Prevalence and Factors Associated with Opisthorchis viverrini Infection in Khammouane Province, Lao PDR

  • Opisthorchis viverrini (OV) liver flukes are common parasites found in central and southern Laos and constitute a major public health problem in the country. Laos people continue to have the habit of extensively consuming raw or half-cooked fish which are intermediate hosts. This study aimed to determine the prevalence and factors associated with OV infection in the population of Thakek district, Khammouane Province. This cross-sectional analytic study covered 237 subjects who filled out structured questionnaires. Fecal examination for OV infection was performed by Kato's thick smear method. Data analysis was carried out using STATA Version 10.0. Multiple logistic regression was applied. The results showed that the infection rate of OV was 54.8%. Factors associated with OV infections were gender, a habit of defecation in fields and raw fish (goi bplaa dip) consumption. Opisthorchiasis and associated cholangiocarcinoma development thus appear to remain as important concerns in Laos.

  • Potential and capabilities of GIS in planning and decision-making is widely known as an experimental tool, but its existence in real-world application is yet to realize. GIS originated in the field of cartography and even at today it is mainly used as a tool for map production. Number of reasons could bring forward for this shortcoming. Limited real-world applications could be a significant one among them. This paper describe an attempt to use GIS technology in Lao PDR where a study team was involved in formulating a Master Plan for national healthcare system. A database was created for the whole country with spatial and descriptive data gathered from various sources. Attempt was made to demonstrate the potential of GIS by using the database for identifying healthcare status, distribution of resources, recognizing deprived areas and locating areas for future investments based on accessibility and social status. The approach justified the investment on a GIS system and introducing the system in the planning sector from grass-root level to central level.

The Effect of the Dr. LEE Jong-wook-Seoul Project on Medical Student Academic Performance in Laos

  • The Dr. LEE Jong-wook-Seoul Project (DLSP) in the Lao People's Democratic Republic (PDR) is a long-term development cooperation initiative aimed at upgrading the medical education capacity at the University of Health Sciences (UHS) in the Lao PDR. Under the project, eight faculty members from UHS were trained at the Seoul National University College of Medicine for a 12-month period during 2010-2011. Using a training-of-trainers model, we estimated the effect of DLSP on the learning outcomes of students, which was measured using the standard test scores of the students. The questionnaire developed by the Medical Education Assessment Consortium in Korea was given to the students and both the percent-correct and standardized scores were examined. The evaluation strategy was to compare the learning outcomes of the students taught by the trainees with those that were not and to compare the medical topics taught by the trainees with other topics. It was found that the first group of trainees in the DLSP was associated with an increase of 1.5 in the percent-correct test scores for basic medicine. While the interpretation requires caution, it is consistent with the view that a training program for faculty members could be effective in improving the teaching capacity of medical schools in developing countries.

  • Mushroom survey was conducted in the Phou Khao Khuoay National Protected Area (PKKNPA) located at east direction 65km from Vientiane, the capital city of Lao-PDR from September 2015 to November 2016. During this periods, mushroom specimens were collected from 10 different survey sites, and then were identified and classified into 149 species, 113 genera, and 55 families by morphological and molecular analyses. The mushrooms belongs to Ascomycota were classified into 13 species, 7 genera, 5 families, and 5 orders, while those belongs to Basidiomycota were classified into 136 species, 113 genera, 55 families, and 18 orders, respectively. Among these mushrooms, the most species-rich families were Polyporaceae (18.1%), Marasmiaceae (11.0%), Ganodermataceae (8.6%), Xylariaceae (5.8%), Russulaceae (5.4%), Agaricaceae (4.7%), Boletaceae (4.7%), Hymenochaetaceae (3.9%), and Amanitaceae (3.6%), and comprised 65.8% of the total specimens identified.

  • Mushroom survey and collection were conducted in the Dong Hua Sao National Biodiversity Conservation Area (DHSNBCA), which is located about 30 km east of Pakse and about 10 km south of the town of Paksong, Champasak Province of Lao-PDR, from from at the early July to October in 2019. During this periods, mushroom specimens were collected from 6 different locations, and then they were identified and classified into 103 species, 74 genera, 37 families, 15 orders and 6 classes by morphological and molecular analyses. The mushrooms belongs to Ascomycota were classified into 10 species, 6 genera, 4 families, 3 orders and 2 classes, while those belongs to Basidiomycota were classified into 92 species, 67 genera, 32 families, 11 orders, and 3 classes, respectively. In addition, 1 species of Myxomycota was also identified. Among these mushrooms, the most species-rich families are Marasmiaceae (17.3%), Polyporaceae (13.1%), Mycenaceae (8.9%), Agricaceae (8.3%), Xylariaceae (6.5%), Auriculariaceae (4.8%), Ganodermtaceae (3.6%), Dacrymycetaceae (3.0%), Meruliaceae (3.0%), Russulaceae (3.0%) and comprised 71.4% of the total specimens identified.

모자보건 증진사업에 대한 비용편익 분석 - 라오스 국가를 중심으로 - (Cost-Benefit Analysis of Maternal and Child Health Program - Focusing on Lao People's Democratic Republic -)

  • Objectives: The purpose of this analysis was to assess the maternal and child health program in Lao PDR, which was implemented to reduce maternal and child mortality in Laos. Target areas for the project included 2 provinces (Xiengkhuang, Houaphan) with all 18 districts within them. The beneficiaries of this project included 121,000 childbearing women and 62,000 children under 5 years old (LSIS, 2012). Methods: In this study, it was done for efficiency evaluation of the assistance projects of the health sector of maternal and child health promotion program in Laos that took place from 2010 to 2013. We conducted a cost-benefit analysis for the evaluation of the effectiveness of that program was being carried out effectively. Results: This evaluation adopted the Cost-Benefit analysis approach. Key findings of the Cost-Benefit analysis are the following: The Benefit-Cost Ratio of 1.012, Net Present Value of 84,250,000 Korean Won, an estimated Internal Rate of Return of 10.12%. These findings suggest that project activities were efficiently implemented. Conclusion: As a result, maternal and child health project in Laos has been analyzed that there was economic efficiency. Therefore, It is considered necessary and continued support expansion of program in the future. The direction of maternal and child health project in Laos, approach and community policy support must be included. Integrated approach between the program approach and overall health should be performed for healthy life habit.

High Prevalence of Haplorchis taichui, Phaneropsolus molenkampi, and Other Helminth Infections among People in Khammouane Province, Lao PDR

  • The prevalence of liver and intestinal helminth infections, including Opisthorchis, Haplorchis, Phaneropsolus, hookworms, Enterobius, and Taenia, was surveyed in Khammouane province, Lao PDR. Fecal specimens were collected from 1,242 people (590 men and 652 women) in 3 Mekong riverside villages and were examined by the Kate-Katz thick smear technique. The overall helminth egg positive rate was 81.1%. The positive rate for small trematode eggs, including Opisthorchis viverrini, heterophyids, and lecithodendriids, was 81.1% and the positive rate for hookworms was 6.7%. To obtain adult worms, 35 people who were positive for small trematode eggs were treated with 20-30 mg/kg praziquantel and 10-15 mg/kg pyrantel pamoate, and then purged. Diarrheic stools were collected from 33 of these people and searched for helminth parasites using a stereomicroscope. Mixed infections with various helminths (Haplorchis taichui, Haplorchis yokogawai, Prosthodendrium molenkampi, Phaneropsolus bonnei, echinostomes, hookworms, Trichostrongylus spp., Trichuris trichiura, Enterobius vermicularis, and/or Taenia saginata) were found. The total number of helminth specimens collected was 20,907 (approximately 634 per person). The most common species was H. taichui, followed by P. molenkampi, 0. viverrini, P. bonnei, E. vermicularis, hookworms, and Trichostrongylus spp. These results show that diverse species of intestinal nematodes, trematodes, and cestodes are infecting humans in Khammouane province, Lao PDR.

Intestinal Parasitic Infections and Environmental Water Contamination in a Rural Village of Northern Lao PDR

  • A field survey studying intestinal parasites in humans and microbial pathogen contamination at environment was performed in a Laotian rural village to identify potential risks for disease outbreaks. A parasitological investigation was conducted in Ban Lak Sip village, Luang Prabang, Lao PDR involving fecal samples from 305 inhabitants as well as water samples taken from 3 sites of the local stream. Water analysis indicated the presence of several enteric pathogens, i.e., Aeromonas spp., Vibrio spp., E. coli H7, E. coli O157: H7, verocytotoxin-producing E. coli (VTEC), Shigella spp., and enteric adenovirus. The level of microbial pathogens contamination was associated with human activity, with greater levels of contamination found at the downstream site compared to the site at the village and upstream, respectively. Regarding intestinal parasites, the prevalence of helminth and protozoan infections were 68.9% and 27.2%, respectively. Eight helminth taxa were identified in fecal samples, i.e., 2 tapeworm species (Taenia sp. and Hymenolepis diminuta), 1 trematode (Opisthorchis sp.), and 5 nematodes (Ascaris lumbricoides, Trichuris trichiura, Strongyloides stercoralis, trichostrongylids, and hookworms). Six species of intestinal protists were identified, i.e., Blastocystis hominis, Cyclospora spp., Endolimax nana, Entamoeba histolytica/E. dispar, Entamoeba coli, and Giardia lamblia. Questionnaires and interviews were also conducted to determine risk factors of infection. These analyses together with a prevailing infection level suggested that most of villagers were exposed to parasites in a similar degree due to limited socio-economic differences and sharing of similar practices. Limited access to effective public health facilities is also a significant contributing factor.

물류환경 분석을 통한 물류시장 진출 방안에 관한 연구 : 메콩강 유역 4개국을 중심으로 (A Study on Entry Strategies Through Analysis of Logistics Environments : Focused on Mekong River Basin 4 Countries)

  • 메콩강은 동남아시아 최대의 하천으로 인도차이나 반도 5개국(태국, 라오스, 베트남, 캄보디아, 미얀마)을 관통한다. 메콩강 유역 국가들은 모두 ASEAN 회원국으로, 특히 저개발국을 중심으로 급속하게 경제성장이 이루어지고 있다. 외국인 투자의 증가와 해외기업의 진출이 증가하고 있고 이에 따라 물류산업의 발전도 기대되는 곳이다. 우리나라 물류기업의 성장을 위해 해외네트워크를 구축할 필요가 있고, 이들 국가는 좋은 투자지역이 될 것이다. 따라서 본 논문은 우리나라 물류기업이 잠재적 성장력을 가지고 있는 베트남, 라오스, 캄보디아, 미얀마를 중심으로 이들 지역에 진출하는데 있어 물류인프라 및 투자법과 제도를 분석한다. 또한 이들 국가 물류시장에 대한 SWOT분석을 하고 이에 따른 진출 방안을 제시한다. 결론적으로 이들 지역에 대한 진출방안은 다음과 같다. 첫째, 이들 지역의 물류기업 진출은 인프라 구축사업과 연계하여 이루어질 필요가 있다. 왜냐하면 이들 지역의 물류 인프라가 아직 많이 취약하고 이러한 인프라 구축사업이 해외원조를 통해 이루어지고 있기 때문이다. 둘째, 취약한 내수시장의 약점을 극복하고 풍부한 천연자원 개발과 연계하여, 제조업 또는 자원 개발기업과 공동으로 진출할 필요가 있다. 또는 해외진출 위험을 분산하기 위해 다른 물류기업과 공동으로 진출하는 것이 바람직하다. 마지막으로 중국과 인도를 잇는 물류요충지 이점을 충분히 활용하고 각 국가가 가진 물류환경에 따른 물류거점 구축에 따른 진출이 필요할 것이다.

라오스 도시·농촌 지역별 40~59세 주민들의 식행동, 식생활변화 및 식품섭취 비교 연구 (Comparison of dietary behavior, changes of diet, and food intake between 40~59 years old subjects living in urban and rural areas in Lao PDR)

  • 본 연구는 라오스의 도시와 농촌 지역에 따른 식행동 및 최근의 식생활의 변화와 이에 따른 건강상태의 차이를 알아보고자 하는 목적으로 수행되었으며 그 결과는 다음과 같다. 1) 연구 대상자는 라오스의 수도 비엔티안과 교외 농촌 지역에 거주하는 성인 40~59세의 979명 (남자 435명, 여자 544명)이었으며, 도시 집단이 농촌 집단에 비해 읽고 쓸 수 있는 비율, 고학력 비율, 가정 내 소유 물품이 많은 비율, 정부기관이나 사 기관에 근무자의 비율이 유의적으로 높게 나타났다. 따라서 경제적 수준은 전반적으로 도시 집단이 농촌 집단에 비해 높은 경향을 보였다. 2) 전반적인 건강의 상태는 도시가 농촌에 비해 몸무게, 허리둘레, 체질량지수, 이완기 고혈압 비율, 알코올 섭취 빈도가 유의적으로 높았으며, 농촌은 도시 집단에 비해 흡연율이 유의적으로 높았다. 주관적 건강의 상태는 안 좋다는 비율이 농촌 집단이 유의적으로 높았으며 매우 좋다는 비율은 도시 집단이 유의적으로 높았다. 3) 거주 지역별 남녀의 건강 상태를 비교 시 도시에 거주하는 여성이 도시에 거주하는 남성에 비해 체질량지수와 비만율이 유의적으로 높았으며, 도시에 거주하는 남성에서 수축기 혈압과 이완기 혈압의 평균이 농촌 지역에 거주하는 남성에 비해 유의적으로 컸다. 4) 농촌에서는 하루 세끼 섭취 비율과 아침식사 섭취 횟수가 도시에 비해 유의적으로 높았으며, 도시 지역에서는 외식 횟수가 농촌 지역에 비해 높았다. 또한 주관적인 식사의 평가와 주관적인 식욕의 정도는 도시가 농촌에 비해 'poor/fair' 비율이 농촌 지역에 비해 유의적으로 높았다. 5) 도시에서는 농촌 지역에 비해 최근 삼 년간 식사가 변화한 비율, 외식의 증가 비율, 서양식 식사가 증가한 비율이 유의적으로 높았다. 6) 도시에서는 우유 및 유제품, 단백질 식품, 과일, 튀기거나 볶은 음식, 지방질 많은 고기의 섭취와 다양하고 균형잡힌 식사의 섭취가 농촌 지역에 비해 유의적으로 높게 나타났으며, 농촌에서는 도시 보다 세끼를 규칙적으로 섭취하고 있었다. 7) 각 식품군별 섭취빈도에서 주간 섭취 횟수는 도시가 과일과 우유의 섭취가 농촌에 비해 유의적으로 높았으나, 채소의 섭취는 낮았다. 하루 섭취량은 도시가 과일, 우유의 섭취 양이 농촌에 비해 유의적으로 높았으며, 채소, 육류, 계란의 섭취 양은 낮았다. 8) 결론적으로 라오스의 도시는 농촌에 비해 식습관에서 서구화의 영향을 더 받으며 전통적인 식사가 건강하지 않은 서구화된 식사로의 변화가 진행되고 있으며, 이는 비전염성질환을 증가 시키는 위험 요소로 작용 할 수 있다. 또한 도시 지역과 농촌 지역 모두에서 높은 만성질환율을 보이므로 건강한 식행동으로의 변화와 라오스의 만성질환율의 감소 및 예방을 위해 지역의 차이에 따른 차별화된 개별적 맞춤형 영양교육이 필요하다고 사료된다.
