A Study on Desirable Breast Type of Women in Their Twenties Based on the Ratio of Breast in Works of Art and Cup Design According to Breast Type (미술작품의 유방 실루엣 비율에 근거한 20대 여성의 바람직한 유방형태와 유형별 컵 설계에 관한 연구)

  • This paper analyzes desirable breast types using measurements from 31 selected female subjects based on the ratio of breast in works of art and to design a brassiere cup design according to the breast type of 182 female subjects in their 20s. The subjects selected for this paper are somewhat larger than a previous study on aesthetically perfect breasts of foreigners according to chest breadth, center neck point to breast point, breast circumference, and breast volume. However, the aesthetic breasts of foreigners are larger than the subjects selected for this study according to chest depth, under-bust circumference, and bust point to bust point. Comparing various breasts types and aesthetic breasts, padding is necessary to complement the form of flat breasts to increase the volume and diameter. Brassiere cups for cone-shaped breasts should be designed to increase breast volume through an increase in nipple height. Hemisphere breasts should be designed to increase the overall volume-leaving diameter. Protrusion breasts should enhance the functions of aggregating and supporting without any increase or decrease of the breast volume. Drooping breasts require the ability to support a large volume to stabilize the breast. Subjects were selected depending on the ratio of breast silhouette as works of art and who have large breasts in disproportional to a slender trunk. Three items, the circumferential length of breast, height of the nipple, and the depth of inner breast using the anthropomorphic measurements of 182 subjects were measured through regression equations for breast volume. Breast volume = -394.86 + 27.52 ${\times}$ (the circumferential length of breast) + 18.73 ${\times}$ (height of the nipple) + 12.85 ${\times}$ (the depth of inner breast). Regression equations to extract the aesthetic breast volume in measurements irrelevant to breasts using the anthropomorphic measurements of 31 subjects were as follows. Aesthetic breast volume = -611.30 + 17.67 ${\times}$ (bust circumference) -24.29 ${\times}$ (under-bust circumference) + 16.31 ${\times}$ (neck point to breast point to waistline) + 22.83 ${\times}$ (bust breadth) + 12.22 ${\times}$ (waist depth) -8.34 ${\times}$ (interscye- front). This prediction equation is useful to develop a breast type brassiere pattern, complement breast enhancement surgery, or minimize the effect a mastectomy.

Multiple Primary Malignant Neoplasms: A Case Report of Breast Mucinous Carcinoma and Extramammary Diffuse Large B-Cell Lymphoma in a Male Patient (다발성 원발성 악성종양: 남성 환자에서 유방의 점액성 선암과 유방외 미만성 거대 B세포 림프종이 병발한 증례 보고)

  • Multiple primary malignant neoplasms refer to two or more malignancies in an individual that are not related. We report a case of a 78-year-old male with concurrent breast mucinous carcinoma and extramammary lymphoma. The patient initially presented with palpable masses in the left breast and the right groin, which were pathologically confirmed after a surgical biopsy as breast mucinous carcinoma and diffuse large B-cell lymphoma, respectively. He underwent whole-body 18-fluorine deoxyglucose PET/CT before surgery, and an enhancing nodular lesion in the left lingual tonsil was found incidentally. It was later confirmed as a diffuse large B-cell lymphoma, a pathology of the same type as the right inguinal mass. Unspecified lymphadenopathies in breast cancer patients may easily be considered as metastatic lesions. However, this case suggests that lymphomas should be included in the differential diagnoses to avoid misdiagnosis and treatment delay, especially in older adult patients.

Experience of Reconstruction of an Extensive Chest Wall Defect with Extended External Oblique Musculocutaneous Flap (거대 흉벽 결손에 대한 확장 외복사근 근피판을 이용한 흉벽재건 치험례)

  • Purpose: The resection of locally advanced or recurred breast cancers frequently result in large chest wall defects and it leads to a great challenges to cover. Generally simple skin grafts are not a practical option for patients because of their poor cosmetic appearance and prognosis. The latissimus dorsi and rectus abdominis musculocutaneous flap have traditionally been recommended for closure of these large defects. Though the cosmetic result of reconstruction using these flaps is often excellent, but has significant drawbacks. Therefore, we thought that chest wall reconstruction using the external oblique musculocutaneous flap can be an alternative method for extensive chest wall defect related to large, locally advanced breast carcinoma. Methods & Results: We present a case of a 50-year-old Korean female, refered to our department with a left breast tumor for 10 months. CT demonstrate a large tumor on the left anterior chest wall and multiple nodules of varying size in the cervical areas and liver. FDG-PET showed areas of hot uptake throughout the left chest wall, mediastinum and liver. Biopsy was consistent with invasive ductal carcinoma (Grade III). The initial tumor was considered inoperable, so a series of chemotherapy was initiated. Though the size of the breast mass was slightly decreased, the patient continued to suffer from purulent discharge, unpleasant odor and contact bleeding of the mass, the salvage mastectomy was performed. Conclusion: We could reconstruct $23{\times}16\;cm$ sized large chest wall defect, resulting from the resection of a locally advanced breast carcinoma, using an external oblique musculocutaneous flap successfully. Immediate postoperatively checked flap was healthy. Overall result was good without any significant complications and discharged 3 weeks after operation.

Breast Reconstruction with Lower Abdominal Tissue Free Transfer (하복부 조직 유리피판에 의한 유방재건술)

  • Lower abdominal tissue is regarded as an ideal donor site for the breast reconstruction because it provides large skin territory and huge amount of soft tissues enough to the breast size. However it is not easy for the surgeon to reconstruct the really natural breast, and needs the learning curve with long time experience. Author represent the various reconstruction procedures for the breast using lower abdominal tissue such as muscle sparing free TRAM, DIEP, and SIEA free flaps to reinsure like breast. Indications, and selection of reconstructive methods, surgical timing, selection of donor sites and recipient vessel of these flaps were reviewed. In addition, detailed procedures, surgical tips and secondary adjuvant procedures are described for more symmetry of reconstructed breast. The muscle sparing free TRAM, DIEP, and SIEA free flaps would be enough to provide supple, huge amount of well vascularized tissue for the breast, if these flaps were selected for the appropriate indication according to patient's general condition, obesity, the opposite breast and abdominal tissue condition. Lower abdominal tissue was able to provide versatile designs with sufficient adipose tissue without compromising the integrity of abdominal wall.

Clinical Applications of Bioactive Milk Components: A Review (우유 생리활성 물질의 임상적 적용)

  • Milk contains essential nutrients and functional compounds, such as calcium, fat-soluble vitamins A, D, E, and K, carotenoids, bioactive peptides, and sphingolipids. The bioactive molecules from milk are not expensive and have an added advantage of being derived from food. Therefore, they are more stable and have a broader spectrum than that of other chemicals. Bioactive milk components are useful for treating non-digestive tract disorders, such as cancer, cognitive decline, and hypertension. However, the clinical application of certain breast milk ingredients is limited due to the lack of a large-scale production technology. Once the scaled-up production of lactoferrin became possible, clinical applications were devised and evaluated. Similarly, human alpha-lactalbumin made lethal to tumor cells (HAMLET) can be produced on a large scale as a recombinant protein in microorganisms or in transgenic cattle using suitable separation systems. HAMLET can be used to treat human skin papilloma and cancer. Studies on breast milk that explored the clinical applications of the bioactive components of breast milk have spurred the development of translational medicine and breast milk-derived therapeutics. Some breast-milk derived therapeutic agents are already available to clinicians. Many components of breast milk have shown efficacy in pre-clinical studies and have valid clinical evaluations.

A Dosimetric Evaluation of Large Pendulous Breast Irradiation in Prone Position (Large Pendulous Breast 환자의 방사선 치료에 있어서 엎드린 자세의 유용성 평가)

  • Purpose: To evaluate dosimetry results of three different techniques for whole breast irradiation after conservative surgery of large pendulous breast patient. Materials and Methods: Planning computed tomography (CT) scans for three techniques were performed on a GE Hi-speed advantage CT scanner in the supine (SP), supine with breast supporting Device (SD) and prone position on a custom prone mattress (PP). Computed tomography images were acquired at 5 mm thickness. The clinical target volumes (CTV), ipsilateral lung and heart were delineated to evaluate the dose statistic, and all techniques were planned with the tangential photon beams (Pinnacle$^3$, Philips Medical System, USA). The prescribed dose was 50 Gy delivered in 25 fractions. To evaluate the dose coverage for CTV, we analysed percent volume of CTV receiving minimum of 95%, 100%, 105%, and 110% of prescription dose ($V_{95}$, $V_{100}$, $V_{105}$, and $V_{110}$) and minimal dose covering 95% ($D_{95}$) of CTV. The dosimetric comparison for heart and ipsilateral lung was analysed using the minimal dose covering 5% of each organs ($D_5$) and the volume that received >18 Gy for the heart and >20 Gy for the ipsilateral lung. Results: Target volume coverage ($V_{95}$ and $V_{100}$) was not significantly different for all technique. The V105 was lower for PP (1.2% vs. 4.4% for SP, 11.1% for SD). Minimal dose covering 95% ($D_{95}$) of target was 47.5 Gy, 47.7 Gy and 48 Gy for SP, SD and PP. The volume of ipsilateral lung received >20 Gy was 21.7%, 11.6% and 4.9% for SP, SD and PP. The volume of heart received >18 Gy was 17.0%, 16.1% and 9.8% for SP, SD and PP. Conclusion: Prone positioning of patient for large pendulous breast irradiation enables improving dose uniformity with minimal heart and lung doses.

Iterative integrated imputation for missing data and pathway models with applications to breast cancer subtypes

  • Tumor development is driven by complex combinations of biological elements. Recent advances suggest that molecularly distinct subtypes of breast cancers may respond differently to pathway-targeted therapies. Thus, it is important to dissect pathway disturbances by integrating multiple molecular profiles, such as genetic, genomic and epigenomic data. However, missing data are often present in the -omic profiles of interest. Motivated by genomic data integration and imputation, we present a new statistical framework for pathway significance analysis. Specifically, we develop a new strategy for imputation of missing data in large-scale genomic studies, which adapts low-rank, structured matrix completion. Our iterative strategy enables us to impute missing data in complex configurations across multiple data platforms. In turn, we perform large-scale pathway analysis integrating gene expression, copy number, and methylation data. The advantages of the proposed statistical framework are demonstrated through simulations and real applications to breast cancer subtypes. We demonstrate superior power to identify pathway disturbances, compared with other imputation strategies. We also identify differential pathway activity across different breast tumor subtypes.

Role of Nuclear Factor-κB in female Breast Cancer: A Study in Indian Patients

  • Introduction: The nuclear factor ${\kappa}B$ (NF-${\kappa}B$) is a super family of transcription factors which plays important roles in development and progression of cancer. The present investigation concerns NF-${\kappa}B$ /p65 activity in human breast cancers with overexpression of ER, PR, HER-2/neu, as well as the significance of p65 expression with regard to menopausal status, stage, grade, tumor size, nodal status, and NPI of invasive ductal carcinomas in Eastern India. Materials and Methods: In this hospital based study 57 breast cancer patients attending a Breast Clinic of a reputed institute of Eastern India were assessed for p65 protein expression in breast tumor tissue samples by Western blotting. ER, PR and HER-2/neu expression was determined by immunohistochemistry. Results: NF-${\kappa}B$/p65 was significantly associated with advanced stage, large tumor size (${\geq}5$ cm), high grade, negative ER, negative PR, and positive HER-2/neu. High NF-${\kappa}B$/p65 expression was more frequent in patients with a high NPI ($NPI{\geq}5.4$, 84.6%) compared with low NPI (<5.4, 44.4%) and this association was statistically significant (p = 0.002). Conclusion: NF-${\kappa}B$/p65 overexpression was associated with advanced stage, large tumor size, high grade, and high NPI which are poor prognostic factors linked to enhanced aggressiveness of the disease. NF-${\kappa}B$/p65 expression implies aggressive biological behavior of breast cancer and this study validates significant association of NF-${\kappa}B$ /p65 overexpression with negative estrogen and progesterone receptor status and overexpression of HER-2/neu oncoprotein. In our good clinical practice, patients with NF-${\kappa}B$ positive tumors need to be treated aggressively.

  • Medullary carcinoma (MC) of the breast is an uncommon histologic form of infiltrating ductal carcinoma and the cytologic features of MC are not well delineated. We recently experienced a case of fine needle aspiration cytology of MC of the breast. The characteristic cytopathologic features were cellular aspirate consisting of large, pleomorphic cells in background of many single to clusters of lymphocytes.

A Study on Classification of Breast Shapes for Women in Their 30s (30대 여성 유방형태 분류에 관한 연구)

  • The purpose of this study is to provide basic information for the development of well-fitting and comfortable brassieres for women in their 30s, based on the classification of breast shapes by utilizing 3D body measurement data of women in the age group. The result of processing the measurements from the 3D body scan data through RapidForm 2006 shows that while the vertical body measurements decrease, the horizontal measurements, which indicates the degree of obesity, increase proportionally with age. Also, the relevant measurements for upper breasts increase proportionally with age, while the measurements for lower part of the breasts decrease as the degree of sagging increases. Four composition factors of the breasts were identified through the factor analysis: Factor 1 is the level of volume in the breasts and the surrounding area; Factor 2 is the position of nipples and the shape of upper part of breasts; Factor 3 is the position and vertical length of the breasts; Factor 4 is the shape of lower part of breasts; and Factor 5 is the shape of inner part of breasts and degree of width between both breasts. The breasts have been categorized into three distinctive shapes: Breast Shape 1, Breast Shape 2 and Breast Shape 3. According to the results of the cluster analysis, the largest percentage (36.68%) of women in their 30s falls into the category of Breast Shape 2 with small volume and flat upper breasts, followed by Breast Shape 1 (32.66%) with large volume in the upper and lower parts of breasts, and large side to side area, and Breast Shape 3 (30.65%) with average volume and width between breasts.
