Research on the Development of the Oriental Medical Model on the Health Examination in the Industry (산업장 건강검진의 한의학적 모델 개발 연구)

  • On the process of research in the plan on oriental medical participation in the industrial health sponsored by BK21 project, we carried out the oriental medical health examination program for workers during former half-year We reached the conclusion as follows, 1. The oriental medical health examination program is contents and formalities that should be determined by present industrial health system, based on the oriental medical system and scholastic character, and included probability of the western and oriental medical cooperation. 2. The oriental medical health examination program can promote capability of individual health management and productive power of workers, and it is capable to manage on the self-conscious symptoms and macroschophically approach to their environment 3. The oriental medical health examination program that we have developed, is flow as questionare, understanding of working environment, information of result and later management. It is composed of three fields as follow , first, use of pulse diagnostic apparatus, understanding of the health promotion life style, and diagnosis of the oriental medical doctor, second, analysis of constitution, third, photographing for understanding of the musculoskeletal disorders, questionare for musculoskeletal self-conscious symptoms, and diagnosis of oriental medical doctor. 4. The oriental medical health examination program that we have developed, progressive from the view point of health, makes the oriental medical doctor's roll more important. It is the first trial at the western and oriental medical cooperation and characterized by excellence about musouloskeletal disorders. But it need to be improved in aspects of time and specialist on the health examination, diagnostic apparatus, control of examinant and later management. So we think that it needs research on the employment of health examination specialist, establishment of later management system, development of significantly diagnosable standard and assessable form on the health examination, and contents of health examination on the western and oriental medical cooperation.

  • With advances in diagnostic technology, radiologic diagnostic methods have been used more frequently, and physical examination may be neglected. The authors report a case of pediatric medial orbital trapdoor fracture in which the surgery was delayed because computed tomography (CT) findings did not indicate bone displacement, incarceration of rectus muscle, or soft tissue herniation. A healthy 6-year-old boy was admitted to the emergency room for right eyebrow laceration. We could not check eyeball movement or diplopia, because the patient was irritable. Thus, we performed facial CT under sedation, but there was normal CT finding. Seven days later, the patient visited our hospital due to persistent nausea and dizziness. We were able to perform a physical examination this time. Lateral gaze of right eye was limited. CT still did not show any findings suggestive of fracture, but we decided to perform exploratory surgery. We performed exploration, and found no bone displacement, but discovered entrapped soft tissue. We returned the soft tissue to its original position. The patient fully recovered six weeks later. To enable early detection and treatment, thorough physical examination and CT reading are especially needed when the patient shows poor compliance, and frequent follow-up observations are also necessary.

Spontaneous Regression of Eyelid Histiocytoma in a Maltese Dog

  • A 1-year-old neutered male Maltese dog was presented with a mass on the upper eyelid of the left eye (OS). A pinkish and alopecic mass was located in the nasal portion of the upper eyelid of OS. The mass was firmly attached to the eyelid and round-shaped with a diameter of 11 mm. Any spread to conjunctiva was not identified. On a cytological examination, cutaneous histiocytoma was confirmed, with the presence of small lymphocytes indicating later regression stage. The patient received no treatment and complete regression of the mass was verified 2 months later. The cytological examination was helpful for the diagnosis and staging of cutaneous histiocytoma. Canine eyelid histiocytoma can regress spontaneously, and thus medical or surgical treatment for removal should be considered carefully.

  • Objectives : The purpose of this study was to figure out interconnections between studies with studies of individual books that information on Listening Examination is included. Through this, what Listening Examination have looked like and how it has been developed were investigated in this study. Methods : Books that information on Listening Examination is included was investigated in order according to the stream of times. Results & Conclusions : Varied information on Listening Examination was mentioned in Neijing. Later, the development that was made in various fields went through the process of integration. Typically, Yixuezhunshengliuyao can serve an example. A previous book mentioned above and Yidengxuyan Wenzhen in the Qing Dynasty were very influential as those were quoted in not a few of books of next generation. In the Qing Dynasty, meanwhile, the new information and a variety of perspectives on the Listening Examination were raised. 'Voice' and 'sound', weakness and firmness, Listening Examination method and theoretical contents became rich in content. Different points of view as follows were raised: divided opinion on two topics such as 'voice' and 'respiration', understanding through Triple Energizer, moisture control and emphasis on relationship between Essence, Qi, Spirit and Life, etc.

Changes in Facial palsy Patient's Quality of life based upon Oriental-Western Medicine Treatment (한양방 협진치료가 안면마비환자의 삶의 질 변화에 미친 영향)

  • Objective : In period of convalescence and aftereffect, facial palsy patients suffer from social and psychological problems, besides experiencing physical inconvenience. So Quality of life is important Evaluation in treatment or facial palsy. Nevertheless the aims of recent study were only trying to explain about objective symptoms. Therefore, Oriental-Western Medicine was performed, effectiveness of treatment were measured in Quality of life. Methods : Acute facial palsy patients who visiting whin 5days completed questionnaire about Quality of life, if he(or she) participated voluntarily. Questionnaire are comprised of general characteristics, Facial Disability Index(FDI), WHOQOL-BREF, VAS and House-Brackmann grade. Questionnaire used two times, the first medical examination and 4weeks later after starting Oriental-Western Medicine. The statistical analysis was performed by GraphPad Prism 4.0. T-test was used to verify effectiveness between the two groups. Results : 1. When we compared the first medical examination with 4weeks later, score of FDI-Physical function and FDI-Social/Well-bieng function increased but they were not valid statistically. 2. When we compared the first medical examination with 4weeks later, in WHOQOL-Brefoverall domain and physical domain, score increased. In WHOQOL-Bref-psychological, Social, Environment domain, score decreased. but, they were not valid statistically. 3. VAS, House-Brackmann grade decreased, but, they were not valid statistically. Conclusion : The number of subjects with facial palsy in our study(N=5) was too small, and the period of study(4 weeks) was short, too. For this reason, our data were not valid statistically. But Facial palsy Patient's Quality of life has risen.

Effects of Orofacial Pain Experience on Psychological Symptoms under Emotional Stress (스트레스하에서 구강안면동통 경험이 심리적 증상에 미치는 영향에 관한 연구)

  • The author studied the prevalence and sites of orofacial pain during the latest 6 months, and psychologic symptoms under the emotional stress through SCL-90-R in thirty-five dental students(nineteen men and sixteen women) of school of Dentistry, Chonbuk National University. 26 students(74.3%), 12 men and 14 women, experienced the orofacial pain; headache, toothache, facial & buccal pain, TMJ pain and intraoral pain were occurred in 51.4%, 37.1%, 25.7%, 17.1% and 17.1% respectively. In psychological scores in before-, during- and after examination, there was no significant difference between men and women. On the other hand, when these scores were compared each other in one group with orofacial pain experience and the other group without pain experience, the former showed significantly higher values in somatization, depression, hostility, and paranoid ideation dimension before examination, interpersonal sensitivity and anxiety dimension during examination, and somatization dimension after examination than those of the later group(P<0.05). The changes of psychologic symptoms in before-, during-, and after examination were observed in one group with pain experience and the other group without pain experience, respectively; in the former group the significantly higher values of anxiety dimension in during examination than after examination, and the significantly higher values of interpersonal sensitivity dimension in before examination than after examination(P<0.05) were evaluated.

A Study on Uihakyimmun (醫學入門) as a Medical Examination Textbook of the Joseon Dynasty (조선시대 의과시험 교재로서의 『의학입문(醫學入門)』에 대한 고찰)

  • Uihakyimmun (醫學入門) was written around 1580 by Li Ting (李梴) during the Ming Dynasty of China. Later, during the mid-Joseon period, Uihakyimmun (醫學入門) was introduced to Joseon and is believed to have contributed to the development of Joseon medicine. The importance of this Uihakyimmun (醫學入門) was especially evident as it was designated as a textbook for medical examination in the late Joseon Dynasty. This paper examines the process of Uihakyimmun (醫學入門) being introduced into Joseon based on historical records. It also considered the reason why Medical Education was selected as the textbook for the medical examination instead of Donguibogam (東醫寶鑑). As a result, it was widely read by court doctors before being selected as a test textbook, and gradually became a formal textbook after being used in informal tests. In addition, it was revealed through historical records that the reason why Uihakyimmun (醫學入門) was chosen instead of Donguibogam (東醫寶鑑) was because Uihakyimmun (醫學入門) fit better with Confucian values.

Usefulness of Audio-visual Methods that is used to Customer to Help Smooth Public Prosecutor at CT Examination (CT Scan Positioning시 고객의 검사진행의 이해를 돕기 위한 시청각 자료의 유용성)

  • It is to improve customer satisfaction measurement and CT Scan process without delay of examination time when is using Scan positioning time(Planning time) that time is happened always between research reactor CT examination to increase fear and examination satisfaction by the customer's comprehension tribe which get the latest contrast enhancement CT examination. Needs and interests that customer wants to compose visual and auditory Contents to be played to Scan positioning time did questionnaire about curiosity later before CT examination to 600 people for October - November 2 months of 2006 to customer whole that get CT examination on source. Data getting through questionnaire investigated examination comprehension and satisfaction through questionnaire after experiment Scan Positioning to 500 coming to help customers to be source CT examination for 3 months February December - 2007 year in 2006 manufacturing Voice and Visual announce media for reference. To customer who interest degree appeared, and answers preparatory audit from preparatory audit about curiosity of CT examination customer to order of examination time required(43%), contrast media side effect(26%), examination region(20%), breath(10%), etc..(1.5%) audio-visual materials in questionnaire that attain after do reclamation among examination age, sex, reception type of irrelatively in 91% of target increase of hailing degree and examination satisfaction appear. Searched that customer hailing and satisfaction are increased greatly when use of audio-visual materials in satisfaction result that use CT Positioning delay time. In experiment process, It took lacking part by method that use hearing in case of do not use sight as is unavoidable in subject position or old age. Through this, audio-visual materials could analogize that it is more useful method that use sight and hearing at the same time.

Intraocular Foreign Body Entering the Anterior Chamber Through the Mouth: A Case Report

  • An 11-year-old, castrated Maltese dog presented with a 3-week history of periocular swelling, epiphora, and intermittent strabismus. On examination, a foreign body was observed in the anterior chamber, along with orbital cellulitis. Severe gingivitis and plaque accumulation were also diagnosed. The foreign body was surgically removed, and dental prophylaxis and dental extraction were performed. The foreign body entrance could not be found intraoperatively, and the foreign body, later identified as a feather, was removed through a clear corneal incision. The right maxillary molar, which had periodontal inflammation, was also extracted. One day postoperatively, severe hypopyon developed, although the periocular swelling was reduced. These signs persisted despite topical and systemic antibiotic and anti-inflammatory therapy; therefore, the right eye was enucleated 1 week later. Intraoperatively, a fistula was found connecting the orbital medial wall, right maxillary molar root, and sclera. The fistula entered the dorsomedial sclera approximately 7 mm behind the limbus. Enterobacteria were cultured from the area. Foreign bodies can enter the anterior chamber not only through the cornea, but also through the mouth. Therefore, when the entry point cannot be found in the cornea, a careful dental examination is required, and the foreign body must be removed through the sclera rather than the cornea.
