Clinical Practice Guideline for Soyangin Disease of Sasang Constitutional Medicine: Chest-Heat congested (Hyunggyeok-yeol) Symptomatology (소양인체질병증 임상진료지침: 흉격열병)

  • Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Soyangin Disease of Sasang Constitutional Medicine (SCM) ; Chest-Heat congested(Hyunggyeok-yeol) Symptomatology. Methods This CPG was developed by the national-wide experts committee consisting of SCM professors. First, collection and organization of literature related to SCM such as Donguisusebowon, Text book of SCM, Clinical Guidebook of SCM and Fundamental research to standardize diagnosis of Sasang Constitutional Medicine was performed. Secondly, journals related to clinical trial or Human complementary medicine of SCM were searched. Finally, 4 articles were selected and included in CPG for Chest-Heat congested(Hyunggyeok-yeol) Symptomatology of Stomach Heat-based Interior Heat disease in Soyangin disease. Results & Conclusions CPG of Chest-Heat congested(Hyunggyeok-yeol) symptomatology in Soyangin disease includes classification, definition and standard symptoms of each pattern. Chest-Heat congested(Hyunggyeok-yeol) symptomatology is classified into mild and moderate pattern by severity. Chest-Heat(Hyunggyeok-yeol) symptomatology Mild pattern is classified into Chest-Heat congested(Hyunggyeok-yeol) initial pattern and Chest-Heat congested(Hyunggyeok-yeol) advanced pattern. And Chest-Heat congested (Hyunggyeok-yeol) moderate pattern is classified into Clear Yang Failure of Stomach(Weguck-cheongyang Bulsagnseung) pattern (Upper wasting-thirst(Sangso) pattern), Clear Yang Failure of Large Intestine (Daejang-cheongyang Bulsangseung) pattern (Middle wasting-thirst (Jungso) pattern).

  • We were trying to study the validity of Puffer fish's poison(Tetrodotoxin- TTX) to make a traditional Korean Medical treatment. The following conclusions were made after literary studies. 1. The first record of the puffer fish dates back 2000 years ago in the Chinese text Book of Mountain and Sea and other texts from the similar period. 2. Puffer fish's poison IS known as tetrodotoxin which is an amino perhydroquinazoline compound. It has a chemical formula of $C_{11}H_{17}N_3O_8$ in the hemiacetal structure and has the molecular weight of 319. 3. Tetrodotoxin (TTX) plays a role as potent neurotransmitter blocker by blocking the $Na^+$ -gate channel which hinders the influx of $Na^+$ ion into the cell. 4. Symptoms of the puffer fish poisoning ranges from blunted sense in the lips and tongue, occasional vomiting in the first degree to sudden descending of the blood pressure, apnea, and other critical conditions in the fourth degree. Intoxication of the puffer fish poison progresses at a rapid pace as death may occur after an hour and half up to eight hours in maximum. Typical death occurs after four to six hours. 5. Ways to treat the puffer fish poisoning include gastric irrigation, induce vomiting, purgation, intravenous fluid injection, and correcting electrolytic imbalance and acidosis. In cases of dyspnea, apply oxygen inhalation and conduct artificial respiration. 6. Tetrodotoxin (TTX) may be applied in treating brain disorders, ocular pain, excess pain in the large intestine and ileum, and relieving tension of the skeletal museles, neuralgia, rheumatism, arthritis, and etc. 7. In terms of Oriental medicine, the puffer fish poison has characteristics of sweet, warm, and poisonous. It's known efficacies are to tonify weakness, dispel damp, benefit the lower back, relieve hemorrhoid, kills parasites, remove edema, and so forth. And the puffer fish eggs processed with ginger are said to be effective against tuberculosis and lung cancer, thus, it's validity must be investigated and further research should be followed.

Ultrastructure and Filtrating Function of the Ciliated Epithelial Cells of Foregut in Urechis unicinctus (개불(Urechis unicinctus) 전장 섬모 상피세포의 미세구조와 여과기능)

  • It is suggested that Urechis unicinctus is a filler feeder as like many tide and watery invertebrates which filtrate food materials by ciliary movement. However, the structure of the filter is not yet known in U. unicinctus, nor the filtering mechanism is not well understood. This study reveals ciliated epithelial cells in the foregut and the features of the cilia are good accord with that of known filtrating apparatus of other tide animals. This may implies that the foregut is in function of filtration and the food materials are filtrated by the ciliary movement. With the observation of the filtrating apparatus in the foregut, the intestine of U. unicinctus can be functionally compartmented into 3 parts. These are already known midgut and hindgut in function of digestion and respiration respectively, and the foregut in function of filtrating apparatus for foods. The filtrating apparatus of U. unicinctus is composed of the pseudostratified columnar epithelial cells with numerous cilia. The cilia are well differentiated kinocilia with the typical microtubule pattern, kinetosome and cilia roots. There are two kinds of striated cilia roots, the main root and the accessory root. The main root is extended perpendicularly from the cell surface to basement membrane and the short accessory root is branched with an acute angle of about $80^{\circ}$ from the main root at level of basal plate of the kinetosome. The spacial approaches of the main root with the large fused form of mitochondria is one of the characteristic features which might be in structural consideration an intimate association between energy source and energy mass consuming cell organelles.

Two Cases Report of Herpes Zoster Patients Improved by Korean Medicine Treatment alone (대상포진에 대한 한방 단독 치료 치험 2례)

  • Objectives : The purpose of this study is to report a 67-year-old male patient with Herpes zoster(caseI) and a 38-year-old female patient with Herpes zoster(caseII), whose symptoms were relieved after Korean medicine treatment alone. Methods : Both patients took Yeonryeonggobondan and Pyungwee-san daily. Everytime they came to the hospital, they received acupuncture treatment, Hwidam's Su-Gi therapy and External Gigong Therapy. Acupuncture was applied by gall bladder seunggyeok(膽勝格, GB-), spleen jeonggyeok(脾正格, SP+), large intestine seunggyeok(大腸勝格, LI-) of Saam's Acupuncture Method. Hwidam's Su-Gi therapy was applied on neck. External Gigong Therapy was applied on skin lesion area. the patients' symptoms were evaluated with photographs and VAS. Results : The results were as follows: 1. In the case of a 67-year-old male patient, it took 14 days to recover the skin lesions and to reduce the pain after treatment started. And he visited 13 times during that period. 2. In the case of a 38-year-old female patient, it took 23 days to recover the skin lesions to reduce and the pain after treatment started. And she visited 10 times during that period. 3. Intensive treatment early in treating herpes zoster helped to shorten the treatment period. 4. Taking Yeonryeonggobondan and Pyungwee-san and receiving acupuncture treatment(SP+) can help to improve immunity and recover skin lesions in herpes zoster diagnosed with spleen deficiency with dampness encumbrance(脾虛濕困) and blood stasis due to qi stagnation (氣滯血瘀). 5. The combination of acupuncture treatment(GB-) and External Gigong Therapy was effective in controlling pain. 6. External Gigong Therapy is considered to be effective for the recovery of the skin as well as the pain of the herpes zoster. Conclusions : Korean medicine treatment alone has a great effect on the above two patients with herpes zoster. I hope the active research about Korean medicine treatment will be done not only for herpes zoster but also for various intractable pain diseases.

An Immunohistochemical study of the gastro-entero-pancreatic endocrine cells of the insectivorous Korean mole, Talpa micrura coreana (한국산 두더지의 위장췌내분비세포에 관한 면역조직학적 연구)

  • The regional distribution and relative frequencies of the gastro-entero-pancreatic endocrine cells were studied immunohistochemically in the GIT and pancreas of the Korean mole. Seven kinds of endocrine cells were identified in this study. Although 5-HT- and somatostatin-immunoreactive cells were seen throughout the GIT, they were most predominant in the pyloric gland region. Glucagon-immunoreactive cells were restricted on the large intestine. Bovine CG-immunoreactive cells were more frequent in the stomach than in the intestines which were not detected in the duodenum. Numerous Gas/CCK-immunoreactive cells were found in the pyloric gland region, but rarely in the jejunum and ileum. BPP-immunoreactive cells were observed to be rare in the stomach and ileum but were a few in number in the intestines. In the pancreas, four types, namely, glucagon-, somatostatin-, BPP- and insulin-immunoreactive cells were identified in the pancreatic islets and exocrine portion. These results suggest that although endocrine cells of the Korean mole is less abundant in the duodenum, the distribution pattern of its gastro-entero-pancreatic endocrine cells is similar to that reported for the Korean hedgehog.

Trematode infections in the small intestine of Egretta alba modesta in Kangwon-do (강원도 횡성지성 백로의 소장 흡충류 감염상태)

  • We surveyed 12 migrating large egrets, Egretta alba modesta, for their infection status with intestinal trematodes, from June to September, 1990. All of them were infected with one or more flukes. Total 5 species were identified. Their infection rates and total numbers of worms were Metagonimus sp. 67% and 1, 412, Centrocestus armatus 67% and 2, 306, Echinochasmus japenicus 50% and 99, Strigea falconis 83% and 635, and a diplostomid cuke 33% and 36. The number of worms in a bird was in the range from 6 to 896. This is the first record that the egret is a reservoir host of Metagonimus sp., C. armatus, E. japonicus, and S. falconis. S. falconis is a new parasite fauna in Korea. Migrating birds must play an important role in the transmission and spreading of these intestinal trematodes in Korea.

Identification and Prevalence of Globocephalus samoensis (Nematoda: Ancylostomatidae) among Wild Boars (Sus scrofa coreanus) from Southwestern Regions of Korea

  • This study describes the first record of Globocephalus samoensis (Nematoda: Ancylostomatidae) recovered in wild boars from southwestern regions of Korea. Gastrointestinal tracts of 111 Korean wild boars (Sus scrofa coreanus) hunted from mountains in Suncheon-si, Gwangyang-si, and Boseong-gun between 2009 and 2012 were examined for their visceral helminths. G. samoensis, as identified by morphological characteristics of the head and tail, were recovered from the small intestine of 51 (45.9%) wild boars. Worms were found from 7 of 28 wild boars (25.0%) from Suncheon-si, 40 of 79 (50.6%) from Gwangyang-si, and all 4 (100%) from Boseong-gun. The length of adult females was $7.2{\pm}0.5mm$, and the thickest part of the body measured the average $0.47{\pm}0.03mm$, while those of males were $6.52{\pm}0.19$ and $0.37{\pm}0.02mm$, respectively. The buccal cavity was equipped with a pair of large and bicuspid subventral lancets near the base of the capsule. The average length of spicules of males was $0.45{\pm}0.02mm$. By the present study, G. samoensis is recorded for the first time in southwestern regions of Korea. Additionally, morphological characteristics and identification keys provided in the present study will be helpful in the faunistic and taxonomic studies for strongylid nematodes in both domestic and wild pigs. The infection of G. samoensis apparently did not elicit pathologic lesions, as revealed by macroscopic observation during the autopsy of all wild boars in this study.

Effects of Rice Bran and Wheat Bran on Intestinal Physiology and Small-bowel Morphology in Rats

  • The present study was aimed at investigating the nutritional and physiological significance of rice bran as a source of dietary fiber as compared to pectin and wheat bran. The parameters for comparison included hypertrophy and morphology of intestines, stool weights and villus marker enzyme activity. For 6 weeks, 10 Sprague Dawley male rats were given one of six experimental diets: 1% cellulose control (CC), 5% pectin (P5), 5% rice bran(RB5), 10% rice bran(RB10), 5% wheat bran (WB5) or 10% wheat bran (WB10) based on the level of dietary fiber. Among experimental groups, food efficiency ratio and body weight gain was comparable. RB10 increased cecal and colonic tissue weights and content weights of cecum and colon as much as P5 did. Stool weight was positiviely correlated with colonic tissue weight (r=0.727, P<0.001), with colonic content weight(r=0.647, P<0.001). Small intestine length increased most in the P5 group, followed by the RB10 group. The scanning electron micrograph of jejunal villi from rice bran groups showed a leaf-shaped, smooth and regular pattern, whereas that of CC group produced a rather long shape. The wheat bran groups showed an irregular leafshaped pattern, and the pectin group typically produced leaf-shaped villi with surface damage. The activities of villus marker enzymes (maltase and sucrase) were higher in the bran-fed rats than in the control or pectin-fed rats. The results indicate than not only dietary fiber amounts but also fiber sources are closely related to the physiology and morphology of the large and small intestines in rats. Rice bran exerted effects on fecal output and trophic effects on the intestines similar to those of pectin.

Development and industrial application of low-calorie food ingredients derived from starches (전분 유래 저열량 식품소재의 개발과 산업적 이용)

  • Indigestible carbohydrates as dietary fiber have attracted interest of consumers due to their several physiological benefits. Recent definitions of dietary fiber have included other indigestible carbohydrates such as resistant starch and resistant maltodextrins, which are natural, colorless, odorless and tasteless low-calorie food ingredients. Unlike some carbohydrates and digestible starches, indigestible starch and maltodextrin resist enzymatic hydrolysis in the upper gastrointestinal tract, resulting in little or no direct glucose absorption. In addition, there is increased microbial fermentation production of short-chain fatty acids in the large intestine. As an emerging functional low-calorie food ingredient, resistant starch and maltodextrin have been shown to have equivalent or superior impacts on human health compared to conventional fiber-enriched food ingredients. In this paper, the definition, strategies to enhance dietary fiber content in foods, some potential health benefits, and applications in food industry for indigestible starch and maltodextrin are summarized and discussed.

  • An in vitro digestibility methods have been developed to mimic an in vivo system in the past decades. Because the in vivo techniques cause high cost, intensive labor, longer research periods and ethnical problems. In this review, the digestive systems from stomach to large intestine of pig as a monogastric animal were addressed to understand an in vivo digestion. The innovative in vitro technique using the Daisy II incubator was performed and the in vitro ileal and fecal dry matter (DM) digestibility of corn, rice, wheat and barley was determined.
