural diameter, height and breadth of the cusps, including commissure, the ratio of rough to clear zone, breadth of the achordal zone, the number of scallops of posterior cusp, the number, length and the pattern of arrangement of the several types of chordae, the number and morphological characteristics of papillary muscles.

Surgical Treatment of VSD with Endocarditis: 2 Cases (심내막염을 합병한 심실중격결손증의 외과적 치료 : 2례 보고)

  • Bacterial endocarditis has been well recognized as an important complication of congenital heart disease. The most common. form of congenital heart disease is the VSD, of which natural history is spontaneous closure, pulmonary vascular disease, symptoms, and endocarditis. The incidence of endocarditis is relatively low. But endocarditis is almost universally fatal if untreated. Two cases of VSD with endocarditis, 4 \ulcorneryear male and 17 \ulcorneryear female, were treated at Department of Thoracic and Cardiovascular Surgery, Chonbuk National University. In the First case, the VSD was perimembranous type and vegetation located on the septal leaflet of the tricuspid valve. After 7 week medical treatment, simple closure of the VSD, removal of vegetation, and tricuspid annuloplasty were performed. In the second case. the VSD was subpulmonic type and the pulmonic valve was destructed due to vegetation. So the VSD was closed with interrupted 4 \ulcorner0 Prolene sutures and the pulmonic valve was excised. Postoperative course of all cases was uneventful.

  • Ebstein's anomaly is a complex congenital defect of the tricuspid valve and right ventricle. Various surgical methods to repair the regurgitant tricuspid valve have been reported, and most of them depend on monocuspidalization with using the anterior leaflet. We report here on our first experience with Ebstein's anomaly in a 31-year-old female patient who underwent cone reconstruction of the tricuspid valve with using three leaflets.

  • In this study, a numerical analysis has been performed for a three-dimensional pulsatile blood flow associated with the elastic blood vessel and curved bileaflet for multiple cycles in terms of fluid-structure interaction. Here, blood has been assumed as a Newtonian, incompressible fluid. Pressure profiles have been used as boundary conditions at the ventricle and the aorta. From this analysis, the motion of the leaflet has been observed with fluttering phenomenon and rebound, and the flow fields of blood have been obtained with recirculation and regurgitation. The results can contribute to the development of design methodology for the curved bileaflet mechanical heart valve.

  • 기존의 철도용량의 개념은 하루동안 최대한 통과시킬 수 있는 최대 열차 횟수를 기본으로 하여왔다. 그러나 이러한 단순한 최대횟수는 그 사용상의 여러 가지 제한사항을 가지고 있다. 최대열차 운행횟수는 열차의 속도, 신뢰성등과 같이 고려하여야 하며, 이러한 고려는 자연스럽게 서비스수준과 같은 과거철도에서는 구체적으로 고려하지 않던 새로운 용량에 대한 접근을 요구하게 된다. 2004년에 세계철도연맹(UIC)에서 제안한 철도용량 산정절차는 이러한 측면에서 많은 점을 시사하고 있다. 새로운 절차의 한국의 지역간 철도에의 적용가능성을 검토해 본다. 본 연구에서는 2006년 11월 새로 개편된 경부선, 호남선의 열차시간표를 이용하여, 문제가 되고 있는 서울역${\sim}$시흥역 구간에 대해 UIC에서 새롭게 제시한 선로용량의 개념을 OpenTrack을 이용한 시뮬레이션을 통해 적용해보고, 이에 따라 선로 용량 산정시 적용할 수 있는 MOE를 찾아내 서비스수준(LOS)을 결정할 수 있는 방안을 제시해보고자 한다.

  • To develop the supplementary heart valve which could reduce the complications of prosthetic heart valve, in vitro experiments were performed to analyze the movement of the supplementary valves. The autologous pericardium was attached to deficient portion of the porcine valve which was dissected completely and partially. The instantaneous motion of the leaflet was pictured by the 35mm camera and the pressure drop through the valve and cardiac output were measured in mock circulatory system. The durability of the valves were tested in severe hemodynamic conditions.
