Background: The causes of the pleural effusion are remained unclear in a the substantial number of patients with exudative effusions determined by an examination of the fluid obtained via thoracentesis. Among the various tools for diagnosing exudative pleural effusions, thoracoscopy has a high diagnostic yield for cancer and tuberculosis. Medical thoracoscopy can also be carried out under local anesthesia with mild sedation. The aim of this study was to determine diagnostic accuracy and safety of medical thoracoscopy. Methods: Twenty-five patients with exudative pleural effusions of an unknown cause underwent medical thoracoscopy between October 2005 and September 2006 in Konyang University Hospital. The clinical data such as age, gender, preoperative pulmonary function, amounts of pleural effusion on lateral decubitus radiography were collected. The vital signs were recorded, and arterial blood gas analyses were performed five times during medical thoracoscopy in order to evaluate the cardiopulmonary status and acid-base changes. Results: The mean age of the patients was 56.8 years (range 22-79). The mean depth of the effusion on lateral decubitus radiography (LDR) was 27.49 mm. The medical thoracoscopic pleural biopsy was diagnostic in 24 patients (96.0%), with a diagnosis of tuberculosis pleurisy in 9 patients (36%), malignant effusions in 8 patients (32%), and parapneumonic effusions in 7 patients (28%). Medical thoracoscopy failed to confirm the cause of the pleural effusion in one patient, who was diagnosed with tuberculosis by a pericardial biopsy. There were no significant changes in blood pressure, heart rate, acid-base and no major complications in all cases during medical thoracoscopy (p>0.05). Conclusions: Medical thoracoscopy is a safe method for patients with unknown pleural effusions with a relatively high diagnostic accuracy.
Objective: The purpose of this study was to compare the cephalometric measurements of obese and non-obese Korean male patients with obstructive sleep apnea syndrome (OSA). Methods: Eighty-seven adults who had visited the Sleep Disorder Clinic Center in Keimyung University, Daegu, Korea were examined and evaluated with polysomnography (PSG) and lateral cephalogram. They were divided into 4 groups (non-obese simple snorers, obese simple snorers, non-obese OSA patients, obese OSA patients) according to AHI (Apnea-Hypopnea Index) and BMI (Body Mass Index). Results: The obese OSA group had the highest AHI among the 4 groups. The non-obese OSA group had a significantly steeper mandibular angle and shorter tongue length than the obese OSA group. The hyoid bone of the obese OSA group was positioned anterior and inferior as compared with the non-obese OSA group. Multiple regression analysis showed that tongue length in the obese OSA group and retroposition of hyoid bone in the non-obese OSA group were significant determinants for the severity of AHI. Conclusions: From a cephalometric point of view, the obese and non-obese pateints with OSA may be characterized by different pathogeneses. Therefore, they have to be managed by individualized treatment. For the obese OSA patients, weight control must be advised as a first choice and for the non-obese OSA patients, oral appliance, nasal CPAP, UPPP and others could be chosen according to the obstructive sites.

By improving the flow pattern in Fontan circuit, total cavopulmonary connection (TCPC) could result in a better outcome than atriopulmonary connection Fontan operation. For the patients with impaired hemodynamics after atriopulmonary Fontan connection, conversion to TCPC can be expected to bring hemodynamic and functional improvement. We studied the results of the revision of the previous Fontan connection to TCPC in patients with failed Fontan circulation. Material and method: From October1979 to June 2002, eight patients who had failed Fontan circulation, underwent revision of previous Fontan operation to TCPC at Yonsei University Hospital. Intracardiac anomalies of the patients were tricuspid atresia (n=4) and other functional single ventricles (n=4). Mean age at TCPC conversion was 14.0$\pm$7.0 years (range, 4.6~26.2 years) and median interval between initial Fontan operation and TCPC was 7.5 years (range, 2.4~14.3 years). All patients had various degree of symptoms and signs of right heart failure. NYHA functional class was 111 or IV in six patients. Paroxysmal atrial fibrillation (n:f), cyanosis (n=2), intraatrial thrombi (n=2), and protein losing enteropathy (PLE) (n=3) were also combined. The previous Fontan operation was revised to extracardiac conduit placement (n=7) and intraatrial lateral tunnel (n=1). Result: There was no operative death. Major morbidities included deep sternal infection (n=1), prolonged pleural effusion over two weeks (n=1), and temporary junctional lachyarrhythrnia (n=1). Postoperative central venous Pressure was lower than the preoperative value (17.9$\pm$3.5 vs. 14.9$\pm$1.0, p=0.049). Follow-up was complete in all patients and extended to 50,1 months (mean, 30.3$\pm$ 12.8 months). There was no late death. All patients were in NYHA class 1 or 11. Paroxysmal supraventricular tachycardia developed in a patient who underwent conversion to intraatrial lateral tunnel procedure, PLE was recurred in two patients among three patients who had had PLE before the convertsion. There was no newly developed PLE. Conclusion: Hemodynamic and functional improvement could be expected for the patients with Fontan circulatory failure after atriopulmonary connection by revision of their previous circulation to TCPC. The conversion could be performed with low risk of morbidity and mortality.

On 4 September 2010 an earthquake of magnitude 7.1 on the Richter scale occurred on the Canterbury Plains in the South Island of New Zealand. The Canterbury Plains are an area of extensive groundwater and spring fed surface water systems. Since the September earthquake there have been several thousand aftershocks (Fig. 1), the largest being a 6.3 magnitude quake which occurred close to the centre of Christchurch on 22February 2011. This second quake caused extensive damage to the city of Christchurch including the deaths of 189 people. Both of these quakes had marked hydrological impacts. Water is a vital natural resource for Canterburywith groundwater being extracted for potable supply and both ground and surface water being used extensively for agricultural and horticultural irrigation.The groundwater is of very high quality so that the city of Christchurch (population approx. 400,000) supplies untreated artesian water to the majority of households and businesses. Both earthquakes caused immediate hydrological effects, the most dramatic of which was the liquefaction of sediments and the release of shallow groundwater containing a fine grey silt-sand material. The liquefaction that occurred fitted within the empirical relationship between distance from epicentre and magnitude of quake described by Montgomery et al. (2003). . It appears that liquefaction resulted in development of discontinuities in confining layers. In some cases these appear to have been maintained by artesian pressure and continuing flow, and the springs are continuing to flow even now. In spring-fed streams there was an increase in flow that lasted for several days and in some cases flows remained high for several months afterwards although this could be linked to a very wet winter prior to the September earthquake. Analysis of the slope of baseflow recession for a spring-fed stream before and after the September earthquake shows no change, indicating no substantial change in the aquifer structure that feeds this stream.A complicating factor for consideration of river flows was that in some places the liquefaction of shallow sediments led to lateral spreading of river banks. The lateral spread lessened the channel cross section so water levels rose although the flow might not have risen accordingly. Groundwater level peaks moved both up and down, depending on the location of wells. Groundwater level changes for the two earthquakes were strongly related to the proximity to the epicentre. The February 2011 earthquake resulted in significantly larger groundwater level changes in eastern Christchurch than occurred in September 2010. In a well of similar distance from both epicentres the two events resulted in a similar sized increase in water level but the slightly slower rate of increase and the markedly slower recession recorded in the February event suggests that the well may have been partially blocked by sediment flowing into the well at depth. The effects of the February earthquake were more localised and in the area to the west of Christchurch it was the earlier earthquake that had greater impact. Many of the recorded responses have been compromised, or complicated, by damage or clogging and further inspections will need to be carried out to allow a more definitive interpretation. Nevertheless, it is reasonable to provisionally conclude that there is no clear evidence of significant change in aquifer pressures or properties. The different response of groundwater to earthquakes across the Canterbury Plains is the subject of a new research project about to start that uses the information to improve groundwater characterisation for the region. Two parallel wide flange built-up beams are widely used as struts in resisting lateral earth pressure because of the effectiveness in structure and construction. In a certain structural system, the reinforced concrete columns are to be placed at the intersection where two perpendicular beams cross each other, the square part of the joint being filled with concrete. In the punching shear mechanism of the beam-column joint, the radial deformation caused due to shear cracking will be constrained by the spring action of the squarely encompassed beam flanges. As a result, the punching shear strength of the joint concrete can be expected to be increased. To verify this phenomenon experiments have been performed for various constraining elements and distances between columns and constraints. Test results are compared with the approximation analysis formula which has been proposed in this study, based on the code formula. The results calculated by the proposed equation show comparatively close agreement with the punching shear strength detected from the test.
The geotectonics and geomorphic structure of Korea resulted from the Song-rim Disturbance and the Daebo orogenic movements. Afterward this mountainous peninsula underwent several geological changes on a small scale, and it was also claimed that the steady rising of the elevated peneplain of the eastern coast and the submerging of the southwestern coastal area are largely due to the tilted block movement. These views have been generally accepted good in several ways, but they are limited in range or lacking in theoretical integration. The present writer investigated the geology of the Mt. Chi-ri-san and the Honam coal mining area for a geological map in 1965, respectively. The results of these studies convinced the present writer that the conventional views, which were based upon a theory of lateral pressure should be reconsidered in many respects, and more recent studies made it clear that the morphological development in the southwestern area can be better explained by the orogenic movement and rock control. The measurement of submerging speed of the western coastal area (Pak. Y. A., 1969) and a new account on the geology and tectonics of the Mid-central region of South Korea (Kim O.J., 1970) act as an encouragement to a new explanation. The present writer's researches on the extreme southwestern portion of the peninsula show that the steady submerging of this area cannot be attributed to a simple downthrown block phenomenon caused by block movement. It is no more than the result of the differential movement of uplifting in the eastern and western coastal areas and the rising of sea-level in the post-glacial period. This phenomenon could be easily explained by the comparison of the rate of rise in sea-level and amount of heat flow between Korea and other areas in the world. The existance of the erosional planes in the Sobaik-San ranges also provide an evidence of an upheaval in the western coast area. Though the Sobaik-San ranges largely follow the direction of the Sinian system. They consist of the numerous branches, whose trends run more or less differently from their main trend because of the disharmonic folding, are converged into Mt. Sobaik-San and Chupungryung. The undulation of the land is not wholely caused by orogenic movements, where as the present writer confirmed that the diversity of morphological development is the direct reflection of geological conditions such as rocks and processes which constitute the basic elements of geomorphic structure. An east-west directed mountain range which could be named as Hansan mountain range, was claimed to be oriented by the joint control. The geological conditions such as a special erosion and weathering of agglomerate and breccia tuff usually produce pot-hole like submarine features which cause the whirling phenomenon at the southwestern coast channel.
The purpose of this study was to investigate whether the ashed tooth powder is utilized as an alternative material of the implant to recovery the bony defect. For this purpose its biocompatibility was evaluated comparing to the synthetic calcium phosphate compounds, such as Syntograft and Calcitite, as well as the vacuum firing porcelain (Ceramco Inc.) which is anticipated to use as a matrix to aid sintering. Bony defects to exposure the bone marrow, $3{\times}5$ mm in size, were created in the right and left tibias of fifteen rabbits, and then the ashed tooth powder at $950^{\circ}C$, the porcelain powder, Syhtograft and Calcitite were inserted in the defects of twelve rabbits of the experimental group and the blood clot only was filled in the defects of three rabbits of the control group. The experimental and control rabbits were sacrificed at 1st, 2nd 3rd week after implantation and the histologic examination was performed. The ashed tooth powder in order to make the needed form of the implant was molded using the cylindrical mold 1 cm high, 1 cm in diameter under the pressure of $1000kg/cm^2$ and the ashed tooth powder was sintered at $1100^{\circ}C$ for 1 hour and the mixture of the porcelain powder and the ashed tooth powder at the weight ratio of 7:3, 6:4, 5:5, 4:6 were molded in the same manner and were sintered at $925^{\circ}C$. From this sintered material, square shaped implants were prepared in the dimension of $2{\times}4{\times}6mm$. The prepared implants were surgically placed in the subperiosteum of lateral surfaces of the right and left mandibular bodies. The dogs were sacrificed at 4 weeks, and then the specimens were examined using the light and scanning electron microscopes. The results of this study were obtained as follows: 1. Any inflammatory response was not noted after implanting of the ashed tooth powder, Syntograft, Calcitite and the porcelain powder during the whole experimental period after implantation. 2. Induction of the new bone formation was significantly shown in the ashed tooth powder, Syntograft and Calcitite. 3. The more the porcelain powder was contained in the implants, the more the porosity was and the bigger the pore size was under the scanning electron microscope. And there was ingrowing of the fibrous connective and the osteoid tissue. 4. The osteoid tissues were found to be directly fused to the implant of the ashed tooth powder, and the mixture implant of the porcelain powder and the ashed tooth powder at the weight ratio of 4:6 under the light and scanning electron microscopes.

This paper describes the continuing effort to develope a single acting free-piston Stirling engine/alternator combination for use of the household cogeneration. Free piston Stirling engines(FPSE) use variations of working gas pressure to drive mechanically unconstrained reciprocating elements. Stirling cycle free-piston engines are driven by the Stirling thermodynamic cycle which is characterized by an externally heated device containing working gas that is continuously re-used in a regenerative, reversible cycle. The ideal cycle is described by two isothermal process connected by two constant volume processes. Heat removed during the constant volume cooling process is internally transferred to the constant volume heating process by mutual use of a thermal storage medium called the regenerator. Since the ideal cycle is reversible, the ideal efficiency is that of Carnot. Free-piston Stirling engine is have no crank and rotating parts to generate lateral forces and require lubrication. The FPSE is typically comprised of two oscillating pistons contained in a common cylinder. The temperature difference across the displacer maintains the oscillations, and the FPSE operate at natural frequency of the mass-spring system. The power is generated from a linear alternator. The purpose of this paper is to describe the design process of the single acting free-piston Stirling engine/alternator. Electrical output of the single acting free-piston Stirling engine/alternator is about 0.95 kW.
The effect of stabilizing piles on cut slopes is checked and the behavior of slope soil and piles are observed throughout the year by field measurements on the large-scale cut slopes. First of all, the behavior of the slope soil was measured by inclinometers during slope modification. Landslides occurred in this area due to the soil cutting for slope modification. The horizontal deformations of slope soil are gradually increased and rapidly decreased at depth of sliding surface. As the result of measuring deformation, the depth of sliding surface below the ground surface can be known. Based on the measuring the depth of the sliding surface, some earth retention system including stabilizing piles were designed and constructed in this slope. To check the stability of the reinforced slope using stabilizing piles, an instrumentation system was installed. As the result of instrumentation, the maximum deflection of piles is measured at the pile head. It is noted that the piles deform like deflection on a cantilever beam. The maximum bending stress of piles is measured at the soil layer. The pile above the soil layer is subjected to lateral earth pressure due to driving force of the slope, while pile below soil layer is subjected to subgrade reaction against pile deflection. The deflection of piles is increased during cutting slope in front of piles for the construction of soil nailing. As a result of research, the effect and applicability of stabilizing piles in large-scale cut slopes could be confirmed sufficiently.

The discharge patterns and peripheral nerve inputs to cardiovascular neurons were investigated in rostral ventrolateral medulla (RVLM) and raphe nucleus of cats. The data from the two were compared to determine their roles in cardiovascular regulation and the endogenous analgesic system. Animals were anesthetized with ${\alpha}-chloralose$ and single cell activities were recorded by carbon-filament microelectrode and their relationships with cardiovascular activity were analyzed. In RVLM area, a total of thirty-three cells were identified as cardiovascular neurons. During one cardiac cycle, the mean discharge rate of the neurons was $1.96{\pm}0.29$ and the peak activity was observed 45 ms after the systolic peak of arterial blood pressure. Thirteen cells could be activated antidromically by stimulation of the the $T_2$ intermediolateral nucleus. Forty-three raphe neurons were identified as cardiovascular neurons whose mean discharge rate during one cardiac cycle was $1.02{\pm}0.12$. None of these cells could be activated antidromically. Study of the interval time histogram of RVLM neurons revealed that the time to the first peak was $128{\pm}20.0\;ms$, being shorter than the period of a cardiac cycle. The same parameter found from the raphe neurons was $481{\pm}67.2\;ms$, which was much longer than the cardiac cycle length. Of seventeen RVLM neurons examined ten received only the peripheral $A{\delta}-afferent$ inputs, whereas six RVLM neurons received both $A{\delta}-$ and C-inputs; the remaining one cell received an inhibitory peripheral C-input. In contrast, nine of eleven raphe neurons were found to receive $A{\delta}-inputs$ only. We conclude that the main output of cardiovascular regulatory influences are mediated through the RVLM neurons. The cardiovascular neurons in the raphe nucleus appear to serve as interneurons transferring cardiovascular afferent information to the raphespinal neurons mediating the endogenous analgesic mechanisms.

① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
