경희의료원 치과병원 교정과에 내원한 부정교합자 중 전신 건강 상태가 양호한 123명의 좌측 수완부골 방사선 사진을 이용하여, 초경과 골격성숙도간의 상관관계를 연구한 결과 다음과 같은 결론을 얻었다. 1. 초경 발현 연령은 평균 12.31세, 최소 9.6세, 최고 15.6세로 개체간 변이가 심하였다. 2. 초경시 골성숙 상태는 SMI 7,8단계($73\%$), 즉 제5중절골의 골단이 골간을 둘러싸거나 제3말절골의 골간과 골단이 융합되는 단계이며, 초경시의 골격성숙도는 초경 발현 연령에 관계없이 일정하였다. 3. 골성숙단계에 따른 초경 후 경과기간간에는 유의한 차이가 있었다. 4. 요골의 융합은 초경 후 평균 1년 8개월이 지나면 시작되었다. 5.초경 발현 연령에 따른 성장 속도의 비교에서, 만기성숙아(초경을 늦게 경험하는 여아)는 초경 후 14개월이 지나면 요골의 융합이 시작되었고, 조기성숙아(초경을 일찍 경험하는 여아)는 초경후 22개월, 평균성숙아는 초경후 20개월이 지나면 요골의 융합이 시작되었다.
This study was performed using histochemical and immunohistochemical methods to investigate the effect of Gami-Shinkiwhan(GS) on reproductive function of male aged mice. 8-weeks-old ICR mice were used as control group, without any treatment, and 15-month-old ICR mice were used as aging elicited group(AE) and Gami-Shinkiwhan treatment group(GS). AE group didn't restrict diets and drinking for 6 months without any treatment. GS was administered 0.56g/kg/day for 6 months. Compared with AE group, the cell division of sertoli cells, spermatids, and spermatogonial cells was increased and the apoptosis of sertoli cells was decreased on GS group. Androgen receptor positive reaction and $17{\beta}$-HSD positive reaction were significantly increased in the GS group compared to AE group. In addition, the DJ-1 positive reaction was significantly increased and the HDAC3 positive response was significantly decreased in the GS group compared with AE group. Based on the above results, GS prevented the apoptosis of sertoli cells in the tubules, and increased the production of sertoli cells, spermatozoa and testosterone. Based on this, it is thought that it improves male reproductive dysfunction caused by late-onset hypogonadism.
Group B Streptococcal sepsis and/or meningitis is one of the most serious and common diseases in the neonatal period with high mortality and frequent complications. We have experienced a case of late onset type group B streptococcal sepsis and meningitis with a complication of subclavian vein catheterization catheterization. This 29-day-old male neonate was admitted to intensive care unit with the presentation of fever and septic shock. He was born with Cesarean delivery at 36 weeks and 3 days of gestational age. He showed multiple episodes of seizure after admission and group B streptococcus was isolated from blood. CSF profiles 10 days after admission showed the features of bacterial meningitis without organism isolated. Diffuse cerebral infarction was detected on brain CT 24 days after admission. In the 13th hospital day, the complication of subclavian vein catheterization occurred; Guide wire was cut during insertion and the distal portion of it(2.5cm) was retained in the left subclavian vein. We removed the retained guide wire with goose-neck snare catheter via right femoral vein. This case was presented with a brief review of the literatures.
Objectives: This study aimed to estimate the risk of bleeding following minor oral surgical procedures and uninterrupted aspirin therapy in high-risk patients or patients with existing chronic diseases compared to patients who did not use aspirin during minor oral surgery at a public hospital. Methods: This retrospective cohort study analyzed the data of 2912 patients, aged 20 years or older, who underwent 5251 minor oral surgical procedures at a district hospital in Thailand. The aspirin group was comprised of patients continuing aspirin therapy during oral surgery. The non-aspirin group (reference) included all those who did not use aspirin during surgery. Immediate and late-onset bleeding was evaluated in each procedure. The risk ratio of bleeding was estimated using a multilevel Poisson regression. Results: The overall cumulative incidence of immediate bleeding was 1.3% of total procedures. No late-onset bleeding was found. A significantly greater incidence of bleeding was found in the aspirin group (5.8% of procedures, p<0.001). After adjusting for covariates, a multilevel Poisson regression model estimated that the bleeding risk in the aspirin group was 4.5 times higher than that of the non-aspirin group (95% confidence interval, 2.0 to 10.0; p<0.001). However, all bleeding events were controlled by simple hemostatic measures. Conclusions: High-risk patients or patients with existing chronic diseases who continued aspirin therapy following minor oral surgery were at a higher risk of hemorrhage than general patients who had not used aspirin. Nonetheless, bleeding complications were not life-threatening and could be promptly managed by simple hemostatic measures. The procedures could therefore be provided with an awareness of increased bleeding risk, prepared hemostatic measures, and postoperative monitoring, without the need for discontinuing aspirin, which could lead to more serious complications.
Objective : Clinical, radiographic, and outcomes assessments, focusing on complications, were performed in patients who underwent pedicle subtraction osteotomy (PSO) to assess correction effectiveness, fusion stability, procedural safety, neurological outcomes, complication rates, and overall patient outcomes. Methods : We analyzed data obtained from 13 consecutive PSO-treated patients presenting with fixed sagittal imbalances from 1999 to 2006. A single spine surgeon performed all operations. The median follow-up period was 73 months (range 41-114 months). Events during peri operative course and complications were closely monitored and carefully reviewed. Radiographs were obtained and measurements were done before surgery, immediately after surgery, and at the most recent follow-up examinations. Clinical outcomes were assessed using the Oswestry Disability Index and subjective satisfaction evaluation. Results : Following surgery, lumbar lordosis increased from $-14.1^{\circ}{\pm}20.5^{\circ}$ to $-46.3^{\circ}{\pm}12.8^{\circ}$ (p<0.0001). and the C7 plumb line improved from $115{\pm}43\;mm$ to $32{\pm}38\;mm$ (p<0.0001). There were 16 surgery-related complications in 8 patients; 3 intraoperative, 3 perioperative, and 10 late-onset postoperative. The prevalence of proximal junctional kyphosis (PJK) was 23% (3 of 13 patients). However, clinical outcomes were not adversely affected by PJK. Intraoperative blood loss averaged 2,984 mL. The C7 plumb line values and postoperative complications were closely correlated with clinical results. Conclusion : Intraoperative or postoperative complications are relatively common following PSO. Most late-onset complications in PSO patients were related to PJK and instrumentation failure. Correcting the C7 plumb line value with minimal operative complications seemed to lead to better clinical results.
Background: Probiotics and prebiotics have strain-specific effects on the host. Synbiotics, a mixture of probiotics and prebiotics, are proposed to have more beneficial effects on the host than either agent has alone. Purpose: We performed a randomized controlled trial to investigate the effect of Lactobacillus and Bifidobacterium together with oligosaccharides and lactoferrin on the development of necrotizing enterocolitis (NEC) or sepsis in very low birth weight neonates. Methods: Neonates with a gestational age ≤32 weeks and birth weight ≤1,500 g were enrolled. The study group received a combination of synbiotics and lactoferrin, whereas the control group received 1 mL of distilled water as placebo starting with the first feed until discharge. The outcome measures were the incidence of NEC stage ≥2 or late-onset cultureproven sepsis and NEC stage ≥2 or death. Results: Mean birth weight and gestational age of the study (n=104) and the control (n=104) groups were 1,197±235 g vs. 1,151±269 g and 29±1.9 vs. 28±2.2 weeks, respectively (P>0.05). Neither the incidence of NEC stage ≥2 or death, nor the incidence of NEC stage ≥2 or late-onset culture-proven sepsis differed between the study and control groups (5.8% vs. 5.9%, P=1; 26% vs. 21.2%, P=0.51). The only significant difference was the incidence of all stages of NEC (1.9% vs. 10.6%, P=0.019). Conclusion: The combination of synbiotics and lactoferrin did not reduce NEC severity, sepsis, or mortality.

Background: This study evaluates the bacterial pathogens of Ventilator-associated pneumonia (VAP) in a tertiary referral hospital. Methods: A total of 109 bacterial pathogens from 91 adult patients with VAP, who were admitted to the medical intensive care unit from January 2008 to December 2009, were examined. Clinical characteristics, bacterial pathogens, and resistance profiles were analyzed. Results: Staphylococcus aureus (44%) was the most frequently isolated. Acinetobacter baumanii (30%), Pseudomonas aeruginosa (12%), Stenotrophomonas maltophilia (7%), Klebsiella pneumoniae (6%), and Serratia marcescens (2%) were isolated from the transtracheal aspirates or bronchoalveolar lavage in patients with VAP. There was no significant difference of bacterial pathogens between early and late onset VAP. All isolated S. aureus were methicillin resistant S. aureus; the imipenem resistance rate of A. baumanii was 69%. Conclusion: The two most frequent pathogens of VAP were S. aureus and A. baumanii. There were no pathogenic differences between early and late onset VAP.
Objectives: This report presents a case of late onset chorea hyperglycemia basal ganglia syndrome treated using traditional Korean medicine therapies. Methods: A patient was treated with Korean medicine therapies, including the herbal medication, Yokukan-san. We evaluated the improvements in the patient's symptoms based on the total score of the Abnormal Involuntary Movement Scale, the activities of daily living portion of the Universidade Federal de Minas Gerais (UFMG) Sydenham's Chorea Rating Scale, the numeric rating scale, and an assessment of the patient's writing. Results: After undergoing Korean medicine treatment for 15 days, there were improvements in the patient's chorea and writing as well as a decrease in the scores for all three scales. Conclusions: This clinical case study suggests that Yokukan-san might be effective for treating dyskinesia in patients with chorea hyperglycemia basal ganglia syndrome.
Parainfluenza virus 감염은 횡문근융해증의 하나의 원인이 될 수 있다. 횡문근융해증은 지속된 금식기간동안 미토콘드리아 지방산 ${\beta}$-oxidation 장애에 의해 악화될 수 있다. 또한 후기 발생 isovaleric 산증을 가진 환아들에게서 고암모니아혈증이 이화작용을 일으키는 상태 후 발생할 수 있다. 본 케이스는 parainfluenza virus 감염과 후기 발생 isovaleric 산증을 가진 4세 남아가 혼수, 경련 및 심호흡 부전으로 빠르게 진행했던 경우이다. 초기 암모니아와 creatinine kinase는 각각 $385{\mu}Mol/L$과 23,707 IU/L 이었으나 지속적 신대체요법 시행 후 암모니아와 creatinine kinase 수치는 정상으로 돌아왔다. 그러므로 생명을 위협하는 횡문근융해증과 고암모니아혈증을 가진 환아들의 치료에 있어서 즉각적인 지속적 신대체요법의 사용을 권하는 바이다.
리 증후군 또는 아급성 괴사성 뇌병증은 드물며, 빠르게 진행하는 신경 퇴행성 장애이다. 일반적으로 생후 1년 이내 호흡곤란, 심장기능 저하 등과 같은 증상이 발생하여, 2~3년 동안 환자의 75%가 사망에 이르는 심각한 질환이다. 리 증후군의 원인은 DNA mutation으로, 약 75%의 환자에서 핵 데옥시리보핵산의 돌연변이가 나타나고, 25%의 환자에서 미토콘드리아 데옥시리보핵산의 돌연변이가 발견된다. 임상 증상은 영향을 받은 뇌 영역에 따라 달라지며, 신경영상은 리 증후군 환자의 진단에 있어 중요한 역할을 한다. 성인에서 발생한 리 증후군은 더욱 드물고 어린 나이에 발생한 경우보다 더 느리게 진행한다. 우리는 성인에서 발생한 베르니케 뇌병증으로 오인된 리 증후군 환자의 증례를 보고하고자 한다.
