The Alluvial Fan Surface Deformation of the Northern Part of the Ulsan(Bulguksa) Active Fault System in the Southeastern Korea

  • The geomorphic deformation of the alluvial fans by tectonic movement was investigated along the lineaments of the northem part of the Ulsan(Bulguksa) fault system. Based on the aerial photographs interpretation and field surveys Ulsan fault system was identified as an active reverse fault which has displaced the Quatemary fan deposits. Buguksa fault system strikes for the direction of NW-SE and N-S. These two lineaments of active fault are crossing at Jinty village in Gyeongju city and the fault plane forms here almost vertical dip. The lateral pressures from the two directions have possibly influenced on the formation of the vertical dip at jinty villagy. It should be resulted from that the two pressures responsible for the active reverse fault at which the one with the NW-SE strike thrusts the hanging wall of Tohamsan block southwestward and the other pressure with the N-S strike thrusts it westward over the foot wall of the fan deposits. The marine oxygen isotope stage 8(0.30-0.25 Ma. BP) and stage 6(0.20-0.14 Ma. BP) are presumed to be the ages of high and middle surfaces of the alluvial fan, repectively. The vertical displacements on the high surfaces along the Bulguksa fault system are about 1.05 m at Ha-Dong, 9.5-10.5 m at Jinhyun-Dong, and about 10 m high at Jinty village. And the vertical displacement on the middle surface was measured about 6 m high at Ha-Dong. The average slip rate of vertical displacements is calculated about 0.03-0.43 mm/y.

  • The purpose of this study was to investigate the dynamic response of the mandible to impact and provide insight into the fracture mechanism of the mandible, by 3-dimensional finite element method. The finite element model of the mandible was developed and calculated using NASTRAN/XL (MSC co. U.S.A.) and the linear dynamic transient analysis was performed according to the impulsive force direction, force type and impulse time to the mandible. At first, the load was applied on the mandibular symphysis, body, angle and subcondylar area in the horizontal mandibular plane and the computed stress-time histories at 14 locations of the mandible were obtained. Secondly, the impulsive force was directed to the symphyseal area with changing the force magnitude and impulse time, and calculated the node displacement at 8 locations of mandible. The conclusions from from this study were as follows. 1. The appearance of impulsive energy transmission was different to the direction of impulse to the mandible. 2. The impulsive stress and deformation were larger in lingual or medial side than buccal or lateral in the mandible. 3. The velocity, appearance of energy transmission and the fracture pattern in mandible were affected rather impulse time than force. 4. The horizontal impact to the one side of mandible did not have effect on the stress and displacement of contralateral mandible. From the above results, fracture pattern in symphysis can be showed as simple or comminuted, multiple or associated in body and angle and solitary in subcondyle area.

  • The purpose of this study was to establish Korean norms that will be valuable aids for diagnosis in orthognathic surgery patients. Lateral cephalometric measurements were established for adult Koreans (60 males, 62 females) with normal occlusion and acceptable profiles. The results were as follows: 1. Means, standard deviation and sexual differentiation were included within each measurement. 2. In skeletal measurements, FH to SN and SN to mandibular plane measurements in females were larger than those in males while all linear measurements in males were larger than those in males were larger than those in females. 3. Significant differences were observed between male and female groups on all of the items that showed vertical relationship in dental measurements. And the measurements of ADH, PDH and overbite in males were larger than those in females while the measurements of incisor exposure in females were larger than those in males. 4. In all measurements of soft tissue except NLA, that is, of FCA, UFH, ULL and LLL, males were larger than those in females.

A ROENTGENOCEPHALOMETRIC STUDY ON MAXILLARY PROTRUSION (상악전돌에 관한 방사선 두부계측학적 연구)

  • This study was undertaken to compare the craniofacial morphology of Class II, Division 1 malocclusion with that of normal occlusion in children, and to investigate the incidence of various Class II, Division 1 craniofacial skeletal patterns. The subjects consist of thirty seven boys and fifty three girls with Class II, Division 1 malocclusion, and forty six boys and eighty one girls 10-15 years with normal occlusion. Measurements were recorded, tabulated and analyzed on the lateral cephalograms by the degree of SNA, SNB and ANB. The following characteristics of the Class II, Division 1 skeletal pattern were observed. 1. The anteroposterior relationship of the maxilla to the cranium in the Class II, Division 1 was very similar to that of normal occlusion. 2, Mandible of the Class II, Division 1 malocclusion was in the posterior position in relation to the cranial anatomy when compared to normal. 3. The chin point as measured by SN Pog and NS Gn showed distal positioning in relation to normal occlusion. 4. SN to mandibular plane angle was large in Class II, Division 1 malocclusion. 5. Mandibular incisor inclination was not significantly different between Class II, Division 1 malocclusion and normal occlusion, but maxillary incisors inclined and positioned labially and consequently overjet was large in Class II, Division 1 malocclusion. 6. Class II, Division 1 malocclusion was divided into four types of craniofacial skeletal pattern. The most common Class II, Division 1 pattern was found to be type C in which SN-Mand. Pl. was above mean range of normal occlusion. The next frequent pattern was found to be type A in which maxilla and mandible were within normal range of protrusion while upper incisors were severly labially inclined.

Simultaneous Augmentation Rhinoplasty with Bony Reduction in Nasal Bone Fracture (비골골절 시 골절정복과 동시에 시행된 융비술)

  • Purpose: The nasal bones are the most common fracture sites of the facial bones, and a careful reduction may still result in secondary deformities, such as saddle nose, deviated nose, hump nose etc, requiring secondary cosmetic rhinoplasty. Therefore, this study examined the clinical characteristics of nasal bone fractures to propose guidelines for patient selection and surgical procedures to achieve more satisfactory results and to prevent secondary deformities with simultaneous augmentation rhinoplasty and bony reduction. Methods: The study was based on 26 out of 149 nasal bone fracture patients who underwent simultaneous augmentation rhinoplasty with bony reduction between May 2008 and April 2009. Retrospective analysis was performed according to the clinical data, surgical techniques and postoperative results. Results: Of the 26 patients, there were 15 males and 11 females. The incidence according to the Stranc's classification revealed that 62% of patients were injured by a frontal impact and 38% by a lateral impact. Frontal impact plane I (50%) was the most frequent type. At the follow up, 18 (81.2%) out of 22 patients were satisfied with their postoperative outcome, and the remaining 4 patients were fair. No one was dissatisfied. However, 5 cases in 3 patients (23%) had some complications; minimal implant deviation in 2 cases, minor irregularity on the nasal dorsum in 2 cases and palpable implant movement under palpation in 1 case. None of these cases required surgical correction. Conclusion: With the proper guidance, simultaneous augmentation rhinoplasty with bony reduction can prevent secondary deformities and satisfy the cosmetic outcomes.

Clinical and cephalometric characteristics in patients with anterior open bite and mandibular condylar resorption (Original Article 2 - 전방개교합이 동반된 하악과두흡수 환자의 임상적 특징 및 악안면 골격형태에 관한 연구)

  • The aim of this study was to investigate clinical and cephalometric characteristics of patients with anterior open bite and mandibular condylar resorption. Total of 45 subjects were recruited from the patients who visited the Department of Oral Medicine, Kyungpook National University Hospital in 2006 for treatment of temporomandibular dysfunction. After taking patient's history concerning temporomandibular joint dysfunction, clinical examination was performed and panoramic, transcranial, and lateral cephalometric radiographs were obtained. The cephalometric data were compared to those of normal Korean population. The results were as follow: 1. Clinical characteristics 1) Female was 4.6 time more prevalent than male, and 82.2% of the subjects were in second and third decade. 2) Angle's Class I occlusal type was 51%, Class II was 29%, and Class III was 20%. 3) The mean value of the overjet and overbite were 3.2mm and -1.1mm, respectively. 4) Most of the patients had parafunctional oral habits. 2. Cephalometric characteristics 1) SNA showed no significant difference between condyle resorption and normal group. However, in female resorption group, SNB was lesser and ANB was greater than those in normal group. 2) SN-GoMe and FMA increased in resorption group. 3) Palatal plane angle did not show significant difference between resorption and normal group. 4) Total posterior facial height was significantly smaller and total anterior facial height showed no significant increase as compared with those of the normal group. 5) Mandibular body length did not show any significant difference between resorption and normal group.

GaN Film Growth Characteristics Comparison in according to the Type of Buffer Layers on PSS (PSS 상 버퍼층 종류에 따른 GaN 박막 성장 특성 비교)

  • GaN is most commonly used to make LED elements. But, due to differences of the thermal expansion coefficient and lattice mismatch with sapphire, dislocations have occurred at about $109{\sim}1010/cm^2$. Generally, a low temperature GaN buffer layer is used between the GaN layer and the sapphire substrate in order to reduce the dislocation density and improve the characteristics of the thin film, and thus to increase the efficiency of the LED. Further, patterned sapphire substrate (PSS) are applied to improve the light extraction efficiency. In this experiment, using an AlN buffer layer on PSS in place of the GaN buffer layer that is used mainly to improve the properties of the GaN film, light extraction efficiency and overall properties of the thin film are improved at the same time. The AlN buffer layer was deposited by using a sputter and the AlN buffer layer thickness was determined to be 25 nm through XRD analysis after growing the GaN film at $1070^{\circ}C$ on the AlN buffer CPSS (C-plane Patterned Sapphire Substrate, AlN buffer 25 nm, 100 nm, 200 nm, 300 nm). The GaN film layer formed by applying a 2 step epitaxial lateral overgrowth (ELOG) process, and by changing temperatures ($1020{\sim}1070^{\circ}C$) and pressures (85~300 Torr). To confirm the surface morphology, we used SEM, AFM, and optical microscopy. To analyze the properties (dislocation density and crystallinity) of a thin film, we used HR-XRD and Cathodoluminescence.

Reconstruction of Greater Trochanteric defect using Lumbar Artery Perforator Free Flap - A Case Report - (요추부 천공지 유리피판을 이용한 대전자부 결손의 재건 - 증례보고 -)

  • There could be several methods for trochanteric reconstruction including local flap, pedicled perforator flaps, free flap, etc. We performed greater trochanteric reconstruction with lumbar artery perforator free flap in some aberrant method. So we report this experience with review of literatures. A 42-year-old man visited our hospital with a large soft tissue defect in his left greater trochanteric area by traffic accident. The patient had wide skin and soft tissue defect combined with open femur fracture. During one month period of admission, he underwent femur open reduction and wound debridement four times. After that we planned thoracodorsal perforator free flap reconstruction. The flap was outlined as large as $20{\times}15\;cm$ and elevated in a suprafascial plane from the lateral border. During intramuscular perforator dissection, we found that two 1.5 mm diametered perforator vessels coursed inferomedially toward second lumbar region. Finally the flap became lumbar artery perforator flap based on second lumbar artery perforator as a main pedicle. After flap transfer, the perforator vessels were connected with inferior gluteal artery and vein microsurgically. The operation was successful without uneventful course. We found no significant postoperative complication and donor site morbidity during six months follow up periods. Lumbar artery perforator flap could be an alternative procedure for thoracodorsal perforator flap in some patients with anatomic variant features.

New Surgical Technique for Harvesting Proximal Fibular Epiphysis in Free Vascularized Epiphyseal Transplantation (혈관부착 근위비골성장판 이식시 공여부 수술의 새로운 술식)

  • Purpose : Propose a surgical technique in donor harvesting method in free vascularized proximal fibular epiphysis. Methodology : Concerned about growth potentials of the transplanted epiphysis in our long term results of the epiphyseal transplanted 13 cases more than 4 years follow-up, anterior tibial artery which contains anterior tibial recurrent artery is most reliable vessel to proximal fibular epiphysis which is the best donor of the free vascularized epiphyseal transplantation. In vascular anatomical aspect proximal fibular epiphysis norished by latearl inferior genicular artery from popliteal, posterior tibial recurrent artery and anterior tibial recurrent artery from anterior tibial artery and peroneal artery through metaphysis. The lateral inferior genicular artery is very small and difficult to isolate, peroneal artery from metaphysis through epiphyseal plate can not give enough blood supply to epiphysis itself. The anterior tibial artery which include anterior tibial recurrent and posterior tibial recurrent artery is the best choice in this procedure. But anterior tibial recurrent artery merge from within one inch from bifucating point of the anterior and posterior tibial arteries from popliteal artery. So it is very difficult to get enough vascular pedicle length to anastomose in recipient vessel without vein graft even harvested from bifucating point from popliteal artery. Authors took recipient artery from distal direction of anterior tibial artery after ligation of the proximal popliteal side vessel, which can get unlimited pedicle length and safer dissection of the harvesting proximal fibular epiphysis. Results : This harvesting procedure can performed supine position, direct anterolateral approach to proximal tibiofibular joint. Dissect and isolate the biceps muscle insertion from fibular head, micro-dissection is needed to identify the anterior tibial recurrent arteries to proximal epiphysis, soft tissue release down to distal and deeper plane to find main anterior tibial artery which overlying on interosseous membrane. Special care is needed to protect peroneal nerve damage which across the surgical field. Conclusions : Proximal fibular epiphyseal transplantation with distally directed anterior tibial artery harvesting technique is effective and easier dissect and versatile application with much longer arterial pedicle.

Cephalometric Study of the Morphology and Position of Mandible in Complete Unilateral Cleft Lip and Palate Patients (완전 편측성 구순구개열자의 하악골 형태와 위치에 관한 측모 두부 방사선학적 연구)

  • This study was designed to evaluate the morphology and the position of the mandible in the complete unilateral cleft lip and palate patients, Craniofacial skeletal morphology pattern was analyzed on the lateral cephalometric radiographs of the 50 subjects of complete unilateral cleft lip and palate, the 50 normal and 50 class III, Each group was divided into child and adult sub-groups, All the data were tested statistically. The results were as follows: I, In the comparison with the normal group, complete cleft group showed smaller angular, condylar length, clockwisely rotated mandible and larger NMe/SGo(p<0.01). 2, In the comparison with the class III group, the complete cleft group showed significantly smaller angular, condylar, ramal, body length of the mandible(p<0,01). 3. As for the position of the mandibular condyle to the cranial base, the class III group was the most anterior, the normal group was the most posterior and the complete cleft group was in the middle(p<0.05). 4. In the comparison with child group, the normal adult group showed smaller mandibular angle and mandibular plane angle, but not the other two groups. And the complete cleft group and the class III group showed the similar change. The normal and class III group showed increased XiCd/XiPog, but not the complete cleft group(p<0.01).
