성인 구개부 측모두부계측방사선 사진과 3D model의 골두께 비교 (Comparison of palatal bone thickness between 3D model and lateral cephalometric radiograph)

  • 본 연구는 동일한 환자에서 촬영한 측모두부계측방사선 사진과 전산화단층촬영 영상을 비교하여 미니 임플랜트 식립 시 측모두부계측방사선 사진을 통해 상악 구개부 골두께를 예측하기 위한 목적으로 시행되었다. 안모가 양호한 정상교합자 남녀 총 30명의 전산화단층촬영 영상을 3D model로 변환하고 측모두부계측방사선 사진과 동일부위의 골두께를 계측하기 위하여 프랑크프루트 평면에 대해 수직이고 상악 제2소구치와 제1대구치 인접면을 지나는 기준평면을 설정한다. 3D model에서 횡적으로 2 mm 간격으로 11개 부위의 구개부 골두께를 계측하고 측모두부계측방사선 사진과 비교하여 다음과 같은 결과를 얻었다. 3D model에서 골두께는 남녀 모두 정중구개부위에서 가장 컸고 측방으로 멀어질수록 작아지다가 남성는 측방 6 mm 여성는 측방 8 mm 부위에서 가장 최소값을 보인 후 다시 커지는 양상이 관찰되었다. 측모두부방사선 사진상 골두께는 남성이 $4.21{\pm}1.03mm$, 여성은 $2.71{\pm}1.62mm$로 남성이 여성보다 유의성 있게 컸다. 측모두부계측방사선 사진상 골두께와 3D model에서 횡적인 부위별로 측정한 골두께는 남성은 정중구개부와 측방 2 mm 부위를 제외한 나머지 부위에서 유의성 있는 상관관계를 보였고 여성은 모든 부위에서 유의성 있는 상관관계를 보였다. 또한 측모두부계측방사선 사진상 골두께와 3D model에서 상관관계를 보이는 부위에 대하여 회귀분석을 시행하였다. 이상의 횡적인 부위별 회귀분석을 통해 미니 임플랜트 식립 시 안전한 스크류의 길이를 결정하기 위한 골두께 예측에 도움이 될 것이다.

  • Purpose: Mid and lower facial convexity is more common in Oriental people than in Caucasian. Bimaxillary dentoalveolar protrusion is characterized by procumbent teeth, protruding lips, acute nasolabial angle, gummy smile, receding chin, facial convexity. Especially, pure maxillary dentoalveolar protrusion is less frequent than bimaxillary dentoalveolar protrusion. Therefore, it is important to make an accurate decision for the operation throughout the history taking, cephalogram, dental cast to arrive at accurate diagnosis and surgical plan. Methods: From December 2002 to June 2004, ten patients with maxillary dentoalveolar protrusion and microgenia were corrected by maxillary anterior segmental osteotomy and advancement genioplasty. 10 patients were analyzed by preoperative and postoperative clinical photography, posteroanterior and lateral cephalograms. Results: No major complications were occurred throughout the follow-up period except one of the over-recessed, otherwise most of the patients were satisfied with the result. Conclusion: We could correct the occulusal relationship with teeth and improve lower facial profile, asthetically and functionally, by maxillary anterior segmental osteotomy and advancement genioplasty.

Ⅰ급 및 Ⅱ급 부정교합자의 과두변위에 관한 두부방사선계측학적 연구 (A ROENTGENOCEPHALOMETRIC STUDY ON THE CONDYLAR DISPLACEMENT IN ANGLE′S CLASS Ⅰ & Ⅲ MALOCCLUSION)

  • The purpose of this investigation was to know correlation of mean values between centric occlusion and centric relation by the cephalogram in Angle's Class Ⅰ and Ⅲ malocclusion subjects. 22 adults with Angle's Class Ⅰ malocclusion (17 men and 5 women, 21 to 27 years of age) and 14 adults with Angle's Class Ⅲ malocclusion (10 men and 4 women, 21 to 27 years of age) were selected from the dental students in Yonsei University. Each subject was given two lateral cephalometric radiographies and cephalometric analysis was performed. All data from these analyses was recorded and statistically processed with CYBER computer system. 1. The results were obtained as follows: There was a strong positive correlation between centric occlusion and centric relation in all subjects with Angle's Class Ⅰ and Ⅲ malocclusion. 2. In Angle's Class Ⅰ malocclusion, measurements in lower facial height revealed significant difference between centric occlusion and centric relation (P<0.05). In Angle's Class Ⅲmalocclusion, measurements in facial axis angle, mandibular plane angle, convexity of A point, lower incisor protrusion, lower facial height revealed significant difference between centric occlusion and centric relation (P<0.05). 3. When the mandible was moved from centric occlusion to centric relation, the mean distance of mandibular movement was 1.27㎜ (0.2-2.8㎜) in Angle's Class Ⅰ malocclusion, 1.70㎜ (0.55-4.15㎜) in Angle's Class Ⅲ malocclusion, and 1.44㎜ (0.2-4.15㎜) in all subjects.

하악 관절와의 형태가 하악 이부편위에 미치는 영향에 관한 3차원 영상 연구 (3-D CT Image Study of Effect of Glenoid Fossa on Menton Deviation)

  • Purpose: The purpose of this study was to define the relation of the degree of menton deviation and 3-D CT (computerized tomography) measurements of the glenoid fossa and the mandible, which are considered to have an influence on menton deviation. Methods: The CT images were obtained in 60 adults and these were transmitted to a computer and reconstructed using computer software. According to the degree of the menton deviation, which was measured on the posteroanterior cephalogram, the subjects were divided into the menton deviated group (30 adults) and the symmetry group (30 adults). A total of 11 measurements that might have an effect on menton deviation were determined and these were measured in the right and left sides using the function of 3-D measurement in the computer program. The 11 measurements consist of 6 measurements in the glenoid fossa (vertical position of the glenoid fossa and articular eminence, the sagittal position of the glenoid fossa and articular eminence, the depth of the glenoid fossa, and the anterior angle of the glenoid fossa), and 5 measurements in the mandible (ramus length, frontal ramal inclination, lateral ramal inclination, body length, body height). Results: The comparison of the differences between the menton deviated and symmetry groups and correlation analysis on the degree of menton deviation were carried out. The results of comparison of the right and the left difference between the menton deviated and symmetry groups showed that the vertical position and depth of the glenoid fossa were significantly increased in the menton deviated group. Conclusion: The results of the present study show that consideration of the shape and position of the glenoid fossa is necessary for making the diagnosis and administering proper treatment in facial asymmetry patients and especially growing patients.

Angle씨 III급 부정교합 환자중 Activator사용 전후의 설골의 위치 변화에 관한 연구 (A STUDY ON POSITIONAL CHANCE OF THE HYOID BONE BEFORE AND AFTER ACTIVATOR THERAPY IN ANGLE'S CLASS III MALOCCLUSION PATIENTS)

  • This study was conducted to assess the positional changes of hyoid bone following the use of activator in Angles class III malocclusion patients with functional factors. For this study, 40 Angle's class I patients and 40 Angle's class III patients, totally 80 subjects were used. They are all in Hellman's dental age IIIB-IIIC ranges. In lateral cephalogram to compare Angle's class I group and Angle's class III group, and the positional changes of the hyoid bone before and after the use of activator in Angle's class III malocclusion group. The results were obtained as follows; 1. Comparison of Angle's class I group and Angle's class III group. In comparison to Angle's class I group, hyoid bone is more anteriorly and superiorly positioned in Angle's class III group. The hyoid bone showed reverse inclination to the mandibular plane in Angle's class III malocclusion group. 2. Comparison of the hyoid positional change before and after use of Activator in Angle's class III malocclusion group. The hyoid bone is displaced posteriorly and inferiorly in vertical relationship. The hyoid bone also showed counter-clockwise rotation. 3. No statistical difference was found between after Activator use data of Angle's class m malocclusion group and Angle's class I group. It is concluded that the hyoid bone in Angle's class III malocclusion group changed its position, similar to Angle's class I malocclusion group.

성장기 아동 하악 과두 골절의 예후에 관한 장기 추적 조사 (THE LONG-TERM RESULT OF CONDYLE FRACTURE IN CHILDREN)

  • The purpose of this study was the long-term clinical and radiological evaluation of conservatively treated condylar fractures in children. This study was therefore undertaken to analyze the long-term effects of treating condylar fractures in children with conservative therapy in order to resolve the controversial question of whether complete remodeling occurs in this age or, if not, whether it is more likely to be associated with certain types of fracture or with other factors. This study was based on a series of 11 consecutive children and adolescents, aged between 3 and 15 years, with fractures of the condylar process who had been treated with conservative therapy at the Department of Oral & Maxillofacial surgery, Inha University Hospital, Inchon, Korea. All patients underwent a clinical investigation with special emphasis on Temporomandibular joint function and facial asymmetry. The patients also underwent a radiologic investigation, focusing on fracture remodeling and symmetry of mandible. It consists of panoramic radiograph, PA & lateral cephalogram and 3-D CT. No patient described impaired temporomandibular joint(TMJ) function or pain on the affected side. 2 of 8 (25%) unilateral and 1 bilateral fractures show slight facial asymmetry. Despite apparently excellent recovery of function, there were marked remodeling changes evident on the CT scan. Such changes are not usually evident on panoramic radiograph. Radiologic investigation showed incomplete remodelling(54.5%) and asymmetry of the mandible (27.3%) in some patients. Conservative treatment of condyle fracture in children results in satisfactory long-term outcome of jaw function despite a relative high frequency of radiologically noted aberrations.

두개하악장애환자에서 악관절강폭과 측모두부형태와의 관계에 관한 연구 (Relationship between Joint Space and Craniofacial Morphology in Patients with Craniomandibular Disorders)

  • This study was performed for the purpose of investigating the relation between width of temporomandibular joint space and craniofacial morphology in patients with CMDs. The subjects utilized here were aged from 16 to 63 years old and 19 male and 46 female. For the study, each patient was taken radiographs by Transcranial and Lateral Cephalographic projection and the films were traced with routine method. The measured items were anterior, superior, posterior joint space and relative condylar position to the deepest part of glenoid fossa in Transcranial film and items related vertical and/or horizontal growth in Cephalogram. The data were processed with SPSS/PC+ package for statistical analysis. The obtained results were as follows : 1. Anterior joint space in affected side were wider than that of unaffected side, but for superior and posterior joint space, the value of unaffected side were more than those of affected side. 2. Superior or posterior joint space of affected side were significantly correlated with items related vertical growth, such as ramus height, Jarabak ratio, FMA. From this, the following cold be proposed, the wider the joint space of superior or posterior aspect of affected side was, the more the patient showed growth pattern of counter clockwise. 3. Superior or posterior joint space of affected side were significantly correlated with items related horizontal growth, such as SNPo, NAPo, APDI and ANB. From this the following could be proposed, the wider the joint space of superior or posterior aspect of affected side was, the more the patient showed anterior displacement of mandible. 4. It would be proposed that the diagnositic value of superior and posterior joint space of affected side in transcranial radiograph was excellent.

하악과성장형과 상악열성장형 골격성 Ⅲ급 부정교합군간의 두개저 형태 비교 (COMPARISON OF CRANIAL BASE MORPHOLOGY BETWEEN THE MANDIBULAR PROGNATHISM AND MAXILLARY RETROGNATHISM IN SKELETAL CLASS III PATIENTS)

  • This study was intended to compare the cranial base morphology between the mandibular prognathism and maxillary retrognathism in skeletal class III patients. The subject of the present study was composed of 88 patients divided into two groups; Group 1 (Skeletal Class III with mandibular prognathism. SNA within normal range, SNB over normal range, n=54) and Group 2(Skeletal Class III with maxillary retrognathism. SNA below normal range, SNB within normal range, n=34). Lateral cephalogram were taken immediate before surgery and 18 landmarks were used to analyze the characteristics of cranial base and maxillomandibular skeleton. The result revealed that cranial base angle is significantly smaller in Group 1 than Group 2, which implies the influence of the cranial base angulation on the mandibular position. However the posterior cranial base length did not influence the mandibular horizontal position and anterior cranial base length did not influence the maxillary horizontal position. As the anterior cranial base length was closely related with ramal height, it is recommendable to investigate the regulatory mechanism of chondrogenesis of cranial base and condyle cartilage in the future research.

폐쇄성 수면 무호흡 환자에서 임시 하악 전방 이동 장치를 이용한 치료결과 분석 (Assessment of Treatment Outcome after Using Temporary Mandibular Advancement Devices in Obstructive Sleep Apnea Patients)

  • Purpose: The aim of this study was to evaluate the effect of temporary mandibular advancement devices (MAD) in obstructive sleep apnea (OSA) patients Methods: 28 patients (male 21, female 7) undergoing temporary mandibular advancement device treatment for OSA were selected from 2011.01. to 2012.02. in the department of Oral & Maxillofacial Surgery at SamsungMedicalCenter. Treatment efficacy was determined by polysomnography (PSG) at baseline & after MAD delivery. The response group was defined as >50% Apnea-Hypopnea Index (AHI) reduction plus post-MAD AHI <10, and the non-response group was defined as <50% AHI reduction. The lateral cephalogram was analysed including SNA, SNB, UL, MPH, PAS, PASU, and PAST using V-ceph$^{TM}$ (Cybermed, USA). Results: The responsers were 23 patients, and non-responsers were 5 patients. The AHI was significantly reduced with temporary MAD ($8.08{\pm}7.93$) compared with baseline ($28.51{\pm}20.56$) in the response group (n=23). No significant difference was observed between pre MAD and post MAD except SNB on cephalometric analysis. Among 11 patients successfully treated with the temporary device, 9 patients said that using permanent device brings better effect too. Conclusion: These results indicate that the Temporary MAD could not be the only effective tools on OSA but also be used to predict patient's reactivity about permanent appliance treatment. Further studies are warranted to evaluate the relations between temporary MAD and permanent MAD.

상악 제 2 대구치 발거에 의한 교정치료의 효과 (A STUDY ON TREATMENT EFFECTS OF MAXILLARY SECOND MOLAR EXTRACTION CASES)

  • Orthodontic treatment in conjunction with second-molar extraction has been a controversial issue among orthodontists over many decades. The aim of this study was to investigate the treatment effects of upper second molar extraction cases. The sample included 19 upper second molar extraction orthodontic cases(ten Angle's Class I's and nine Class II's, average age=13Y 6M) cared at Kyung-Hee University Department of Orthodontics. Lateral cephalometric radiographs were taken before and immediately after treatment. Seventy-nine points were digitized on each cephalogram and 38 cephalometric parameters were computed comprising 22 angular measurements, 13 linear measurements, and 3 facial proportions. The data obtained from each malocclusion group were analyzed by paired t-test. The statistical results disclosed that there was no significant change in skeletal pattern after treatment except for that accountable by growth while there was statistically significant change in dentoalveolar and soft tissue patterns. There were no significant changes in Bjork sum, posterior facial height /anterior facial height and lower anterior facial height /anterior facial height. No significant changes in anteroposterior position of maxilla and palatal plane were manifested. Although facial axis and lower facial height was slightly increased and the mandible was rotated backward and downward, there was no remarkable change in the mandibular plane. There were statistically significant changes in distal movement of upper first molar, molar key correction and overjet reduction while there was no change in the occlusal plane. The upper lip was slightly retracted simultaneously with slight increase in nasolabial angle. These results signify that distalization of upper dentition with the second molar extraction does change occlusal relationship without gross modifications in the craniofacial skeletal configurationson. Henceforth the second molar extracted would be recommended to treat severe anterior crowding and protrusion with minor skeletal discrepancy.
