  • When the midfacial fractures involve the upper maxilla (LeFort I, II, III), there is a chance that the nasolacrimal duck may have been injured. When this suspected, We must observe the presence of epiphora carefully. If the epiphora was not improved following conservative treatment, dacryocystorhinostomy would be the treatment of choice. We performed two cases of DCR and got the favorable results.

  • This case report describes the treatment of a 23-year-old woman who had lip protrusion with gummy smile and mentalis muscle strain. Orthognathic surgery was performed in conjunction with orthodontics. Minimum dental decompensation was performed with presurgical orthodontics followed by an anterior segmental osteotomy for the majority of dental decompensation. Counterclockwise rotation of the maxillomandibular complex was applied by LeFort I osteotomy, and bilateral sagittal split ramus osteotomies with anterior segmental osteotomy to achieve overall facial balance. The active treatment period was 15 months. Stable occlusion and skeletal relationship were observed after a 10-month follow-up period.

  • The posttraumatic complications of jaw fractures related to jaw function and facial deformity include nonunion, malunion, malocclusion, temporomandibular joint dysfunction and facial asymmetry. This report presents cases referred to our department for revision of malunion and malocclusion following inadequate reduction of jaw fractures. Three patients with posttraumatic malocclusions caused by malunion were treated with a LeFort I osteotomy in one case and re-fracture in two cases. All of the patients exhibited stable results without further complications (e.g., malunion or malocclusion). Accurate preoperative diagnosis and proper anatomical reduction of the fracture segments are essential to preventing post-surgical malunion and malocclusion.

악교정 수술을 받은 골격성 III급 부정교합 환자의 악관절 상태에 관한 연구 (The Evaluation of TMJ Status after Orthognathic Surgery for Skeletal Class III Malocclusion)

  • This study was performed to investigate the influence of orthognathic surgery on the temporomandibular dysfunction in skeletal class III malocclusion. The temporomandibular joint status in 22 patients(mean age: 23.7 years) who received orthognathic surgery such as mandibular BSSRO(14 patients), maxillary Le Fort I osteotomy with mandibular BSSRO(8 patients) was evaluated by craniomandibular index. All these patients received orthognathic surgery at least 6 months ago. The mean score and standard deviation was obtained and compared with that of 22 normal individuals(mean age: 24.8years) by Student's t-test. In mandibular movement, the score of orthognathic surgery group was higher than that of the normal group. All the items except mandibular movement did not show any differences between the two groups.

편측성 구순구개열 환자에서의 편측성 분절 구개골 신장술 (Unilateral Segmental Palatal Distraction in Unilateral Cleft Lip and Palate Patient)

  • Patients with unilateral cleft lip and palate (UCLP) usually present unilateral cross bite due to collapse of the maxillary minor segment. Unequal expansion of the palate is needed to resolve this problem in UCLP patient. Unilateral segmental palatal distraction (USPD) after Le Fort I osteotomy and the oblique placed orthodontic expansion screw (Hyrax) can be used to correct the unilateral cross bite. 1his case report describes the effects of USPD of the collapsed maxillary minor segment on patient with unilateral cleft lip and palate.

  • This is a case report of orthognathic surgery for the correction of maxillary retrusion and mandibular protrusion. The summary and results are as follows, 1. The maxillary retrusion was corrected by LeFort I osteotomy. 2. The mandibular protrusion was corrected by sagittal split osteotomies in the rami. 3. And, for the correction of the discrepancy between max8llary and mandibular arches, the mandibular arch was widened by the midsymphyseal step osteotomy. 4. The ratios of horizontal changes of soft tissue to hard tissue at the points, Subnasale (Sn), Labrale superius (Ls), Labrale inferius (L9), and Supramentale (B) were 67.6%, 43.2$, 70.2% and 87.7%, respectively.

Application of Monitored Anesthesia Care Using Dexmedetomidine to Common Oral and Maxillofacial Trauma Cases

  • Oral and maxillofacial surgery (OMFS) trauma cases are commonly treated under general anesthesia. The purpose of this case report is to introduce an alternative method of anesthesia in patients who refuse general anesthesia. A combination of dexmedetomidine and ketamine for sedation anesthesia in 3 frequent fracture types in the field of OMFS-Le Fort I fracture, mandibular fracture, and alveolar bone fracture-was used. Dexmedetomidine as the single agent has not shown stable success rates for invasive procedures. To overcome some of the pitfalls with dexmedetomidine, combination sedation using ketamine was performed. Visual analogue scale scores were recorded postoperatively. Dexmedetomidine combined with ketamine administration provided safe and effective sedation and anxiolysis for surgical reduction and internal fixation of OMFS fractures. It showed advantages of decreased admission time, reduced expenses, minimal pain, and reduced anesthetic burden for the patient thus ultimately increasing overall satisfaction.

Surgery-first Approach for Facial Asymmetry with Transverse Discrepancy Using Hyrax-type Palatal Expansion Appliance

  • This report presents a case of successful treatment of skeletal Class III malocclusion with transverse discrepancy in adult by surgery-first approach. Traditionally dental decompensation is necessary prior to surgery in 2-jaw surgery to correct transverse and rotational discrepancy. However, the hyrax-type palatal expansion appliance was used to improve maxillary expansion accuracy and postoperative stability in this surgery-first approach (segmental Le Fort I osteotomy and mandibular setback surgery). It was established to be an effective means of precisely predicting postoperative occlusion and achieving stable retention after surgery of skeletal Class III malocclusion with maxillary transverse discrepancy.

골격성 III급 부정교합자에서 상악골 전방 이동술 후 코의 변화에 관한 연구 (Nose Changes after Maxillary Advancement Surgery in Skeletal Class III Malocclusion)

  • 본 연구는 골격성 III급 부정교합 환자에서 상악골 전방이동 수술 후 야기되는 코의 형태변화에 대해 연구하고 이를 예측할 수 있는 요소를 찾아 교정-악교정 수술 복합 치료계획의 수립과 결과의 예측에 이용하고자 시행되었다. 부산대학교병원 치과진료처 교정과에 내원하여 골격성 III급 부정교합으로 진단되어 술전 교정치료를 받고 Le Fort I 골절단술로 상악골의 전방 이동시키는 동시에 하악골을 후방 이동시킨 남녀 성인 환자 25명을 대상으로 하였다. 이들의 수술전, 후 측모두부규격방사선사진과 정모 및 측모 안면사진을 계측, 분석하여 다음과 같은 결과를 얻었다. 1. 상악골 전방이동에 따른 비첨의 수직적 위치 변화는 상관성이 높았으며 ANt, SNt에서 A point의 전방이동에 의해 ${\beta}_0$는 각각 0.228, 0.257로 나타났다. 2. 상악골 전방이동에 따른 비첨의 수평적 위치변화는 상관성이 높았으며 ANt, SNt, Sn point에서 A point의 전방이류에 의해 ${\beta}_0$는 각각 0.484, 0.431, 0.806으로 나타났다. 3. 상악골의 전방이동에 따른 비익부의 확장정도는A point의 전방이동에 의해 0.002의 ${\beta}_0$를 나타내었으나, 임상적으로 유의한 변화는 없었다. 4. 수술 후 비첨의 수직적 및 전후방적 위치변화를 예측하는 지표로 사용된 ADV, DRI, Prominence of nose, Pre-Op CA 중 ADV 만이 설명변수로서 유의성이 있었다.

III급 부정교합자의 양악 수술과 하악 편악 수술 시 연조직 변화에 관한 비교 연구 (A comparative study of soft tissue changes with mandibular one jaw surgery and double jaw surgery in Class III malocclusion)

  • 악교정 수술 시행 시 연조직 변화에 대한 대부분의 연구는 II급 부정교합을 대상으로 하였다. 또한 수술 방법에 따른 연조직 변화에 대한 비교 연구가 부족하였다. 따라서 이 연구는 골격성 III급 부정교합자에서 상악골 전진술과 하악골 후퇴술을 동시에 시행한 양악 수술과 하악골 편악 수술 시행 시, 경조직과 연조직 측모 및 연조직 두께의 변화를 관찰하고, 수술 전후 변화의 상관성을 산출하며, 경조직 변화에 수반하는 연조직 변화의 비율을 산출하여 교정-악교정 수술 복합 치료 계획 수립과 결과의 예측에 이용하고자 시행되었다. 경희대학교 치과병원 교정과에 내원하여 골격성 III급 부정교합자로 진단받은 환자 52명을 대상으로 하여, Le Fort I osteotomy를 이용한 상악골 전방이동술과 시상분할골절단술을 이용한 하악골 후퇴술을 시행한 양악 수술군 26명과 하악골 후퇴술만을 시행한 편악 수술군 26명으로 구분하여, 수술 전후 측모두부방사선규격사진을 계측, 분석하여 다음과 같은 결과를 얻었다. 양악 수술군은 골격 변화량의 72.4% 비율로 상순 부위 연조직의 전방 이동을 보였으며, 편악 수술군은 통계적으로 유의한 변화를 보이지 않았다. 비순각은 양악 수술군에서 편악 수술군에 비해 더 크게 증가하였다. 이순각은 편악 수술군에서 더 크게 감소하였다. 하악골의 후방 이동에 따른 연조직 pogonion의 후방 이동은 양악 수술군에서 98%, 편악 수술군에서 109%로 편악 수술군에서 더 크게 나타났다. 양악 수술군에서는 편악 수술군에 비하여 경조직의 변화에 수반하는 상순에서의 큰 변화가 나타났다. 편악 수술군에서는 상순의 변화는 미약하였으며, 하순 및 이부의 변화가 양악 수술군에 비해 두드러지게 나타났다.
