Successful Obliteration of Unclippable Large and Giant Middle Cerebral Artery Aneurysms Following Extracranial-Intracranial Bypass and Distal Clip Application

  • Large to giant middle cerebral artery aneurysm is a challenging disease, especially when incorporating important perforating arteries. Surgical risk increases by perforator infarction and anatomical complexity. In this clinical setting, extensive consideration of surgical options is needed. The two cases described here were unruptured and had rather stable wall. Because of their large and giant size, hardness and incorporated arteries, it was not affordable to isolate them by means of clipping or trapping. The procedure as the alternative to conventional treatment modalities, extracranial-intracranial bypass followed by clipping of only the efferent artery successfully treated the aneurysms.

Endosaccular Treatment of Very Large and Giant Intracranial Aneurysms with Parent Artery Preservation : Single Center Experience with Long Term Follow-up

  • Objective : Very large (20-25 mm) and giant (${\geq}25mm$) intracranial aneurysms have an extremely poor natural course, and treatment of these aneurysms remains a challenge for endovascular and surgical strategies. This study was undertaken to describe our experiences of endosaccular treatment of very large and giant intracranial aneurysms with parent artery preservation. Methods : From January 2005 to October 2016, twenty-four very large or giant aneurysms in 24 patients were treated by endosaccular coil embolization with parent artery preservation. Nine (37.5%) aneurysms were ruptured and 15 were unruptured, and of these 15, 11 were symptomatic cases and 4 were incidentally discovered. The cohort comprised 17 women and 7 men of mean age 58.5 years (range, 26-82). Mean aneurysm size was 26.0 mm (range, 20-39) and 13 of the 24 aneurysms were giant. Results : Immediate angiographic results were complete occlusion in nine (37.5%) cases, remnant neck in six (25.0%), and remnant sac in nine (37.5%). Overall procedural related morbidity and mortality rates were 12.5% and 4.2%, respectively. Angiographic follow-up was available in 16 patients (66.7%). Mean and median follow-up periods were 27.2 (range, 2-77) and 10.5 months, respectively. In 12 cases (12/16, 75%) stable occlusion was achieved, four cases (4/16, 25%) had recanalized, and two of these were retreated with additional coiling. At clinical follow-up of the nine ruptured cases, three patients (33.3%) achieved a good clinical outcome (Glasgow outcome scale [GOS] score of 4 or 5), two (22.2%) a poor outcome (GOS score of 2 or 3), and four patients (44.4%) expired (GOS 1). On the other hand, of the 15 unruptured cases, 13 patients (86.7%) achieved a good clinical outcome (GOS 4 or 5), one patient a poor outcome (GOS score of 2 or 3), and one patient expired (GOS 1). Conclusion : The present study shows endosaccular treatment of very large or giant intracranial aneurysms with parent artery preservation is both feasible and effective with acceptable morbidity and mortality.

Giant Serpentine Aneurysm of the Middle Cerebral Artery

  • Giant serpentine aneurysms are rare and have distinct angiographic findings. The rarity, large size, complex anatomy and hemodynamic characteristics of giant serpentine aneurysms make treatment difficult. We report a case of a giant serpentine aneurysm of the right middle cerebral artery (MCA) that presented as headache. Treatment involved a superficial temporal artery (STA)-MCA bypass followed by aneurysm resection. The patient was discharged without neurological deficits, and early and late follow-up angiography disclosed successful removal of the aneurysm and a patent bypass graft. We conclude that STA-MCA bypass and aneurysm excision is a successful treatment method for a giant serpentine aneurysm.

거대억새 단지 조성에 따른 토양 특성 변화 (Change in Soil Properties after Planting Giant Miscanthus)

  • Miscanthus has received wide attention as an option for biomass production in Korea, recently. New strain of giant miscanthus has been developed and was planted in two large trial sites (184 ha) in the lower reaches of the Geum River. To evaluate the susceptibility of the giant miscanthus as an bioenergy crop for the future, we investigated the influence of the giant miscanthus on the soil properties. The particle size, CEC, pH, EC, T-N, T-P, heavy metal total concentration, and heavy metal fractions of soil samples collected from abandoned field, 1 year old giant miscanthus field (1st Year GM), and 2 year old giant miscanthus field (2nd Year GM) at different depths of 0~15, 15~30, and 30~45 cm in April and August were analyzed. Results showed that the CEC and pH of the soil of the giant miscanthus field were lower than those of the soil of abandoned field. The EC of 2nd GM was lower than that of abandoned field, indicating that the giant miscanthus may facilitate soil desalination process. The organic and sulfide fraction and residual fraction of heavy metals in the giant miscanthus field was higher than in abandoned field, due to the low pH of the giant miscanthus field and the excretion of phytosiderophores by rhizome of the giant miscanthus. This study showed that the giant miscanthus can influence on the soil properties and further study for long term is needed to elucidate the interaction between the soil and the giant miscanthus.

  • The cultivation of biomass crops is now global demand for decreasing emissions of carbon dioxide ($CO_2$) from fossil fuel. Miscanthus species have been studied as a suitable crop for biomass production, due to its characteristics of fast growth and high biomass. In Korea, Miscanthus species have gained wide attention as an option for biomass production alternative to fossil fuels, recently. New strain of giant Miscanthus has been developed and two large trial sites for the giant Miscanthus production were built in the lower reaches of the Geum River. To evaluate the ecological influence of the giant Miscanthus as an bioenergy crop for the future, we investigated the impact of the construction of the giant Miscanthus production fields on the biota and also compared it with biota in paddy fields near the study sites. The biota including plants, amphibians, reptiles, mammals, avifauna, insects, and bugs was investigated. The plant diversity of the giant Miscanthus production fields was poorer than the paddy fields because the high height of the giant Miscanthus might hinder the growth of other plants. However, the giant Miscanthus production fields serves habitat to animals, leading to rich diversity of animals including avifauna, insects, and bugs. The rich diversity of the animals in the giant Miscanthus production fields coincides with the fact that the giant Miscanthus was grown without any pesticide, herbicide, and fertilizer. This study showed that the giant Miscanthus can influence on biota and further long term study is needed to elucidate the interaction between the diversity of biota and the giant Miscanthus.

편평상피암과 연관된 거대피각 1례 (Giant Cutaneous Horn Associated with Squamous Cell Carcinoma: A Case Report)

  • Cutaneous horn is a morphologic designation for a projectile, conical, dense hyperkeratotic nodule that resembles the horn of an animal. The lesion varies in size from only a few millimeters to several centimeters, in color(white or yellowish) and in form (straight, curved, or twisted). It arises from a wide range of epidermal lesions, which include benign lesions, premalignant lesions and malignant lesions. An 83-year-old women came to our clinic with a giant cutaneous horn on the right chin and a small horn on the left upper eyelid. The patient had no palpable cervical lymph node. A wide elliptical skin incision was made and the horn was totally excised. In pathology, the giant cutaneous horn on the right chin revealed a moderately differentiated squamous cell carcinoma with subcutis invasion at its base. "Giant cutaneous horns" have often been associated with invasive squamous cell carcinoma. Cutaneous horns are common lesions usually found on the face, rarely larger than 2 cm. As large cutaneous horns are often associated with underlying malignancy, histopathologic examination of the base of the lesion is necessary to rule out carcinoma and full excision is recommended.

악하선에서 발생한 거대 타석증: 증례 보고 (Giant sialolithiasis of the submandibular gland: a case report)

  • Sialolithiasis is the common pathology of salivary gland. The size of sialoliths vary from 1 mm to a few cm, but most of that are less than 10 mm. Large sialoliths (larger than 15 mm) are extremely rare. It is called Giant sialolithiasis or megalith. Symptom of the giant sialolithiasis is similar to that of regular sialolithiasis. First choice of treatment is removal of the stone. Many literatures reported various methods to remove the sialoliths. For this case report, we accidentally found the giant sialolith on the computed tomography taken for dental implant, and successfully removed the stone by minimal invasive surgical approach. Base on this result, we report this case with literature reviews.

  • 아프리카왕달팽이 Achatina fulica의 내장신경 절과 우체벽신경절은 좌, 우 양반구로 구성된 나비모습을 하고 있으며, 이들 신경절의 피질부와 수질부의 표피부위에는 신경세포가 밀집되어 있는 반면 중앙부위에는 신경섬유가 망상형으로 신경망을 구성하고 있었다. 두 신경절의 피질부 및 수질부에 위치한 신경세포들은 초대형신경세포(200 $\mu\textrm{m}$ 이상)와 대형신경세포(직경 60-70 $\mu\textrm{m}$ 이상), 중형신경세포(직경 30-40 $\mu\textrm{m}$) 그리고 소형신경세포(직경 10-15 $\mu\textrm{m}$) 등 4종류로 구분할 수 있었다. 초대형 및 대형신경세포는 20-22개 정도로 매우 소수가 관찰된 반면, 중형신경세포(약 400-500개)와 소형신경세포(약 700-800개)는 다수가 관찰되었다. AB/AY 이중염색반응에서 초대형 신경세포는 light Yellow cell(LYC)로, 대형 및 중형신경세포는 yellow green cell (YGC)과 dark green cell(DGC) 등 두 종류로 그리고 소형신경세포는 yellow cell(YC)과 blue cell(BC) 등으로 각각 확인되었다. Somatostatin 면역염색반응에서 양성반응을 나타낸 DGC는 성장조절물질 분비의 억제에 관여하는 것으로 확인되었으며, 초대형 및 대형신경세포는 신경분비기능 이외 포식작용을 수행하는 것으로 각각 확인되었다.

간의 파골세포모양 거대세포 종양의 세침흡인 세포학적 소견 (Fine Needle Aspiration Cytology of Osteoclast-like Giant Cell Tumor of the Liver)

  • Osteoclast-like giant cell tumor of the liver is an extremely rare malignancy with poor prognosis. To our knowledge, 5 cases have been reported in English literatures, but there was no report about fine needle aspiration cytologic(FNAC) features. We experienced a case of osteoclast-like giant cell tumor of the liver obtained by computed tomography(CT)-guided FNAC and needle biopsy. The cytologic findings mimicked slant cell tumor of the bone. A large hepatic mass of the left lobe with abdominal wall invasion was found by CT in a 46- year-old female complaining of epigastric pain. The FNAC showed moderately cellular smears consisting of osteoclast-like giant cells and mononuclear cells, which were individually scattered or intermingled in clusters. The osteoclast-like giant cells had abundant cytoplasms and multiple small round nuclei with fine chromatin and distinct nucleoli. The mononuclear cells had moderate amount of cytoplasm and relatively bland-looking oval nuclei with single small nucleoli. All of the cytologic features recapitulated the histologic findings of bland-looking osteoclast-like multinucleated giant cells evenly dispersed throughout the background of mononuclear cell. The immunohistochemical study showed positive reaction for CD68 and vimentin, but negative for cytokeratin in both osteoclast-like slant cells and mononuclear cells.

Discovery of Giant Magnetostriction in Amorphous RFe$_2$B (R = Sm, Tb) Alloys

  • Compared with the conventional magnetostriction in Ni alloys which are in the order of several tens ppm (Parts Per Million =10-6), RFe$_2$(R = rare earth element) Laves Phase intermetallic compounds show large saturation magnetostriction in the range of a few thousands ppm. However, the large external magnetic field necessary to obtain saturatio magnetostriction has due to large magnetocrystalline anisotropy energy restrained the applicationof magnetostriction materials in RFe$_2$intermetallic compounds. As a result of its solution, the largest published value of effective giant magnetostriction in a low external magnetic field (less than a few hundred Oe) is reported in this paper by means of amorphisation of RFe$_2$intermetallic compounds with the addition of boron, as a half metal. For the amorphous (SmFe$_2$)0.97 B0.03 alloys, the effective magnetostriction of -545 and -610 $\times$ 10-6 is obtained at 400 and 1,000 Ie, respectively. Moreover, the effective magnetostriction of 590 and 630$\times$10-6 in the amorphous (TbFe$_2$)0.98 B0.02 alloys is also found at 400 and 1,000 Oe, respectively. This result will provide a clue to understanding the effect of half metal on anomalous increase of the effective giant magnetostriction and attract the great attention for magnetostriction applications.
